Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Enhancing hand motor recovery in neonatal total brachial plexus palsy with a novel surgical approach.

Neonatal total brachial plexus palsy (NBPP) is one of the most challenging birth injuries, especially if it is associated with hand palsy and sensory loss. Current techniques offer limited success for both extrinsic and intrinsic hand muscles recovery, especially for the latter. We present an innovative combined surgical method designed to reinnervate the hand in complete palsies: (1) the ulnar nerve of the affected limb, which is insensate and nonfunctional, is harvested as an autologous graft and used to connect the contralateral C7 root to the medial (motor) trunk contribution to the median

Brachialis Branch to Radial Nerve Transfer for Reanimation of Wrist and Finger Extension: An Anatomical Feasibility Study.

PURPOSE: The motor branch to the brachialis muscle has been described as a versatile donor in upper limb nerve reconstruction. This includes patients with peripheral nerve injuries and tetraplegia. However the anatomic feasibility of transfers from the musculocutaneous nerve to radial nerve branches around the elbow has not been systematically evaluated. This study aims to assess this feasibility. Such a transfer may be particularly relevant in patients with high-level tetraplegia or in patients with spastic upper limb involvement, in which selective denervation of spastic elbow flexors could

A Tactile Trans-Flexor Carpi Radialis Surface Landmark Technique for Median Nerve Local Injection: A Cadaveric Study.

PURPOSE: Median nerve (MN) block under local anesthesia at the wrist is commonly performed but can be limited by anatomic variation, particularly absence of the palmaris longus tendon. This study investigates a novel trans-flexor carpi radialis (trans-FCR) technique for MN anesthesia that uses the flexor carpi radialis (FCR) tendon as a consistent surface landmark. METHODS: Fourteen fresh frozen upper extremity cadaver specimens were used. A 25-gauge needle was inserted through the FCR sheath approximately 4 cm proximal to the wrist crease in a transverse fashion, angled approximately 0° to t

Anomalous Flexor Carpi Radialis Anatomy During Volar Distal Radius Approach: Two Case Reports.

Volar locking plates become the standard treatment for distal radius fractures and are mostly applied through volar approaches that rely on the flexor carpi radialis (FCR) as a superficial landmark. Although usually straightforward, anomalous flexor musculature may complicate exposure, misdirect dissection and increase the risk of iatrogenic neurovascular injury. We report two rare anatomical variants encountered during open reduction and internal fixation: FCR brevis (FCRB) associated with anomalous insertion of the pronator quadratus, and complete agenesis of the FCR. Both substantially alte

Postoperative Reduction and Persistent Enlargement of Median Nerve Cross-Sectional Area in Carpal Tunnel Syndrome: Influence of Metabolic Syndrome.

PURPOSE: Carpal tunnel syndrome (CTS) causes enlargement of the median nerve cross-sectional area (CSA), which often decreases after carpal tunnel release. However, the clinical relevance of postoperative median nerve CSA changes remains unclear. This study aimed to assess the association between postoperative changes in median nerve CSA and clinical outcomes and to identify factors linked to a persistent or increased CSA. METHODS: We enrolled 117 patients (136 wrists) with CTS who underwent endoscopic carpal tunnel release. Preoperative and 12-month postoperative assessments included ultraso

Coordinate-Independent Ultrasound Assessment for Complex Tissue Kinematics: A Preliminary Study on Median Nerve Mobility.

OBJECTIVE: This paper presents an ultrasound strain-divergence imaging technique, which quantifies complex tissue kinematics without being constrained by imaging coordinates. An in vivo application of the technique was demonstrated on median nerve (MN) mobility. Transverse mobility of MN, a potential biomarker for evaluating MN entrapment in carpal tunnel syndrome (CTS), is featured with large variations across different active gestures, individuals, and operators, so it remains inconclusive in literature. METHODS: Ultrasound data of the MN in the transverse view during active thumb and finge

Comparison of Immunohistochemical and Histopathological Findings between Patients with Diabetic and Idiopathic Carpal Tunnel Syndrome.

