OBJECTIVE: To investigate the operative procedure and the short-term therapeutic effects of medial plantar venous flaps for restoration of soft-tissue defects on the volar aspect of fingers.
METHODS: From May 2007 to July 2009, 13 cases (15 fingers) ofvolar soft tissue defects were treated with medial plantar venous flaps, including 7 males (9 fingers) and 6 females (6 fingers) with an average age of 30 years (range, 17-55 years). Soft tissue defects were caused by electric saws in 4 cases (5 fingers), by crush injury in 6 cases (6 fingers), and by burned scar removal in 3 cases (4 fingers).

INTRODUCTION: Reconstruction of the dorsal surface of hand defects requires thin, pliable, well vascularized tissue with a gliding surface for the extensor tendon course. Also defects of the palmar hand and degloved fingers need non-bulky soft tissue for reconstruction. We present a retrospective analysis of nine patients with free serratus anterior fascia flaps used to cover defects of the palmar and dorsal hand and of degloved fingers.
METHODS: Three of the patients presented limited range of hand motion due to full-thickness burns; one patient had defects of the dorsum of one hand after ac

OBJECTIVE: To approach a new procedure of microsurgery to repair thumb fingertip amputation with forward homodigital ulnaris artery flap coverage for bone and nail bed graft.
METHODS: From March 2005 to October 2007, 6 cases of amputated thumb fingertip (6 fingers) were treated, including 4 males and 2 females and aging 23-63 years. Six patients' (3 crush injuries, 2 cut injuries and 1 other injury) amputated level was at nail root (2 cases), mid-nail (3 cases), and the distal one third of nail bed (1 case). The time from injury to surgery was 3-10 hours, they were treated with forward homodi

OBJECTIVE: To investigate the surgical methods and clinical results of repairing soft tissue defects in the thumb with distally-based dorsal thumb neurocutaneous vascular flap.
METHODS: From January 2006 to October 2007, 23 patients with soft tissue defect in the thumb were treated, including 20 males and 3 females aged 19-46 years old (average 27.5 years old). The defect was caused by crush injury in 1 case, electric planer accident in 6 cases, incised injury in 8 cases, and avulsion injury in 8 cases. The defect was located on the palmar aspect of the thumb distal phalanx in 3 cases, the do

The aim of this follow-up study was to evaluate the functional and subjective results after thumb replantation. Twenty-four patients with replantation of the thumb, performed during the period 1992-1997, were reexamined after 6.5 years (range, 4.2-9.1 years post-injury). In 10 cases the amputations were isolated, 14 amputations were combined with other injuries of the hand, 15 amputations resulted from crush/avulsion injuries, and 9 amputations were sharp. Range of motion, grip strength, cutaneous sensibility, and upper-extremity functioning using the DASH questionnaire were determined. A corr

PURPOSE: The pocket principle suggested by Brent in 1979 is an alternative method for use when microsurgical replantation is not feasible. The application and the amputation level for which the method is available, however, have not been well examined.
METHODS: Between 1999 and 2003 we treated 6 patients (7 fingers) by nonmicrosurgical replantation using a subcutaneous pocket (the Brent technique). All patients had sustained complete fingertip amputations across or proximal to the lunula in digits other than the thumb. In every case the amputation was a crush or avulsion-type injury and micro

Restoration of finger length and function are the goals of replantation after fingertip amputation. Methods include microsurgical replantation and nonmicrosurgical replantation, such as composite graft techniques. To increase the survival rates for composite grafts, the subcutaneous pocket procedure has been used as a salvage procedure. The subdermal pocket procedure, which is a modification of the subcutaneous pocket procedure, was used for replantation of 17 fingertips in 16 consecutive patients. Eight fingertips experienced guillotine injuries and the other nine fingertips experienced crush

This report presents a case of microsurgical replantation of a volar skin segment of the thumb. In a 24-year-old patient, a heavy object falling over the dominant thumb resulted in a crush-avulsion injury of a pure skin segment measuring 4 x 2 cm. Examination revealed that the distal fingertip as well as the bone-tendon structures remained intact. Exploration demonstrated that both neurovascular bundles were included in the avulsed skin segment. Microsurgical replantation was achieved successfully, repairing the radial digital artery at both ends with vein grafts as well as anastomosing a palm

PURPOSE: Regaining free tendon gliding after reconstruction of flexor tendons is essential to restore full function to the affected finger. Mantero et al. described a pull-out suture technique for the repair of flexor digitorum profundus (FDP) lesions in zone 1, allowing early postoperative active mobilisation and thus minimizing the risk of tendon adhesions. In a retrospective study we examined the results after Mantero tendon repair and compared these with the results after different reconstructive procedures in the literature.
METHODS AND MATERIAL: Between 1995 and 2001, the FDP tendon in

Technical advances in revascularization and in skin cover have resulted in increases in the possibilities of digital replantation. In cases of multiple digital lesions and isolated avulsions of the thumb, all attempts to ensure recovery of basic pinch capacity are justified. On the other hand, in cases of proximal amputations with extensive avulsion or crush injury such attempts should be abandoned if results on function are important. Primary and secondary surgery must confront two difficult problems namely, repair of the avulsed flexor tendons and resensibilization. But in the field of repla

In Tampere Central Hospital, Finland, 30 thumb replantations or revascularisations were carried out between March 1977 and March 1980. 22 were complete amputations and 8 were incomplete severances. Half of this material was due to avulsion injury and the other half consisted of local crush or sharp cut injuries. Ten cases in each group were considered equal and functional comparison was possible. Some technical points in replanting avulsed thumbs are presented. In particular the reconstruction of the flexor pollicis longus tendon and muscle is described. The avulsed flexor tendon was rerouted

Report about two cases of severe injuries to the thumb. One severe crush injury was treated primarily; in the other case of complete amputation a late reconstruction was performed. Methods and modern concepts for these problems are discussed. The importance of long vein grafts in microsurgery is pointed out. The advantage of reconstructing all structures primarily is stressed. Good functional results are presented to support these concepts.

Two cases of aneurysmal bone cyst occurred in the hand. One involved the distal phalanx and followed a crushing injury to the tip of the finger; amputation of the part resulted in cure. The other involved almost all of the first metacarpal and was resected and replaced by an iliac bone graft. The appearance of an aneurysmal bone cyst in roentgenograms and after angiography usually is not diagnostic, although benign, progressive growth and potential for recurrence require complete removal.

This report describes a new hand injury seen in passengers in dune buggies having rollover accidents. Since these vehicles are usually driven off the road and far from treatment centers the treatment of these injuries is often delayed. As the rollover takes place, the reflex action--usually of the passenger--is to grasp the roll bar. A crush-avulsion injury occurs at the level of the metacarpals, with the thumb frequently spared. Wound contamination, delay in treatment, and the frequency of other serious injuries add to the seriousness of this injury. Redesign of the roll bar, provision of bet
