Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

The widespread use of smartphones and electronic devices has raised concerns about their impact on hand and wrist health. This scoping review synthesised current evidence on musculoskeletal and soft-tissue disorders associated with device use. Thirty-two studies were included, examining smartphones, computers and other digital tools. Smartphones were primarily linked to De Quervain's tenosynovitis and flexor pollicis longus tendinopathy, while computers and visual display units were associated with carpal tunnel syndrome and repetitive strain injuries. Key risk factors included repetitive thum

BACKGROUND: Digital nerve injuries are the most prevalent form of peripheral nerve injuries. We aimed to conduct a systematic review and Bayesian network meta-analysis to determine the optimal technique for sensory restoration in patients undergoing digital nerve repair.
METHODS: An electronic search was conducted from inception to September 2025. Thirteen studies were included comparing neurorrhaphy, allograft, autograft, and conduit. Outcomes analyzed included static 2-point discrimination (S2PD) and moving 2-point discrimination (M2PD), presented as mean difference (MD) with 95% confidence

PURPOSE: We describe a technique to reconstruct fingertip defects with dorsal digital artery perforator flaps innervated with terminal branches of the proper digital nerve in a propeller manner.
METHODS: To reconstruct fingertip defects, a dorsal digital artery perforator flap innervated with terminal branches of the proper digital nerve was performed. This retrospective study includes 44 fingers and 4 thumbs in 45 cases. Thirty-seven males and eight females with a mean age of 43.7 years were enrolled. The sizes of large paddles ranged from 2.0 cm × 1.3 cm to 6.0 cm × 1.6 cm, and the sizes of

BACKGROUND: Peripheral nerve injuries (PNI), particularly digital nerve injuries, can lead to significant loss of function, pain, and sensory deficits. Autologous nerve grafting (ANG) has been the gold standard for nerve gap reconstruction, but it is associated with donor site morbidity and other complications. Nerve conduits, including collagen-based, chitosan-based, and muscle-in-vein conduits, have emerged as potential alternatives with the advantage of avoiding donor site morbidity. The objective of this systematic review and meta-analysis was to compare the efficacy and safety of nerve co

Tetraplegia is a significant impairment of mobility in all four limbs, often following a cervical spinal cord injury. Recent studies in the management of tetraplegia describe the use of nerve transfer in addition to tendon transfer for upper extremity rehabilitation. While the popularity of these techniques is growing, there are limited studies describing the long-term outcomes and functional recovery following such procedures. This case describes a patient who sustained a C7 spinal cord injury, right olecranon fracture, and left distal radius fracture following a fall. Surgeons performed open

Thumb amputation is a severe injury that significantly affects hand function. Replantation remains the preferred treatment, particularly in young and active individuals. However, prolonged ischemia time is traditionally considered a limiting factor for successful replantation. We present a case of a 25-year-old right-hand-dominant male who sustained a traumatic saw-cut amputation of the left thumb at the base of the proximal phalanx and underwent successful replantation approximately 24 hours post-injury. Surgical management included skeletal fixation with Kirschner wires, microvascular anasto

Background/Objectives: Finger amputation is frequently followed by complications, with reported revision rates of up to 20%. One of the most disabling sequelae is the formation of painful neuromas, occurring in approximately 3-9% of cases. Several biological and mechanical risk factors have been proposed, but the potential influence of psychological traits remains poorly understood. This study aimed to investigate whether a correlation exists between patients' personality traits and the development of neuropathic pain or related symptoms. Methods: A retrospective study was conducted at a Level

Background: Although surgical digit amputations are common procedures in hand surgery, substantial differences in technical approaches have been reported, with the development of symptomatic neuromas posing a major challenge. Given the absence of a universally accepted technique, especially concerning painful neuroma prevention, this survey study's rationale was to explore the preferred surgical approach for traumatic noninfected digital amputation injuries (excluding the thumb) at the interphalangeal (IP) joint levels. Methods: A 10-point online questionnaire was sent to ÖGH, DGH and SGH memb

INTRODUCTION: A traumatic nerve injury that is improperly treated or left untreated may result in a painful neuroma with compromised sensory and motor function. Affected individuals might be hesitant to donate a healthy nerve for an autologous nerve graft procedure. The use of processed nerve allografts (PNA) to reconstruct nerve injuries with painful neuromas is less often discussed. We aim to offer perspectives on treating painful neuromas with PNA when autograft reconstruction is considered not an option.
METHODS: This series involves five individuals with neuroma formation and varying pai

BACKGROUND: Indications for pediatric digital replantation/revascularization (replant/revasc) are broader than those for adults. Pediatric patients suffer many of the same challenges seen in adult replant/revasc patients with similar rates of digit salvage. The purpose of this study was to review the incidence of and long-term outcomes after pediatric replant/revasc.
METHODS: Data was collected from medical records of patients presenting to our Level 1 Trauma Pediatric Hospital Emergency Room between January 2011 and March 2017. All charts with ICD-9 codes for upper extremity trauma were quer

