Open transtrapezium injuries of the thumb are not common and sufficiently described injuries. We have clinically and radiologically evaluated six patients with transtrapezium injury of the thumb. The average follow-up time was 65 months. All injuries occurred in young people, who had high-energy trauma to their thumb. Fracture of the trapezium with exposure of the first carpometacarpal joint was a constant along with soft tissue damage. The latter included thumb tendons, digital nerves and arteries, motor branch of the median nerve, palmar or dorsal branch of the radial artery, and thenar musc

Hypothenar hammer syndrome (HHS) is an uncommon form of secondary Raynaud phenomenon, occurring mainly in subjects who use the hypothenar part of the hand as a hammer; the hook of the hamate strikes the superficial palmar branch of the ulnar artery in the Guyon space, leading to occlusion and/or aneurysm of the ulnar artery. In patients with HHS, such injuries of the palmar ulnar artery may lead to severe vascular insufficiency in the hand with occlusion of digital artery. To date, only a few series have analyzed the long-term outcome of patients with HHS. This prompted us to conduct the curre

The characteristic pathological finding in carpal tunnel syndrome (CTS) is noninflammatory fibrosis of the synovium. How this fibrosis might affect tendon function, if at all, is unknown. The subsynovial connective tissue (SSCT) lies between the flexor tendons and the visceral synovium (VS) of the ulnar tenosynovial bursa. Fibrosis of the SSCT may well affect its gliding characteristics. To investigate this possibility, the relative motion of the flexor tendon and VS was observed during finger flexion in patients undergoing carpal tunnel surgery, and for comparison in hands without CTS, in an

OBJECTIVE: To study the skin temperature changes as a sign of altered blood flow regulation due to abnormal sympathetic nerve function in carpal tunnel syndrome (CTS) and the efficacy of carpal tunnel release (CTR) operation in the alleviation of those signs and other symptoms of CTS.
SUBJECTS AND METHODS: Forty-one healthy hands (n=41) of 22 volunteers and 22 hands (n=22) with clinically diagnosed carpal tunnel syndrome of 16 patients were examined. A series of infrared photos of the hands of each subject were taken and stored by using digital infrared thermography (DIRT) before and 6 months

OBJECTIVES: The aim of this cadaveric study was to demonstrate the utility of some visible landmarks in the hand in avoiding probable complications during percutaneous or open releases performed for the trigger thumb associated with flexor tendon tenosynovitis.
METHODS: In this cadaveric study, we dissected 20 thumbs of 10 fresh cadavers (10 males; mean age 46 years; range 31 to 62 years) from the interphalangeal to the metacarpophalangeal creases to expose all digital arteries, nerves, the flexor tendon, and the A1 pulley. The following distances were measured: from the proximal edge of the

Attempting to separate frozen food with a knife can result in serious injury to the hand. This injury is prevalent and is a common reason for referral to hand surgeons. Ten cases were reviewed over a 3-month period; the causative frozen food varied widely. Seven of the 10 patients required surgical exploration, and of these 7 patients, 3 were found to have significant injury. The injuries varied from digital nerve injuries to damaged flexor tendons. The other 4 patients were all found to have only soft tissue damage.

Stenosing tenosynovitis, or trigger finger, is an entity seen commonly by hand surgeons. This problem generally is caused by a size mismatch between the flexor tendon and the first annular (A-1) pulley. Conservative management includes splinting, corticosteroid injection, and other adjuvant modalities. Surgical treatment consists of release of the A-1 pulley by open or percutaneous techniques. Complications are rare but include bowstringing, digital nerve injury, and continued triggering. Some patients require more extensive procedures to reduce the size of the flexor tendon. Comorbid conditio

Dupuytren's contracture is a common hand problem. Its treatment, other than percutaneous fasciectomy, is an outpatient operation performed with the patient under regional or general anesthesia. If local anesthetics are used, they are used without epinephrine, and an arm or forearm tourniquet is essential. Multiple reviews have described the safety of local anesthetics with epinephrine in the digits. This study is a retrospective review of operations performed by the same surgeon in the hospital (43 digits) versus the office using local anesthetics with epinephrine and no tourniquet (60 digits)

