Wraparound chimeric radial collateral artery perforator flap for reconstruction of thumb loss.

OBJECTIVE: We present our clinical experience and demonstrate surgical methods to reconstruct the thumb by using a wraparound chimeric radial collateral artery perforator flap. METHODS: Surgical procedures were performed in 12 patients. Flaps with a skin paddle and humeral bone segment were created on the basis of independent perforators. The sizes of the flaps and humeral fragments ranged from 5.5 cm × 2.0 cm to 7.5 cm × 4.5 cm and from 1.5 cm × 0.5 cm to 4.5 cm × 1.5 cm, respectively. The flap pedicle was divided and ligated above the level at which the radial collateral artery was divided

Percutaneous trigger thumb release in children: neither effective nor safe.

BACKGROUND: Although percutaneous trigger thumb release has been extensively used in adults, the technique is not widespread in children. The purpose of this study was to evaluate the efficacy and safety of percutaneous trigger thumb release in the pediatric age group. METHODS: Twenty consecutive thumbs of 15 patients scheduled for surgical release of the A1 pulley were included in this cohort. Each patient received first the percutaneous release (PR) followed by an open release (OR) and served as self-controls. Thumb extension was assessed immediately before PR, after PR, and finally after O

The relationship of the superficial radial nerve and its branch to the thumb to the first extensor compartment.

PURPOSE: The superficial radial nerve and its branches are vulnerable during surgery for de Quervain tenovaginitis. We studied the proximity of the nerve branches to the first extensor compartment. METHODS: We dissected 20 forearms of 11 cadavers and measured the branching point of the superficial radial nerve relative to the radial styloid. We defined the midline of the first extensor compartment and measured distances of nerves adjacent to it. RESULTS: The superficial radial nerve gave the lateral dorsal digital branch to the thumb at 50 ± 13 mm (minimum, 26 mm; maximum, 72 mm) proximal to

Value of anatomic landmarks in carpal tunnel surgery.

This study investigated which anatomic landmarks were most useful for correct and safe incision placement in carpal tunnel surgery. Kirschner wires were attached to the hands to mark previously defined landmarks. The bony attachments of the transverse carpal ligament, which were identified previously, were drawn on an anteroposterior digital x-ray of the hand, with the thumb in full abduction. The relationship between anatomic landmarks and these bony attachments were examined. In all hands, either the line along the third web space or the crease between the thenar and the hypothenar regions,

Early radial digital neuropathy of the thumb due to flexor pollicis longus tendinitis: value of ultrasound in an uncommon mild neuropathy.

INTRODUCTION: A 50-year-old man developed intermittent paresthesia of the outer portion of the right thumb. Paresthesia increased with thumb abduction and flexion. The remainder of the physical examination was normal. METHODS: Routine motor and sensory examinations of the median and ulnar nerves were within normal ranges. RESULTS: Ultrasonographic studies revealed that a tenosynovitis of the flexor pollicis longus of the right thumb was compressing the radial digital nerve of the thumb. CONCLUSIONS: Ultrasonographic evaluation was used in conjunction with nerve conduction studies to diagnos

Restoration of thumb sensibility with innervated reverse homodigital dorsoradial flap.

PURPOSE: This study investigates the use of homodigital reverse dorsoradial flap with neurorrhaphy for thumb soft tissue defect. METHODS: From 1996 to 2010, the homodigital dorsoradial flap was performed on seven adult patients and one 3-year-old boy. The flaps ranged from 1.2 to approximately 3.0 cm × 2.0 to approximately 4.2 cm in size. In six of the eight patients, the dorsal collateral branch of the radial nerve supplying the flap was also included in the pedicle and coapted to the proper digital nerves of the thumb. At final follow-ups, flap sensation, thumb motion, and donor-site morbid

Finger replantation: surgical technique and indications.

In this article, we discuss the surgical technique of finger replantation in detail, distinguishing particularities of technique in cases of thumb amputation, children fingertip replantation, ring finger avulsion, and very distal replantations. We emphasize the principles of bone shortening, the spare part concept, the special importance of nerve sutures and the use of vein graft in case of avulsion or crushing. However, even if finger replantation is now a routine procedure, a clear distinction should be made between revascularization and functional success. The indications for finger replant

Collagenoma in a child with tuberous sclerosis complex causing carpal tunnel syndrome and thumb overgrowth: case report.

