Crossover innervation after digital nerve injury: myth or reality?

Several clinical studies promulgate the concept that some degree of crossover innervation occurs after digital nerve injuries are sustained and that the intact digital nerve might even substitute for the loss of nerve function on the injured side. Other studies strongly dispute the existence of this phenomenon. An excellent model for evaluation of crossover innervation is bilateral sharp digital nerve lacerations because there is no confusion of anomalous innervation from an intact contralateral nerve. This model avoids problems seen with replanted digits such as the inherent ischemia, multist

A model instrument for the documentation of outcome after nerve repair.

We present a new model for documentation and quantification of the functional outcome after nerve repair at the wrist or distal forearm level and a protocol that includes a numerical scoring system. The model, presented here along with validation and reliability test results, supports our hypothesis that the summarized test results reflecting specific functional limitations correlates well with the patient's opinion of the impact of the nerve injury on activities of daily living. Seventy patients with nerve repair were examined using the protocol. Analysis included 3 factors: sensory domain (s

Can a battery of functional and sensory tests corrobrate the sensorineural complaints of subjects working with vibrating tools?

OBJECTIVES: The objective of the present paper is to study the relationship between the early sensorineural symptoms, classified according to the Stockholm scale, and the results of the main functional and sensory tests described in the literature, in subjects working with vibrating tools. METHODS: Three groups of male workers were selected from industry: one group (69 subjects) exposed to hand-arm vibration in several workplaces, one group (62) performing heavy and repetitive hand and arm work but without exposure to vibration, and one control group (46) performing light and non-repetitive t

Partial nerve injuries in the upper extremity.

The level of injury of a peripheral nerve is a critical factor that has a great impact on the result of the repair. At the level of the wrist, the median and ulnar nerves have pure motor and sensory fascicular groups. Proximal to the wrist, the motor fascicular groups combine with sensory fascicles and become mixed nerves. Mapping the fascicular orientation with electrical stimulation is indicated for injuries located from the wrist to the distal third of the forearm. Successful application of this technique depends on the level of injury, anesthetic technique, and careful patient selection. C

Repair of muscle and musculotendinous junction injuries with an autogenous fascial patch.

We have used an autogenous fascia lata patch to repair muscle and musculotendinous junction injuries affecting 99 individual muscles in 23 patients with multiple incised wounds to the upper limbs. All patients were followed up for at least 2 years. Two patients with severe dystrophic changes following nerve injuries were excluded from the final analysis. Satisfactory grip strength was restored 6 months following the repair in 19 of the 21 patients we evaluated. Complete active and passive range of wrist and digital motion was seen in 18 of these 21 patients. Tightness of the flexor pollicis lo

Anatomic variations in sensory innervation of the hand and digits.

Anatomic dissections under microscopic magnification were performed on 30 fresh cadaveric hands to depict the course and interconnections of the sensory nerves to the digits. The dissections included the median nerve, the ulnar nerve, the superficial branch of the radial nerve, the dorsal branch of the ulnar nerve, and the dorsal branch of the proper digital nerve. The communicating branches between the median and ulnar nerves in the palm were found in 20 of the 30 (67%) specimens. The dorsal branch of the proper digital nerve was found to arise at or proximal to the A1 pulley zone in 62% of t

[The evaluation of the use of Shafiroff and Palmer modification (arterial reconstruction prior to bony stabilization) of thumb replantation].

The B. B. Shafiroff and A. K. Palmer modification of thumb replantation procedure proposed in 1982 was evaluated. The reconstruction or repair of the artery and sometimes digital nerve repair on the ulnar side of the thumb prior to bony fixation is the mainstay of the modification. It was used in 11 thumb replantations done by the author since 1987. The access to arteries and nerves is grossly improved and operating time shortened. Short description of 6 cases is given. All the replanted thumbs survived.

[Treatment of sections of the long flexor tendon of the thumb using the Rouhier technic. Apropos of 25 cases].

We treated 28 cases of Flexor Pollicis Longus tendon injury by Rouhier's technique between 1989 and 1996 and reviewed 25 cases, with a minimum follow-up of 6 months and maximum of 8 years. Nine patients presented an associated collateral nerve section. We used the International Federation of Hand Surgery Societies (IFSSH) topographic classification. The surgical technique consisted of Flexor Pollicis Longus lengthening and transosseous pull-out in 16 cases and distal suture in 9. The immobilization time ranged from 4 to 7 weeks. The results were evaluated according to Tubiana's classification,

Digital nerve repair by autogenous vein graft in high-velocity gunshot wounds.

Gunshot wounds to the hands are high-energy injuries that cause widespread tissue damage, including to the nerves. Great difficulty is encountered in later reconstruction with nerve grafting of gaps in these destructive and scarred wounds. We present our experience with three patients with digital nerve repair by autogenous vein graft performed at an early stage in this type of injury. Based on our experience and that of others, this simple and rapid technique suggests a high rate of satisfactory results. It also avoids extensive and destructive late dissection and the morbidity associated wit

The heterodigital reversed flow neurovascular island flap for fingertip injuries.

We report the results of pulp reconstruction with a new heterodigital reverse flow island flap. A dorsolateral flap from the middle phalanx, based on the digital artery is raised from the adjacent uninjured finger. The common digital artery, between the injured finger and the donor finger, is ligated and transected just before its bifurcation. The two converging branches of the digital arteries can be entirely mobilized as a continuous vascular pedicle for the flap. Thus the vascularization is now supplied by reverse flow through the proximal transverse digital palmar arch of the injured finge

Accuracy of the preoperative examination in Zone 5 wrist lacerations.

