[Loss of digital tissues and carpal canal syndrome. Apropos of 3 cases].

We have observed three cases of carpal tunnel syndrome (CTS) complicated by cutaneous ulcerations and acroosteolysis. Although our first patient had a mild case of CTS, he had a concomitant amphetamine addition. This led to a mania for gnawing away at the digits in the territory of the median nerve. Our other two patients had severe cases of CTS. One suffered from diabetes and the other from primary amyloidosis. Surgical decompression of the median nerve was carried out in these two cases and led to rapid healing of the cutaneous lesions. The latter patients suffered from the entity known in t

Restoration of sensibility to anesthetic scarred digits with free vascularized nerve grafts from the dorsum of the foot.

Five cases of segmental vascularized nerve grafts that bridge scarred beds for digital sensory nerve reconstruction where previous nonvascularized nerve grafts have failed are reported. Average follow-up in this study was 27 4/5 months. Three patients were men and two were women. Average age was 35 1/2 years. The thumb was the recipient digit in one patient; the index finger in two patients; and the long finger in two patients (primary opposing digit). Three digits had suffered amputation and two had crush lacerations. Average graft length was 6.6 mm. Pin prick, touch, and vibratory sensation

An investigation of the digital nerves of the thumb.

With the use of a stereoscopic microscope, observations were made on the course of the proper digital nerve of the thumb on ten upper limbs in adult cadavers. By observation from the dorsal side after removal of the bone, it was possible to clearly see the ramification of the digital nerve from the subcutaneous tissue to the dermis. Ramification from the palm to the level of the interphalangeal joint was segmental. The digital nerve of the thumb trifurcates from the proximal site of the distal palmar thumb crease into the main, medial, and lateral branches. The main branch is thick and extends

Evaluation of sensibility after sensory reconstruction of the thumb.

We have studied the restoration of sensation in 24 patients after operations involving the digital nerves of the thumb. These comprised 10 neurorrhaphies, two nerve graftings, five replantations, one toe-to-thumb transfer and six neurovascular island flaps. The average follow-up period was 46 months. Greater sensitivity was found in the moving two-point discrimination (2PD) test than in the static 2PD test. The results of the Semmes-Weinstein test showed good correlation with the static 2PD test. Porter's letter test showed correlations with both 2PD tests, and the pulp-writing test showed goo

Secondary repair of digital flexor tendons in outpatient clinic.

A total of 32 digits, including 3 thumbs, were reconstructed in an outpatient clinic. The patients were allowed to go home 3-4 hours after the operation. The patients had injury to the flexor tendon in no man's land without other complications except for an injury to the digital nerve in 6 cases. The tendon was reconstructed secondarily using a free graft. The minimum follow-up time was 6 months. The results were classified in three ways. With all methods the results, which were deemed satisfactory in 79% and poor in 21%, reached the average standard for this type of repair. Increasing age of

[Intersesamoid resection-suture of the metacarpophalangeal joint of the thumb. Sesamoidorrhaphy of the metacarpophalangeal joint of the thumb].

In some cases, congenital or acquired posterior instabilities of the MP joint of the thumb may cause pain or create functional disorders. It is possible to stabilize the metacarpophalangeal joint by partial resection of the ligament joining the two sesamoids which makes it possible to move and suture them toward each other. An enlargement plasty of the pulley then is required. This technique prevents hyperextension but without restricting flexion or opposition. The digital nerves must be carefully identified and protected.

Results of digital neurorrhaphy in adults.

Forty-two digital neurorrhaphies utilizing contemporary techniques and suture material are reviewed. 17% regained near normal function. Another 30% regained imperfect, but very pleasing function. Half of the patients were left with moderate to marked hypoaesthesia, but protective sensation. Protective sensation did not return to those nerves left unrepaired. Immediate repair did not allow better recovery than repair within six days. Delay up to eleven months did not obviate pleasing return of sensibility. Similarly, simultaneous tenorrhaphy did not seem to affect the quality of the result. Cru

Surgical significance of the motor fascicular group of the ulnar nerve in the forearm.

The topography of the fascicular group of the ulnar nerve at the wrist and forearm was studied focusing on the motor (muscle) fascicular group. In 109 of 111 specimens studied (98%), the motor fascicular group of the ulnar nerve is located at the ulnar dorsal or straight dorsal position at the wrist and the distal forearm. This motor fascicular group may be identified as a distinct entity up to 90 mm proximal from the level of the radial styloid. The relatively constant location of the motor fascicular group is significant since most lacerations of the major peripheral nerves of the upper extr

Restoration of sensibility to a thumb by the technique of digital nerve advancement: a new surgical procedure.

A groin flap was used to repair avulsion injuries of the nondominant thumb and index finger in a 13-year-old boy. The patient demonstrated full range of motion in the involved digits but lacked sensibility and a measurable two-point discrimination of the thumb. A two-point discrimination of 3 mm and excellent subjective sensibility were restored by distal advancement of the digital nerves and separation into fascicles across the ulnar aspect of the thumb. A 1 1/2-year follow-up period is recorded.

[Effectiveness of the rehabilitation of patients with combined injuries of the flexor tendons and nerves of the fingers].

