Reported complications of endoscopic carpal tunnel release have increased as more surgeons use this technique to release the flexor retinaculum. We used a cadaver model to compare the results of endoscopic carpal tunnel release through a one-portal distal (Group A, 15 specimens) and a one-portal proximal (Group B, 15 specimens) entry site with a new endoscopic technique. Our custom-made glass tube of three different sizes (5, 7, and 9 mm in diameter) is designed to house an endoscope and accommodate a meniscus knife for releasing the flexor retinaculum. Complete release of the flexor retinacul

The anatomical characteristics of the flexor carpi radialis tendon, the tendon sheath, the tunnel boundaries, and the adjacent structures were examined in twenty-five adult cadaveric specimens. Transverse and sagittal sections of the wrist and forearm, tenograms, and histological sections of the tendon were made from two additional specimens each. The musculotendinous portion of the flexor carpi radialis tendon begins an average of fifteen centimeters (range, twelve to seventeen centimeters) proximal to the radiocarpal joint; the muscular fibers end an average of eight centimeters (range, six

Endoscopic carpal tunnel release has been shown to decrease postoperative recovery time in patients undergoing surgery for carpal tunnel syndrome. There is, however, some controversy concerning the safety and reliability of the procedure. We performed endoscopic carpal tunnel release utilizing the two portal technique described by Chow on 20 fresh cadaver specimens. There were no injuries noted in any of the vital structures including the ulnar artery, superficial palmar arch, third common digital nerve or flexor tendons. The transverse carpal ligament was found to be completely transected in

Endoscopic carpal tunnel release was performed on 13 fresh-frozen cadavers using the Agee 3-M "Inside Job" Endoscopic Carpal Tunnel Release System, by a surgeon specifically trained in the technique and with a large clinical experience. Independent dissections showed no inadvertent lacerations to nerves, vessels, or tendons. The superficial palmar arch, the communicating branch between the ulnar and median nerves at the palm, and the common digital nerves to the third and fourth web spaces were the structures most at risk. Measurements of the distances between external landmarks were not helpf

To define the role of two-portal endoscopic carpal-tunnel release as a method for the treatment of compression of the median nerve at the wrist, a prospective, randomized, multicenter study was performed on 169 hands in 145 patients. Either open or endoscopic carpal-tunnel release was performed in all of the patients who had clinical signs and symptoms consistent with carpal tunnel syndrome, had not responded to or had refused non-operative management, and had had electrodiagnostic studies consistent with carpal tunnel syndrome. Follow-up evaluations were performed at twenty-one, forty-two, an

Anatomic relationships of an endoscopic carpal tunnel device to surrounding soft tissue structures along the ring finger and the long-ring interspace axis were investigated in 28 adult cadaver hands. The average distance from the center of the device to the median nerve in the carpal tunnel averaged 3.3 mm in the ring finger axis and 2.5 mm in the long-ring interspace axis. The average distance from the distal edge of the transverse carpal ligament to the superficial palmar arch was 4.8 mm in the ring finger axis and 5.5 mm in the long-ring interspace axis. These and other more subtle anatomic

A reversed radial forearm fascial flap has been used as a vascularized tube through which tendon grafts may glide and in which nerve grafts may rest. It may have applications in the treatment of the severely damaged flexor system at wrist level. A further advantage of this technique is that it does not [corrected] sacrifice the radial artery in an already injured hand.

The ulnar palmar cutaneous nerve (UPCN) is potentially useful to the electrodiagnostician. However, no definitive nerve conduction study techniques for the UPCN have been reported. The UPCN supplies sensory innervation to the hypothenar palm. This study describes an orthodromic sensory conduction technique for the hypothenar palm. Data were collected from 20 normal limbed patients (ages 22-58). Potentials were recorded over the ulnar nerve 10cm proximally and at the elbow. Distal latency was 2.19 +/- 0.17msec, and distal amplitude was 12.7 +/- 6.9uv. Additionally, a modified collision techniqu

To determine the relationship of neurovascular structures to the sites of portal placement and transverse carpal ligament division during two-portal endoscopic carpal tunnel release, a study of 20 fresh cadaver specimens was carried out. Open dissection of the carpal tunnel after endoscopic surgery showed complete ligamentous release in 18 hands (90%). In 10 specimens, the procedure was performed as described by Chow. There was one partial transection of the superficial palmar arch (5%), and five specimens (50%) had complete divisions of the superficial palmar fascia with considerable pressure

