The widespread use of smartphones and electronic devices has raised concerns about their impact on hand and wrist health. This scoping review synthesised current evidence on musculoskeletal and soft-tissue disorders associated with device use. Thirty-two studies were included, examining smartphones, computers and other digital tools. Smartphones were primarily linked to De Quervain's tenosynovitis and flexor pollicis longus tendinopathy, while computers and visual display units were associated with carpal tunnel syndrome and repetitive strain injuries. Key risk factors included repetitive thum

PURPOSE: We describe a technique to reconstruct fingertip defects with dorsal digital artery perforator flaps innervated with terminal branches of the proper digital nerve in a propeller manner.
METHODS: To reconstruct fingertip defects, a dorsal digital artery perforator flap innervated with terminal branches of the proper digital nerve was performed. This retrospective study includes 44 fingers and 4 thumbs in 45 cases. Thirty-seven males and eight females with a mean age of 43.7 years were enrolled. The sizes of large paddles ranged from 2.0 cm × 1.3 cm to 6.0 cm × 1.6 cm, and the sizes of

Tetraplegia is a significant impairment of mobility in all four limbs, often following a cervical spinal cord injury. Recent studies in the management of tetraplegia describe the use of nerve transfer in addition to tendon transfer for upper extremity rehabilitation. While the popularity of these techniques is growing, there are limited studies describing the long-term outcomes and functional recovery following such procedures. This case describes a patient who sustained a C7 spinal cord injury, right olecranon fracture, and left distal radius fracture following a fall. Surgeons performed open

Thumb amputation is a severe injury that significantly affects hand function. Replantation remains the preferred treatment, particularly in young and active individuals. However, prolonged ischemia time is traditionally considered a limiting factor for successful replantation. We present a case of a 25-year-old right-hand-dominant male who sustained a traumatic saw-cut amputation of the left thumb at the base of the proximal phalanx and underwent successful replantation approximately 24 hours post-injury. Surgical management included skeletal fixation with Kirschner wires, microvascular anasto

Background: Although surgical digit amputations are common procedures in hand surgery, substantial differences in technical approaches have been reported, with the development of symptomatic neuromas posing a major challenge. Given the absence of a universally accepted technique, especially concerning painful neuroma prevention, this survey study's rationale was to explore the preferred surgical approach for traumatic noninfected digital amputation injuries (excluding the thumb) at the interphalangeal (IP) joint levels. Methods: A 10-point online questionnaire was sent to ÖGH, DGH and SGH memb

INTRODUCTION: A traumatic nerve injury that is improperly treated or left untreated may result in a painful neuroma with compromised sensory and motor function. Affected individuals might be hesitant to donate a healthy nerve for an autologous nerve graft procedure. The use of processed nerve allografts (PNA) to reconstruct nerve injuries with painful neuromas is less often discussed. We aim to offer perspectives on treating painful neuromas with PNA when autograft reconstruction is considered not an option.
METHODS: This series involves five individuals with neuroma formation and varying pai

Volkmann ischemic contracture is a severe and largely preventable complication of untreated or delayed compartment syndrome. In children, it is most commonly associated with supracondylar humerus fractures, especially when external compression from casts or splints is not promptly recognized. We report the case of a six-and-a-half-year-old right-handed girl who developed severe left upper-limb dysfunction after a closed supracondylar humerus fracture treated in a rural setting with tight traditional splint immobilization, locally known as "Jbira." No fasciotomy was performed during the acute p

BACKGROUND: Loss of key-pinch sensation after median nerve injury poses significant functional detriment. Nerve transfers are utilized to improve function after nerve injury and size matching of donor and recipient nerves is important to optimize success. This anthropometric study investigates the anatomy of the superficial branch of the radial nerve (SBRN) to the thumb and index finger and explores radial to median sensory nerve transfers, a necessary but not heavily discussed facet of nerve transfers for the hand.
METHODS: Anatomic dissection of SBRN in 15 cadaver hands was performed to ass

Hand injuries from firecrackers require precise repair, especially when the thumb is involved due to its crucial role in grasping, pinching, and daily activities. In this study, we evaluate the outcomes of selective soft tissue pollicization in a patient who sustained fireworks injuries, including multiple wounds, amputations with bone exposure, digital nerve transections, and extensive soft tissue damage across several fingers. Numerous methods exist to achieve optimal results, each with benefits and downsides. Through comprehensive assessment and meticulous surgical technique, we present the

