Successful replantation of an avulsed middle finger.

In avulsion amputations of the digits, soft-tissue injuries are extensive and often require tendon, nerve, and vessel transfers or grafts. The functional results of such digital replantations are frequently less than ideal. Therefore, avulsion amputation of a single digit proximal to the insertion of the flexor digitorum superficialis has been a contraindication to replantation, because the anticipated poor functional result may interfere with overall hand function, and is not worth the sacrifice of a tendon, nerve, or vessel from another digit or transfer. The authors report a patient with av

Median mononeuropathy among active workers: are there differences between symptomatic and asymptomatic workers?

The objective was to determine whether symptomatic workers with an abnormal sensory nerve conduction study consistent with carpal tunnel syndrome differed, in terms of electrophysiologic measures, psychosocial, demographic, anthropometric, or ergonomic variables, from workers with an asymptomatic median mononeuropathy. This was a cross-sectional study of active workers at six different work sites. Cases were defined as workers with electrodiagnostic findings of a median mononeuropathy in either hand, based on a 0.5-msec prolongation of the median sensory evoked peak latency compared to the uln

Distance between the median nerve and ulnar neurovascular bundle: clinical significance with ultrasonographically assisted carpal tunnel release.

In ultrasonographically assisted carpal tunnel release, the zone of the transverse carpal ligament between the median nerve and ulnar neurovascular bundle is of significance because proximity of the cutting device to these structures depends on its width. In this study, we measured the width of the zone at 5 levels in 60 wrists from 54 patients with surgery-indicated idiopathic carpal tunnel syndrome. The width of the 5 levels ranged from 2.8 to 11.2 mm (mean, 7.1 mm) at the hook of the hamate, 3.0 to 12.5 mm (mean, 7.9 mm) at the distal one fourth of the carpal tunnel, 2.8 to 12.4 mm (mean, 8

Neurophysiological evidence of neuroplasticity at multiple levels of the somatosensory system in patients with carpal tunnel syndrome.

The human somatosensory cortex (S1) is capable of modification after partial peripheral deafferentation, but it is not known whether spinal and brainstem changes contribute to this process. We recorded spinal, brainstem and cortical somatosensory evoked potentials following ulnar nerve stimulation in patients affected by unilateral carpal tunnel syndrome with EMG evidence of chronic alterations in median nerve sensorimotor conduction at the wrist lasting at least 4 weeks, and compared them with those from the unaffected hand and with those obtained in a control group. Amplitudes of spinal N13

Femoral nerve transfer for treatment of brachial plexus root avulsion.

Femoral nerve transfer to the muscular branches of the thenar and hypothenar muscles was performed to determine its protective effect on the hand intrinsic muscles. Seven cases of brachial plexus root avulsion treated from May of 1989 to October of 1991 were involved. The femoral nerve transfer to the muscular branches of the thenar and hypothenar muscles was done at the same stage of multiple neurotization. The muscular branches derived from the femoral nerve were isolated and coapted with the thenar muscle branch of the median nerve and the deep branch of the ulnar nerve. A groin flap was ha

[Carpal tunnel syndrome in childhood. A case of early onset].

INTRODUCTION: The carpal tunnel syndrome (CTS) is the commonest neuropathy due to compression to be seen in adults. There are very few cases in the literature referring to patients of paediatric age, particularly those under ten years old. Most of these young patients had a metabolic disorder (mucopolysaccharidosis (MPS) or mucolipidosis (ML). In fact, as many as 90% of the MPS had CTS, sometimes subclinically. This syndrome is caused by compression of the median nerve at the level of the carpal tunnel, to which multiple factors may contribute, both local and systemic, as reviewed in this pape

The effects of tendon load and posture on carpal tunnel pressure.

Two pressure measurement techniques (catheter and bulb) were used to decompose the effects of tendon loads on carpal tunnel pressure (CTP). The catheter technique measures true hydrostatic pressure, whereas the bulb technique is a estimate of contact force or pressure on the median nerve. Eight cadaveric wrists were moved through a range of flexion-extension (0 degrees, 10 degrees, 20 degrees, 30 degrees, and 45 degrees of each) and radioulnar deviation (10 degrees and 20 degrees radial and 0 degrees, 10 degrees, 20 degrees, and 30 degrees ulnar) while CTPs were measured under 4 muscle loading

[Repair of pulp defect of thumb by free palmaris brevis musculocutaneous flap].

It is difficult to repair the pulp defect of finger with good function. Here reported two cases of pulp defect of the thumb which were repaired with free palmris brevis musculo-cutaneous flap. The flap was designed as the following: taken the line crossing the pisiform and metacarpo-phalangeal joint of the little finger as the longitudinal axis of the flap. The proximal end of the flap was at the level of pisiform and the distal end was the distal transverse palmar crease. The radial border was the radial side of the palmris brevis and ulnar border was the ulnar edge of the palm. The flap shou

Effect of long-term implanted nerve cuff electrodes on the electrophysiological properties of human sensory nerves.

During a long-term implantation (307 days) of a tripolar split cuff electrode around the palmar digital nerve to the radial side of the left index finger, branching off the median nerve in a medullary lesioned C6 patient, the physiological state of the nerve was intensively monitored. The resulting sensory nerve action potential (SNAP) amplitude was recorded, using both near-nerve electrodes and the implanted cuff electrode. The SNAP amplitude declined within 10 days to approximately 50% of the first SNAP cuff amplitude measured on Day 2 after implantation and recovered to the initial amplitud

[Replacement operations in radial paralysis].