Fifty patients were studied to determine the accuracy of the preoperative physical examination in flexor tendon Zone 5 wrist lacerations. Initially, a retrospective review of the charts of 33 patients who were examined by a second- or third-year postgraduate physician was done. An average of three errors per examination was found. Then a prospective study of 17 patients was performed to see if an examiner's level of training in hand surgery influenced the accuracy of the examination. Error rates improved slightly with experience. Ninety-seven examinations were performed in the combined prospec

Percutaneous A1 Pulley Release a Clinical Study.

Percutaneous A1 pulley release is being increasingly used as an alternative to open surgical release and injection of local steroids for the treatment of the trigger digit. In 31 cases (26 patients), a percutaneous release was performed with a 14-gauge intravenous catheter needle under local anaesthesia in the outpatient setting. We report a 97% successful release and only one case of incomplete release. There were no digital nerve injuries, flexor tendon injuries, bowstringing or infections. By two months, all the patients had no pain at the operative site, and their preoperative range of mot

Surgical treatment of carpal tunnel syndrome and trigger digits in children with mucopolysaccharide storage disorders.

The role of surgical intervention for carpal tunnel syndrome (CTS) and trigger digits in children with mucopolysaccharide storage disorders (MPSDs) has not been clearly defined, particularly as the treatment of the underlying disease has advanced to include bone marrow transplantation. This study reviews our experience in the treatment of CTS and trigger digits in 22 children with MPSDs who were evaluated for CTS by electromyographic (EMG)/nerve conduction velocity (NCV) testing. Seventeen children were diagnosed with CTS by EMG/NCV testing and were treated with bilateral open surgical release

Snowblower injuries to the hand.

A retrospective review of 22 patients who sustained snowblower injuries to the hand was performed. There were 17 men and 5 women, ranging in age from 20 to 68 years (average age, 39.7 years). Fifty percent were manual laborers, 25% were unemployed, 15% were office workers, and 10% were not categorized. The dominant hand was involved in 86% of patients. In all patients, injuries occurred during an attempt to unclog manually the snowblower of wet snow. Patients were evaluated initially in the emergency room, where their wounds were irrigated and debrided, subungual hematomas drained, and nail be

Evaluation of the improvement of sensibility after primary median nerve repair at the wrist.

In the Istanbul Hand Surgery and Microsurgery Centre, between 1991-1996, 28 out of 54 patients who had clean-cut median nerve laceration at the wrist level were evaluated in a detailed manner regarding the improvement of sensibility following primary repair. Semmes-Weinstein monofilament and vibration tests form the group of threshold tests, and static and dynamic two-point discrimination tests and the Moberg pick-up test form the group of functional tests which were applied to the patients. Follow-up was from 1 to 5 years. Moreover, subjective findings such as cold intolerance, pain, and para

Acute carpal tunnel syndrome.

Although the carpal tunnel is open at both ends, it has the physiologic properties of a closed compartment bounded by synovium proximally and distally. When the intracarpal canal interstitial pressure rises above a critical threshold pressure, capillary blood flow is reduced below the level required for median nerve viability. Acute carpal tunnel syndrome is recognized frequently as occurring secondary to wrist trauma and infrequently due to a variety of infectious, rheumatologic, and hematologic disorders. This condition warrants prompt recognition and the treatment is early carpal tunnel rel

Predictors of rate of return to work after surgery for carpal tunnel syndrome.

OBJECTIVE: To evaluate the impact of patient demographics, clinical features, and job-related factors on the time until return to work after carpal tunnel release surgery. METHODS: We employed a cross-sectional community-based study of 59 patients who had undergone carpal tunnel release surgery. Sociodemographic, clinical, and job-related characteristics and time to return to work were obtained by interview and from medical records. Exposure to ergonomic risk was derived from an independently validated job matrix. Time to return to work after surgery was analyzed by survival techniques. RESU

Ulnar nerve entrapment at the wrist: cases from a hand surgery practice.

BACKGROUND: Eight patients with 11 instances of wrist-level ulnar nerve entrapment, a fairly uncommon compression syndrome, were identified in a hand surgery practice from 1992 through 1996. METHODS: Presentations, causes, and surgical outcomes were examined, and the pertinent literature was reviewed. RESULTS: All eight patients had extrinsic, nonidiopathic compression of the ulnar nerve caused by tumor, vascular disease, anomalous muscle development, or a tight fibrous arch at the origin of the flexor digiti minimi. In all cases, sensory symptoms resolved with removal of the cause of ulnar

Correlation of segmental carpal tunnel pressures with changes in hand and wrist positions in patients with carpal tunnel syndrome and controls.

We investigated pressures at 1 cm intervals along the carpal tunnel in 39 patients with carpal tunnel syndrome (CTS) and 12 controls. Pressures were measured for relaxed and gripping hand positions in combination with neutral, extended, and flexed wrist positions. Patient pressures exceeded control pressures, were below the previously reported 30 mmHg threshold for four of five locations in the relaxed neutral position and were typically greater in extension than in flexion. In the neutral position, both patient and control pressures were slightly above threshold levels just distal to the tunn
