The carpal tunnel is most narrow at the level of the hook of the hamate. The median nerve is the most superficial structure. It has specific relationships to surrounding structures within the carpal tunnel to the ulnar bursa, flexor tendons, and endoscopic devices placed inside the canal. The importance of the ring finger axis is stressed. Knowledge of topographical landmarks that mark the borders of the carpal tunnel, the hook of the hamate, superficial arch, and thenar branch of the median nerve ensure appropriate incision placement for endoscopic as well as open carpal tunnel release surger

The unilateral distal arterial lesions still give problems in classification and differential diagnosis specially in younger patients. We report on a 45 years old male patient with an aneurysm of the distal ulnar artery and superficial palmar arch. The chronic occupational trauma of the artery led to the clinical picture of hypothenar hammer syndrome.

During a routine anatomic dissection of a 60-year-old female cadaver, a large branch of the ulnar artery in the carpal tunnel was found in the left forearm. This branch crossed the median nerve anteriorly. Guyon's canal contained another branch of the ulnar artery and the ulnar nerve. There was no superficial palmar arch in the left hand. The right forearm was without any vascular anomalies. This anomaly has considerable consequences for surgeons considering endoscopic carpal release, because there is a significant risk of injury to the ulnar artery branch.

Twenty-six patients (30 hands) who underwent endoscopic carpal tunnel release were physically examined and asked to complete questionnaires on their symptoms, functional limitations and satisfaction. Their mean grip strength improved considerably from 17.5 kg before the operation to 31.3 kg at the final follow-up. Symptoms at the final follow-up were hypesthesia in 9 hands, muscle atrophy in 10, positive Tinel sign in 3, and positive Phalen sign in 1. The mean symptom severity score improved from 3.01 before the operation to 1.44 at the final follow-up and the mean functional status score impr

The distally-based radial forearm flap is safe, easy, and versatile as a regional, one-stage procedure to reconstruct soft tissue defects of the hand. However, there is a general perception that the deep and superficial palmar arches have to be intact to raise a distally-based radial forearm flap. We successfully used two flaps supplied by the dorsal carpal arch, or the palmar carpal arch, or both, despite the fact that the deep and the superficial palmar arches were damaged in mutilating injuries.

BACKGROUND: Patients with anorexia nervosa can develop distal vasculopathy sometimes leading to severe Raynaud's phenomenon or acrocyanosis. We report a cas of anorexia nervosa-related vasculopathy associated with iatrogenic injury to the radial artery that led to digital necrosis.
CASE REPORT: An 18-year-old woman, with a history of severe anorexia nervosa of 5 years duration and who acknowledged regular use of tobacco and cannabis, was hospitalized for necrosis of the left index and thumb that had occurred shortly after left radial artery puncture for blood gas analysis. Acrocyanosis of the

PURPOSE: To investigate the reasons and disease course of Hypothenar Hammer Syndrome.
INTRODUCTION: Occlusion of the ulnar artery at the level of the hamate bone due to repetitive trauma to the hypothenar eminence is implicated as the cause of the rarely diagnosed hypothenar hammer syndrome (HHS). The thrombotic occlusion and the formation of an aneurysm of the ulnar artery and the superficial palmar arch with possible peripheral embolism of the digital arteries are a direct cause of the chronic damage to the vessel wall. Generally, HHS is diagnosed too late for recanalization to be a viable

For surgical treatment of the unstable Colles fracture we developed a new form of osteosynthesis, which consists in a modification of the dynamic Kirschner wire fixation described by Kapandji. It allows early motion without the typical risk of palmar dislocation noted with the Kapandji method. We prefer this method in elderly patients with reduced bone quality. The analysis of a collective of 110 fractures including a clinical and radiological follow-up examination of 72 patients revealed good anatomical reconstruction of the distal radius. The loss of motion was minimal on average and consist

Two cases of ulnar nerve lesions at the wrist are reported. The lesions had an acute onset and exclusively impaired the ulnar motor deep branch. The coexistence of carpal tunnel syndrome in each case allowed an early diagnosis but was somewhat misleading. In both cases, the use of classic motor and sensory conduction studies did not provide clear abnormalities that would have precisely determined the site of the nerve lesion. In both cases, only palmar stimulation of the ulnar motor deep branch showed an important conduction block. This electrodiagnostic finding showed definitively the site of