OBJECTIVES: Segmental sensory nerve conduction velocity (SNCV) was measured from the wrists to the hands and digits of a population of vibration-exposed shipyard workers. This study was designed to investigate whether SNCV was selectively slowed in the fingers and whether a laboratory approach could be adapted for robust field use.
METHODS: Wrist-palm, palm-proximal digit, and digital segments were determined from stimulation at the wrist with recording electrodes placed distally and adjusted to individual anatomy. The cohort was selected on the basis of current use of vibratory tools.

A randomized, double-blind study was performed in 50 patients to compare the transthecal and traditional subcutaneous infiltration techniques of digital block anesthesia regarding the onset of time to achieve anesthesia and pain during the infiltration. All the patients had sustained injury involving two or four fingers of the hand. Each patient served as his or her own control, having one finger infiltrated with the transthecal technique and the other with the subcutaneous infiltration technique. Time to loss of pinprick sensation and pain (at the time of the infiltration and 24 hours postope

This report presents a case of microsurgical replantation of a volar skin segment of the thumb. In a 24-year-old patient, a heavy object falling over the dominant thumb resulted in a crush-avulsion injury of a pure skin segment measuring 4 x 2 cm. Examination revealed that the distal fingertip as well as the bone-tendon structures remained intact. Exploration demonstrated that both neurovascular bundles were included in the avulsed skin segment. Microsurgical replantation was achieved successfully, repairing the radial digital artery at both ends with vein grafts as well as anastomosing a palm

A patient suffering from recalcitrant carpal tunnel syndrome despite 4 surgical decompressions was treated successfully with revision carpal tunnel release, neurolysis, and autogenous saphenous vein wrapping of the median nerve. When he subsequently developed compression of the common digital nerve to the middle finger his carpal tunnel was re-explored and the previously applied vein graft underwent a biopsy examination. Histopathologic analysis of the biopsy specimen was remarkable for neovascularization of the vein graft and structural transformation of the vein endothelium, neither of which

We studied the vascular anatomy of dorsal digital skin flaps and the vascular anastomoses between the dorsal cutaneous branch of the palmar digital artery (PDA) and the dorsal digital branches (the terminal branches) of the dorsal metacarpal artery (DMA) at the level of the proximal phalanx. Eight fresh cadavers (16 hands) were injected with a lead oxide-gelatin mixture. Our radiographic assessment revealed that there were 2 sources of the arterial supply to the dorsum of the digit, the dorsal digital branches of the DMA, and the dorsal cutaneous branches of the PDA. These branches anastomose

In motor nerve conduction studies compound muscle action potentials (CMAPs) appear later than sensory nerve action potentials (SNAPs). This time lag originates from the conduction delay at the distal motor axon, neuromuscular transmission time and muscle action potential induction time. To investigate the latency difference between CMAPs and SNAPs we studied 46 healthy individuals, 46 patients with diabetes mellitus and 33 patients with carpal tunnel syndrome, using the lumbrical and interossei recording method. In this method the recording active electrode was placed on the 2nd lumbrical musc

Twelve patients with posttraumatic defects of the thumb were treated in the years 1990-1999 by the trimmed great toe method. Eight of them had additional injuries or loss of long fingers (two patients lost all fingers). Reconstruction methods were chosen basing on the amputation level. A fragment of the trimmed great toe of an appropriate length was used distally to the distal part of the first metacarpal according to the technique described by Wei (9 patients). In cases of a more proximal amputation only a distal part of the great toe was used. Pollicisation of other elements of the index wit

Repetitive trauma to a finger can result in compressive neuropathy of one or more digital nerves. The thumb is commonly affected in bowlers, hence the term "bowler's thumb". However, this condition can involve other fingers and non-bowlers. To our knowledge, none of the cases reported had undergone neurophysiologic testing. We describe the neurophysiologic findings in two patients who had a history consistent with digital neuropathy of the thumb.
