Radiographic changes consisting of alterations in mineral content, osteopaenia or destructive neuropathy that occur following successful finger replantation have already been described. We report our experience about four fingers in three individuals in whom bone changes developed in the first three months postoperatively with complete "restitution ad integrum". Three patients, 21-49 years old (average 36 years) sustained a clean-cut amputation of four fingers. The first patient had an amputation at the base of the middle phalanx of the index finger and the second patient at the base of the pr

PURPOSE: We report on a modified kite flap for the reconstruction of thumb pulp defects. We performed nerve repair to improve thumb pulp sensation.
METHODS: From May 2005 to December 2008, 42 thumbs in 42 patients were treated. The average size of the thumb pulp defects was 2.1 × 2.6 cm (range, 1.6 × 1.8 cm to 2.8 × 3.1 cm). The mean flap size was 2.5 × 2.9 cm (range, 1.8 × 2.2 cm to 3.2 × 3.5 cm). The radial branch of the second dorsal digital nerve was coapted to one of the proper digital nerves of the thumb. The required average length of the nerve branch was 1.2 cm (range, 0.7 to 1.6 cm).

PURPOSE: We report on the results of reconstruction of fingertip sensation by very distal nerve transfer in 8 patients with high median nerve lesions.
METHODS: Before surgery, patients underwent sensory testing of the hand using Semmes-Weinstein monofilaments. All patients had surgery within 1 year of trauma. For sensory reconstruction, branches of the radial nerve on the proximal phalanx of the index and thumb were sutured to the ulnar proper digital nerve of the thumb and radial proper digital nerve of the index finger. Patients were followed up for 12 months.
RESULTS: After median nerve l

BACKGROUND: The aim of this study was to conduct a systematic review of the English literature on replantation of distal digital amputations to provide the best evidence of survival rates and functional outcomes.
METHODS: A MEDLINE search using "digit," "finger," "thumb," and "replantation" as keywords and limited to humans and English-language articles identified 1297 studies. Studies were included in the review if they (1) present primary data, (2) report five or more single or multiple distal replantations, and (3) present survival rates. Additional data extracted from the studies meeting

In this study, median nerves (MNs) and ulnar nerves (UNs) were dissected in 200 palmar sides of hands (left and right) of 100 (50 male, 50 female) spontaneously aborted fetuses with no detectable malformations. The fetuses, whose gestational ages ranged from 13 to 40 weeks, were dissected under an operating microscope. The MN divided first into a lateral ramus and a medial ramus and then formed a common digital nerve. The first common digital nerve trifurcated in all of the studied cases. The branching patterns were classified into two types (Type 1 and Type 2) based on the relationship with t

Irreducible open dislocation of the thumb interphalangeal joint is a rare injury with several factors responsible for blocking reduction. We describe a case of a 32-year-old man who, on surgical exploration, was found to have an irreducible thumb dislocation due to flexor pollicis longus tendon, ruptured volar plate, and the digital nerve interposed in the interphalangeal joint.

BACKGROUND: Digital anesthesia in the pediatric population has traditionally been accomplished using a ring block that requires multiple injections. A modified transthecal digital nerve block is a single-injection technique of the midproximal phalanx that has been shown to be technically simple and highly effective in adults.
OBJECTIVE: To describe the success rate of the modified transthecal digital nerve block in children.
METHODS: : A convenience sample of children requiring digital anesthesia for minor surgical procedures on the fingers or thumb at an urban tertiary-care pediatric emerge

PURPOSE: We report 2 modifications of the classic Brunelli flap to avoid vascular compromise and first web contracture, common when using medium or large flaps.
METHODS: We treated 15 patients with posttraumatic loss of palmar surface and distal amputation of the thumb, with dorsoulnar flap associated with 2 refinements. The first technical tip was an adipofascial extension proximal in the flap aimed to decrease the morbidity of the donor site without changing the potential covering of the flap. The second was always to graft the pedicle of the flap, avoid vascular compression problems, and m

INTRODUCTION: Surgical treatment of the injuried flexor tensons is the important part of hand surgery. Tendon adhesions, ruptures, joint contcatures-stifness are only one part of the problem one is faced during the tendon treatment. In spite of improvement in surgical technique and suture material, the end result of sutured flexor tendons still represent a serious problem.
THE AIM: To present of operative treatment of flexor pollicis longus injury with Krakow suture technique.
METHODS: All patients are treated in the first 48 hours after the accident. The regional anesthesia was performed wi

BACKGROUND: The functional outcome of a flexor tendon injury after repair depends on multiple factors. The postoperative management of tendon injuries has paved a sea through many mobilization protocols. The improved understanding of splinting techniques has promoted the understanding and implication of these mobilization protocols. We conducted a study to observe and record the results of early active mobilization of repaired flexor tendons in zones II-V.
MATERIALS AND METHODS: 25 cases with 75 digits involving 129 flexor tendons including 8 flexor pollicis longus (FPL) tendons in zones II-V

PURPOSE: To investigate the adequacy and safety of percutaneous trigger finger release on symptomatic patients.
METHODS: Two orthopedic non-hand surgeons performed percutaneous A1 pulley release on the thumb, index, middle, and ring fingers with the use of a 19-gauge needle in 25 fingers of 24 patients. Open inspection was then performed to determine the extent of release and any injury to the surrounding anatomic structures.
RESULTS: Triggering was eliminated in all fingers. Of the 25 A1 pulleys, 19 were partially released; only 6 were completely released. Noted injury included only superfi

Two rare cases of bowler's thumb (traumatic neuropathy of the ulnar digital nerve of the thumb) were treated surgically with encouraging results. These had different types of lesion, involving nodular neuroma and epineural mass, which were distinguished by magnetic resonance imaging. The surgical outcome for active bowlers is unclear and postoperative protection from the repetitive trauma of throwing is important. Possible technical solutions include changing the size and weight of the ball, the spacing between holes, the fitting of thumb to ball, and the slope and size of thumb holes. It is i

Reverse axial adipofascial flaps use subcutaneous tissue of the laterodigital and dorsal metacarpal or digital areas. They are sited on the arterial branches anastomosing the volar and dorsal arterial networks of the fingers. These flaps allow coverage of wide and distal defects. Metacarpal flaps were used for defects on the proximal phalanx, and digital flaps for the defects over the proximal interphalangeal joint (PIPj) and further distally of the thumb and fingers. A series of 18 cases is reviewed. Dissection of the flap is easy, fast and preserves the collateral nerve and artery of the fin
