Early mobilization following carpal tunnel release. A prospective randomized study.

A prospective randomized study was undertaken of 50 consecutive patients undergoing surgery for idiopathic carpal tunnel syndrome to determine the value of splintage of the wrist following open carpal tunnel release. Patients were randomized to either be splinted for 2 weeks following surgery or to begin range-of-motion exercises on the first post-operative day. Subjects were evaluated at 2 weeks, 1 month, 3 months, and 6 months after surgery by motor and sensory testing, physical examination, and a questionnaire. Variables assessed included date of return to activities of daily living, dates

Sensitivity and specificity of vibrometry for detection of carpal tunnel syndrome.

A cross-sectional study was performed to assess the utility of vibrotactile thresholds (VTs) obtained before and after a 10-minute period of wrist flexion as a method for detection of carpal tunnel syndrome (CTS) among adult subjects. Subjects with hand discomfort were recruited from patients referred to a university-based electromyography laboratory. Asymptomatic subjects were recruited from among office and technical staff at a professional school. In addition to electrophysiologic evaluation (EP), all subjects were offered VT measurement of the index and small fingers, bilaterally, before a

Reverse Phalen's maneuver as an aid in diagnosing carpal tunnel syndrome.

A reverse Phalen's maneuver involves wrist and finger extension held for 1 minute. We showed that this maneuver results in a significantly higher intracarpal canal hydrostatic pressure as compared to a traditional Phalen's or a modified Phalen's maneuver. Additionally, 31 individuals with complaints of carpal tunnel syndrome symptoms and 20 normal controls were evaluated to see what effect the reverse Phalen's maneuver would have on median sensory latency and amplitude. Both groups demonstrated a prolongation of the median sensory revoked response after 1 minute of this maneuver. The control g

[The sensory potential of the ring finger. The value of electromyographic diagnosis of carpal tunnel syndrome].

Accurate diagnosis of carpal tunnel syndrome relies on electromyography. The usual criteria (sensory conduction velocity and motor latency) fail to identify 10% of cases of carpal tunnel syndrome. The sensory innervation of the ring finger (digit IV) is shared by the median and ulnar nerves. This anatomic feature is the basis of a novel electromyographic test: measurement of the sensory potential of digit IV by an orthodromic method with stimulation of the finger via a ring electrode and recording at the wrist. Analysis of the sensory potential of digit IV in 200 subjects with no median nerve

The effect of wearing a flexible wrist splint on carpal tunnel pressure during repetitive hand activity.

We investigated how repetitive hand activity normally affects carpal tunnel pressure and whether a flexible wrist splint can influence this effect. Nineteen healthy subjects were evaluated under four test conditions: at rest with and without a wrist splint (baseline) and while performing a repetitive task with and without a wrist splint. The task involved loading and unloading 1 lb. cans from a box at a rate of 20 cans per minute for period of 5 minutes. Carpal tunnel pressure and wrist angles were continuously monitored by means of a fluid-filled catheter inserted into the carpal canal and a

Sensory recovery following total and subtotal amputations distal to the wrist and digital replantations.

Apart from the basic aim of revascularization, other criteria for the success of digital replantation are functional and sensory recovery. Tactile gnosis can only be restored in cases of replantation distal to the wrist by paying great attention to the technical details for nerve repair. Of 253 patients who underwent 312 digital replantations, after total or subtotal amputations, over 16 years, 162 with 215 successful replantations and 377 nerve reconstructions returned for follow-up evaluations. Of 320 patients who underwent 395 nerve repairs after isolated severed digital nerves, 198 with 24

An analysis of the results of late reconstruction of 132 median nerves.

132 patients with injuries to the median nerve were assessed on average 10.4 years after repair. There were 106 males and 26 females, with a mean age of 28.4 years. Most of the nerve lesions were sharp (76) or blunt (47) injuries. Division was total in 87 cases, and most were at the wrist level. Secondary repair was performed in 34 cases and fascicular grafting in 98 cases. The average gap was 5.8 cm. A quantitative evaluation and grading according to the MRC classification was created to reflect sensory and motor recovery. Excellent or good results were obtained in only 49.2%. The age of the

Neurophysiological investigation of hands damaged by vibration: comparison with idiopathic carpal tunnel syndrome.

Forty-seven men with numbness and paresthesiae in their hands after long-term occupational exposure to vibrating hand-held tools were examined neurophysiologically and clinically. The vibration thresholds of the finger tips were assessed and fractionated neurography of the median nerve motor and sensory fibres carried out. They were compared with control groups of healthy subjects and patients with idiopathic carpal tunnel syndrome. The patients exposed to vibration had major increases in their finger vibration thresholds, shown with both the Goldberg-Lindblom vibrameter and the Lundborg vibro

Changes in carpal tunnel shape during wrist joint motion. MRI evaluation of normal volunteers.

This study evaluated dynamically the morphology of the carpal tunnel during the wrist motion. The object was to determine the transverse dimension (TD), anteroposterior (AP) dimension and cross-sectional area (CA) of the carpal tunnel in the neutral, flexed and extended positions. The wrists of eight male and eight female volunteers aged 20 to 38 years were studied. With wrist extension, AP and CA decreased while the TD increased at the pisiform level, and CA increased at the hamate level. With flexion, TD and CA decreased at the pisiform level and CA decreased at the hamate level. The results

Clinical application of motorsensory differentiated nerve repair.

