Inhibition of motoneuron discharge by peripheral nerve stimulation: an F response analysis.

F waves were recorded from the abductor pollicis brevis stimulating the median nerve at the wrist. These data were compared to responses obtained after preceding supramaximal stimulation of digital fibers of the second (II) finger or the median nerve at the wrist. The time between conditioning and test stimuli were 50 msec. Following conditioning stimuli, F wave latencies were significantly increased while F amplitudes, durations, and persistences were all decreased. Chronodispersion was not significantly affected. These changes were associated with increased repetition of individual F respons

Avulsion injuries of the thumb.

Avulsion amputations of the thumb are generally thought to have a worse prognosis after replantation than other amputations. We report the results of 17 thumbs that had an avulsion amputation and were replanted. Fourteen of the 17 survived (82 percent). Our experience indicated that the survival rate was improved by restoring continuity of at least two veins and two arteries, using a Y-shaped vein graft and the princeps pollicis artery for the source of arterial circulation. Nerve grafts were used to bridge defects in avulsed digital nerves. When possible, avulsed tendons were reattached to th

[Free toe transfer with plantar metatarsal arteries and dorsal venous pedicle. Report of 18 cases].

Eighteen free second toe were used for pollicization other than the conventional method. From the first and second plantar metatarsal arteries junction of the both digital arteries of the second toe were dissected retrogradely and anastomosed to the deep of superficial palmar arch or terminal branch of the radial artery. The venous return was established between the dorsal venous arch and cephalic vein. These vessels were found to have much bigger diameters and less anatomical variation than those used by conventional methods. Seventeen transplantation were considered successful both functiona

[Nerve graft of collateral branches of fingers. Report of a series of 16 reviewed cases].

Sixteen digital nerve grafts (14 patients) were reviewed with an average follow-up of 43 months. Average age was 27 years (7-53 yrs). Eleven grafts were performed on the thumb or the index finger. The nerve lesion was always proximal to the proximal interphalangeal joint, and associated with a tendinous lesion in one case out of two. One graft was done in emergency, 6 for failure of sutures and 7 for neglected wounds. Delay before grafting was on average of 4 months (0-16 months). The n. cutaneus antebrachii medialis was used in 11 patients. An hypoesthetic area of an average of 78 cm2 was not

Sensory recovery following digital replantation.

The recovery of sensibility following digital replantation is essential in the restoration of hand function. We reviewed 12 series of digital replantations between 1977 and 1989. Three hundred sixty-seven fingers and 87 thumbs were successfully replanted. Mean age was 32.5 years. Mean follow-up was 33.5 months. Mean static two-point discrimination (S2PD) was 9.3 mm in clean thumbs vs. 12.1 mm in crush/avulsion thumb replantations. Mean S2PD was 8 mm in clean finger vs. 15 mm in crush/avulsion finger replantations. Overall mean S2PD was 11 mm in thumb and 12 mm in finger replantations. Sixty-on

Isolated complete denervation of the flexor pollicis longus.

Isolated complete denervation of the flexor pollicis documented by electromyography has not been reported. We report a healthy 26-year-old man who sustained a gunshot wound at the base of his left index finger. He reported ipsilateral flexor pollicis longus (FPL) paralysis one week later, after undergoing surgical exploration of the radial digital artery and nerve supplying the left index finger under axillary block anesthesia. Forearm pronation and distal interphalangeal flexion of the second through fifth digits were normal. Electromyographic findings revealed complete isolated denervation o

Transection of radial digital nerve of the thumb during trigger release.

Iatrogenic laceration of the radial digital nerve of the thumb during trigger release occurred in four patients. Histologic cross sections through the thumb in cadaveric specimens at the level of the metacarpophalangeal crease showed the radial digital nerve to be 2.19 mm beneath the dermis and 1.15 mm directly anterior to the radial sesamoid bone. In this position the nerve is vulnerable to transection when it is trapped between the sesamoid and the knife blade.

Diagnostic specificity of sensory and motor nerve conduction variables in early detection of carpal tunnel syndrome.

In the carpal tunnel syndrome (CTS) sensory nerve conduction is more sensitive than motor conduction. However, 8%-25% of the sensory distal latencies in symptomatic hands may still be normal. A systematic study was made of the median, ulnar and radial orthodromic nerve conduction velocities (SNCV) stimulating each of the fingers separately. Four SNCVs from the median nerve, two SNCVs from the ulnar nerve and one from the radial nerve were obtained, and the ratio of the median to radial SNCV and the ratios of the median and ulnar SNCVs were estimated. The significance of these parameters in the

Free serratus anterior muscle transplantation for hand reconstruction.

Free transplantation of the serratus anterior muscle has allowed salvage or reconstruction of difficult hand injuries with advantages compared to traditional methods. Free muscle tissue adds vascularity to poorly vascularised and potentially infected wounds and allows hand elevation and early mobilisation. Dynamic reinnervation is possible with segmental preservation of the long thoracic nerve. The serratus anterior muscle is easily harvested and has a long, reliable vascular pedicle. It is thin, easily contoured to the defect and can be separated into its component slips. The muscle provides

Dual innervated index to thumb cross finger or island flap reconstruction.

