Staged Reconstruction of a Complex Zone 3 Injury Involving Neurovascular and Tendon Repair.

We present a case of a 67-year-old male who sustained a complex glass injury to the right palm involving Zone 3. Intraoperative exploration revealed full-thickness lacerations of the flexor digitorum profundus (FDP) and flexor digitorum superficialis (FDS) tendons to all four fingers, a 22 mm median nerve gap requiring cabled nerve allograft repair, laceration of the deep motor branch of the ulnar nerve and fourth common digital nerve requiring conduit-assisted repairs, and a segmental laceration of the ulnar artery requiring microsurgical reconstruction. The patient underwent staged reconstru

Quantification of Palmar and Digital Epidermal Thickness and Sweat Duct Density.

Palmar digital skin, with its high density of epidermal sweat glands and unique stratum corneum, derives its autonomic innervation from peripheral nerves. Understanding the changes to these structures could serve as objective proxy markers following peripheral nerve injury and interventions to promote regeneration. There are limited data in humans for these measures, and normative reference ranges are yet to be defined. Light-based imaging techniques, such as optical coherence tomography (OCT), can detect quantifiable changes in these epidermal parameters. This study aimed to define the normal

Digital Nerve Coaptation vs Standard of Care for Partial and Ray Digital Amputations.

BACKGROUND: Traction neurectomy (TN) remains the standard of care for digital nerve management in partial and ray digital amputations but is associated with both neuroma formation and chronic postoperative pain. The purpose of this study is to evaluate if digital nerve coaptation (DNC) is associated with lower postoperative pain and complication rates compared to standard TN. We hypothesize that patients undergoing digital amputations with DNC will demonstrate lower postoperative complications, reduced rates of persistent pain, and fewer reoperations for nerve-related pain. Risk of Injury to the Superficial Branch of the Radial Nerve in Dorsal and Anterolateral Approaches to the First Carpometacarpal Joint.

BACKGROUND: Complications of thumb basal joint arthritis surgery include surgical wound pain and cheiralgia paresthetica through involvement of the radial nerve superficial branch (RNSB). The aim of this study is to compare the risk of nerve injury between anterolateral and dorsal approaches to the first carpometacarpal joint (1CMCJ) by measuring the distance between the incisions and RNSB, recording crosses between them, and evaluating the density of skin nerve endings at incision sites. METHODS: In this descriptive study of 20 cryopreserved cadaver specimens, the anatomical distribution of

Minimizing risk to the dorsal sensory branch of the ulnar nerve in wrist arthroscopy: an anatomical study.

Wrist arthroscopic procedures using ulnar wrist portals, such as the 6-ulnar and direct foveal portals, have a risk of injuring the dorsal sensory branch of the ulnar nerve. This study investigated the anatomy of the dorsal sensory branch of the ulnar nerve and its positional changes during supination, neutral rotation and pronation at specific landmarks to find an optimal forearm position to minimize the risk of iatrogenic nerve injury during arthroscopy. The concept of 'nerve density' was introduced to quantify the likelihood of encountering the dorsal sensory branch of the ulnar nerve withi

Radial sensory nerve to anterior interosseous nerve transfer for symptomatic neuromas- clinical outcomes and a transfer classification system.

BACKGROUND: Radial sensory nerve (RSN) injuries occur during common surgical procedures or injuries to the wrist and often result in the formation of painful neuromas. Management strategies of primary repair or secondary reconstruction are limited by poor patient satisfaction. We present a targeted muscle reinnervation (TMR) technique, its clinical outcomes, and a novel classification system for the treatment of recalcitrant RSN neuromas with transfer of the RSN to the anterior interosseous nerve (AIN). METHODS: Cadaveric specimens were used to devise a classification system for the transfer.

Case Report: Palmaris Brevis Syndrome Resulting From Acute Wrist Hyperextension.

A 24-year-old, 19-week pregnant female presented to a community hospital emergency department following an acute hyperextension injury to the left wrist. The pertinent findings on the initial examination included pain, swelling of the ulnar aspect of her palm, and decreased, painful flexion of her small finger. The emergency room physician suspected a flexor tendon injury, and urgent referral to a tertiary level plastic surgery center was made. After plastic surgery evaluation, it was determined that her symptoms were related to the ulnar nerve opposed to an acute tendon injury. After ruling o

Targeted Muscle Reinnervation Using the Anterior Interosseous Nerve for Symptomatic Wrist Level Neuromas.

BACKGROUND: Iatrogenic and traumatic sensory nerve injury at the level of the wrist can lead to debilitating neuroma. Targeted muscle reinnervation (TMR) is an effective treatment for the management of symptomatic neuromas. We investigate the use of the anterior interosseous nerve (AIN) as a recipient nerve for the treatment of iatrogenic neuromas. This case series describes 4 patients treated for neuromas of the lateral antebrachial cutaneous nerve (LABC), palmar cutaneous branch of median nerve (PCB), and radial sensory nerve (RSN). METHODS: Four cases involved a symptomatic neuroma of the

Wrist Extension Reconstruction Using Distal Anterior Interosseous to Extensor Carpi Radialis Brevis Nerve Transfer in Brachial Plexus Injuries.

