The hatchet flap for reconstruction of fingertip amputations.

BACKGROUND: Emmett has described a single triangular flap that contains a partial skin bridge on one of its sides and called it the hatchet flap. It was successfully used for defects located on various body parts. The aim of this study was to present the authors' experience and results obtained with this flap in fingertip amputations. METHODS: Nine patients who have completed their 1-year follow-up period were included in the study. Two-point discrimination and stiffness were tested, and the patients were queried about the existence of cold intolerance, hypersensitivity, numbness, and pain in

Nonmicrosurgical replantation using a subcutaneous pocket for salvage of the amputated fingertip.

PURPOSE: The pocket principle suggested by Brent in 1979 is an alternative method for use when microsurgical replantation is not feasible. The application and the amputation level for which the method is available, however, have not been well examined. METHODS: Between 1999 and 2003 we treated 6 patients (7 fingers) by nonmicrosurgical replantation using a subcutaneous pocket (the Brent technique). All patients had sustained complete fingertip amputations across or proximal to the lunula in digits other than the thumb. In every case the amputation was a crush or avulsion-type injury and micro

Free dorsoulnar perforator flap transfers for the reconstruction of severely injured digits.

The aim of this study was to investigate the feasibility of transferring the free dorsoulnar perforator flap nourished by the cutaneous perforator branched dorsoulnar artery to reconstruct severely injured fingers under upper arm anesthesia. Between April of 2001 and April of 2002, 13 free dorsoulnar perforator flaps were used in 13 patients. There were 11 men and two women ranging in age from 18 to 64 years, with an average age of 38 years. The affected fingers were one thumb, four index fingers, five middle fingers, two ring fingers, and one little finger. All cases were performed under uppe

[Results of reconstruction of the thumb by emergency pollicization of mutilated index finger].

Traumatic amputations of the thumb are frequently associated with mutilation of one or more of the remaining digits. The most common digital injuries associated with thumb loss are partial or complete amputation of the index finger. This was the case in 7 of our patients (men, 37.3 years of age on average), who had had emergency pollicization of the mutilated index finger performed. In 4 patients the palmar neurovascular bundles, in one dorsal metacarpal artery were used for this procedure. In 2 patients, because of lesion of digital blood circulation caused by a trans-metacarpal wound, a two-

Fingertip replantation using the subdermal pocket procedure.

Restoration of finger length and function are the goals of replantation after fingertip amputation. Methods include microsurgical replantation and nonmicrosurgical replantation, such as composite graft techniques. To increase the survival rates for composite grafts, the subcutaneous pocket procedure has been used as a salvage procedure. The subdermal pocket procedure, which is a modification of the subcutaneous pocket procedure, was used for replantation of 17 fingertips in 16 consecutive patients. Eight fingertips experienced guillotine injuries and the other nine fingertips experienced crush

Basic principles on toe-to-hand transplantation.

Within the last three decades, toe-to-hand transplantation has become a well-established method for function and appearance reconstruction after trauma and in congenital hand anomalies. An otherwise healthy and cooperative patient is the ideal candidate for toe transplantation after trauma. In such patient, even primary toe transplantation is possible, if the stump is clean and viable. If secondary reconstruction after completed wound healing is considered, emphasis should be laid on tissue sacrifice during the acute management of non-replantable amputations at the hand. Specific consideration

Ring avulsion replantation by extended debridement of the avulsed digital artery and interposition with long venous grafts.

Ring avulsion replantation is a technically challenging procedure with a very low success rate. Because the zone of arterial injury is more extensive than what it appears to be in such avulsion amputations, a technique was developed to debride the avulsed digital artery for a long distance, extending well into the digital pulp, thereby creating healthy arterial ends to be bridged by one or two segments of long venous grafts. Using this technical approach, 7 patients with complete amputations of ring avulsion injury (Urbaniak's class III) were operated. In all amputations the bone was disrupted

[The use of fillet flaps in upper extremity and shoulder reconstruction].

Extensive soft tissue defects of the upper extremities were reconstructed with major fillet flaps in nine patients over a 5-year period at two institutions. Etiologies included trauma and tumor resection. Defect locations were the shoulder ( n=3), combined defects of the shoulder plus neck, arm or chest wall ( n=4) as well as one upper arm and one forearm defect. Seven of nine flaps required microsurgical free tissue transfer. Fillet flaps harvested from amputated parts represent the "spare part" concept, providing coverage of defects without additional donor site morbidity. Unlike the classic

[Popliteal trauma due to posterior dislocation of the knee].

Popliteal artery injuries have an incidence ranging between 5% and 19% of all traumatic arteria lesions. A high index of amputation, when associated with lesions of the infrapopliteal branches, and a 4-5% overall mortality are recorded in these conditions. Three patients with popliteal artery injury due to posterior knee dislocation were operated upon in our department during the last 12 months. In one case, the physical examination revealed a warm limb, with peripheral pulse; angiography showed a pseudoaneurysm of the popliteal artery with intimal dissection and partial thrombosis. In the las

Replantation of fingertip amputation by using the pocket principle in adults.

There are several treatment modalities for zone 1 or zone 2 fingertip amputations that cannot be replanted by using microsurgical techniques, such as delayed secondary healing, stump revision, skin graft, local flaps, distant flaps, and composite graft. Among these, composite graft of the amputated digit tip is the only possible means of achieving a full-length digit with a normal nail complex. The pocket principle can provide an extra blood supply for survival of the composite graft of the amputated finger by enlarging the area of vascular contact. The surgery was performed in two stages. Snowblower injuries to the hand.

