Sensitivity and specificity of vibrometry for detection of carpal tunnel syndrome.

A cross-sectional study was performed to assess the utility of vibrotactile thresholds (VTs) obtained before and after a 10-minute period of wrist flexion as a method for detection of carpal tunnel syndrome (CTS) among adult subjects. Subjects with hand discomfort were recruited from patients referred to a university-based electromyography laboratory. Asymptomatic subjects were recruited from among office and technical staff at a professional school. In addition to electrophysiologic evaluation (EP), all subjects were offered VT measurement of the index and small fingers, bilaterally, before a

Importance of crossover innervation in digital nerve repair demonstrated by nerve isolation technique.

This study was conducted to determine how testing of nerve repair using nerve isolation techniques compares with standard testing. Nerve isolation consisted of double-gloving, leaving the study finger free, and administering local anesthetic blocks to all other sensory contributions in the exposed digit. Ten patients with 13 single digital nerve repairs of equally severe injuries were studied at a mean of 41 months postoperatively. Standardized tests yielded an excellent result in 77% for static two-point discrimination (S2pd), 85% for moving two-point discrimination (M2pd), and 46.2% for Semm

[Transposition of flexor pollicis brevis muscle for reconstruction of opposition function of the thumb: anatomical study and clinical application].

Anatomical study was performed on 20 cadaveric hands and the results showed that M. flexor pollicis brevis overlap the M. abductor pollicis brevis for about half of its width at their origins and the overlapping reduced to about 1/3 of the width at the muscle belly level. The M. flexor pollicis brevis chiefly inserts on the palmar aspect of the base of proximal phalanges. The M. abductor pollicis brevis primarily inserts to the radial side of MP joints, the M. flexor pollicis brevis is entirely innervated by the deep branch of the ulnar nerve. In order to increase the angle bettween longitudin

Safety and efficacy of percutaneous trigger finger release.

Twenty-five A1 pulleys in 5 fresh cadaveric hands and 13 trigger fingers in 11 patients were released percutaneously with a 19 gauge needle, as described by Eastwood et al., to determine the efficacy and safety of the technique. Over 90% of the length of each individual finger and thumb A1 pulley were successfully released in the cadaveric digits with no injuries to the A2 pulley, nerves, or vessels. Superficial abrasions were noted in four superficialis tendons. In our surgical series, complete clinical release (eradication of triggering) was achieved in each digit. In 8 of 13 digits, the A1

Percutaneous A1 pulley release: a cadaveric study.

Percutaneous A1 pulley release was performed on 17 fresh-frozen cadaveric hands with a 14-gauge angiocath needle. Each hand was then explored to assess the adequacy of release and the degree of injury to adjacent structures. Complete release of the A1 pulley was obtained in 45 of the 66 fingers and in 10 of the 17 thumbs. Significant injury to the flexor tendons was observed in two digits. All tendon injuries occurred along the line of the fibers. There were no digital nerve injuries. The release was within 2 mm of a thumb digital nerve in seven hands and the little finger ulnar digital nerve

[The strategy of the finger bank].

The authors present 40 cases of multidigital amputations with a "bank-finger" reconstruction. They use of flaps, tendons, nerves grafts .... from the injured finger. Moreover, they propose a guide in the use of heterotopic implantations, in emergency and in secondary. The indications of the digital translocation is discussed about the priority finger and the localisation of the others fingers to be reimplanted. They make mention of the rare possibility of temporary ectopic implantation.

Phentolamine reversal of epinephrine-induced digital vasospasm. How to save an ischemic finger.

A 17-year-old girl accidently injected her thumb with an adult autoinjector epinephrine syringe, resulting in rapid digital ischemia. Local infiltration of 0.5% phentolamine mesylate injected at the puncture site immediately resolved the ischemia and resulted in no long-term sequelae. Similar cases of epinephrine-induced digital ischemia are reviewed herein, revealing phentolamine to be the drug of choice for reversal of this type of ischemia. Alternative attempts to restore blood flow included warm water immersion, amyl nitrite inhalations, metacarpal nerve block, and application of topical n

Sensory neural conduction of median nerve from digits and palm stimulation in carpal tunnel syndrome.

The median sensory nerve conduction between ring finger and wrist is a suitable parameter for early detection of carpal tunnel syndrome (CTS), although shorter segments of median nerve have also been proposed for the same goal. In order to assess the relative diagnostic value of the sensory nerve conduction velocity (SNCV) of the third palmar branch versus the SNCV of the second palmar branch, generally performed until now, we studied 62 patients with typical signs and symptoms of CTS. The following parameters were evaluated by surface recording: orthodromic SNCVs in digit-wrist segments for m

A comparative study of the heterodigital neurovascular island flap in thumb reconstruction, with and without nerve reconnection.

41 heterodigital neurovascular island flaps were used to cover defects of the tactile pad of the thumb in 17 years. With an average follow-up of 75.5 months, 30 patients were reviewed. 17 were treated by the original Littler technique and 13 were treated with the same flap reconstruction but with division of the digital nerve innervating the flap and re-anastomosis of this nerve to the proximal nerve end of the ulnar digital nerve of the thumb. Good aesthetic and functional results were achieved in both groups. Sensory acuity did not appear to decrease with time. The nerve reconnection techniq

Reverse Phalen's maneuver as an aid in diagnosing carpal tunnel syndrome.

A reverse Phalen's maneuver involves wrist and finger extension held for 1 minute. We showed that this maneuver results in a significantly higher intracarpal canal hydrostatic pressure as compared to a traditional Phalen's or a modified Phalen's maneuver. Additionally, 31 individuals with complaints of carpal tunnel syndrome symptoms and 20 normal controls were evaluated to see what effect the reverse Phalen's maneuver would have on median sensory latency and amplitude. Both groups demonstrated a prolongation of the median sensory revoked response after 1 minute of this maneuver. The control g

[The sensory potential of the ring finger. The value of electromyographic diagnosis of carpal tunnel syndrome].

