Epidemiology of Flexor Tendon Injuries of the Hand in a Northern Finnish Population.

BACKGROUND: Flexor tendon injuries cause significant morbidity in working-age population. The epidemiology of these injuries in adult population is not well known. The aim of this study was to describe the epidemiology of flexor tendon injuries in a Northern Finnish population. MATERIAL AND METHODS: Data on flexor tendon injuries, from 2004 to 2010, were retrieved from patient records from four hospitals, which offer surgical repair of the flexor tendon injuries in a well-defined area in Northern Finland. The incidence of flexor tendon injury as well as the gender-specific incidence rates was

Bowler's thumb: ultrasound diagnosis of a neuroma of the ulnar digital nerve of the thumb.

Bowler's thumb is a rare traumatic neuropathy of the ulnar digital nerve of the thumb. We present a case of bowler's thumb in a 21-year-old male recreational bowler who presented with a painful mass on the ulnar side of the right thumb. Magnetic resonance (MR) imaging of the hand was inconclusive. However, subsequent ultrasound (US) showed asymmetric enlargement of the ulnar digital nerve of the thumb with marked epineural thickening corresponding to the palpable mass, confirming the clinical diagnosis of bowler's thumb. Although this condition is typically diagnosed clinically, imaging can he

Cadaveric Study of a Z-Plasty Modification to the Moberg Flap for Increased Advancement and Decreased Morbidity.

BACKGROUND: The Moberg advancement flap is a well-established tool to provide sensate, vascularized tissue for thumb reconstruction. Modifications providing additional length have been described, but no studies have examined how much additional advancement can be achieved consistently, and at what cost. The authors hypothesized that Z-plasty modification at the base of the Moberg flap would allow additional advancement compared with the traditional technique, and maintain primary closure of the donor-site and avoid additional morbidity. METHODS: Standard Moberg flaps were performed and advanc

Evaluation of Percutaneous First Annular Pulley Release: Efficacy and Complications in a Perfused Cadaveric Study.

PURPOSE: Trigger finger is the most common entrapment tendinopathy, with a lifetime risk of 2% to 3%. Open surgical release of the flexor tendon sheath is a commonly performed procedure associated with a high rate of success. Despite reported success rates of over 94%, percutaneous trigger finger release (PFTR) remains a controversial procedure because of the risk of iatrogenic digital neurovascular injury. This study aimed to evaluate the safety and efficacy of traditional percutaneous and ultrasound (US)-guided first annular (A1) pulley releases performed on a perfused cadaveric model. METH

Thumb proximal phalanx reconstruction with nonvascularized corticocancellous olecranon bone graft.

Large segmental bone defects of the phalanges reportedly have been treated with free vascularized grafts from the hand, foot, or knee, or with nonvascularized grafts from the iliac crest. A nonvascularized structural corticocancellous graft from a local site would be advantageous. The olecranon has been used as a source of both cancellous and corticocancellous graft. The authors describe a unique case of the use of nonvascularized corticocancellous olecranon bone graft for structural purposes in a mutilating thumb injury. The patient injured the left thumb with a miter saw, resulting in a larg

Ancient schwannoma of the thumb pulp.

A 21-year-old woman had become aware of a mass on the palmar aspect of the distal phalanx of her left thumb 11 years earlier. Her medical history was unremarkable, with no traumatic injury of the left thumb. Initial examination revealed a hard but elastic, immovable tumor of 30 × 25 mm on the palmar aspect of the distal phalanx of the left thumb, with an ulcer formed at the center. No pain, sensory disturbance, or motor dysfunction was noted. The patient underwent punch biopsy at the outpatient clinic, and the tumor was histopathologically diagnosed as benign schwannoma. Total excision of the

Extrusion of a NeuroTube: A Case Report.

BACKGROUND: Peripheral nerve injury is a common result of trauma. In cases of nerve gap, treatment may involve placement of a nerve conduit. This case involves a polyglycolic acid nerve conduit tube that was extruded through soft tissue. To our knowledge, this reactive process has only been previously documented in one article. This complication is not commonly known among hand surgeons, hence our interest in documenting it. CASE REPORT: We present the case of a 33-year-old male who injured his right nondominant thumb in a workplace saw accident. His complex wound involved the radial digital

A Cadaver Study of the Feasibility of Multidigit Allotransplantation for Reconstruction of the Metacarpal Hand.