We retrospectively reviewed the records of 62 patients who sustained serious hand injuries caused by snowblowers between 1981 and 1990. Frequency of injuries to digits tended to correlate with length (i.e., middle, index, ring, or small finger or thumb). Damage to tendons did not seem to follow any particular pattern. The majority of victims sustained multiple digital involvement. Complete versus partial amputation followed the same length distribution as did injured digits. Most of the injuries occurred to the dominant hand. When patients were further questioned regarding the circumstances an

Flexor tendon tears in the hand: use of MR imaging to diagnose degree of injury in a cadaver model.

OBJECTIVE: Treatment of flexor tendon lacerations of the finger partly depends on the degree of injury, which is difficult to determine clinically. We used a cadaver model to investigate the potential of MR imaging in evaluating these injuries. MATERIALS AND METHODS: A scalpel was drawn transversely across the volar surface of four cadaver hands, producing various flexor tendon injuries. MR imaging of each hand was performed using axial two-dimensional spin-echo and three-dimensional gradient-recalled-echo sequences. The three-dimensional data sets were interactively reformatted along the lon

Pollicization of the mutilated hand by transposition of middle and ring finger remnants.

Traumatic injuries to the thumb are frequently associated with mutilation of one or more of the remaining digits. The most common digital injuries associated with thumb loss are partial or complete amputation of the index finger or index and long fingers. Occasionally, satisfactory index finger function will be preserved with damage or amputation of the middle or ring fingers, making them the best candidates for transposition. Although pollicization of the index finger has received considerable attention in the literature, pollicization of injured middle and ring finger remnants has not. Trans

Reconstruction of four damaged or destroyed ipsilateral fingers with free toe-to-hand transplantations.

Three patients with traumatic partial or complete loss of all ipsilateral digits, excluding the thumb, were reconstructed with free microvascular toe transplantations for all involved digits. In two cases, two pairs of combined second and third toe transplantations were utilized; in the third case, four separate toes were employed. Clinical follow-up was 8 1/2 years, 2 years, and 1 2/3 years. Detailed functional analysis was performed. All patients except the first had initially undergone reconstruction of only two digits and had requested a second procedure to transfer two additional toes. Wh

The use of island and free flaps in crush avulsion and degloving hand injuries.

Reconstructive options for early microsurgical tissue and island flap transfer are discussed in 23 patients with extensive avulsion and degloving injuries of the hands and fingers. The patients were divided into three groups (1) degloving thumb injuries; (2) crush avulsions with or without degloving of the palm and fingers; (3) complete degloving injuries of the hand and distal forearm. There were 11 free and 12 island flap transfers. There was partial loss in two flaps with satisfactory esthetic and functional results in these patients. The advantages and indications for the use of distally b

Results after replantation and revascularization in the upper extremity in children.

The rates of survival of the amputated part and the functional outcomes were studied retrospectively after seventy-three replantations and eighty-nine revascularizations in the upper extremity in 120 children. All operations were performed between January 1974 and December 1988 after partial and complete amputations at various levels. The ages of the patients ranged from three days to sixteen years. The average duration of follow-up was thirty-six months (range, fourteen months to seven years) for the patients who had had a replantation and thirty months (range, fourteen months to eight years)

Replantation of amputated facial tissues with microvascular anastomosis.

A challenge to the microsurgeon is perfecting the technique of replantation of small pieces of facial tissue, mainly because of the extremely small size of the arteries as well as a lack of suitable veins for drainage. In the past 4 years, we have had seven cases of facial amputations, which included one scalp, two nasal tips, two ears, one lower lip, and one eyebrow. All of these patients were replanted/revascularized by microvascular anastomosis. Only two of the cases had suitable veins for anastomosis. Alternative techniques used for improving venous outflow were arterio-venous fistula, che

Changes in sensation after nerve injury or amputation: the role of central factors.

Dynamic changes in somatosensory cortical maps are known to occur in experimental animals subjected to peripheral nerve transection or amputation. To study the sensory effects of central nervous system adaptation to temporary or permanent loss of input from a part of the hand, multimodality quantitative sensory tests were carried out in 11 patients with complete traumatic division and repair of the median or ulnar nerves and in six patients who had undergone amputation of one or more digits. As expected, vibration, two point discrimination, and tactile thresholds were raised in the territory o

[Forum: reconstruction of the traumatic thumb. Reconstruction of the thumb in a child by double transfer. Apropos of a clinical case].

The authors present a case of atypical thumb reconstruction in children, using a combined vascularized joint transfer and a free partial big toe transfer. They describe a burned hand with first metacarpal amputation and transproximal phalanx amputation of the index. According to the "bank finger" principle the metacarpophalangeal joint was transferred on the top of the first metacarpal, vascularized by the two collateral pedicles. The second metacarpal shaft was removed to create the first web and kept for bone grafting. A "wrap-around" flap (Morrison) with an intercalated bone graft completed

[Forum: reconstruction of the traumatic thumb in practice...the surgeon passes!].

The aim of thumb reconstruction is to obtain, within a reasonable time, a thumb with an aesthetic appearance and function as close as possible to those of a normal thumb, leaving a minimum of sequelae at the donor site. The author reviews the various modalities of reconstruction, according to the severity of mutilation of the thumb. In pulp amputations, partial lesions are repaired by a homodigital island pulp flap (subcutaneous or unilateral or bilateral neurovascular pedicle advancement flap); total pulp defects should be treated by toe pulp transfer rather than the neurovascular pedicle con