PURPOSE: Reconstruction of wrist extension in C5-C8 root brachial plexus injuries (BPIs) is challenging, because of limited nerve and tendon donors. The purpose of this study was to report outcomes relating to the pronator quadratus branch of the anterior interosseous nerve (AIN) to extensor carpi radialis brevis (ECRB) nerve transfer from three BP surgeons. METHODS: Ten patients with C5-C8 BPI underwent AIN to ECRB nerve transfer. The mean age of the patients was 30.5 ± 15.9 years. All patients were men. The mean time to nerve surgery following initial injury was 4.5 ± 1.2 months. In all pat

Epidemiology, risk factor, and economic analysis of peripheral nerve injury following distal radius fractures.

PURPOSE: Distal radius fractures (DRFs) are a common orthopaedic injury with a bimodal age and gender distribution, comprising male children after high-energy trauma and older women after low-energy falls. Complications like malunion, ligament ruptures, and nerve injuries can significantly impact recovery. This study aimed to address the gap in population-based data by analyzing the prevalence and risk factors for peripheral nerve injuries associated with DRFs. METHODS: The National Inpatient Sample database was used to analyze DRFs patients with associated ulnar, median, or radial nerve inju

Extending reconstructive possibilities in tetraplegia; the results of 137 nerve transfers.

We present the results of 137 nerve transfers in 56 limbs (35 participants, 21 bilateral) with special reference to the International Classification for Surgery of the Hand in Tetraplegia, as outcomes for Groups 0 and 1 are rarely reported. These participants were assessed for a minimum of 20 (median 36) months after operation. Medical Research Council grades and strength measurements showed that for Group 0, reconstruction of useful wrist extension was achieved in six out of eight limbs. Reanimation of finger and thumb flexion in this group remains difficult and in only one out of four limbs

Topographic anatomical landmarks for targeted nerve infiltration in distal radius fracture surgery.

The study focuses on the increasing incidence of distal radius fractures, particularly in individuals under 15 and over 50, and the critical need for effective perioperative pain management strategies. Distal radius fractures often require surgical intervention and pain control, essential to enhancing recovery and minimizing complications like chronic regional pain syndrome (CRPS). Standard analgesic options include hematoma blocks, Intravenous regional anesthesia (IVRA), regional nerve blocks, sedation, and general anesthesia. This research proposes a nerve block approach targeting the anteri

Combined nerve and tendon transfer (CNaTT) for grasp and release function in patients with tetraplegia: a matched prospective pilot study.

INTRODUCTION: Traditional single-stage tendon transfer procedures in patients with tetraplegia are designed to restore active pinch and grasp function. However, digital extension is usually achieved by wrist tenodesis. More recently, nerve transfer has allowed potential restoration of digital extension. This matched observational pilot study compares clinical outcomes after traditional tendon transfers vs. a study group using two-stage using combined nerve and tendon transfer. METHODS: Eighteen patients in the traditional tendon transfer control group and 18 patients in the combined nerve and

Long Term Analysis of 11 Consecutive Contralateral C7 to Lower Trunk Transfer via a Pre-spinal Approach in Adult Pan Brachial Plexus Injury Patients.

Background: Contralateral C7 (CC7) is an important donor nerve for reconstruction in global brachial plexus palsy. In 2013, Shu Feng Wang introduced the direct repair of lower trunk (LT) to CC7. The outcomes of hand recovery reported in the study was significantly better than the traditional techniques. We did a retrospective analysis of 11 consecutive adult patients who underwent reconstruction for pan-brachial plexus injury by repair of the CC7 to LT along with other extraplexal nerve transfers. Methods: Patients were evaluated for motor recovery of flexion and extension at elbow, wrist and

Preserved finger flexion following high median nerve transection: a rare case report and review of literature.

BACKGROUND: High-level complete transection of the median nerve will impair the flexion function and sensation of the thumb and index finger, and will also result in weakened wrist flexion strength. In rare cases, atypical clinical manifestations may arise, potentially due to ulnar-to-median nerve anomalies, such as Marinacci communication (MC), or the function of muscles directly innervated by ulnar nerve branches. CASE PRESENTATION: A 52-year-old male sustained a chainsaw injury to his left elbow and forearm, resulting in complete transection of the median nerve. Despite this, he retained p

Does Incorporation of the Dorsal Cutaneous Branch of the Proper Digital Nerve in Anterograde Neurovascular Island Flap Improve Outcomes? Results of A Retrospective Case Series.

BACKGROUND: Fingertip injuries present a challenge for surgeons. Factors such as appearance, sensation, and grip strength influence the choice of procedure. The neurovascular island (NVI) flap is commonly used due to its reliability and favorable results. However, it can lead to reduced sensation and a limited range of motion, which are significant concerns. METHODS: This study analyzed 56 adult patients with 66 finger injuries treated with a modified NVI flap by a single surgeon. The flap is based on branches of the proper digital artery to the volar and dorsal skin, including two nerves in

A Two-Year Retrospective Comparison of Clinical Outcomes Between Open Release and Percutaneous Release of the Trigger Finger.

BACKGROUND: Trigger finger is often found in adults, accompanied by pain, swelling, and limited movement. If conservative treatment fails, surgical release of the A1 pulley will be performed. Percutaneous trigger finger release is a new surgery method with rapid recovery that includes a good outcome, but there is also more reluctance to use this standard technique than open surgery. This article, therefore, presents a comparison of the long-term outcomes of both methods across different factors, similarities, and patient groups. The study hypothesized that the percutaneous trigger finger relea