BACKGROUND: Amputation of all fingers (metacarpal hand) can be functionally equivalent to hand amputation. Multidigit allotransplantation can benefit patients who require a full complement of fingers to return to their preinjury activities. The authors investigated the feasibility of the transfer of four fingers and thumb as a single allograft. METHODS: Four fingers and the thumb were harvested from 16 hands as an allograft based on the radial and ulnar arteries. Dorsal digital veins were dissected until confluent in the major veins. The common digital nerves were divided at the origin. The f

Modified first dorsal metacarpal artery island flap for sensory reconstruction of thumb pulp defects.

UNLABELLED: Restoration of tactile sensation after reconstruction of a thumb pulp defect is import for hand function. We describe our clinical experience using a modified first dorsal metacarpal artery island flap innervated by the radial dorsal branch of the proper digital nerve and the terminal branch of the superficial radial nerve in 20 consecutive cases. The results were compared with 25 patients treated by the conventional Foucher's first dorsal metacarpal artery flap without nerve repair. At the final follow-up, flap sensation was assessed using static two-point discrimination and Semme

Use of the dorsal digital sensate free flap for reconstruction of volar soft tissue defect of digits.

Sensate cross-finger flaps and homodigital and heterodigital island flaps can usually be used for sensory reconstruction in the volar aspect of the digit. However, when the donor areas are damaged by concomitant injuries, these flaps are not available. The free dorsal digital flap, including both dorsal branches of the proper digital nerves, can be used as an alternative to resolve this problem. The flap was used in 16 patients with soft tissue defects on the volar aspect of the digits. For comparison, we collected a consecutive series of 31 patients with similar defects treated using a cross-

Extended reverse dorsal metacarpal artery flap for coverage of finger defects distal to the proximal interphalangeal joint.

UNLABELLED: Finger reconstruction distal to the proximal interphalangeal (PIP) joint is a challenging task for hand surgeons. Although extended reverse dorsal metacarpal artery (RDMA) flaps were described for coverage of finger defects, reports on repairs beyond the PIP joint are limited. The occurrence of venous congestion of the flaps and their treatments require further clarification. METHODS: Extended RDMA flaps were used to cover the finger defects extending from the PIP joints to the fingertips in a series of 16 patients. The reconstructed fingers included the index, middle, ring, and l

Compression neuropathy of the ulnar digital nerves in the thumbs of a massage therapist.

Compression neuropathies of digital nerves, caused by hypertrophied or anomalous muscles, are rare compared with such occurrences above the wrist. We reported a case of compression neuropathy of the ulnar digital nerves in bilateral thumbs of a massage therapist. Entrapment of the digital nerves by the hypertrophied first dorsal interosseous and adductor pollicis muscles over the first web space of the right hand was detected by magnetic resonance imaging. Surgical debulking of the muscles and neurolysis were performed on the dominant right hand. The left hand was successfully treated with bot

Innervated thenar pedicle flap with the palmar cutaneous branch of the median nerve for treating a thumb pulp defect: a case report.

A 51-year-old male plumber with a thumb pulp defect was treated with a reversed innervated thenar pedicle flap. The flap was based on the radial digital artery originating from the princeps pollicis artery with the palmar cutaneous branch of the median nerve. The flap survived and achieved good innervation with a moving 2-point discrimination of 6 mm at 12 months after surgery. This flap is indicated for patients who hesitate to have tissue taken from the foot. We believe that this flap is a feasible option for reconstructing thumb pulp defects.

Transfer of the Fifth Finger to Replace an Amputated Thumb: A Preliminary Study.

Amputation of the thumb causes not only loss of significant functional use of the hand but also psychological and social problems. The procedures of toe-to-thumb transfer, pollicization, or metacarpal extension are recognized and well-documented options in thumb reconstruction. Although these techniques have been successfully applied, they have some disadvantages. This study aims to test the feasibility of the fifth finger for thumb reconstruction. The fifth finger was released following exploration of the digital artery, vein, nerve, and flexor and extensor tendons, including the flexor retin