Background: Carpal tunnel syndrome (CTS) is the most common nerve entrapment syndrome, caused by median nerve compression at the wrist. Its aetiology remains unclear, with idiopathic and diabetic-related forms. Diabetic patients (type 1 and 2) have a higher CTS risk, likely due to oedema, neo-angiogenesis and increased non-inflammatory fibrosis in the sub-synovial connective tissue (SSCT). Vascular endothelial growth factor (VEGF) A plays a key role in vascular permeability and angiogenesis, particularly in diabetic CTS. This study compared histopathological differences and VEGF expression in

Surgically Oriented Ultrasound-Based AI of Median Nerve Morphology as a Decision Support for Carpal Tunnel Release: A Calibrated ConvNeXt-CBAM Framework With Supervised Contrastive Warm-Up.

AIM: Carpal tunnel syndrome (CTS) is a compressive neuropathy commonly encountered in hand surgery, and decisions regarding whether operative decompression should be conducted rely on accuracy and clinical interpretability of imaging findings. Ultrasound is commonly used to directly visualize the median nerve in this condition, but remains operator-dependent and is limited by inconsistent diagnostic thresholds. This study aimed to develop a surgically oriented ultrasound-based artificial intelligence (AI) model that provides strong discriminative power, reliable probability calibration for pre

Surgical Management of Wrist Arthritis Is Linked to Increased Carpal Tunnel Syndrome/Carpal Tunnel Release Risk: Rethinking Preoperative Evaluation.

PURPOSE: This study aimed to determine whether patients with wrist arthritis undergoing corrective surgeries, such as 4-corner fusion (4CF), proximal row carpectomy (PRC), wrist fusion, or total wrist arthroplasty, have a higher incidence of carpal tunnel syndrome (CTS) and subsequent carpal tunnel release (CTR) compared with those managed nonsurgically. METHODS: The PearlDiver database was used to identify two patient groups: the surgical group (patient with wrist arthritis who underwent corrective surgery) and the conservative group (patients with wrist arthritis who did not have surgery).

MRI Assessment of Median Nerve Size in Patients with Proximate Electrodiagnostic Studies.

BACKGROUND: Carpal tunnel syndrome (CTS) diagnosis has traditionally relied on electrodiagnosis (EDX) to confirm the diagnosis and to assess severity. Ultrasound has shown potential in measuring median nerve cross-sectional area (CSA) for CTS diagnosis, and magnetic resonance imaging (MRI) can be used for wrist soft-tissue evaluation. This study explored the correlation between CTS diagnosis and median nerve CSA measured on MRI at different wrist levels. METHODS: A retrospective review of an electronic medical record database identified patients who underwent both wrist MRI and EDX within a 9

Ultrasonographic parameters comparison for pre- and postoperative carpal tunnel syndrome release: A single-center prospective study.

Carpal tunnel syndrome (CTS) is a prevalent condition characterized by median nerve compression, impacting a significant portion of the population worldwide. While neurophysiological testing has been traditionally employed for diagnosis, recent advancements in ultrasonography have shown promise in offering a noninvasive alternative. In this study, we aimed to evaluate the mid-term postoperative statistical changes of the median nerve and flexor retinaculum, share our ultrasonographic measurement methodologies for standardized reporting, and suggest a set of ultrasonographic parameters for post

Repeated Measurements of Ultrasound Median Nerve Cross-Sectional Area Are Associated with Symptomatic and Functional Improvement after Carpal Tunnel Release.

Background: The potential for ultrasound (US) to be used as an objective measurement of improvement after carpal tunnel release is poorly understood. The purpose of this study is to evaluate whether measurements of US cross-sectional area (CSA) of the median nerve in the postoperative recovery period are associated with improvement of Boston Carpal Tunnel Questionnaire (BCTQ) scores for the Symptom Severity Scale (SSS) and Functional Severity Scale (FSS). Methods: A total of 167 wrists from 137 patients who underwent CTR with availability of US and BCTQ at 2-weeks, 6-weeks, 6-months and 1-year

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Targeted Muscle Reinnervation Using the Anterior Interosseous Nerve for Symptomatic Wrist Level Neuromas.

BACKGROUND: Iatrogenic and traumatic sensory nerve injury at the level of the wrist can lead to debilitating neuroma. Targeted muscle reinnervation (TMR) is an effective treatment for the management of symptomatic neuromas. We investigate the use of the anterior interosseous nerve (AIN) as a recipient nerve for the treatment of iatrogenic neuromas. This case series describes 4 patients treated for neuromas of the lateral antebrachial cutaneous nerve (LABC), palmar cutaneous branch of median nerve (PCB), and radial sensory nerve (RSN). METHODS: Four cases involved a symptomatic neuroma of the
