Revisitation of the vascular anatomy of the lumbrical and interosseous muscles.

Functional outcome after transmetacarpal replantations and revascularizations is discouragingly poor and often associated with a high incidence of intrinsic-related complications. In order to explore the hypothesis that intrinsic muscle ischemia may play a significant role, we revisited the vascular anatomy of the lumbrical and interosseous muscles. Six fresh-frozen cadaver hands were injected with latex-barium sulfate, and dissections were performed focusing on the contributions of the deep and superficial palmar arches and their branches to the intrinsic muscle vasculature. We found that the

Ulnar nerve lesion in the palm: report of an unusual case.

We report the case of a patient who had weakness of the fourth dorsal interosseous (DI) and third palmar interosseous (PI) muscles resulting in the abducted position of the little finger at rest. Electromyographic abnormalities were limited to only these muscles with partial denervation findings. The results of routine nerve conduction studies and imaging techniques were normal. Clinical and electrophysiological findings suggested that the lesion had occurred distal to the point where the very small motor branches to these muscles exit the deep palmar branch of the ulnar nerve.

[Free sensory or retrograde pedicled fasciocutaneous thenar flap: anatomic study and clinical application].

In 1993, Kamei et al. presented the "free thenar flap" based on the superficial palmar branch of the radial artery. In light of the microsurgical importance of this vessel, we decided to perform an anatomic study. 20 fresh upper extremity specimens were injected with 30 cc colored latex and examined. The superficial palmar branch of the radial artery was constantly found, originating on an average 23 mm proximal to the scaphoid tubercle with an average diameter of 0.8 mm. Normally, five cutaneous branches to the thenar skin and a branch of the antebrachial nerve, securing the sensibility of th

Endoscopic carpal tunnel release using a single-portal system.

In this prospective clinical study, we present our experience with a new single portal carpal tunnel release kit. The safety and efficacy of this new device was assessed and compared with a consecutive control group treated with conventional open carpal tunnel release. 126 patients were enrolled in this study, 64 of them were treated endoscopically (group 1) and 62 by open release of the carpal ligament (group 2). Follow-ups were conducted at 1, 3, 6, 12 and 24 weeks postoperatively. A serious intraoperative complication in group 1 was a transection injury of a branch of the superficial palmar

Hypothenar hammer syndrome: an uncommon cause of digital ischemia.

Unilateral digital ischemia may occur as a result of repetitive use of the hypothenar eminence as a hammer, with resultant damage to the wall of the ulnar artery and the superficial palmar arch. Secondary thrombus formation from intimal damage and potential embolization may cause signs and symptoms of arterial insufficiency in the second through fifth digits. Three automobile mechanics with unilateral ischemia caused by hypothenar hammer syndrome are described. All three patients had improvement with conservative treatment and continued to work with only minimal disability.

[Technique, results and risks of endoscopic carpal tunnel release].

Endoscopic carpal tunnel release was performed in 168 cases of carpal tunnel syndrome using the two-portal technique described by Chow. There were 164 cases of idiopathic carpal tunnel syndrome and four cases following a wrist trauma. Sensory symptoms improved within few days in 113 patients (67%) and in 141 cases within eight weeks (84%). Nighttime pain relief was achieved immediately in 87% of the patients and in 94% within eight weeks. Grip strength increased to preoperative values within eight weeks and pinch strength reached preoperative levels within six weeks. In four cases an iatrogeni

[Anatomic observation of the dorsal metacarpal arteries].

Anastomosis between the first to the fourth dorsal metacarpal arteries was observed in 43 hands of adult corpses. It was found that 86% of the first dorsal metacarpal artery arises from the radial artery, 14% is replaced by the main artery of thumb or superficial branches of the radial arteries; eighty-six percent of the second dorsal metacarpal artery originates from the dorsal carpal artery. Sixty-seven percent of the third and the fourth dorsal metacarpal arteries is formed by anastomosis of the dorsal carpal artery and its deep palmar arch. The first to the fourth dorsal metacarpal arterie

Free vascularized deep peroneal nerve grafts.

An ideal donor site for vascularized nerve grafts should have a constant anatomy, minimal functional loss after the nerve has been sacrificed, and a dependable blood supply parallel to the nerve over a relatively long distance. Creating a pedicle for a free vascularized deep peroneal nerve graft with the anterior tibial vessels seems to be a most suitable method for repairing long nerve gaps of over 20 cm and digital nerve defects with severe finger damage. Applications of this nerve graft to digital nerve losses with severely scarred beds created by avulsion injury, and two-stage reconstructi

Clinical use of microvascular PTFE grafts.

