Free transfer of a large sensory flap from the first web space and dorsum of the foot including the second toe for reconstruction of a mutilated hand.

A 19-year-old man sustained a mutilating crush injury in August, 1978, in an 80-ton punch press. The injury resulted in transmetacarpal amputations of the index through little fingers. The thumb was revascularized by microvascular anastomoses. The replanted index to the little finger position failed. A free transfer of the patient's second toe to the fifth metacarpal was done to provide a post to oppose the thumb. The first web space of the toe was used in expanding the contracted single cleft of the hand. The innervated dorsal foot flap provided coverage for the dorsal surface of the hand. An

Carpal and Guyon tunnel syndrome in burns at the wrist.

A series of 22 patients with burns of the upper extremities have been evaluated by clinical and electromyographic (EMG) studies. As suggested by others, a significantly higher incidence of carpal tunnel syndrome and sensory involvement of the ulnar nerve was discovered. Those findings suggest that we pay more attention to the median and ulnar nerves at the wrist level in burns and treat them when compression is demonstrated by clinical and EMG evaluation.

[Anomalies of muscles and tendons in hand and wrist (author's transl)].

Anomalies of muscles and tendons in hand and wrist are not uncommon. Their signification in practicing handsurgery must be known. They can rarely be foreseen. When found at random during operation, they may be often neglected. But they are sometimes the cause of different syndromes such as disability of some artist's hands, painful wrist by sports of efforts, nerve compression simulating a carpal tunnel syndrome, camptodactylia, etc. They may also necessitate a modification of planning tendon transfers or grafting. On the dorsal aspect one must point out the extensor digitorum brevis; the abse

A case of forearm amputation with additional multilevel partial amputations.

A right-handed 4 1/2-year old boy had a sharp amputation 3 cm proximal to his left wrist joint in a hay-cutting machine accident. In addition, he sustained 5 incisions on the anterior and ulnar aspects of the same forearm, the proximal and deepest including radius, ulna and the deep branch of the radial nerve. In a 19 1/2 hours operation, his left hand was replanted and the mainly cold anoxaemia time was 16 hours. Despite several reoperations, the hand could not be saved but the proximal portion of the amputated part did survive. On the 21st day the hand was reamputated at the radiocarpal join

[The neurovascular forearm island flap].

We describe a neurovascular island flap of forearm skin based on the radial artery at the wrist level with reversed arterial blood flow. The flap may cover almost any skin and soft tissue defect of the hand. The flap is drained via venous anastomosis between one of the forearm veins and a suitable vein of the hand. Sensation is provided through a microneural anastomosis of one of the sensitive nerves of the forearm to a corresponding nerve of the hand.

Distal digital nerve grafts: clinical and anatomical studies.

An anatomical study of the terminal branches of the digital nerves of thumb and finger indicated a constant trifurcation at the level of the distal interphalangeal joint. Twelve microscopic interfascicular nerve grafts were done in 11 patients who had nerve division at this level with insensitive fingertips. All patients exhibited improved sensibility. Eight of 12 or 67% achieved 6 mm of two-point discrimination. Although there were no children in this study, the subjects under 30 years of age enjoyed better final results.

Funicular localization in partial median nerve injury at the wrists.

The funiculi of the median nerve at the wrist carrying the motor and sensory fibers innervating the hand were described in 1945 by Sunderland after an extensive anatomic study. In an attempt to confirm clinically the findings of that study, 7 patients with partial median nerve laceration at the wrist are presented. The findings in these patients support the schema developed from Sunderland's study except for the funiculi subserving the 1st and 2nd lumbricals which appear to be more dorsally placed. They also tend to confirm Sunderland's finding that the mixing of nerve fibers from different fu

Control of vasospasm following trauma and microvascular surgery.

Vasospasm may result in marked reduction of blood flow through injured or revascularized tissues. Neurogenic vasoconstriction is mediated through sympathetic pathways which may be blocked by long-acting local anesthetics resulting in optimal intravascular pressures and elevated flow. An experimental study performed on normal human volunteers has shown that distal sympathetic blockade using Bupivacaine protects the digital circulation against vasospasm elicited by a strong stimulus for a minimum of 7 to 8 hours. A catheter, placed adjacent to the median or ulnar nerve at the wrist level after d

Rerouting vessels and nerves from other digits in replanting an avulsed and degloved thumb.

In avulsion and degloving injuries of amputated thumbs, we have rotated nearby neurovascular bundles and dorsal digital vessels to restore circulation and sensation. This has been done in 5 patients, and 3 thumbs survived with preservation of length and good function.

Traumatic false aneurysm of the hand in hemophilia.

An unusual case of traumatic false aneurysm following a puncture wound in the hand of a hemophiliac, despite Factor VIII replacement, demonstrates the necessity for careful evaluation of such patients when there is profuse and continued bleeding. Following surgical repair of the false aneurysm and the ulnar proper digital nerve of the thumb 17 days postinjury the patient had an uncomplicated recovery. Partial arterial laceration, the underlying pathology of this lesion, may be demonstrated by arteriography or surgical exploration.

Digital replantation: clinical experiences.

In a series of 130 digital replantations successful revascularization has been obtained in 74% of cases. Arterial thrombosis is the most common cause (65%) of all failures. A useful digit can be obtained following replantation and the surgery is particularly indicated for amputated thumbs and in hands suffering multiple injuries. Precautionary measures against bleeding are absolutely essential. Primary tendon and nerve repairs are advocated. Vascular occlusion may be corrected by revisional surgery and secondary surgery can improve tendon nerve or joint function.

[Microvascular and microneural repair: technique and results (author's transl)].

Microsurgical technique permits repair of arteries and veins of a diameter of 0.5-2 mm. The success rate rose to 80-90% because of the frequent use of vascular grafts. This led to a high success rate in replantation of amputated parts. But repair of digital vessels is indicated even if the basal circulation is present to improve healing of other structures (tendons, nerves) and so to avoid late ischemic complaints. Free transfer of toes to replace digits in cases of multiple digital loss has its place in reconstructive hand surgery. It became possible because of the satisfactory return of sens

Nerve grafting in the repair of complicated peripheral nerve trauma.

Peripheral nerve trauma has been a challenge to surgeons, with significant advances in the surgery of repair occurring in major wars. The standard method of treatment involving wide mobilization and end-to-end suture to close traumatic gaps in peripheral nerves has not produced consistently acceptable results, particularly with large gaps. However, with the recent development of microsurgical techniques and instrumentation, the method of interfascicular nerve grafting has been applied to selected patients with trauma problems of the peripheral nerve, by resecting and autogenous nerve grafting

Treatment of tendon injuries in children.

Tendon injuries in the child's hand can be repaired primarily with better results than in the adult; this is true at all levels, including the digital sheath. In less than ideal circumstances, secondary conventional tendon grafting also provides predictable satisfactory results and can also salvage a failed primary repair. In children under the age of six years with digital flexor tendon repairs, the limb must be immobilized above the elbow. In the cooperative child over the age of six years with a primary repair of a lacerated flexor tendon, the dorsal splint with rubber band traction as desc

[Computerized axial tomography of the skull - diagnostic possibilities and clinical results (author's transl)].

Computerized axial tomography is a new method of tissue examination with x-rays whereby a picture can be produced which is a representation of a slice of the skull. This is done by irradiating the skull from 180 or 225 incremental angles and measuring the absorption at each of these angles. Then with the aid of a computer a tomogram is produced which can be displayed on a screen. These tomograms are representations of a cross-section of the skull composed of 160 X 160 points showing the various intracranial structures with great detail. The present study demonstrates the diagnostic possibiliti