Neonatal total brachial plexus palsy (NBPP) is one of the most challenging birth injuries, especially if it is associated with hand palsy and sensory loss. Current techniques offer limited success for both extrinsic and intrinsic hand muscles recovery, especially for the latter. We present an innovative combined surgical method designed to reinnervate the hand in complete palsies: (1) the ulnar nerve of the affected limb, which is insensate and nonfunctional, is harvested as an autologous graft and used to connect the contralateral C7 root to the medial (motor) trunk contribution to the median

Spaghetti wrist injuries are complex distal forearm lacerations involving flexor tendons, nerves, and vessels, associated with significant functional morbidity. We present the case of a 64-year-old right-handed man with a deep cut wound to his left forearm and clinical signs of distal ischemia, complete loss of digital flexion, and palmar anesthesia. An urgent examination was performed, identifying sections of the radial and ulnar arteries, median nerve, and injuries to the palmaris longus, flexor carpi ulnaris, flexor carpi radialis, superficial and deep flexor tendons of the fingers, and fle

PURPOSE: The motor branch to the brachialis muscle has been described as a versatile donor in upper limb nerve reconstruction. This includes patients with peripheral nerve injuries and tetraplegia. However the anatomic feasibility of transfers from the musculocutaneous nerve to radial nerve branches around the elbow has not been systematically evaluated. This study aims to assess this feasibility. Such a transfer may be particularly relevant in patients with high-level tetraplegia or in patients with spastic upper limb involvement, in which selective denervation of spastic elbow flexors could

PURPOSE: Median nerve (MN) block under local anesthesia at the wrist is commonly performed but can be limited by anatomic variation, particularly absence of the palmaris longus tendon. This study investigates a novel trans-flexor carpi radialis (trans-FCR) technique for MN anesthesia that uses the flexor carpi radialis (FCR) tendon as a consistent surface landmark.
METHODS: Fourteen fresh frozen upper extremity cadaver specimens were used. A 25-gauge needle was inserted through the FCR sheath approximately 4 cm proximal to the wrist crease in a transverse fashion, angled approximately 0° to t

PURPOSE: Unstable fractures of the distal ulna present a clinical challenge. The purpose of this study was to describe the outcomes following fixation of unstable distal ulnar neck fractures via a retrograde intramedullary threaded nail (IMTN).
METHODS: We retrospectively identified patients at a single level 1 trauma center, between 2022 and 2025, who underwent surgical fixation of an unstable distal ulnar neck fracture with a retrograde IMTN. We included patients at least 1 year postinjury. Demographics, fracture characteristics, functional outcomes, radiographic outcomes, and complications

Volar locking plates become the standard treatment for distal radius fractures and are mostly applied through volar approaches that rely on the flexor carpi radialis (FCR) as a superficial landmark. Although usually straightforward, anomalous flexor musculature may complicate exposure, misdirect dissection and increase the risk of iatrogenic neurovascular injury. We report two rare anatomical variants encountered during open reduction and internal fixation: FCR brevis (FCRB) associated with anomalous insertion of the pronator quadratus, and complete agenesis of the FCR. Both substantially alte

PURPOSE: Complex regional pain syndrome (CRPS) is a debilitating complication reported after upper-extremity surgeries, yet its incidence following isolated cubital tunnel surgery (CuTS) remains undefined. This study aimed to estimate the incidence of CRPS after isolated CuTS and to identify independent patient-level risk factors.
METHODS: A retrospective cohort study was conducted using the TriNetX research database. Patients who underwent a single isolated CuTS between 2000 and 2024 were identified. One-year cumulative incidences of CRPS type I and type II were calculated, excluding individ

Reconstruction of the interosseous membrane of the forearm has gained popularity over recent decades and several surgical techniques have been described to treat the longitudinal instability of the forearm after an Essex-Lopresti injury (ELI). We propose a synthetic reconstruction of the interosseous ligament (IOL) with double Arthrex FiberTape® and present data from cadaveric specimens used to test the reconstruction. After reconstruction, proximal migratory forces were applied to the radius to assess the radioulnar displacement at wrist level. Safety of the technique was also assessed by mea

Volkmann ischemic contracture is a severe and largely preventable complication of untreated or delayed compartment syndrome. In children, it is most commonly associated with supracondylar humerus fractures, especially when external compression from casts or splints is not promptly recognized. We report the case of a six-and-a-half-year-old right-handed girl who developed severe left upper-limb dysfunction after a closed supracondylar humerus fracture treated in a rural setting with tight traditional splint immobilization, locally known as "Jbira." No fasciotomy was performed during the acute p
