Plastic and reconstructive surgery. Global open2026eng
Hypothenar hammer syndrome is a rare vascular disorder resulting from repetitive trauma to the hypothenar region of the hand, leading to ulnar artery injury and potential aneurysm formation. The aneurysm can eventually release distal emboli and lead to acute digital ischemia. Hypothenar hammer syndrome is often underdiagnosed, particularly in the plastic surgery setting. Management strategies are not standardized, and literature offering detailed surgical techniques, particularly involving palmar arch aneurysms, is sparse. We present the case of a 63-year-old manual laborer with a known ulnar

Understanding anatomical variations in hand vasculature is critical for hand and vascular surgery. This case report describes rare unilateral variations in the course and branching of the superficial palmar branch of the radial artery (SPRA) in the hand of a 100-year-old Caucasian female donor. The forearm, wrist, and hand were dissected bilaterally to demonstrate the musculature and neurovasculature. Two anatomical variations were observed in the right hand of the donor body. The first variant was the SPRA coursing superficial to the thenar muscles after emerging from the radial artery rather

BACKGROUND: It is known that the incidence and mortality rate of gastric cancer is high among Japanese and Chinese populations, but extremely low in Thai and Vietnamese populations. The aim of this study was to investigate the prevalence of Helicobacter pylori infection and the differences in the glandular atrophy and intestinal metaplasia scores in stomach specimens of Asian adult subjects of different races.
MATERIALS AND METHODS: Chinese, Thai, Vietnamese and Japanese patients were matched by age, gender and endoscopic diagnosis, in order to compare the differences in incidence of H. pylor

BACKGROUND: The management of extensive volar soft tissue defects in multiple fingers remains challenging for plastic surgeons. To date, although various types of flaps have been developed as reconstruction options, the applications remain limited by the shortcomings of bloated appearance, insufficient flap area, prolonged treatment time, persistent donor complications, and the sharply increased surgical complexity with the increased number of injured fingers. The superficial circumflex iliac artery perforator (SCIP) flap avoids the shortcomings of conventional flaps whilst allowing harvesting

The variations in the branching pattern of the axillary artery are clinically significant. We report an extremely rare combination of arterial variations discovered during the routine anatomical dissection of the left upper limb in a 73-year-old male cadaver. The most significant findings were a superficial ulnar artery originating directly from the axillary artery, proximal to the ansa pectoralis. The superficial ulnar artery coursed superficially in the forearm and terminated as the primary contribution to the superficial palmar arch. Additionally, a common trunk for the circumflex scapular

BACKGROUND: Carpal tunnel syndrome (CTS) is the most prevalent peripheral nerve entrapment disorder, often requiring surgical intervention. While Ultrasound-guided Looped Thread Carpal Tunnel Release (LTCTR) offers a minimally invasive alternative to carpal tunnel release, its implementation faces limitations due to insufficient specialized instruments. This cadaveric study assesses the Hybrid-J Shape Needle (HJSN), a blunt-tip, J-shaped device designed to improve safety in LTCTR procedures.
METHODS: Twenty fresh-frozen cadaveric specimens (40 wrists) were included. Ten junior surgeons were r

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. Anterograde ultrasound-guided carpal tunnel release is now a widely adopted technique. However, identifying the distal limit of the transverse carpal ligament (TCL) on a longitudinal view remains challenging. This study investigates whether the 'V'-shaped structure seen on longitudinal ultrasound can serve as a reliable landmark for complete TCL release.
MATERIALS AND METHODS: Ten fresh cadaveric upper limbs were examined. Four anatomical landmarks were identified and marked under ultrasound guidance

Background Carpal tunnel syndrome (CTS) is one of the most prevalent types of entrapment mononeuropathies, necessitating surgical treatment. The median nerve and its branches within the carpal tunnel have anatomical variances that may have clinical implications due to the possibility of iatrogenic injury while undergoing decompression treatments. Methods A total of 40 upper limb specimens (17 right and 23 left) from the Department of Anatomy were used in the dissection investigation. The recurrent thenar branch (RTB) was discovered and identified based on Lanz's classification system. Four cha

Rupture of a pseudoaneurysm of the superficial palmar arch after surgical release of the carpal tunnel is an extremely rare and still debated occurrence. Only a few cases have been described in the literature, none of which were fatal. In this report, we present a single case that came to our attention, in which an elderly man suffered a fatal haemorrhage due to the rupture of a palmar pseudoaneurysm 22 days after surgical carpal tunnel release. Autopsy examination confirmed the presence of a breach in the large palmar hematoma as the sole source of bleeding. Histologic examination in some of

