Evaluation of moving and static two point discriminations of volar forearm skin before and after transfer as a sensate radial forearm island flap in reconstruction of degloving injury of the thumb.

In degloving injury of the thumb the large skin defect needs cover with sensate, glabrous and pliable skin. Although coverage of this defect with a sensate free flap from the foot is the best choice, most commonly, cover is achieved using a non-sensate distant pedicle flap. Between 2001 and 2003, degloving injuries of the thumb in eight patients were reconstructed using a sensate radial forearm flap in the sensory territory of the lateral ante-brachial nerve of the forearm which was repaired to the digital nerve of the thumb (six cases) or to a branch of the sensory radial nerve (two cases). [References to avoid complications in releases of the trigger thumb: a cadaveric study].

OBJECTIVES: The aim of this cadaveric study was to demonstrate the utility of some visible landmarks in the hand in avoiding probable complications during percutaneous or open releases performed for the trigger thumb associated with flexor tendon tenosynovitis. METHODS: In this cadaveric study, we dissected 20 thumbs of 10 fresh cadavers (10 males; mean age 46 years; range 31 to 62 years) from the interphalangeal to the metacarpophalangeal creases to expose all digital arteries, nerves, the flexor tendon, and the A1 pulley. The following distances were measured: from the proximal edge of the

Innervated lateral middle phalangeal finger flap for a large pulp defect by bilateral neurorrhaphy.

BACKGROUND: The dorsal middle phalangeal finger flap is a reliable flap. However, it requires skin grafting on the whole surface of the dorsum of the middle phalanx of the donor finger for an exposed extensor tendon and leaves a conspicuous contour deformity at the donor site. In an effort to overcome this problem, the authors have been using a heterodigital innervated lateral middle phalangeal finger flap for resurfacing sizable defects of the pulp. METHODS: Flaps were designed on the lateral ulnar side of the middle finger for five thumb defects and the ring finger for two little finger def

Percutaneous release of the trigger thumb: is it safe, cheap and effective?

Percutaneous release of the trigger finger and trigger thumb has recently gained popularity. The aim of this study was to determine the clinical results and safety of percutaneous release in trigger thumbs. Twenty-five thumbs of 21 patients were released percutaneously in the polyclinic under local anaesthesia. Steroid injection was performed following the release procedure using the same needle. The thumb function was evaluated by a patient questionnaire, and functional thumb scores (VAS) were calculated in the preoperative and postoperative periods. At the 1 week follow-up, four patients had

Trigger digits: principles, management, and complications.

Stenosing tenosynovitis, or trigger finger, is an entity seen commonly by hand surgeons. This problem generally is caused by a size mismatch between the flexor tendon and the first annular (A-1) pulley. Conservative management includes splinting, corticosteroid injection, and other adjuvant modalities. Surgical treatment consists of release of the A-1 pulley by open or percutaneous techniques. Complications are rare but include bowstringing, digital nerve injury, and continued triggering. Some patients require more extensive procedures to reduce the size of the flexor tendon. Comorbid conditio

Pathologic conditions of the hypothenar eminence: evaluation with multidetector CT and MR imaging.

Pain, weakness, and sensory loss occur frequently in the hypothenar eminence. However, clinical examination is difficult and nonspecific, and the prescribed imaging technique may be inadequate, or images may be misinterpreted. Different imaging modalities have various degrees of usefulness for the diagnosis of painful pathologic conditions of the hypothenar eminence. Radiography, multidetector computed tomography (CT), multidetector CT arthrography, and magnetic resonance (MR) imaging of the wrist are useful for surveying the anatomy of the hypothenar eminence, the Guyon canal, and the ulnar n

[Applied anatomy of the second dorsal metacarpal artery island flap with double pivot points].

OBJECTIVE: To provide the anatomic bases for clinical application of the second dorsal metacarpal artery (SDMA) island flap with double pivot points. METHODS: The origin, branches and distribution of the recurrent cutaneous branch of the SDMA were observed in 30 adult fresh cadaver specimens, which were illustrated with special dye. Eighteen cases of skin defects of the thumb were repaired with the SDMA island flap. The defect locations were the dorsal part in 11 cases and palmar part in 7 cases, including 3 cases of defect in association with long pollical extensor defect and 2 cases of defe

Painful foot neuromas after toe-to-thumb transfer.

Reconstruction of the thumb by transfer of a toe has evolved technically to the point that this complex procedure can result in a mobile, sensate, and aesthetically pleasing digit that contributes to an almost-normally functioning hand. Donor site deformity is well recognized, primarily as it relates to the appearance of the foot after transfer of the hallux to the thumb position and stiffness of the remaining portions of the big toe. The present report describes donor site disability related to painful neuromas of the superficial and deep peroneal nerves and the common plantar digital nerve t

[Clinical experiences and dosage pattern in subcutaneous single-injection digital block technique].

100 patients with injuries to their fingers were treated using the subcutaneous digital block as described by Low et al.. Different dosages ranging from 2 to 3 millilitres of a local anesthetic were required to obtain appropriate anesthesia according to the location of injury. 108 finger injuries were treated, 18 thumb injuries, 90 finger injuries. The anesthetic was administered using a 0.55 x 25 mm needle and injected strictly subcutaneously into the flexor crease at the base of the finger or thumb. Injuries to the palmar aspect of the fingers were anesthetized using 2 ml of a local anesthet

Regeneration of peripheral nerve gaps with a polyglycolic acid-collagen tube.

