Three patients with traumatic partial or complete loss of all ipsilateral digits, excluding the thumb, were reconstructed with free microvascular toe transplantations for all involved digits. In two cases, two pairs of combined second and third toe transplantations were utilized; in the third case, four separate toes were employed. Clinical follow-up was 8 1/2 years, 2 years, and 1 2/3 years. Detailed functional analysis was performed. All patients except the first had initially undergone reconstruction of only two digits and had requested a second procedure to transfer two additional toes. Wh

Reconstructive options for early microsurgical tissue and island flap transfer are discussed in 23 patients with extensive avulsion and degloving injuries of the hands and fingers. The patients were divided into three groups (1) degloving thumb injuries; (2) crush avulsions with or without degloving of the palm and fingers; (3) complete degloving injuries of the hand and distal forearm. There were 11 free and 12 island flap transfers. There was partial loss in two flaps with satisfactory esthetic and functional results in these patients. The advantages and indications for the use of distally b

The rates of survival of the amputated part and the functional outcomes were studied retrospectively after seventy-three replantations and eighty-nine revascularizations in the upper extremity in 120 children. All operations were performed between January 1974 and December 1988 after partial and complete amputations at various levels. The ages of the patients ranged from three days to sixteen years. The average duration of follow-up was thirty-six months (range, fourteen months to seven years) for the patients who had had a replantation and thirty months (range, fourteen months to eight years)

A challenge to the microsurgeon is perfecting the technique of replantation of small pieces of facial tissue, mainly because of the extremely small size of the arteries as well as a lack of suitable veins for drainage. In the past 4 years, we have had seven cases of facial amputations, which included one scalp, two nasal tips, two ears, one lower lip, and one eyebrow. All of these patients were replanted/revascularized by microvascular anastomosis. Only two of the cases had suitable veins for anastomosis. Alternative techniques used for improving venous outflow were arterio-venous fistula, che

Dynamic changes in somatosensory cortical maps are known to occur in experimental animals subjected to peripheral nerve transection or amputation. To study the sensory effects of central nervous system adaptation to temporary or permanent loss of input from a part of the hand, multimodality quantitative sensory tests were carried out in 11 patients with complete traumatic division and repair of the median or ulnar nerves and in six patients who had undergone amputation of one or more digits. As expected, vibration, two point discrimination, and tactile thresholds were raised in the territory o

The authors present a case of atypical thumb reconstruction in children, using a combined vascularized joint transfer and a free partial big toe transfer. They describe a burned hand with first metacarpal amputation and transproximal phalanx amputation of the index. According to the "bank finger" principle the metacarpophalangeal joint was transferred on the top of the first metacarpal, vascularized by the two collateral pedicles. The second metacarpal shaft was removed to create the first web and kept for bone grafting. A "wrap-around" flap (Morrison) with an intercalated bone graft completed

The aim of thumb reconstruction is to obtain, within a reasonable time, a thumb with an aesthetic appearance and function as close as possible to those of a normal thumb, leaving a minimum of sequelae at the donor site. The author reviews the various modalities of reconstruction, according to the severity of mutilation of the thumb. In pulp amputations, partial lesions are repaired by a homodigital island pulp flap (subcutaneous or unilateral or bilateral neurovascular pedicle advancement flap); total pulp defects should be treated by toe pulp transfer rather than the neurovascular pedicle con

Ten or nine-digit amputation is a very rare injury. From March, 1987 to June, 1988, we replanted two cases of ten-digit complete amputation and two cases of nine-digit complete amputation with success. The maximum ischaemic time was 39 h and the operation time was between 25 and 31 h. All digits survived completely except for partial necrosis of one finger. The follow-up has been 13-20 months, and all the patients have a pair of good, functional and aesthetically acceptable hands.

Among the various alternative forms of treatment of digital tip amputations, replantation of the amputated tip as composite graft or conservative treatment (healing by secondary intention) are of special interest for the management of fingertip injuries in children. From 1986 until 1987, the authors treated thirteen fingertip amputations in children between one and eight years of age (mean 3.5 years). In twelve cases, the amputated tip was reattached as composite graft, one injury healed by secondary intention. Twelve children were reexamined according to a prospective protocol after a mean fo

Our experience with simultaneous transfer of two or more toe units to the same hand where multiple digits were missing is presented. Forty-six toes from 38 feet were transferred to reconstruct 19 hands in 19 patients. The transfers consisted of 7 combined second and third toe units and 32 single toes. Three patients had a primary and 16 patients had a secondary reconstruction. There was one complete and one partial failure. The two-point discrimination ranged from 6 mm to protective sensation. Total active movement averaged 57 degrees in the thumb and 127, 93, 71, and 68 degrees, respectively,

Thirty adults (17 male and 13 female patients) with spastic disorders were treated by hamstring releases of 46 extremities. The diagnoses were stroke, traumatic brain injury, spinal cord injury, multiple sclerosis, and anoxia. The mean age at surgery was 38.6 years. Three extremities had partial release of the hamstring tendons behind the knee; the remaining 43 extremities had a complete release. The average position of the knee was 61.4 degrees of flexion preoperatively and 6 degrees postoperatively. The follow-up period averaged 21.3 months. Preoperatively, 87% of patients were nonambulatory

The results of a light and electron microscopy study of seven amputation neuromas are presented. The neuromas were removed during surgery for traumatic lesions of limbs. The histological study showed a complete disappearance of the nerve architecture at the neuroma level, where the fibres are collected in small bundles, enveloped in an abnormal perineural sheath, surrounded by an overgrowing connective tissue. In two cases a partial and incomplete reinnervation of the distal stump was found. The ultrastructural study shows abnormalities of the nervous fibers, both of the axon and of the relati

Two cases are presented in which multiple vein grafts were used to revascularize several digits in hands injured by severe crushes. One case had partial amputations of 4 digits, and the other had 4 complete amputations. After debriding the damaged parts of the vessels, we interposed vein grafts in both the arterial and venous circulations to the digits. Seven of 8 revascularized digits survived. In crushing injuries, it is essential to debride the vessels proximally and distally to where the intima has not been injured. To bridge the gaps, vein grafts can be used successfully where other metho