The deep palmar arch is very important in the blood supply to the hand. Consequently, the radial artery and the deep palmar arch were studied in 60 hands from 30 cadavers of adult Brazilian individuals, of both sexes. The cadavers belong to the Universidade Federal de São Paulo, Brazil. The hand arteries were injected with red stained latex neoprene: the deep palmar arch was observed in 59 preparations (98.3%). The arch conformation was classified in two groups, according to the course of the radial artery through the interosseous spaces from the dorsal to the deep palmar region. In group I th

Fifty patients were studied to determine the accuracy of the preoperative physical examination in flexor tendon Zone 5 wrist lacerations. Initially, a retrospective review of the charts of 33 patients who were examined by a second- or third-year postgraduate physician was done. An average of three errors per examination was found. Then a prospective study of 17 patients was performed to see if an examiner's level of training in hand surgery influenced the accuracy of the examination. Error rates improved slightly with experience. Ninety-seven examinations were performed in the combined prospec

Despite its demonstrated advantages in postoperative recovery, endoscopic carpal tunnel release has not been adopted by most surgeons because of the associated complications of neurovascular injury. A technique of carpal tunnel release is presented that utilizes a 1.0 to 1.5-cm palmar incision and a specially designed carpal tunnel "tome." Any aberrant anatomy of adjacent neurovascular structures may be identified under direct vision. Anatomic dissection in 28 cadaveric specimens following the procedure showed complete decompression of carpal tunnel and preservation with safe margins of the pa

This article describes the anatomy and clinical application of the lumbrical muscle flap. Anatomic and radiologic studies were performed in 20 fresh human cadaver hands injected with latex-lead-oxide solution. Only the 2 radial lumbrical muscles were found suitable for flap transposition. The vascular supply of the first and second lumbrical muscles is from branches originating in the superficial palmar arch and from the common palmar digital artery, respectively. The dominant branches invariably enter the muscle bellies at the junction of their proximal and middle thirds. Pedicled on these ve

OBJECT: The goal of this paper is to present a critical review of the endoscopic procedures currently in use for the treatment of carpal tunnel syndrome. Endoscopic techniques and outcomes are discussed.
METHODS: An extensive review of published articles on the subject of endoscopic carpal tunnel release surgery is presented, encompassing six endoscopic techniques used to treat carpal tunnel syndrome. Since the first report in 1987, 7091 patients have undergone 8068 operations. The overall success rate has been 96.52%, with a complication rate of 2.67% and a failure rate of 2.61%. The mean ti

Based on a retrospective clinical series, the authors evaluated the results of endoscopic nerve release according to Chow's technique, particularly in terms of the complications. The population consisted of 96 patients, corresponding to 102 hands, operated between January 1993 and January 1996. Paraesthesiae had completely resolved at 3 months in 87.2% of cases, while 29.7% of patients complained of discomfort in the hypothenar region. At last review, with a mean follow-up of 13.5 months, paraesthesiae had completely resolved in the same proportion of patients. 93.3% of patients with discomfor

Deterioration of pre-existing signs or appearance of a nerve deficit raise difficult problems during the complicated course following endoscopic carpal tunnel release. One possible explanation is transient aggravation of nerve compression by passage of the endoscopy material, but these signs may also be due to incomplete section of the flexor retinaculum or an iatrogenic nerve lesion. Each case raises the problem of surgical revision. The authors report three cases of open revision in which MRI allowed a very precise preoperative diagnosis of the lesions and all of the MR findings were confirm

In ultrasonographically assisted carpal tunnel release, the zone of the transverse carpal ligament between the median nerve and ulnar neurovascular bundle is of significance because proximity of the cutting device to these structures depends on its width. In this study, we measured the width of the zone at 5 levels in 60 wrists from 54 patients with surgery-indicated idiopathic carpal tunnel syndrome. The width of the 5 levels ranged from 2.8 to 11.2 mm (mean, 7.1 mm) at the hook of the hamate, 3.0 to 12.5 mm (mean, 7.9 mm) at the distal one fourth of the carpal tunnel, 2.8 to 12.4 mm (mean, 8
