Hand injuries are among the most frequent accidents seen in sports medicine. All too commonly they are considered trivial since the athlete may continue to participate actively and neglect his injury. The consequent delay in diagnosis and proper treatment may result in long-standing or even permanent disability. This paper describes the more commonly encountered hand injuries, their diagnosis and their optimal treatment. Included are soft-tissue injuries, ligamentous injuries, fractures and tendon avulsions. The basic principles applicable to skeletal and soft-tissue trauma of the hand, which

The principles illustrated in this brief survey are germane to all surgery. They cross whatever boundaries exist between specialties; in the hand, there are no divisions into parts for general, plastic, orthopedic, or neurosurgeons. They are essential to the success and parallel Boyes' description of Bunnell's influence in "the restoration of function, a careful detailed evaluation of the problem and a skillful manipulation of the parts, to accomplish the planned result. Solutions were sought and planned by thinking in broad terms of function, but the surgery is executed with maximum attention

A review of 429 cases of carpal tunnel syndrome in 329 patients confirms the prevalence of this entity among disorders of the hand. Three symptoms are almost diagnostic--daytime numbness, nighttime pain, and morning stiffness of the fingers. The diagnosis is confirmed by findings on physical examination. A relatively simple technique is presented for the exposure and release of the median nerve, using a longitudinal incision in the palm and stopping short of the wrist skin. Limiting the incision to the palmar skin results in an inconspicuous scar, while still providing adequate exposure for a

Clinical and electrophysiological studies were performed on 250 patients with carpal tunnel syndrome. Acroparaesthesia was often referred to all digits (64%) and proximal pain was not uncommon (46%). The male patients as a group differed from the female in that nocturnal acroparaesthesia was less frequent and a causal of predisposing factor was identifiable in the majority (76%). The most common associated condition in the females was trigger finger (11%). An associated ulnar nerve lesion was rare. Delay in diagnosis was attributable to cultural factors or lack of awareness of the occurrence o

Patients with Hurler's syndrome (MPS-1H), I-cell disease (ML-II) and pseudo-Hurler's syndrome (ML-III) had median nerve compression and triggering of the fingers which limited finger extension. To our knowledge, this combination has not been reported previously in patients with mucopolysaccharidoses and related disorders. In all of our 3 cases the median nerve was compressed by thickened flexor tenosynovium. Synovectomy and resection of the volar carpal ligament improved the hand function in all, including the mentally retarded patient with Hurler's syndrome. Release of the fibroosseous tunnel

Among 450 patients examined for neck pain following automobile accidents, carpal tunnel syndrome (CTS) was diagnosed in four and trigger finger (TF) in three. All seven patients sustained cervical sprains and were drivers or passengers in automobiles which received impacts from the rear. All seven patients reported the onset of hand symptoms during or shortly after the accident. This unusual occurrence, not previously reported, appears to be related to the events which occurred at the time of the accident. The mechanism of injury for CTS is hypothesized to be median nerve compression by moment

Volar fracture dislocation of the second and third metacarpal bases associated with acute carpal tunnel syndrome in a 17-year-old football player were treated with open reduction, via volar incision to decompress the carpal tunnel, and via a second dorsal incision for internal fixation of the fracture dislocations with Kirschner wire fixation and reinsertion of the avulsed extensor carpi radialis:longus tendon. Anatomic reduction by closed or open reduction is recommended to avoid leaving patients with a weak grip and pain over the site of the fracture.

In a follow-up study 60 hands of 49 patients were examined 1-12 years after operation for carpal tunnel syndrome. The syndrome was diagnosied in the majority of cases on clinical examination only. Paresthesia, hypesthesia or anesthesia in the 3 1/2 radial fingers of the hand are typical signs. Some patients complained of severe pain irradiating towards the elbow and shoulder. The method of surgical treatment is described in detail. Of the 49 patients only 2 were dissatisfied with the result. Both patients suffered from stenosing arteriosclerotic disease of the peripheral arteries of the hand a

Many PIP joint injuries which appear to be innocuous are actually very severe injuries to this small hinged joint and carry a high percentage of disability for the hand as well as the individual digit. A flexion contracture of the PIP joint of one digit may significantly reduce the functional capacity of the entire hand. To prevent such physical impairment, the type of PIP joint injury must be defined and the proper treatment for the particular injury must be initiated. Second, it is paramount that prolonged immobilization of this joint be prevented. It has been found, through clinical experie

A survey was made of the work and hobbies of 658 patients with the carpal tunnel syndrome. Seventy-nine per cent of the patients were employed in work requiring light, highly repetitive movements of the wrists and fingers. Fifty-one per cent of the unemployed housewives knitted. It is suggested that this type of manual activity can be a causal factor in the development of the, so-called, 'spontaneous' carpal tunnel syndrome.
