Conservative Management of a Traumatic Deep Palmar Arch Pseudoaneurysm with Delayed Ulnar Motor Dysfunction: A Case Report.

Traumatic pseudoaneurysms of the palmar arch are rare vascular lesions and are typically managed with surgical repair or endovascular exclusion. Deep palmar arch involvement is particularly uncommon, and neurological deficits are often considered indications for exploration. We report the case of a 26-year-old male with a penetrating palm injury resulting in a bilobulated deep palmar arch pseudoaneurysm, a thenar hematoma, and delayed ulnar motor dysfunction. Because there was no digital ischemia, ongoing bleeding, or lesion expansion, conservative management with structured clinical and radio

Anatomical Variation of the Superficial Palmar Branch of the Radial Artery: A Cadaveric Case Report.

Understanding anatomical variations in hand vasculature is critical for hand and vascular surgery. This case report describes rare unilateral variations in the course and branching of the superficial palmar branch of the radial artery (SPRA) in the hand of a 100-year-old Caucasian female donor. The forearm, wrist, and hand were dissected bilaterally to demonstrate the musculature and neurovasculature. Two anatomical variations were observed in the right hand of the donor body. The first variant was the SPRA coursing superficial to the thenar muscles after emerging from the radial artery rather

Striatal (a-)symmetry reveals sex-specific autonomic vulnerabilities in early Parkinson's disease.

BACKGROUND: It is known that the incidence and mortality rate of gastric cancer is high among Japanese and Chinese populations, but extremely low in Thai and Vietnamese populations. The aim of this study was to investigate the prevalence of Helicobacter pylori infection and the differences in the glandular atrophy and intestinal metaplasia scores in stomach specimens of Asian adult subjects of different races. MATERIALS AND METHODS: Chinese, Thai, Vietnamese and Japanese patients were matched by age, gender and endoscopic diagnosis, in order to compare the differences in incidence of H. pylor

Rare combined variations of the axillary artery: superficial ulnar artery and circumflex humeral arteries.

The variations in the branching pattern of the axillary artery are clinically significant. We report an extremely rare combination of arterial variations discovered during the routine anatomical dissection of the left upper limb in a 73-year-old male cadaver. The most significant findings were a superficial ulnar artery originating directly from the axillary artery, proximal to the ansa pectoralis. The superficial ulnar artery coursed superficially in the forearm and terminated as the primary contribution to the superficial palmar arch. Additionally, a common trunk for the circumflex scapular

Proximity of the Ulnar Neurovascular Structures in Endoscopic Carpal Tunnel Release Surgery: A Cadaveric Study.

PURPOSE: To evaluate the proximity of the ulnar neurovascular structures to the endoscopic blade during endoscopic carpal tunnel release (CTR). METHODS: Ten fresh-frozen cadaver hands were used to perform endoscopic CTR using devices from two manufacturers. The skin was excised from the palm, and the endoscopic carpal tunnel blade was deployed at the distal edge of the transverse carpal ligament (TCL). The blade's proximity to the ulnar neurovascular bundle, deep ulnar motor branch, superficial palmar arch, and median nerve was recorded. Following release of the TCL, the device was turned uln

Median artery persistence in coexistence with a bifid median nerve and interconnections of the median with the ulnar nerve and vice versa.

PURPOSE: This report describes a bilateral persistent median artery (PMA) originating from the ulnar artery and terminating at different levels of the upper limb. The PMA coexisted with a bilateral bifid median nerve (MN) and two bilateral interconnections (ICs characterized with the symbol -) of the MN with the ulnar nerve (UN) (MN-UN) and a unilateral reverse IC (UN-MN). Emphasis was given to the artery's developmental background. METHODS: The PMA was identified in an 80-year-old formalin-embalmed donated male cadaver. RESULTS: The right-sided PMA terminated at the wrist, posterior to the

Relevant landmarks to navigate the suture locations for the arthroscopic triangular fibrocartilage complex foveal reattachment.

INTRODUCTION: Key step of arthroscopic triangular fibrocartilage complex (TFCC) foveal reattachment is to suture dorsal and palmar deep components of radioulnar ligaments (RULs) to the fovea of ulnar head to ensure distal radioulnar joint stability. However, the deep components are covered and cannot be identified arthroscopically from standard radiocarpal joint view. Suturing sites described in previous studies have not been proved gasping of the deep components. The purpose of this study was to investigate our TFCC suture locations using relevant landmarks on securing the RULs' deep componen

A Case of Thenar Hammer Syndrome.

Thenar hammer syndrome (THS) is characterized by vascular injury and subsequent digital ischemia from acute high-energy trauma or repetitive low-energy trauma to the thenar eminence of the palm. Here, we report the case of a 41-year-old male construction worker who presented with unilateral, cold, painful, and blue-colored fingertips in his left hand. Angiography of his left upper extremity showed abrupt occlusion of the radial artery at the level of the radial styloid process with a poorly developed but patent deep palmar arch, consistent with THS. The ulnar artery and superficial palmar arch

A unique bilateral accessory forearm flexor muscle.

