The perfused, pulseless supracondylar humeral fracture: intermediate-term follow-up of vascular status and function.

BACKGROUND: This study provides intermediate-term follow-up data on the vascular status of a cohort of children with a perfused, pulseless supracondylar humeral fracture who were managed with closed reduction, percutaneous pinning, and observation. METHODS: Pediatric patients who sustained a perfused, pulseless supracondylar humeral fracture in the period from 2007 to 2011 and who had at least six months of clinical follow-up were identified and returned for evaluation. The primary outcome was vascular status as indicated by palpation of the radial pulse, the wrist brachial index, and arteria

Variation in the hypothenar muscles and its impact on ulnar tunnel syndrome.

Compression of the ulnar nerve at Guyon's canal can be caused not only by tumor-like structures, a fibrotic arch, a ganglion, lipoma, aneurysm or thrombosis but also by anomalous hypothenar muscles which are reviewed here. For the search of relevant papers, PubMed and crucial anatomical textbooks were consulted. The abductor digiti minimi is the most variable hypothenar muscle. It can possess one to three muscle bellies. Additional heads can arise from the flexor retinaculum, the palmaris longus tendon, the pronator quadratus tendon or the deep fascia of the palmar side of the forearm. Our own

Anatomical study and clinical relevance of the flexor superficialis synovial flap: an assessment of 31 hand dissections.

PURPOSE: The authors present an anatomical study of the flexor digitorum superficialis synovial flap and its clinical application for palmar soft tissue defects in hand trauma. METHODS: Green latex was injected into the brachial artery of thirty-one human upper limbs from corpses donated to science. Anatomical features of this pedicled flap were assessed: arterial vascularization, dimensions and covering surface. RESULTS: The anatomical support of this flap is the synovial tissue of the flexor tendons, and particularly the superficial layer of the synovial system, covering the flexor digitor

Arcus venosus dorsalis pedis graft to restore the superficial palmar arch in devascularized hands.

For complex amputation injuries and anatomical characteristic of the metacarpus, restoration to the superficial palmar arch in severe devascularized or amputated hands remains a daunting challenge. In this article, we treated a series of five cases using an arcus venosus dorsalis pedis (AVDP) graft to restore the superficial palmar arches in devascularized hands. All hands survived and satisfying function was gained. The AVDP graft may be a suitable option to restore the superficial palmar arch in devascularized hands.

Pseudoaneurysm in the hand of a three-year-old boy: a case report.

BACKGROUND: Post-traumatic, true aneurysm of the hand is a well-documented phenomenon. Hypothenar hammer syndrome is often a consequence of repeated blunt trauma to the palm, but a false aneurysm, or pseudoaneurysm, of the palm is much rarer. A pseudoaneurysm is often the result of focal penetrating trauma. OBJECTIVES: The goal of this article is to describe a rare case of a child with a pseudoaneurysm of the superficial palmar arch and his subsequent surgical treatment. CASE REPORT: A 3-year-old boy sustained an accidental laceration to his palm while picking up a knife from a counter. The

Superficial palmar arch aneurysm after carpal tunnel decompression, a rare complication: a case report.

False aneurysms of the palmar arteries are rare. They are usually associated with traumatic injuries to the hand vasculature. We present a case of superficial palmar arch aneurysm (SPAA), complicating carpal tunnel decompression which presented as a pulsatile mass at the site of previous surgery. Initial diagnosis was made on clinical examination and confirmed on doppler ultrasound (US) and computed tomographic angiography (CTA). The feeding vessel of the aneurysm was subsequently occluded using coil embolization.

Reconstruction of a fingertip with a thenar perforator island flap.

We raised thenar island flaps that were supplied by perforators that originated in the superficial palmar arch or the superficial palmar branch of the radial artery for the reconstruction of fingertip defects in eight patients. The flap was so well-vascularised that a large flap with increased mobility could be raised. The donor site was covered with well-vascularised thick tissue, and skin grafting of the donor site was avoided in all cases. No patient developed a complication, and all flaps survived. Functional and cosmetic results of both fingertips and donor sites were excellent. A perfora

Use of superficial palmar arch for bridging the gap in digital revascularisation.

Traumatic hand injuries resulting in segmental loss of common digital artery are not uncommon. To bridge the gap and repair of transected common digital artery, the superficial palmar arch could be divided proximally on radial side and turned distally to the common digital artery for revascularisation. Revascularisation of the transversely crushed common digital artery can be performed based solely on superficial palmar arch for revascularisation of the segmental loss of common digital artery. However, because of known variability in the palmar arch, intraoperative verification is needed to en

Persistent median artery: cadaveric study and review of the literature.

The persistent median artery (PMA) may compress the median nerve (MN) and may be a significant supply of blood to the hand. Two cases of unilateral PMA (4%) were detected during the dissection of 50 upper limbs. The first case was a 75-year-old, right-handed male who suffered from chronic pain in both upper limbs, especially the left side. A dissection of his left upper limb revealed a PMA piercing both the MN and the medial branch of the anterior interosseous nerve. This artery coursed distally, deep to the transverse carpal ligament (TCL), forming a median-ulnar pattern for the superficial p

Hypothenar hammer syndrome: a case and brief review.

