Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Brachialis Branch to Radial Nerve Transfer for Reanimation of Wrist and Finger Extension: An Anatomical Feasibility Study.

PURPOSE: The motor branch to the brachialis muscle has been described as a versatile donor in upper limb nerve reconstruction. This includes patients with peripheral nerve injuries and tetraplegia. However the anatomic feasibility of transfers from the musculocutaneous nerve to radial nerve branches around the elbow has not been systematically evaluated. This study aims to assess this feasibility. Such a transfer may be particularly relevant in patients with high-level tetraplegia or in patients with spastic upper limb involvement, in which selective denervation of spastic elbow flexors could

A Tactile Trans-Flexor Carpi Radialis Surface Landmark Technique for Median Nerve Local Injection: A Cadaveric Study.

PURPOSE: Median nerve (MN) block under local anesthesia at the wrist is commonly performed but can be limited by anatomic variation, particularly absence of the palmaris longus tendon. This study investigates a novel trans-flexor carpi radialis (trans-FCR) technique for MN anesthesia that uses the flexor carpi radialis (FCR) tendon as a consistent surface landmark. METHODS: Fourteen fresh frozen upper extremity cadaver specimens were used. A 25-gauge needle was inserted through the FCR sheath approximately 4 cm proximal to the wrist crease in a transverse fashion, angled approximately 0° to t

Anomalous Flexor Carpi Radialis Anatomy During Volar Distal Radius Approach: Two Case Reports.

Volar locking plates become the standard treatment for distal radius fractures and are mostly applied through volar approaches that rely on the flexor carpi radialis (FCR) as a superficial landmark. Although usually straightforward, anomalous flexor musculature may complicate exposure, misdirect dissection and increase the risk of iatrogenic neurovascular injury. We report two rare anatomical variants encountered during open reduction and internal fixation: FCR brevis (FCRB) associated with anomalous insertion of the pronator quadratus, and complete agenesis of the FCR. Both substantially alte

Physeal-Sparing Medial Femoral Condyle Flap for Treatment of Scaphoid Nonunion With Avascular Necrosis in Skeletally Immature Patients: A Case Series With Long-Term Follow-Up.

BACKGROUND: Scaphoid nonunion is a rare but challenging condition in the pediatric population. While acute scaphoid fractures in children typically heal well with cast immobilization, nonunion can result in persistent pain and functional impairment. The medial femoral condyle (MFC) flap offers potential advantages for reconstruction by providing a robust blood supply and structural support. However, its application in skeletally immature patients is limited by concerns over damaging the open distal femoral physis. METHODS: We present 2 cases of skeletally immature patients with scaphoid nonun

Radial sensory nerve to anterior interosseous nerve transfer for symptomatic neuromas- clinical outcomes and a transfer classification system.

BACKGROUND: Radial sensory nerve (RSN) injuries occur during common surgical procedures or injuries to the wrist and often result in the formation of painful neuromas. Management strategies of primary repair or secondary reconstruction are limited by poor patient satisfaction. We present a targeted muscle reinnervation (TMR) technique, its clinical outcomes, and a novel classification system for the treatment of recalcitrant RSN neuromas with transfer of the RSN to the anterior interosseous nerve (AIN). METHODS: Cadaveric specimens were used to devise a classification system for the transfer.

Targeted Muscle Reinnervation Using the Anterior Interosseous Nerve for Symptomatic Wrist Level Neuromas.

BACKGROUND: Iatrogenic and traumatic sensory nerve injury at the level of the wrist can lead to debilitating neuroma. Targeted muscle reinnervation (TMR) is an effective treatment for the management of symptomatic neuromas. We investigate the use of the anterior interosseous nerve (AIN) as a recipient nerve for the treatment of iatrogenic neuromas. This case series describes 4 patients treated for neuromas of the lateral antebrachial cutaneous nerve (LABC), palmar cutaneous branch of median nerve (PCB), and radial sensory nerve (RSN). METHODS: Four cases involved a symptomatic neuroma of the

Epidemiology, risk factor, and economic analysis of peripheral nerve injury following distal radius fractures.

PURPOSE: Distal radius fractures (DRFs) are a common orthopaedic injury with a bimodal age and gender distribution, comprising male children after high-energy trauma and older women after low-energy falls. Complications like malunion, ligament ruptures, and nerve injuries can significantly impact recovery. This study aimed to address the gap in population-based data by analyzing the prevalence and risk factors for peripheral nerve injuries associated with DRFs. METHODS: The National Inpatient Sample database was used to analyze DRFs patients with associated ulnar, median, or radial nerve inju

Topographic anatomical landmarks for targeted nerve infiltration in distal radius fracture surgery.

The study focuses on the increasing incidence of distal radius fractures, particularly in individuals under 15 and over 50, and the critical need for effective perioperative pain management strategies. Distal radius fractures often require surgical intervention and pain control, essential to enhancing recovery and minimizing complications like chronic regional pain syndrome (CRPS). Standard analgesic options include hematoma blocks, Intravenous regional anesthesia (IVRA), regional nerve blocks, sedation, and general anesthesia. This research proposes a nerve block approach targeting the anteri

Long-term outcomes of vascular grafting of medium-sized vessels of wrist and hand in systemic sclerosis patients with medication refractory Raynaud's.