This paper reports on the first clinical case where a polytetrafluoroethylene (PTFE) graft of less than 3 mm in diameter was implanted in the arterial system of a patient to bridge a vascular defect. A 1.5 cm long, 1.5 mm diameter prosthesis was interposed in the superficial palmar arch of a man who sustained a laceration of the palm of his dominant hand. The graft was implanted by means of the 3M Precise Microvascular Anastomotic System on one end, and by conventional microsurgical technique on the other end. There were no postoperative complications. The patient resumed his pre-injury activi

Carpal tunnel release using limited direct vision.

A novel method to release the carpal tunnel is presented. It combines the advantages of endoscopic and open techniques while utilizing standard instruments. A lighted Killian nasal speculum is introduced between the palmar fascia and the transverse carpal ligament. The ligament is incised under direct vision. The procedure was performed in 28 cadaver hands and followed by inspection utilizing the open method through an interthenar incision. The transverse carpal ligament was completely divided in all hands without nerve or tendon injury. The superficial palmar arch was injured in one hand (3.6

The use of topographical landmarks to improve the outcome of Agee endoscopic carpal tunnel release.

A modified approach to endoscopic carpal tunnel release has been developed and tested in 60 cadaveric specimens by three surgeons using the Agee endoscopic carpal tunnel release system. The modified approach, which includes specific localization of the hook of the hamate, flexor retinaculum, and the superficial palmar arch utilizing topographical landmarks, avoids entry into Guyon's canal and injury to the ulnar artery and nerve, median nerve, and common digital nerves. Use of the anatomic approach resulted in significantly superior results. There were fewer incomplete releases, and fewer surg

Risk and complications in endoscopic carpal tunnel release.

Single portal endoscopic carpal tunnel release was carried out in 107 hands of 88 patients. There were 11 complications. These included incomplete release (2), post operative scarring around the median and ulnar nerves (2), laceration of the superficial palmar arterial arch (1), reflex sympathetic dystrophy (2), palmar fasciitis (1), and wound inflammation (3). In two cases there was no relief of symptoms. In one there was incorrect diagnosis and in another, incorrect indication for endoscopic carpal tunnel release. The follow-up was from 3 to 18 months with an average of 6.8 months. The overa

A new technique for endoscopic carpal tunnel decompression.

A new one-portal technique for endoscopic carpal tunnel release (ECTR) is introduced with its clinical results. The incision is made at the palmar aspect of the hand. A custom-made glass tube with a groove is inserted, and under endoscope observation, a meniscus knife is pushed forward along the groove to release the flexor retinaculum. This new technique has been studied in ten fresh cadaver hands and used in 123 patients' hands. Results of the cadaver study showed that the flexor retinaculum was released completely in all ten hands. No injuries to tendons, nerves, or arteries were noted. In

Palmar uniportal extrabursal endoscopic carpal tunnel release.

A new technique of endoscopic carpal tunnel release using a 1.5-cm longitudinal palmar incision was used in 280 cases. The incision allows identification of the superficial palmar arch as well as the median nerve and its branches. A new knife/sleeve device that attaches to a standard 4-mm endoscope was created to simplify the procedure. The flexor retinaculum is endoscopically divided proximally into the distal forearm; the "interthenar fascia" (fascia superficial to transverse carpal ligament) can be preserved. Early postoperative results include a mean overall return to work and full activit

[Endoscopic decompression of the median nerve in the carpal tunnel. Apropos of 1,400 cases].

We report the results of endoscopic carpal tunnel release in 1,400 patients over a two year period (minimum 6-month follow-up). All patients were operated on using a technique derived from that of Chow. Complications were limited to two partial lesions of the superficial palmar arch and one interdigital nerve lesion. The technique is described. Analysis of the results indicates the advantages of this method. Though immediate post-operative comfort was greater and restoration of the grip quicker, the results were roughly similar to those of the classic method in the 6 months following surgery.

Major neurovascular complications of endoscopic carpal tunnel release.

Endoscopic carpal tunnel release is becoming an increasingly popular method of surgically correcting median nerve compression. Several complications have been suggested as possibilities following the technique; however, to date, there have been only isolated reports of iatrogenic injury to major neurovascular structures in the hand. We report both a case of transection of the median nerve and a pseudoaneurysm of the superficial palmar arch following endoscopic carpal tunnel release.

[An anatomical study of the carpal tunnel for endoscopic carpal tunnel release].

An anatomical study of the carpal tunnel was performed to characterize the transverse carpal ligament, enabling improved endoscopic carpal tunnel release. One hundred wrist joints obtained from 50 cadavers were studied. The width of the transverse carpal ligament (TCL) was found to be narrower in females than in males, and there was no significant difference in the width of the ligament between the right and the left wrist joints. The length of the TCL was not significantly different between males and females or between the right and left wrist joints. On the sagittal plane, the TCL was spindl