OBJECTIVE: The aim of this study was to report by means of objective methods on the effectiveness of a nerve reconstruction procedure using a bioresorbable tube in two patients. Our previous successes in regenerating canine peripheral nerves across long distances (80-mm gaps) using a bioabsorbable tube have led us to investigate the value of such a tube for the treatment of human patients with chronic nerve injuries. METHODS: The device was made from a cylindrically woven polyglycolic acid tube filled with a collagen sponge. It was designed to be resorbed after nerve regeneration. Peripheral

Microsurgical replantation of a small segment of thumb volar skin.

This report presents a case of microsurgical replantation of a volar skin segment of the thumb. In a 24-year-old patient, a heavy object falling over the dominant thumb resulted in a crush-avulsion injury of a pure skin segment measuring 4 x 2 cm. Examination revealed that the distal fingertip as well as the bone-tendon structures remained intact. Exploration demonstrated that both neurovascular bundles were included in the avulsed skin segment. Microsurgical replantation was achieved successfully, repairing the radial digital artery at both ends with vein grafts as well as anastomosing a palm

Results of 1245 endoscopic carpal tunnel decompressions.

Between 1994 and 2000, the senior author performed 1245 endoscopic carpal tunnel decompressions. There were 291 bilateral cases. A telephone review was conducted of those patients who had not had an early excellent result, or conversion to an open procedure. Ninety-four per cent follow-up of patients in this group was achieved; 88% had an early excellent result with no subsequent problems. In 33 hands (3%) the carpal tunnel was opened, because of abnormal anatomy or poor view. A further 5% had a late excellent result. Seventeen hands (1%) have required subsequent open decompression, with adher

Trigger thumb: results of a prospective randomised study of percutaneous release with steroid injection versus steroid injection alone.

One hundred and twenty-seven trigger thumbs in 115 adult patients were randomised to either percutaneous release with steroid injection (n=66) or steroid injection alone (n=61). Two patients, one from each group, were lost to follow-up. Percutaneous release with steroid injection produced satisfactory results in 91% of cases whereas steroid injection alone produced satisfactory results in 47% of cases. One case in the percutaneous group developed stiffness and one in the injection group developed cellulitis. No digital nerve injury occurred in either group. We conclude that percutaneous trigge

[Free microvascular transplantation of parts of the great toe for thumb reconstruction].

PURPOSE/BACKGROUND: A reconstructed thumb has to fulfil the function of opposition, pinch-grip, stability, sufficient length, sufficient sensibility, and painlessness. Next to pollicisation, these requirements can only be met satisfactory by the microvascular free great toe-to-hand transplantation. The goal of a thumb reconstruction is a compromise between an optimal functional and aesthetic outcome and a minimal morbidity of the donor site. METHOD AND CLINICAL MATERIAL: Depending on the extent of the traumatic thumb defect, not only the fibular part of the ipsilateral great toe but also its

[The first dorsal metacarpal artery neurovascular osteocutaneous flap--a new method of distal thumb reconstruction].

Basing on the earlier experience with neurovascular flap from the dorsal part of the index when reconstructing soft tissues of thumb and thenar, first of the authors elaborated a method of harvesting this flap together with a dorsal aponeurosis and bone fragment of the proximal phalanx. Blood flow comes from the periosteum of the proximal phalanx. Many small vessels penetrate into it from the dorsal soft tissues thus piercing central part of the dorsal aponeurosis in many points. The flap was used in 25 year old male who had secondary reconstruction of the mutilating thumb pulp when the nail a

Pitfalls in diagnosing carpal tunnel syndrome.

The aim of this paper is to point out the differential diagnosis be-tween a thumb hypoplasia type 1/11 and a carpal tunnel syndrome. Both syndromes have symptoms in common. Two children and an adult with hypoplastic thumbs were sent to our department with the diagnosis carpal tunnel syndrome. In both children, the diagnosis was changed after an exact history and examination, and in one child an opponensplasty was done. The adult patient had already been operated unsuccessfully for a carpal tunnel syndrome elsewhere and again at our department. The exact diagnosis was only made after the second

Transthecal digital block at the proximal phalanx.

This study assessed the efficacy of a modified transthecal digital block. Three-hundred-and-sixty consecutive digits were anaesthetised with this technique for the treatment of fractures, infections and foreign bodies. Complete palmar and dorsal anaesthesia was achieved in 357 of the 360 digits (99%), including 52 of 53 thumbs (98%). The technique was extremely easy to perform and no complications occurred.

Innervated dorsal adipofascial turnover flap for fingertip amputations.

Traumatic or thermal injury to the fingertip may result in composite tissue loss. Exposed tendon, bone, or joint surface is best treated by flap coverage. The authors present their experience with a new technique that provides coverage for fingertip defects using the innervated dorsal adipofascial turnover flap, which consists of adipofascial tissue only and relies on the arterial anastomotic network of this tissue layer to sustain its vascularization. Eight digital amputations between the distal phalanx proximal to the nail matrix and midportion of the middle phalanx were resurfaced successfu