Objective: Systemic sclerosis is characterized by vasculopathy at the microvascular level and less commonly the macrovascular level. The vasculopathy in systemic sclerosis, based on its functional and structural changes, leads to symptoms of vasospasm (Raynaud's phenomenon) and tissue injury. In most cases, preventive actions and pharmacotherapy provide beneficial treatment options; however, in severe cases, painful ulcerations, gangrene, and digital amputations may occur with treatment. In medication refractory cases, revascularization of medium and small vessels has been pursued. Other studi

What Is the Probability of Radial Nerve Recovery After Surgical Repair of Humerus Fractures Accounting for Time Since Injury?

BACKGROUND: Radial nerve injury after operative treatment of humeral shaft fracture usually improves, but a subgroup of patients does not recover. Understanding the probability of recovery over time is important in deciding on watchful waiting, nerve exploration, or tendon or nerve transfer. Bayesian analysis is helpful in estimating such probabilities. This type of analysis is predicated on estimating the probability that an event will occur and subsequently updating that estimate as new information becomes available-for instance, when nerve recovery does not occur over time. Reconstruction of Thumb and Index Flexion in High Median Nerve Paralysis Using a Single Radial Wrist Extensor Tendon Transfer.

PURPOSE: Chronic paralysis of the flexor pollicis longus (FPL) and the index flexor digitorum profundus (FDP) is commonly treated with brachioradialis transfer to the FPL and side-to-side suturing of the index FDP to the FDPs of the ulnar digits. We report the clinical outcomes of nine patients in whom the extensor carpi radialis brevis (ECRB) or longus (ECRL) was transferred to both the FPL and index FDP. METHODS: We reviewed nine patients who underwent tendon transfer of the ECRB/L to both the FPL and index FDP. Postoperative assessments included strength grading of the FPL and index FDP us

Anconeus Muscle Transfer to Correct or Prevent Wrist Radial Deviation in Radial and Posterior Interosseous Nerve Injuries.

PURPOSE: Wrist radial deviation is a possible complication of tendon transfer for restoration of wrist extension in cases of radial nerve paralysis. In posterior interosseous nerve (PIN) injury, this is because of the imbalance caused by the intact extensor carpi radialis longus and paralysis of the extensor carpi ulnaris (ECU). This deformity may also occur following transfer of the pronator teres (PT) to the extensor carpi radialis brevis (ECRB) for radial nerve palsy. To address wrist radial deviation, we propose transferring the anconeus muscle, extended by the intermuscular septum between

Radial and median nerves distal peripheral tension after reverse shoulder arthroplasty: a cadaveric study.

BACKGROUND: Peripheral nerve injury is a recognized complication after reverse shoulder arthroplasty (RSA) that has mainly been studied at the level of the brachial plexus and its proximal branches. However, the impact of RSA on distal peripheral nerves and the influence of elbow and wrist position is not known. This cadaveric study aimed to analyze the effect of RSA implantation and upper limb position on tension in the distal median and radial nerves. The hypothesis was that RSA increased distal nerve tension, which could be further affected by elbow and wrist position. METHODS: 12 upper li

SKIN SURFACE TEMPERATURE CHANGES BEFORE AND AFTER MEDIAN NERVE DECOMPRESSION IN CARPAL TUNNEL SYNDROME.

Carpal tunnel syndrome is the most common upper extremity compression neuropathy caused by compression of the median nerve at the wrist. Along with motor and sensory fibers, sympathetic fibers also pass through the median nerve, playing an important role in the regulation of blood flow to the skin which interacts between the body interior and its environment. The aim of this study was to examine the preand postoperative correlation between skin temperature changes in the area innervated by compressed median nerve compared to the skin areas innervated by radial and ulnar nerve and median nerve

Outcomes and prognostic factors for nerve grafting following high radial nerve injury.

In this study, we examined the prognostic factors affecting outcomes following nerve grafting in high radial nerve injuries. Thirty-three patients with radial nerve injuries at a level distal to the first branch to the triceps and proximal to the posterior interosseous nerve were retrospectively studied. After a follow-up of at least 1 year, 24 patients (73%) obtained M3+ wrist extension, 16 (48%) obtained M3+ finger extension and only ten (30%) obtained M3+ thumb extension. Univariate, multivariate and receiver operating characteristic analyses showed that a delay in the repair of less than 6

In-house 3D-printed custom splints for non-operative treatment of distal radial fractures: a randomized controlled trial.

We compared patient satisfaction and clinical effectiveness of 3D-printed splints made of photopolymer resin to conventional fibre glass casts in treating distal radial fractures. A total of 39 patients with minimally displaced distal radius fractures were included and randomized. Of them, 20 were immobilized in a fibre glass cast and 19 in a 3D-printed forearm splint. The 3D-printed splints were custom-designed based on forearm surface scanning with a handheld device and printed in-house using digital light processing printing technology. Patient satisfaction and clinical effectiveness were a