Accurate diagnosis of carpal tunnel syndrome relies on electromyography. The usual criteria (sensory conduction velocity and motor latency) fail to identify 10% of cases of carpal tunnel syndrome. The sensory innervation of the ring finger (digit IV) is shared by the median and ulnar nerves. This anatomic feature is the basis of a novel electromyographic test: measurement of the sensory potential of digit IV by an orthodromic method with stimulation of the finger via a ring electrode and recording at the wrist. Analysis of the sensory potential of digit IV in 200 subjects with no median nerve

The effect of wearing a flexible wrist splint on carpal tunnel pressure during repetitive hand activity.

We investigated how repetitive hand activity normally affects carpal tunnel pressure and whether a flexible wrist splint can influence this effect. Nineteen healthy subjects were evaluated under four test conditions: at rest with and without a wrist splint (baseline) and while performing a repetitive task with and without a wrist splint. The task involved loading and unloading 1 lb. cans from a box at a rate of 20 cans per minute for period of 5 minutes. Carpal tunnel pressure and wrist angles were continuously monitored by means of a fluid-filled catheter inserted into the carpal canal and a

Sensory recovery following total and subtotal amputations distal to the wrist and digital replantations.

Apart from the basic aim of revascularization, other criteria for the success of digital replantation are functional and sensory recovery. Tactile gnosis can only be restored in cases of replantation distal to the wrist by paying great attention to the technical details for nerve repair. Of 253 patients who underwent 312 digital replantations, after total or subtotal amputations, over 16 years, 162 with 215 successful replantations and 377 nerve reconstructions returned for follow-up evaluations. Of 320 patients who underwent 395 nerve repairs after isolated severed digital nerves, 198 with 24

Successful reconstruction of a high-pressure injection injury of the hand using a first web flap of the foot.

High-pressure injection injury to the hand often results in loss of tissue and hand function. The successful reconstruction of a hand following high-pressure injection injury is reported. A free neurovascular flap was transferred from the first web space of the foot to cover a skin defect in the first web space area of the hand. The digital nerves of the thumb and index finger were repaired using sural nerve grafts.

Long-term follow-up of hand function after digit replantation.

A long-term follow-up study of 26 patients with 28 replanted digits was performed with respect to circulatory, sensory, and motor function. Peripheral circulation in the replanted digits was comparable with corresponding uninjured digits on the opposite hand. Excellent sensory recovery was observed in the territories supplied by 30 of the 36 repaired digital nerves. Best functional results were noted with the thumb and with fingers amputated through the middle phalanx. By objective criteria, 21 of the 28 replanted digits demonstrated satisfactory function. Subjectively, however, only 1 patient

An analysis of the results of late reconstruction of 132 median nerves.

132 patients with injuries to the median nerve were assessed on average 10.4 years after repair. There were 106 males and 26 females, with a mean age of 28.4 years. Most of the nerve lesions were sharp (76) or blunt (47) injuries. Division was total in 87 cases, and most were at the wrist level. Secondary repair was performed in 34 cases and fascicular grafting in 98 cases. The average gap was 5.8 cm. A quantitative evaluation and grading according to the MRC classification was created to reflect sensory and motor recovery. Excellent or good results were obtained in only 49.2%. The age of the

Neurophysiological investigation of hands damaged by vibration: comparison with idiopathic carpal tunnel syndrome.

Forty-seven men with numbness and paresthesiae in their hands after long-term occupational exposure to vibrating hand-held tools were examined neurophysiologically and clinically. The vibration thresholds of the finger tips were assessed and fractionated neurography of the median nerve motor and sensory fibres carried out. They were compared with control groups of healthy subjects and patients with idiopathic carpal tunnel syndrome. The patients exposed to vibration had major increases in their finger vibration thresholds, shown with both the Goldberg-Lindblom vibrameter and the Lundborg vibro

Changes in carpal tunnel shape during wrist joint motion. MRI evaluation of normal volunteers.

This study evaluated dynamically the morphology of the carpal tunnel during the wrist motion. The object was to determine the transverse dimension (TD), anteroposterior (AP) dimension and cross-sectional area (CA) of the carpal tunnel in the neutral, flexed and extended positions. The wrists of eight male and eight female volunteers aged 20 to 38 years were studied. With wrist extension, AP and CA decreased while the TD increased at the pisiform level, and CA increased at the hamate level. With flexion, TD and CA decreased at the pisiform level and CA decreased at the hamate level. The results

Clinical application of motorsensory differentiated nerve repair.

Since 1979 acetylcholinesterase has been used in clinical practice for motor sensory differentiation. It was first used for median and ulnar nerve injuries at the wrist. Recently the application was extended to secondary nerve repair, including plexus reconstruction. The aim of this study was to present, for the first time, clinical results of motor sensory differentiated median and ulnar nerve repair and to show the advantage of this method in secondary repair. We compared a group of nine patients with motor sensory differentiated median and ulnar nerve repair with a group of 13 patients with

[Bicycle rider's ulnar nerve paralysis].

The ulnar neuropathy in bicycle riders is a less common occurrence, due to a local damage of the nerve on the level of the ulnar tunnel. It has been described in neurological literature only a few times, and hence its good prognosis is not sufficiently known. Within a short time two patients of this disease were transferred to our clinic for operation. After a local space-occupying growth in the tunnel could be excluded by MR or sonography of the wrist, we discouraged performance of an operative procedure. The patients' complaints and the neurological deficits vanished in a few weeks. Ulnar ne