Muscular and neurovascular variations in the upper extremity are of utmost clinical significance. Here we report a unique bilateral accessory muscle in the forearm and palm of an 89-year-old male cadaver. The accessory muscle presented two bellies on the right side, one in the forearm, innervated by the anterior interosseous nerve, and the other in the palm, innervated by a branch of the median nerve. A long tendon interconnected the two bellies. On the left side, the muscle had a single belly in the palm, which began at the end of a long tendon that extended from the forearm. However, on both

Management of a Multi-Level Forearm Microsurgical Reconstruction and the Following Transfusion-Related Acute Lung Injury.

Transfusion-related acute lung injury (TRALI) is a clinical syndrome characterized by acute respiratory distress following blood transfusion. This case-based technical report documents a case on the management of a multi-level forearm microsurgical reconstruction and the following TRALI syndrome that developed shortly after blood transfusion in a 29-year-old male. Multilevel microsurgical revascularization was performed via saphenous vein autograft arterial reconstruction from the proximal 1/3 ulnar artery to the ulnar side of the deep superficial palmar arterial arch. TRALI was resolved with

Superficial Ulnar Artery: A Case Report of its Unusual Course.

After arising from the brachial artery in the cubital fossa the ulnar artery usually passes deep into the superficial flexor muscles of the forearm. In the lower two-thirds, it typically follows a sub-fascial course. In the present case, during a routine undergraduate course dissection of a cadaver, it was found that the ulnar artery arose normally as a terminal branch of the brachial artery in the cubital fossa, followed a sub-fascial course by lying superficial to the flexor muscles then completed the superficial palmar arch in hand. This artery gave only minute muscular branches in the fore

Hypothenar hammer syndrome from ice hockey stick-handling.

Ulnar artery thrombosis and hypothenar hammer syndrome are rare vascular complications that could potentially occur with repeated blows or trauma to the hand. Although initially reported as an occupational hazard among laborers and craftsmen, it has been observed more recently among recreationalists and athletes. Until now, it has never been reported as a complication in ice hockey players. In this case report, a 26-year-old Canadian professional ice hockey player presented with acute dominant right hand paleness, coolness, and pain with hand use. The patient used a wooden hockey stick with a

[Scapular/parascapular double skin-paddle free flap for transfixing blast injuries of the hand. A case report].

Balistic transfixing hand traumas require a complex reconstruction management. Stabilization and reconstruction of the injuried tissues need a double skin-paddle coverage. We present an original case report of a double skin-paddle scapular/parascapular free flap used to cover a through and through gunshot injury of the right hand. A 14-years-old patient presents a severe and extensive wound with full-thickness palmar and dorsal skin defects, dislocation of the carpal bones, median nerve and flexor tendons losses. Distal vascularization is maintained by the deep palmar arch system supplied by t

Variation in the hypothenar muscles and its impact on ulnar tunnel syndrome.

Compression of the ulnar nerve at Guyon's canal can be caused not only by tumor-like structures, a fibrotic arch, a ganglion, lipoma, aneurysm or thrombosis but also by anomalous hypothenar muscles which are reviewed here. For the search of relevant papers, PubMed and crucial anatomical textbooks were consulted. The abductor digiti minimi is the most variable hypothenar muscle. It can possess one to three muscle bellies. Additional heads can arise from the flexor retinaculum, the palmaris longus tendon, the pronator quadratus tendon or the deep fascia of the palmar side of the forearm. Our own

Persistent median artery: cadaveric study and review of the literature.

The persistent median artery (PMA) may compress the median nerve (MN) and may be a significant supply of blood to the hand. Two cases of unilateral PMA (4%) were detected during the dissection of 50 upper limbs. The first case was a 75-year-old, right-handed male who suffered from chronic pain in both upper limbs, especially the left side. A dissection of his left upper limb revealed a PMA piercing both the MN and the medial branch of the anterior interosseous nerve. This artery coursed distally, deep to the transverse carpal ligament (TCL), forming a median-ulnar pattern for the superficial p

Anatomy of the palmar branch of the ulnar nerve: application to ulnar and median nerve decompressive surgery.

OBJECT: The palmar cutaneous branch of the ulnar nerve (PCUN) has received little attention in the literature, and to the authors' knowledge, has received no attention in the neurosurgical literature. The present study was performed to help the surgeon minimize postoperative complications of nerve decompression at the wrist. METHODS: Forty cadaveric upper limbs underwent dissection of the ulnar nerve in the forearm, at the wrist, and in the palm. The PCUN was investigated and when identified, measurements were made and relationships documented between this cutaneous branch and the ulnar arter