Hypothenar hammer syndrome is an uncommon cause of upper-extremity ischemia that is often overlooked in the absence of a thorough occupational and recreational history. Importantly, it is a reversible cause of hand ischemia that, if missed, can lead to significant morbidity and even amputation. The occupational ramifications and quality of life of those affected can be significant. Its relative rarity, set against the ubiquitous use of the hand as a 'hammer' is noteworthy. Several other causes of hand ischemia can present similarly; therefore, consideration of other etiologies must be thorough

Hypothenar hammer syndrome: distal ulnar artery reconstruction with autologous descending branch of the lateral circumflex femoral artery.

Repetitive trauma to the hypothenar eminence can cause the ulnar artery to become aneurysmal, thrombose, or send emboli to the digital arteries. The symptoms and signs are also known as the hypothenar hammer syndrome. We postulate that an arterial autograft is a superior conduit to the traditional vein graft. In this article, we report 3 cases (2 in 1 patient) of reconstruction of the distal ulnar artery with the descending branch of the lateral circumflex femoral artery (LCFA). Two patients, ages 45 to 50 years, had severe ischemic complaints of the ulnar fingers. Arteriograms confirmed occlu

Hand ischemia associated with elbow trauma in children.

OBJECTIVES: Previous observational studies suggest that children with hand ischemia following elbow trauma can be safely observed if Doppler signals are present in the wrist arteries (pink pulseless hand, PPH). Nonoperative management of PPH is predicated on the assumption that PPH results from local arterial spasm, but the mechanism of arterial compromise has not been investigated. We hypothesized that PPH signifies a brachial artery injury that requires surgical repair. METHODS: Retrospective review of operations performed on children with hand ischemia following elbow trauma at a level I t

Thumb reconstruction using the radial midpalmar (perforator-based) island flap (distal thenar perforator-based island flap).

BACKGROUND: Although the reconstruction of palmar defects of the thumb with exposed vital structures is difficult because of a lack of locally available tissue, several local flaps are available for the reconstruction of these defects. The majority of these flaps are raised from the dorsal or dorsolateral aspect of the thumb and index finger or from the forearm. METHODS: Between 1998 and 2008, 23 patients underwent reconstruction of palmar defects of the thumb at Chonnam National University Medical School, using the radial midpalmar (perforator-based) island flap (distal thenar perforator-bas

[Pseudoaneurysm of the superficial palmar arch, following endoscopic carpal tunnel release. A case report].

Pseudo-aneurysm of the superficial palmar arch following carpal tunnel release is exceptional and thus poorly reported. We present one observation, recorded on more than 1300 hands treated by endoscopic release using the "Chow" technique by a single surgeon. Ultra-sonography could be used in an attempt to locate the superficial palmar arch and thus prevent vascular damage at surgery.

Bridging the gap in hand replantation: use of the common digital artery for completion of the superficial palmar arch.

BACKGROUND: Traumatic hand amputations along a longitudinal axis are possible but, to the authors' knowledge, have not been described in the literature. To bridge the gap and repair the transected superficial palmar arch at its apex, a common digital artery can be sacrificed and turned toward the plane of the arch. An anatomical study was performed to investigate the possibility of using the common digital artery to the third web space for completion of the superficial palmar arch. METHODS: Fifteen fresh cadaveric hands were dissected to evaluate the caliber and origin of the common digital a

Anatomy of the palmar branch of the ulnar nerve: application to ulnar and median nerve decompressive surgery.

OBJECT: The palmar cutaneous branch of the ulnar nerve (PCUN) has received little attention in the literature, and to the authors' knowledge, has received no attention in the neurosurgical literature. The present study was performed to help the surgeon minimize postoperative complications of nerve decompression at the wrist. METHODS: Forty cadaveric upper limbs underwent dissection of the ulnar nerve in the forearm, at the wrist, and in the palm. The PCUN was investigated and when identified, measurements were made and relationships documented between this cutaneous branch and the ulnar arter

An anterior-lateral thigh perforator flap on a recipient brachial-radial vein graft for complex wound reconstruction: a case report.

A case of challenging microsurgical reconstruction of a difficult defect in a radiated upper limb is reported. A difficult wound, with tendon and bone exposition, developed on the dorsum of the forearm in a 76-year-old patient; she had been radiated since almost 50 years and her left hand had also been revascularized twice with venous grafts between the humeral artery and the superficial palmar arch. After failure of a local flap, an anterior-lateral thigh perforator flap was successfully transferred with end-to-side anastomoses on the arterialized venous graft. Up to date follow-up shows a go

Definition of a safe-zone in open carpal tunnel surgery: a cadaver study.

Carpal tunnel decompression is one of the most common surgical procedures in hand surgery. Cutaneous innervation of the palm by median and ulnar nerves was evaluated to find a suitable incision preserving cutaneous nerves. A morphometric study was designed to define the safe-zone for mini-open carpal tunnel release. Sixteen fresh-frozen (8 right, 8 left) and 14 formalin-fixed (8 right, 6 left) cadaveric hands were dissected. Anatomy of the palmar cutaneous branch of the median and the ulnar nerve, motor branch of the median nerve, superficial palmar arch were evaluated relative to the surgical

The palmar fat pad is a reliable intraoperative landmark during carpal tunnel release.

PURPOSE: During endoscopic or open carpal tunnel release, a palmar fat pad is visualized immediately proximal to the distal end of the transverse carpal ligament (TCL). Visualization of the fat pad allows anticipation of complete release of the TCL without unnecessary distal dissection that could risk iatrogenic injury. This study defines the anatomic relationship of the distal edge of the TCL to the fat pad, the superficial palmar arch, and the motor branch of the median nerve. METHODS: Eighteen fresh-frozen cadaver hands were dissected, and the proximal aspect of the palmar fat pad, the dis