On the basis of a comparative analysis of results of the early (71 patients) and late (128 patients) rehabilitative treatment of injured tendons of flexors and nerves at the level of the lower third of the forearm and hand articulation it was established that early treatment resulted in more complete restoration of the extremity function and working capacity of the patients.

The late functional results of upper limb revascularization and replantation.

The functional results in 25 of 30 patients after successful upper limb revascularization or replantation were evaluated by subjective-patient surveying and objective measurements. Young patients with complete, sharply amputated extremities at the wrist level or those with incomplete injuries and uninjured peripheral nerves had the best functional results. Multiple-level, diffuse crush, or avulsion injuries, even if the injuries were incomplete, and patients with high-level nerve injury had less return of function.

Ulnar digital compression neuropathy of the thumb caused by splinting.

Acute compression neuropathy of the ulnar digital nerve of the thumb is described in two women, 23 and 53 years of age. The injury was caused by pressure from a splint. The pathogenesis and clinical features resemble the classic "bowler's thumb." Early recognition and proper treatment will prevent irreversible neurologic damage.

Variations in digital sensory patterns: a study of the ulnar nerve-median nerve palmar communicating branch.

Eighty percent of 50 dissected cadaver palms showed a communicating branch between the fourth and third common digital nerves (ulnar and median origins, respectively). The branch commonly crossed the palm with the superficial arterial arch and usually provided sensory fibers from a branch of the superficial ulnar nerve into the ring finger radial digital nerve. Caution should be taken during carpal tunnel release or other surgery along the axis of the fourth ray to avoid injury to this structure.

Radial nerve paralysis associated with fractures of the humerus. A review of 62 cases.

Sixty-two patients with radial nerve paralysis associated with fractures of the humerus were reviewed. Seventy-three per cent of the patients had primary radial nerve paralysis and 27% had secondary paralysis. Conservative management has led to excellent nerve recovery in all groups of patients. Overall, 95% of the patients with radial nerve paralysis recovered normal or near normal function. All patients with secondary paralysis had full functional recovery of the radial nerve. Conservative management is recommended in patients with radial nerve paralysis associated with fractures of the hume

Complex dislocations of the metacarpophalangeal joint.

A dorsal longitudinal incision provides excellent exposure for reducing complex dislocations of the metacarpophalangeal joints of the thumb and index finger. Within the past four years, we have successfully treated five patients with irreducible dislocations of the metacarpophalangeal joint of the thumb and four with dislocations of the index finger by open reduction through a dorsal incision. This approach exposes the joint and the major structure blocking reduction, the volar plate. The digital nerves, which are tented volarly over the metacarpal head, are not prone to injury and osteochondr

Digital nerve grafts in replanted digits.

In most cases where digits have been amputated by avulsion and the digital nerve extensively injured, primary nerve suture cannot be performed. For such a digit when replanted, secondary nerve grafting was done under the operating microscope using a lateral femoral cutaneous nerve. Two years after operation, two-point discrimination of these cases proved to be 10.5 mm on average. And in the case of the transplanted three strand cable graft, using a lateral femoral cutaneous nerve yielded the most favourable results. The replacement was superior to the side receiving primary crushed nerve sutur

Interfascicular nerve grafting.

Interfascicular nerve grafting is a useful method to repair nerves with gaps. Good motor recovery can be obtained, and return of some degree of sensibility is possible. The procedure can be used to overcome small gaps that result from neglected sharp lacerations or after failure of primary repair, or large gaps that result from loss of nerve substance or traction lesions. Primary nerve repair should be done for acute lacerations. However, in old lacerations (more than three weeks old) and in nerve gaps of more than 2 cm in length, a functional recovery can still be expected after nerve graftin

[Carpal tunnel syndrome. Cosmetic incision (author's transl)].

The median nerve compressed in the carpal tunnel can be released by an incision in the "line of life", a natural fold along the axis of the third metacarpal bone. By sectioning the carpal tunnel in the palm of the hand, on the cubital side of the median nerve, division of the external branch and of the sensory cutaneous rami of the nerve can be avoided. Hypertrophic scars are prevented. The part of the carpal tunnel situated in the forearm can be freed by a subcutaneous incision if needed, but compression at that level is much less frequent than in the palm of the hand.

[Autologous vein transplants for digital revascularization after hand injuries].

In this study, reverse autogenous vein grafts were employed to bypass vascular defects in replantations and occluded arteries in hands with chronic post-traumatic ischemia. In 32 thumb replantations, vein grafts were used as short interposition segments or longer bypass grafts. The bypass grafts have the advantage of permitting proximal and distal anastomoses in undamaged tissue. In 5 post-traumatic chronically ischemic hands, vein grafts were employed to bypass arterial occlusions. All grafts remained open and cold intolerance improved as tactile discrimination returned. Alternative hand flaps for amputations and digital defects.

Traumatic or thermal injury to the hand may result in a composite tissue loss. Exposed tendon, bone, or joint surface is best treated by flap coverage. This paper will discuss four types of alternative hand flaps that can be used to treat difficult areas and/or minimize complications. (1) A thumb flap elevated from the metacarpophalangeal joint flexion crease covers a fingertip injury and the donor site is closed primarily. (2) A proximally based side-finger flap, preferably from the ulnar side of the donor finger, can be used to cover a fingertip defect in a young person or a thumb tip in any