Partial volar amputations of the entire thumb pulp require soft tissue coverage that provides not only padding and protection for exposed bone and tendons, but also restoration of discriminative sensation. A modification of the radial sensory innervated cross finger flap from index to thumb is described, which includes separate reinnervation of the flap by repair of the dorsal sensory branch of the index radial digital nerve to the ulnar digital nerve of thumb. The anatomical basis for this flap is discussed and two example cases presented. Sensory reinnervation is rapid and does not require c

Digital replantations distal to the distal interphalangeal joint.

Microvascular replantations of digits distal to the distal interphalangeal (DIP) joint were reviewed in 33 digits of 29 patients. Twenty-five digits were completely amputated and eight were incompletely amputated. Ten of 33 replants failed and 23 digits survived. Even without venous reconstruction, good results were obtained in Zone I. Of eight completely amputated digits in Zone 1 in which venous reconstruction was not performed, seven digits survived. Of four completely amputated digits in Zone 2 without venous reconstruction, all became necrotic. In digits followed for six months or more af

Thumb avulsion: results of replantation/revascularization.

During a 9-year period, 39 thumb avulsions required emergency replantation/revascularization at our institution. Amputation was complete in 28 (72%) cases and involved the dominant thumb in 31 (80%). Farm machinery was the source of injury in 33 (85%) cases. Follow-up ranged from 7 months to 6 years. Only 10 of the revascularization attempts (26%) were successful; the remaining attempts required amputation at the level of the injury. In two (18%) of the 11 partial amputations and in eight (29%) of the 28 complete amputations, revascularization was successful. In eight (31%) of the 26 thumbs in

An investigation of the digital nerves of the thumb.

With the use of a stereoscopic microscope, observations were made on the course of the proper digital nerve of the thumb on ten upper limbs in adult cadavers. By observation from the dorsal side after removal of the bone, it was possible to clearly see the ramification of the digital nerve from the subcutaneous tissue to the dermis. Ramification from the palm to the level of the interphalangeal joint was segmental. The digital nerve of the thumb trifurcates from the proximal site of the distal palmar thumb crease into the main, medial, and lateral branches. The main branch is thick and extends

Evaluation of sensibility after sensory reconstruction of the thumb.

We have studied the restoration of sensation in 24 patients after operations involving the digital nerves of the thumb. These comprised 10 neurorrhaphies, two nerve graftings, five replantations, one toe-to-thumb transfer and six neurovascular island flaps. The average follow-up period was 46 months. Greater sensitivity was found in the moving two-point discrimination (2PD) test than in the static 2PD test. The results of the Semmes-Weinstein test showed good correlation with the static 2PD test. Porter's letter test showed correlations with both 2PD tests, and the pulp-writing test showed goo

Secondary repair of digital flexor tendons in outpatient clinic.

A total of 32 digits, including 3 thumbs, were reconstructed in an outpatient clinic. The patients were allowed to go home 3-4 hours after the operation. The patients had injury to the flexor tendon in no man's land without other complications except for an injury to the digital nerve in 6 cases. The tendon was reconstructed secondarily using a free graft. The minimum follow-up time was 6 months. The results were classified in three ways. With all methods the results, which were deemed satisfactory in 79% and poor in 21%, reached the average standard for this type of repair. Increasing age of

[Intersesamoid resection-suture of the metacarpophalangeal joint of the thumb. Sesamoidorrhaphy of the metacarpophalangeal joint of the thumb].

In some cases, congenital or acquired posterior instabilities of the MP joint of the thumb may cause pain or create functional disorders. It is possible to stabilize the metacarpophalangeal joint by partial resection of the ligament joining the two sesamoids which makes it possible to move and suture them toward each other. An enlargement plasty of the pulley then is required. This technique prevents hyperextension but without restricting flexion or opposition. The digital nerves must be carefully identified and protected.

Restoration of sensibility to a thumb by the technique of digital nerve advancement: a new surgical procedure.

A groin flap was used to repair avulsion injuries of the nondominant thumb and index finger in a 13-year-old boy. The patient demonstrated full range of motion in the involved digits but lacked sensibility and a measurable two-point discrimination of the thumb. A two-point discrimination of 3 mm and excellent subjective sensibility were restored by distal advancement of the digital nerves and separation into fascicles across the ulnar aspect of the thumb. A 1 1/2-year follow-up period is recorded.

Ulnar digital compression neuropathy of the thumb caused by splinting.

Acute compression neuropathy of the ulnar digital nerve of the thumb is described in two women, 23 and 53 years of age. The injury was caused by pressure from a splint. The pathogenesis and clinical features resemble the classic "bowler's thumb." Early recognition and proper treatment will prevent irreversible neurologic damage.

Complex dislocations of the metacarpophalangeal joint.

A dorsal longitudinal incision provides excellent exposure for reducing complex dislocations of the metacarpophalangeal joints of the thumb and index finger. Within the past four years, we have successfully treated five patients with irreducible dislocations of the metacarpophalangeal joint of the thumb and four with dislocations of the index finger by open reduction through a dorsal incision. This approach exposes the joint and the major structure blocking reduction, the volar plate. The digital nerves, which are tented volarly over the metacarpal head, are not prone to injury and osteochondr