Palmar digital skin, with its high density of epidermal sweat glands and unique stratum corneum, derives its autonomic innervation from peripheral nerves. Understanding the changes to these structures could serve as objective proxy markers following peripheral nerve injury and interventions to promote regeneration. There are limited data in humans for these measures, and normative reference ranges are yet to be defined. Light-based imaging techniques, such as optical coherence tomography (OCT), can detect quantifiable changes in these epidermal parameters. This study aimed to define the normal

BACKGROUND: Complications of thumb basal joint arthritis surgery include surgical wound pain and cheiralgia paresthetica through involvement of the radial nerve superficial branch (RNSB). The aim of this study is to compare the risk of nerve injury between anterolateral and dorsal approaches to the first carpometacarpal joint (1CMCJ) by measuring the distance between the incisions and RNSB, recording crosses between them, and evaluating the density of skin nerve endings at incision sites.
METHODS: In this descriptive study of 20 cryopreserved cadaver specimens, the anatomical distribution of

BACKGROUND: Fingertip injuries present a challenge for surgeons. Factors such as appearance, sensation, and grip strength influence the choice of procedure. The neurovascular island (NVI) flap is commonly used due to its reliability and favorable results. However, it can lead to reduced sensation and a limited range of motion, which are significant concerns.
METHODS: This study analyzed 56 adult patients with 66 finger injuries treated with a modified NVI flap by a single surgeon. The flap is based on branches of the proper digital artery to the volar and dorsal skin, including two nerves in

PURPOSE: Median nerve injuries can result in substantial functional deficits with both motor and sensory components. Sensory loss results in impaired hand function. The most important areas are the ulnar and radial aspects of the thumb and index finger, respectively. This study aimed to evaluate the feasibility of nerve transfer from the lateral antebrachial cutaneous nerve (LACN) to the median nerve for restoring hand sensation.
METHODS: This study used 16 cadaveric upper limbs. After dissecting the LACN and its branches, the length and size were recorded. The median nerve was identified via

Bowler's thumb is an uncommon traumatic neuropathy of the thumb ulnar digital nerve. We present a case of bowler's thumb in an individual who is an avid bowler, however, only manifesting following an unrelated injury. We describe the clinical and imaging findings, differential diagnostic considerations, and the treatment options. Knowledge of this entity and its sonographic features is important in order to identify this uncommon entity.

PURPOSE: Sensory nerve transfers may be performed to restore protective sensation and tactile perception after peripheral nerve injury in the upper extremity. There is a paucity of literature on the available donor-recipient configurations for sensory nerve transfers. This article presents a systematic review of reported sensory nerve transfers in the upper extremity.
METHODS: Original articles published between 1982 and 2022 were searched in MEDLINE and EMBASE. Articles describing a sensory nerve transfer were included if patient sensory outcomes were reported. Outcomes were categorized acco

OBJECTIVE: Cervical spinal cord injury (SCI) and lower trunk brachial plexus injury (BPI) commonly result in hand paralysis. Although restoring hand function is complex and challenging to achieve, regaining volitional hand control drastically enhances functionality for these patients. The authors aimed to systematically review the outcomes of hand-opening function after supinator to posterior interosseous nerve (PIN) transfer.
METHODS: A systematic literature review was performed according to the PRISMA guidelines.
RESULTS: A total of 16 studies with 88 patients and 119 supinator to PIN tran

BACKGROUND: Tension-free end-to-end digital nerve repair or reconstruction under loupe or microscope magnification are surgical treatment options for lacerated digital nerves in patients with multiple injured digits, injuries to the border digits, or injuries to the thumb, with the goal of improved or restored sensation and a decreased risk of painful traumatic neuroma formation. Different techniques for primary repair have been described and include epineurial sutures, nerve "glues" including fibrin-based gels1,2, biologic or synthetic absorbable or nonabsorbable nerve wraps or conduits, or a

The thumb is acknowledged to contribute significantly to hand function, accounting for up to 40% of its overall capability. Consequently, injuries affecting the thumb can have a profound impact on the quality of life experienced by patients. When it comes to the surgical reconstruction of thumb injuries, the primary objective is to promptly provide coverage of the affected area with glabrous skin, thus preserving both the length and functionality of the thumb. Managing injuries specifically involving the thumb pulp can be particularly challenging due to the digit's size and vital role. Acquiri
