Combined nerve and tendon transfer (CNaTT) for grasp and release function in patients with tetraplegia: a matched prospective pilot study.

INTRODUCTION: Traditional single-stage tendon transfer procedures in patients with tetraplegia are designed to restore active pinch and grasp function. However, digital extension is usually achieved by wrist tenodesis. More recently, nerve transfer has allowed potential restoration of digital extension. This matched observational pilot study compares clinical outcomes after traditional tendon transfers vs. a study group using two-stage using combined nerve and tendon transfer. METHODS: Eighteen patients in the traditional tendon transfer control group and 18 patients in the combined nerve and

Comparison of two different orthoses used in the treatment of lateral epicondylitis.

BACKGROUND: Lateral epicondylitis (LE) is one of the most common causes of lateral elbow pain, and orthoses are often used in its conservative treatment. However, the number of studies examining the effectiveness of the orthoses used and their differences compared to each other is limited. OBJECTIVE: To determine the effects of 2 orthoses types, wrist orthosis (WO) and counterforce brace (CB), commonly used in the treatment of lateral epicondylitis on muscle activation, grip strength, hand function, and satisfaction level in healthy individuals. STUDY DESIGN: Quasi-experimental study. METHO

High Prevalence of Work-related Musculoskeletal Disorders and Limited Evidence-based Ergonomics in Orthopaedic Surgery: A Systematic Review.

BACKGROUND: The Centers for Disease Control defines work-related musculoskeletal disorders as disorders of the nerves, muscles, tendons, joints, spinal discs, and cartilage that are caused or exacerbated by the environment or nature of work. Previous meta-analyses have characterized work-related musculoskeletal disorders among interventionists, general surgeons, and other surgical subspecialties, but prevalence estimates, prognosis, and ergonomic considerations vary by study and surgical specialty. QUESTIONS/PURPOSES: (1) What is the career prevalence of work-related musculoskeletal disorders

Investigating the relationship between pain level and grip and wrist muscles strength in individuals with lateral epicondylitis: is pain a barrier to strength assessment?

PURPOSE: This study was carried out to examine the relationship between rest, activity, and nighttime pain and grip and isokinetic muscle strength of the wrist muscles in individuals with lateral epicondylitis. METHODS: Fifty-six sedentary individuals aged between 18 and 65 years diagnosed with unilateral lateral epicondylitis volunteered to participate in the study. The level of rest, activity, and nighttime pain was evaluated with visual analog scale (VAS). The grip strengths of both arms were evaluated by averaging a maximum of three grip strength measurements using a hand dynamometer. The

The Healing Effects of Piano Practice-Induced Hand Muscle Injury.

Источник пометил публикацию как отозванную

BACKGROUND: The muscles related to piano practice are mainly concentrated in the fingers and upper limbs, and the muscles related to other parts of the body are weak. Compared with other sports injuries, the injuries caused by piano practice are mainly chronic injuries caused by long-term strain of the upper limbs, and acute injuries rarely occur. The purpose of this study was to analyze the therapeutic effect of hand muscle injury caused by piano practice. METHOD: A total of 60 patients with hand muscle injury caused by piano practice admitted to our hospital from January 2019 to June 2020 w

Platelet-rich Plasma or Autologous Blood Do Not Reduce Pain or Improve Function in Patients with Lateral Epicondylitis: A Randomized Controlled Trial.

BACKGROUND: Platelet-rich plasma (PRP) and autologous blood are commonly used therapies for lateral epicondylitis, but the evidence from randomized, placebo-controlled trials is conflicting. Thus, it is still unclear if patients benefit from these treatments. QUESTIONS/PURPOSES: In the setting of a randomized, placebo-controlled trial, we compared PRP, autologous blood, and saline injections in the treatment of lateral epicondylitis with respect to: (1) VAS pain scores, and (2) functional outcomes (DASH score and grip strength) 1 year after treatment. METHODS: We performed a parallel-group,

Lateral epicondylalgia exhibits adaptive muscle activation strategies based on wrist posture and levels of grip force: a case-control study.

OBJECTIVES: To investigate forearm muscle activity in individuals with lateral epicondylalgia (LE) when gripping at different wrist postures, and investigate the association between muscle activity and clinical characteristics of LE. METHODS: Eleven LE and 11 healthy participants performed isometric handgrips at 15% and 30% of maximum grip force (MVC). Gripping was performed in wrist extension, wrist flexion, and wrist neutral. Surface electromyography was collected from six forearm muscles. Standard clinical and tendon structural measures for LE were obtained. RESULTS: LE group had reduced

Nerve Transfer Surgery for Penetrating Upper Extremity Injuries.

CONTEXT: Nerve transfer surgery is an option for repair of penetrating injuries of the upper extremity. In the right setting, it has advantages over tendon transfers and nerve grafting. OBJECTIVE: To review our experience since 2006 of nerve transfer surgery in the upper extremities. DESIGN: We included cases performed to repair penetrating trauma within three months of injury with at least three years' follow-up. MAIN OUTCOME MEASURES: Preoperative and postoperative muscle strength of the affected extremity. RESULTS: All 16 patients were males aged 16 to 43 years. Six patients underwent n

Hand Amputations at the Radiocarpal Level With Proximal Neuromuscular Avulsion.

PURPOSE: To report a series of 6 radiocarpal avulsion amputations in 5 patients. METHOD: We replanted 6 radiocarpal amputations in 5 patients with proximal musculotendinous and nerve avulsion between 2005 and 2013 and reviewed them retrospectively. All 5 patients were men, age range from 21 to 32 years. Functional results were evaluated through total active motion (TAM) of the fingers, 2-point discrimination, monofilament test, grip strength, and Disabilities of the Arm, Shoulder, and Hand questionnaire. Follow-up was 4 to 10 years. RESULTS: All replanted parts survived without systemic comp

Practical anatomy of the carpal tunnel.

The carpal tunnel is most narrow at the level of the hook of the hamate. The median nerve is the most superficial structure. It has specific relationships to surrounding structures within the carpal tunnel to the ulnar bursa, flexor tendons, and endoscopic devices placed inside the canal. The importance of the ring finger axis is stressed. Knowledge of topographical landmarks that mark the borders of the carpal tunnel, the hook of the hamate, superficial arch, and thenar branch of the median nerve ensure appropriate incision placement for endoscopic as well as open carpal tunnel release surger

[Pathogenesis of insertion tendinitis of the elbow in insurance medicine].

The pathogenesis of insertion tendinosis of the elbow,--lateral as well as medial epicondylitis--is difficult to define precisely. Different hypotheses have been postulated with a correspondingly large number of alternative therapeutic proposals. At first view, the prime factor may be a repetitive stress of the groups of muscles involved, respectively of the extensors of the hand and the wrist. In fact, the etiology is multifactorial and includes a cluster of causes. A series of 661 cases of insertion tendinosis of the elbow--lateral and medial epicondylitis--seen over 16 years in a private ou