Radial nerve palsy results in a lack of extrinsic extensors of the wrist, fingers and thumb. The degree of dysfunction depends on the level of trauma to the radial nerve. If the wrist is not stable there is a loss of prehensile grip. Patients have great difficulty picking up large or heavy objects. In high radial nerve palsy the loss of triceps function can cause additional problems. Based on our knowledge of anatomy and biomechanics, radial nerve function can be restored sufficiently. The indications, preoperative planning, technique of transposition and postoperative management are explained

Free flaps based on the anterior interosseous artery.

Free flaps based upon the anterior interosseous artery were defined anatomically by 15 dissections. The anterior interosseous artery runs distally for the entire length of the anterior interosseous nerve on the palmar surface of the interosseous membrane. A dorsal branch of the anterior interosseous artery penetrates the interosseous membrane about 5 cm proximal to the distal radioulnar joint and gives off a tributary radially. This tributary ascends the ulnar aspect of the radius to provide blood to the radius and the dorsal wrist skin. Based on these anatomical findings, four types of free f

[Washington regime after-care of flexor tendon injuries in zone 2].

The Washington-regimen for the rehabilitation of flexor tendon injuries (Chow et al., 1987) represents a combination of the established Kleinert-method and the controlled passive motion of Duran and Houser. This paper presents the results of a study which was carried out in 99 patients with 113 injured fingers treated in the Department of Burns, Plastic and Hand Surgery of the Accident Hospital Ludwigshafen. 55 patients with injuries of the fingers and 29 patients with injuries of the thumb were evaluated according to functional and subjective criteria and compared to a group of 15 patients tr

Toe-to-hand transfer. Current concepts, techniques, and research.

The current status of toe-to-hand transfer at the Chang Gung Memorial Hospital is presented. Various combinations of toe-to-hand transfers have been used routinely for reconstruction of lost digits as well as parts of the digits distal to the sublimis insertion. Early, one-stage reconstruction is recommended in patients with clean wounds when replantation is not feasible. The decision of which technique to be used in certain patients is highly individualized. Technical refinements have been introduced aimed at simplifying the technique of toe harvesting and improving the function and appearanc

Ulnar tunnel syndrome.

Compression neuropathies are a significant source of pain in the upper extremity. Although ulnar tunnel syndrome occurs much less frequently than cubital tunnel syndrome, compression of the nerve at this level is a readily treatable condition. Ulnar tunnel syndrome should be kept in the differential diagnosis when the patient complains of numbness of the ring and small fingers, hand weakness, and pain on the ulnar side of the wrist.

Replantation of ring avulsion of index, long, and ring fingers.

The salvage rate of degloved amputation of the fingers is as low as 50%. This case report consists of a multiple-finger degloving amputation characterized by intact tendons, pulleys, and bone, but avulsed neurovascular bundles. After replantation, the range of motion of the fingers was nearly full. Protective-to-light touch sensation was obtained with transpositional digital nerve transfers. Grasp strength was 80% that of the uninjured side. In addition to good functional recovery, the aesthetic appearance obtained by replantation of the degloved skin was far better than that obtained by other

Ethylene oxide neurotoxicity: a cluster of 12 nurses with peripheral and central nervous system toxicity.

Ethylene oxide (EO) is commonly used to sterilize heat-sensitive products used by hospital patients and personnel. Ethylene chlorohydrin (EC), a by-product, is considered highly toxic. We report a cluster of 12 operating-room nurses and technicians who developed symptoms after a 5-month exposure to high levels of EO and EC in disposable surgical gowns. All patients reported a rash on the wrist where contact was made with the gowns, headaches, and hand numbness with weakness. Ten of 12 patients complained of memory loss. Neurologic evaluation revealed neuropathy on examination in nine of the 12

Free vascularized deep peroneal nerve grafts.

An ideal donor site for vascularized nerve grafts should have a constant anatomy, minimal functional loss after the nerve has been sacrificed, and a dependable blood supply parallel to the nerve over a relatively long distance. Creating a pedicle for a free vascularized deep peroneal nerve graft with the anterior tibial vessels seems to be a most suitable method for repairing long nerve gaps of over 20 cm and digital nerve defects with severe finger damage. Applications of this nerve graft to digital nerve losses with severely scarred beds created by avulsion injury, and two-stage reconstructi

Transposition of flexor pollicis brevis muscle for reconstruction of thumb opposition. Anatomical study and clinical application.

OBJECTIVE: Based on the anatomical study and clinical trial of transposition of the flexor pollicis brevis muscle (M. f. p.b.) for the reconstruction of thumb opposition, we suggested a new method for the treatment of irreparable median nerve injury which causes paralysis of the opponens pollicis and the abductor pollicis brevis muscles (M.a.p.b) and leads to loss of thumb opposition. MATERIAL AND METHODS: Anatomical study and biomechanic analysis were performed on 20 cadaveric hands and 8 patients who had been treated and followed up on an average for 12 months. RESULTS: The M.f.p.b. overla

Two immediate synchronous free flap transfers in a severe traumatic mid-hand defect: a case report.

A 66-year-old patient sustained a severe right hand injury resulting in a thumb pulp defect and unreplantable amputation of the index, long, and ring fingers at the metacarpophalangeal joint with joint exposure. There was also an 8 cm defect of the radial digital nerve of the little finger. Immediate one-stage total reconstruction of all of the defects was accomplished by a free groin flap and a free glabrous skin flap from the foot. Opposable function of the hand was thus preserved.