BACKGROUND: The superficial palmar arch is a crucial blood supply to the palm. However, it exhibits significant variations, posing challenges in surgical procedures. Gaining a comprehensive understanding of the relationship between different types, physiological indices, and the clinical significance of the superficial palmar arch will enhance the accuracy of diagnosing and treating patients.
MATERIALS AND METHODS: In this study, we dissected a total of 72 specimens, comprising 39 males and 33 females. We observed the type, length, and diameter of the superficial palmar arch and analysed its

A 45-year-old male prisoner was referred to the orthopedic outpatients clinic after he sustained a blunt palmar injury when he tried to break-up a bag of ice with the volar aspect of his hand. A few months later a pulsatile expansile mass developed at the site of blunt trauma affecting flexion and extension of the fingers and inability to hold objects. A computed tomography angiogram confirmed the mass to be a true aneurysm of the superficial palmar arch 3 cm in size. The patient was referred to the vascular surgery department where the aneurysm was dissected and ligated with removal of the th

OBJECTIVE: This study explored the surgical technique and clinical application of the dorsal metacarpal cutaneous branch flap of the radial digital artery of the index finger.
METHODS: Through the anatomical dissection of 10 hand specimens, we examined the origin and characteristics of the artery and its branches towards the hand dorsum. Furthermore, the soft tissue defects of the index finger in 12 patients admitted to our hospital between 2017 and 2021 were surgically repaired using the dorsal metacarpal cutaneous branch flap of the radial digital artery of the index finger.
RESULTS: The r

Entrapment neuropathy of the median nerve is typically caused by compression at specific anatomical points. However, idiopathic cases, where the standard anatomical compression points are normal, pose diagnostic challenges. This report highlights a unique case discovered during an anatomical dissection of the right upper limb in a 62-year-old male cadaver, where the median nerve was compressed by an unusual branch of the brachial artery, termed the superficial brachioulnar artery (SBUA). The median nerve formed at the distal half of the arm, receiving additional components from the lateral cor

We present the case of a 22-year-old male patient who developed "double" pseudoaneurysm of the superficial palmar arch of the left hand after trivial trauma with a kitchen knife. Following an unsuccessful embolization attempt, surgical excision of the pseudoaneurysm was performed during which the pseudoaneurysm was found to arise from the anterior wall of the palmar arch. Intraoperatively, a second pseudoaneurysm arising from the deeper surface of the superficial palmar arch was found and excised. This is probably the only reported case of double pseudoaneurysm of the palmar arch in literature

Pseudoaneurysm of the palmar arch is a rare entity. Diagnosis is dependent on high clinical suspicion. We present a case referred to the emergency department, with a history of glass penetrating trauma to the palmar surface with a pulsatile mass and jet bleeding. Doppler ultrasound evidenced a partially thrombosed pseudoaneurysm. A CT angiography examination showed a saccular formation arising from the superficial palmar arch. A conventional surgical approach was indicated. A clinical suspicion must be ventured to arrive at the correct diagnosis. Imaging modalities are needed to identify the p

The individual arterial pattern of the upper limb is considerably variable (11-24%) and relevant for intraarterial interventions performed by cardiologists, plastic and vascular surgeons, radiologists, anaesthesiologists, transplant specialists, orthopaedists and neurosurgeons. Arterial variants in the upper limb result from modifications in the maintenance and regression of the initial capillary plexus, which forms dominant arterial channels and gradually expands into the growing upper limb bud between stages 12 and 21. In this case report we present the superficial brachioulnar artery with i

PURPOSE: Carpal tunnel syndrome is the most common compressive neuropathy. There is limited evidence to support endoscopic compared to open carpal tunnel release according to the 2016 American Academy of Orthopaedic Surgeons Clinical Practice Guideline on carpal tunnel syndrome. The purpose of the present study was to assess differences between the two procedures by comparing 30- and 90-day complications and mean hospital costs in a large patient population.
METHODS: Using the national Mariner15 Database by PearlDiver Technologies, we retrospectively studied 27,192 carpal tunnel syndrome pati

PURPOSE: To evaluate the proximity of the ulnar neurovascular structures to the endoscopic blade during endoscopic carpal tunnel release (CTR).
METHODS: Ten fresh-frozen cadaver hands were used to perform endoscopic CTR using devices from two manufacturers. The skin was excised from the palm, and the endoscopic carpal tunnel blade was deployed at the distal edge of the transverse carpal ligament (TCL). The blade's proximity to the ulnar neurovascular bundle, deep ulnar motor branch, superficial palmar arch, and median nerve was recorded. Following release of the TCL, the device was turned uln
