Carpal and Guyon tunnel syndrome in burns at the wrist.

A series of 22 patients with burns of the upper extremities have been evaluated by clinical and electromyographic (EMG) studies. As suggested by others, a significantly higher incidence of carpal tunnel syndrome and sensory involvement of the ulnar nerve was discovered. Those findings suggest that we pay more attention to the median and ulnar nerves at the wrist level in burns and treat them when compression is demonstrated by clinical and EMG evaluation.

Funicular localization in partial median nerve injury at the wrists.

The funiculi of the median nerve at the wrist carrying the motor and sensory fibers innervating the hand were described in 1945 by Sunderland after an extensive anatomic study. In an attempt to confirm clinically the findings of that study, 7 patients with partial median nerve laceration at the wrist are presented. The findings in these patients support the schema developed from Sunderland's study except for the funiculi subserving the 1st and 2nd lumbricals which appear to be more dorsally placed. They also tend to confirm Sunderland's finding that the mixing of nerve fibers from different fu

Control of vasospasm following trauma and microvascular surgery.

Vasospasm may result in marked reduction of blood flow through injured or revascularized tissues. Neurogenic vasoconstriction is mediated through sympathetic pathways which may be blocked by long-acting local anesthetics resulting in optimal intravascular pressures and elevated flow. An experimental study performed on normal human volunteers has shown that distal sympathetic blockade using Bupivacaine protects the digital circulation against vasospasm elicited by a strong stimulus for a minimum of 7 to 8 hours. A catheter, placed adjacent to the median or ulnar nerve at the wrist level after d

Nerve grafting in the repair of complicated peripheral nerve trauma.

Peripheral nerve trauma has been a challenge to surgeons, with significant advances in the surgery of repair occurring in major wars. The standard method of treatment involving wide mobilization and end-to-end suture to close traumatic gaps in peripheral nerves has not produced consistently acceptable results, particularly with large gaps. However, with the recent development of microsurgical techniques and instrumentation, the method of interfascicular nerve grafting has been applied to selected patients with trauma problems of the peripheral nerve, by resecting and autogenous nerve grafting