After traumatic loss of the index finger the second metacarpal bone is often used as a free graft to reconstruct bony defects of the thumb. Since clinical experience has shown numerous advantages of using a blood supplied bone graft, an anatomical study was performed in 104 cadaveric hands to investigate the vascularization supply of this bone. After dye injections into the arterial system the vascular situation of the second metacarpal was studied and a classification was made. Six variations in arterial vascularization were found. The nutrient vessels to the bone originate from the radial ar

Ischaemic injury to the hand after arterial cannulation is a rare but well documented complication and routine testing of the adequacy of collateral circulation is widely advocated. The widespread availability of the pulse oximeter in the operating theatre. its applicability in circumstances where the patient is unable to cooperate, and its dependence on pulsatile blood flow suggest that this device could potentially be usefully applied to the assessment of collateral blood flow. The reliability of the pulse oximeter to detect the presence or absence of collateral circulation was prospectively

Eighteen free second toe were used for pollicization other than the conventional method. From the first and second plantar metatarsal arteries junction of the both digital arteries of the second toe were dissected retrogradely and anastomosed to the deep of superficial palmar arch or terminal branch of the radial artery. The venous return was established between the dorsal venous arch and cephalic vein. These vessels were found to have much bigger diameters and less anatomical variation than those used by conventional methods. Seventeen transplantation were considered successful both functiona

The arterial pattern in 13 cases of radial deficiency and six cases of ulnar deficiency was studied by arteriography. The radial artery was absent or hypoplastic in 84.6% of cases of radial deficiency and in 50% of cases of ulnar deficiency. A persistent median artery was seen in 76.9% of cases of radial deficiency and in 16.7% of cases of ulnar deficiency. In 12 hands, the deep palmar arch was small or absent, depending upon the degree of radial arterial dysplasia. The digital artery to the deficient digits was missing in some cases. The persistence of an embryonic vascular pattern in cases o

Digital periarterial sympathectomy was performed on 11 digits in three patients with chronic digital ischaemia which was a manifestation of either Raynaud's disease, C.R.E.S.T. syndrome or traumatic ulnar artery thrombosis. Before operation, all patients had pain in the affected fingers and five digits had ulcers, two of which were infected. Using the operating microscope, the adventitia was stripped circumferentially over the distal 2 cm. of the common digital arteries, the bifurcation and the proximal 1 cm. of the proper digital arteries distal to the bifurcation. The same procedure was repe

This article describes a subcutaneous endoscopic operative procedure for carpal tunnel syndrome and analyzes its effectiveness using electrophysiological data. Subcutaneous transverse carpal ligament release under universal subcutaneous endoscope (USE) was performed using local anesthesia without pneumotourniquet in 54 hands of 45 patients since June 1986. The mean follow-up period was 13.8 months. Sensory disturbances began to subside immediately after the operation and disappeared within 2 months in all cases. After the disappearance of sensory disturbances, we performed postoperative electr

The article deals with the results of the analysis of morphological changes in the perineural tissue of 54 hands (in 47 patients) which were treated by operation for compression of the ulnar nerve on the hand. The main variants of localization of compression of the ulnar nerve in Guyon's canal were revealed. The main trunk of the ulnar nerve is compressed at the level of the volar carpal ligament in most cases. Two or three levels of affection of the main trunk of the nerve or its branches are encountered in half of the cases. The causes of compression of the ulnar nerve on the hand are thicke

Fractures of the hook of the hamate frequently fail to unite. In symptomatic nonunion, surgical excision of the hook has provided relief of pain in the few reported cases. In a survey of surgeons, 133 cases of excision of the hook to treat nonunion were reviewed retrospectively. The complication rate associated with excision is three percent. These complications, such as injury to the ulnar nerve or the superficial palmar arch and median nerve paresthesias secondary to retraction are due more to the surgical exposure than the actual excision.

The use of an interpositional vein graft to restore inflow to the digits by recreating the superficial palmar arch is presented. This technique is best reserved for severe, devascularizing injuries to the hand, significant damage to the palmar vessels, and when they may a paucity of donor vein available.

The mini condylar plate fixation system is a new method for stable internal metaphyseal bone fixation about the MP and PIP joint areas of the hand. Indications for its use are rigid stabilization of fractures, osteotomies, and bony components of composite tissue transfers. The device should be used in deference to other techniques when early active range of motion is pursued. Its best use is in the face of combined injuries, or osteotomies with tenolysis/capsulotomy. The advantages of this implant's design permit stabilization of small epiphyseal fragments with minimal interference of joint fu

A case is reported of an unusual double injury, in which a lower limb salvage procedure employed a free radial forearm flap from an amputated upper limb. The use of amputated parts for salvage procedures is well documented but none has previously involved a free forearm flap. This clinical case shows that the entire forearm and hand skin can be adequately perfused on the radial artery along with the superficial palmar arch.