Since 1979 acetylcholinesterase has been used in clinical practice for motor sensory differentiation. It was first used for median and ulnar nerve injuries at the wrist. Recently the application was extended to secondary nerve repair, including plexus reconstruction. The aim of this study was to present, for the first time, clinical results of motor sensory differentiated median and ulnar nerve repair and to show the advantage of this method in secondary repair. We compared a group of nine patients with motor sensory differentiated median and ulnar nerve repair with a group of 13 patients with

Intrinsic reinnervation--myth or reality?

A clinical and electromyographic study of major nerve transections at the wrist in 22 patients has shown that electrical recovery nearly always occurs in the intrinsic muscles, despite the absence of clinically detectable function. There appears to be a level of electrical reinnervation above which clinically detectable intrinsic muscle power is usually present. This level would appear to be 50% of the contralateral maximum evoked muscle action potential; above this the "myth" of clinical recovery becomes a reality.

Obesity as a risk factor for slowing of sensory conduction of the median nerve in industry. A cross-sectional and longitudinal study involving 429 workers.

As part of a longitudinal study of the etiology of carpal tunnel syndrome in industry, we evaluated weight and body mass index (BMI) as risk factors for slowing of sensory conduction of the median nerve. Using nerve conduction studies, we evaluated 858 hands of 429 industrial workers in 1984 and reevaluated 630 hands of 316 of these same workers in 1989. Weight and BMI were strongly and positively correlated with the maximum latency difference (MLD). In 1984 and 1989 the risk for abnormal nerve conduction averaged 3.5-fold and 4.1-fold greater, respectively, in the obese workers than in the sl

Inhibition of motoneuron discharge by peripheral nerve stimulation: an F response analysis.

F waves were recorded from the abductor pollicis brevis stimulating the median nerve at the wrist. These data were compared to responses obtained after preceding supramaximal stimulation of digital fibers of the second (II) finger or the median nerve at the wrist. The time between conditioning and test stimuli were 50 msec. Following conditioning stimuli, F wave latencies were significantly increased while F amplitudes, durations, and persistences were all decreased. Chronodispersion was not significantly affected. These changes were associated with increased repetition of individual F respons

Carpal tunnel changes and median nerve compression during wrist flexion and extension seen by magnetic resonance imaging.

Magnetic resonance imaging was done on the wrists of 14 volunteer subjects with the wrists in the neutral position, then flexed and extended at 45 degrees. Computer enhanced cross-sectional areas were measured on each subject at the level of the hook of the hamate. In the neutral position the distance between confining structures around the median nerve is 2.0 +/- 0.2 mm in the anteroposterior direction, and the mean cross-sectional area of the carpal tunnel is 1.52 cm2. With the wrist in the extended position the distance available for the median nerve measures 2.2 +/- 0.4 mm and the cross-se

Results of median nerve repair in children.

Seven children ranging in age from 1 to 13 years (mean 8 years) were evaluated at a range of 8 months to 15 years (mean 4.7 years) following group fascicular repair of the median nerve at the wrist following a sharp transection of the nerve. At the follow-up evaluation, they were measured with Semmes-Weinstein nylon monofilaments, moving and static two-point discrimination, and were judged for the presence or absence of paresthesias in their hands. The children did not receive sensory reeducation in the postoperative period. All children recovered to normal levels of moving and static two-poin

Effects of elevation on nerve function in an acute upper extremity nerve compression model.

External compression was applied to the palmar surface of the wrist over the carpal tunnel in eight healthy volunteer subjects. With the arm in the maximum elevated position, the carpal tunnel pressure was elevated to 50 mm Hg, being continuously monitored by a slit catheter inserted into the carpal canal. Sensory and motor latencies and amplitudes were evaluated at 1-min intervals. When the sensory amplitude decreased by 50% (correlating with subjective sensory changes), the hand was lowered to heart level, still maintaining the carpal tunnel pressure at 50 mm Hg. Nerve conduction velocity an

Patterns of reinnervation and motor unit recruitment in human hand muscles after complete ulnar and median nerve section and resuture.

Following complete ulnar or above-elbow median nerve sections, there was no significant correlation between motor unit size (twitch amplitude) and recruitment threshold, as assessed by spike triggered averaging. This absence of orderly recruitment was attributed to misdirection of motor axons during regeneration. Following median nerve section at wrist level, where the reinnervated muscles have more synergistic actions, orderly recruitment by size appeared to be re-established. Thus, the size principle of motor unit recruitment can be re-established after nerve section in humans, if motor axon

Interfascicular nerve grafting.

Interfascicular nerve grafting is a useful method to repair nerves with gaps. Good motor recovery can be obtained, and return of some degree of sensibility is possible. The procedure can be used to overcome small gaps that result from neglected sharp lacerations or after failure of primary repair, or large gaps that result from loss of nerve substance or traction lesions. Primary nerve repair should be done for acute lacerations. However, in old lacerations (more than three weeks old) and in nerve gaps of more than 2 cm in length, a functional recovery can still be expected after nerve graftin

[Carpal tunnel syndrome. Cosmetic incision (author's transl)].

The median nerve compressed in the carpal tunnel can be released by an incision in the "line of life", a natural fold along the axis of the third metacarpal bone. By sectioning the carpal tunnel in the palm of the hand, on the cubital side of the median nerve, division of the external branch and of the sensory cutaneous rami of the nerve can be avoided. Hypertrophic scars are prevented. The part of the carpal tunnel situated in the forearm can be freed by a subcutaneous incision if needed, but compression at that level is much less frequent than in the palm of the hand.