We report on a child with tuberous sclerosis complex who had progressive enlargement of a collagenoma of the right hand. The tumor resulted in median nerve compressive neuropathy and overgrowth of the thumb. Debulking of the tumor along with median nerve decompression was performed with relief of neurological symptoms. No recurrence of the symptoms was observed during the 1-year follow-up after surgery. To the best of our knowledge, this is the first report of a deep collagenoma in any anatomical location.

Open versus percutaneous release for the treatment of trigger thumb.

The purpose of this retrospective study was to compare the outcomes and complications of conventional open surgical release and percutaneous needle release in the treatment of trigger thumb. The study comprised 87 patients with trigger thumb who were treated with either open pulley (n=52) or percutaneous (n=32) release between 2008 and 2011. All patients were reevaluated at a mean follow-up of 22.7±9.6 months (range, 9-44 months). Main outcome measures were the rate of recurrence, pain on movement or tenderness over the pulley, infection rate, digital nerve injury, tendon bowstringing, joint s

A coincidental anterior interosseous nerve palsy after volar plate fixation of a distal radius fracture diagnosed by 3D-CT.

A case presented dysfunctional flexion of the thumb and index fingers after volar plate fixation for of distal radius fractures was diagnosed anterior interosseous nerve palsy and confirmed by 3D-CT. 3D-CT was useful to confirm the continuities of tendons, which excluded the most common complication of tendon ruptures after volar plate fixation. Also, it was superior to needle electromyography which is unreliable due to the accompanying damage of the pronator quadratus following volar plate fixation.

Use of twin dorsal middle phalangeal finger flaps for thumb or index finger reconstruction.

Amputation or degloving injuries of the thumb or index finger are highly disabling. We describe the use of twin dorsal middle finger flaps harvested from the dorsal aspects of the middle and ring fingers, and based on one palmar proper digital artery, its venae comitantes, and the dorsal branches of the palmar digital nerves of the middle and ring fingers, respectively. These flaps offer advantages when large soft tissue defects of the thumb or index finger are present. In this study, twin dorsal middle finger flaps were used in nine patients (six thumbs, three index fingers). All flaps comple

Prognosis for recovery of posterior interosseous nerve palsy after distal biceps repair.

BACKGROUND: There is very little information on the incidence of and usual recovery period for posterior interosseous nerve (PIN) palsies after distal biceps repair. This study examined the incidence and the time to resolution of PIN palsies in a large consecutive series of primary distal biceps repairs. MATERIALS AND METHODS: A retrospective review was performed of a consecutive series of patients treated by 34 fellowship-trained upper extremity surgeons with primary distal biceps repair through a single anterior incision technique. Patients' records were reviewed to determine how many exper

Reconstruction of proper digital nerve defects in the thumb using a pedicle nerve graft.

BACKGROUND: Traumatic defects of the proper digital nerve in the thumb affect tactile perception of the thumb pulp. This article reports on the treatment of the defect using a pedicle nerve graft taken from the dorsal branch of the proper digital nerve of the index or long finger, or both. METHODS: From May of 2006 to March of 2010, the pedicle nerve graft was used in 16 thumbs in 16 patients. There were 13 male and three female patients, with an average age of 33 years. Nerve repair was performed on one side in six thumbs and on both sides in 10 thumbs. The average length of the defects was

Clinical findings in C5-C6 and C5-C7 root palsies with brachial plexus traction lesions.

Stretch injuries of the C5-C7 roots of the brachial plexus traditionally have been associated with palsies of shoulder abduction/external rotation, elbow flexion/extension, and wrist, thumb, and finger extension. Based on current myotome maps we hypothesized that, as far as motion is concerned, palsies involving C5-C6 and C5-C7 root injuries should be similar. In 38 patients with upper-type palsies of the brachial plexus, we examined for correlations between clinical findings and root injury level, as documented by CT tomomyeloscan. Contrary to commonly held beliefs, C5-C7 root injuries were n

Tumescent technique in digits: a subcutaneous single-injection digital block.

BACKGROUND: A modified subcutaneous single-injection approach to achieve digital block using a tumescent technique is described. METHOD: A convenient sample of patients requiring digital anesthesia for minor surgical procedures on the fingers or thumb in the emergency and plastic departments were enrolled into the study. Digital nerve block was performed by injecting 1% lidocaine into the volar subcutaneous space at the proximal digit to create a firm, turgid feel to the tissue, the so-called tumescent state. The volume of anesthetic was based on the size of the digit. All nerve blocks were p
