[Efficacy of free perforator flap combined with great toe transplantation in repairing thumb damage combined with thermal crush injuries in the hand].

Objective: To explore the efficacy of free perforator flap combined with great toe transplantation in repairing thumb damage combined with thermal crush injuries in the hand. Methods: This study was a retrospective observational study. From May 2014 to March 2024, seven cases (5 males and 2 females, aged from 25 to 46 years) with thumb injuries combined with thermal crush injuries in hands were admitted to the 988th Hospital of Joint Logistics Support Force of the PLA. After surgical debridement, the thumb defects were classified as grade Ⅱ in 2 cases, grade Ⅲ in 3 cases, and grade Ⅳ in 2 case

Functional outcome of multiple toe transfer procedure for open multiple transmetacarpal amputation secondary to a crushing injury: A case report.

INTRODUCTION: Reconstruction for open multiple transmetacarpal amputation secondary to a crushing injury is really challenging. Some treatment approaches could be proposed. To avoid the drawbacks of a prosthesis and hand transplantation such as the high cost, and long-term side effects of anti-rejection drugs, toe transfers were chosen. PRESENTATION OF CASE: A 33 year-old man suffered a crushing injury that led to a left transmetacarpal amputation and a right arm amputation. The left hand was reconstructed by toe transfers to restore the thumb, index and middle fingers. After 75 months of fol

A case series study in new restorative surgery in thumb amputation: The Adiposofaciocutaneous flap technique for distal thumb amputation replantation.

Thumb distal amputation refers to the loss of a portion of the thumb at or near the tip, which can be caused by various injuries such as crush injuries, lacerations, or avulsions. Several surgical methods can be used to repair thumb distal amputations, including composite graft, flap reconstruction, replantation, and amputation revision. In this case report, we describe a successful surgical procedure performed on three healthy men (19, 26, and 44 years old) who suffered a sharp amputation of their left and right hands thumb. In one case initial fixation of the amputated part was performed by

Reoperation After Operative Treatment of Open Finger Fractures.

BACKGROUND: Our primary aim was to develop a prediction model for return to the operating room (OR) after open finger fractures by studying the reoperation rate of open finger fractures based on patient demographics, injury mechanism, injury severity, and type of initial surgical fixation. The secondary aim was to study the predictors for secondary surgery due to nonunion, postoperative infection, and secondary amputation. METHODS: In the retrospective chart review, 1321 open finger fractures of 907 patients were included. Demographic-, injury-, and treatment-related factors were gathered fro

Replantation of a circumferentially degloved thumb in an occupational crush injury - A case report and review of the literature.

BACKGROUND: Degloving injuries of the fingers and especially the thumb are highly demanding problems in hand surgery and replantation often is not successful because of severe soft tissue contusion. Only few cases of successfull replantation for hand degloving injuries in thumbs are reported in literature. CASE PRESENTATION: We present the case of a young right hand dominant worker experiencing an occupational accident with a circumferential avulsion of his right thumbs' soft tissue at the level of the metacarpophalangeal (MCP) joint with complete skeletization of his thumb and a deep lacerat

Thumb fingertip reconstruction with palmar V-Y flaps combined with bone and nail bed grafts following amputation.

INTRODUCTION: The aim of treating thumb fingertip amputations with no indication for replantation is to reestablish functional and esthetic properties. MATERIALS AND METHODS: From March 2005 to October 2008, we treated 14 patients with thumb fingertip amputation using palmar V-Y flaps combined with bone and nail bed grafts. There were 10 men and 4 women, whose ages at surgery ranged from 19 to 63 years (mean 35.8 years). In all, 11 of the injuries occurred in the dominant hand. According to Allen's classification, two were type II, seven were type III, and five were type IV. All patients unde

Percutaneous Distraction Pinning for Metacarpophalangeal Joint Stabilization After Blast or Crush Injuries of the Hand.

BACKGROUND: Unstable, severely comminuted fractures of the metacarpophalangeal (MCP) joint are difficult to treat. Closed treatment and casting of these fractures often fail to maintain proper alignment and impede wound care where concomitant open injuries such as gunshot wounds are present. Conventional pinning or plating techniques are not feasible if extensive bone loss and comminution are present. A distraction pinning technique represents a potential alternative, but results with this approach, to our knowledge, have not been reported. QUESTIONS/PURPOSES: The purposes of this study were

Digit replantation: experience of two U.S. academic level-I trauma centers.

BACKGROUND: Despite advances in microsurgery, digit replantation now is performed less frequently in the U.S. compared with fifteen years ago. There has been uncertainty regarding whether previously reported U.S. replantation success rates and results reported from other countries reflect the current experience in the U.S. We hypothesized that the success of digit replantation at two academic level-I referral hospitals in the U.S. would be similar to previously published results. METHODS: In this retrospective case series, we examined all cases of digit replantation that were performed from 1

Three-year activity report of the replantation service for amputation of the hand in Poland.

UNLABELLED: A permanent on-call service for hand amputation (Replantation Service) was established in 2010 as the initiative of the Council of Polish Society for Surgery of the Hand. It is run by three qualified hand centres in Trzebnica, Poznań and Szczecin. The aim of the study was to present a summary of the almost three-year activity of this service. MATERIAL AND METHODS: Over this period, a total of 435 cases of total amputations, subtotal amputations and other severe injuries to the hand were referred. Of these, 290 referrals (67%) were accepted and 141 (33%) rejected. Among accepted, t
