BACKGROUND: Peripheral nerve regeneration via electrical stimulation on nerve repair has been shown to potentiate axonal growth and functional recovery. However, it remains unknown whether increasing the frequency of electrical stimulation improves, diminishes, or has no effect on the quality of nerve regeneration after repair. Additionally, few animal studies have investigated the potential beneficial effects of serial stimulations on accelerating and improving end-organ functionality over time. We therefore sought to evaluate the additive effect of weekly versus biweekly serial electrical st

INTRODUCTION: Various patient-reported outcome measures (PROMs) are available to assess functional outcomes in patients treated for wrist-hand fractures. Among the most frequently applied instruments are the Patient-Rated Wrist/Hand Evaluation (PRWHE), Quick Disabilities of the Arm, Shoulder and Hand and Short Musculoskeletal Function Assessment. Computerized adaptive tests (CATs) utilizing the PROM-Information System (PROMIS) offers a standardized alternative. This study compared the psychometric properties of PROMIS CATs with these three legacy instruments and assessed independent factors as

INTRODUCTION: Artificial intelligence (AI) has become increasingly tested for improving fracture detection. This scoping review evaluates how AI can improve human performance in detecting traumatic wrist fractures.
METHODS: A scoping review was conducted to identify studies comparing AI and human performance for wrist fracture detection. Nine resources were searched, including key databases Embase, MEDLINE, and SCOPUS. Studies which did not include AI being used as an assistant were excluded.
RESULTS: Nine studies were included in the review, where AI models demonstrated sensitivity from 83%

Neonatal total brachial plexus palsy (NBPP) is one of the most challenging birth injuries, especially if it is associated with hand palsy and sensory loss. Current techniques offer limited success for both extrinsic and intrinsic hand muscles recovery, especially for the latter. We present an innovative combined surgical method designed to reinnervate the hand in complete palsies: (1) the ulnar nerve of the affected limb, which is insensate and nonfunctional, is harvested as an autologous graft and used to connect the contralateral C7 root to the medial (motor) trunk contribution to the median

INTRODUCTION: Scapholunate ligament (SLL) injuries can lead to instability and osteoarthritis through stages of scapholunate dissociation and scapholunate advanced collapse. Although cartilage damage is traditionally seen as a late finding, recent evidence suggests that it may occur earlier. This study aimed to assess the prevalence and distribution of cartilage damage in patients with isolated SLL lesions confirmed by arthroscopy and to explore associated sociodemographic and clinical factors.
METHODS: A retrospective cohort study was conducted in patients with arthroscopically confirmed SLL

BACKGROUND: Extensor tendon release for tennis elbow is fraught with complications as persistence of pain because of adhesions together with weakness of wrist extension. Denervation of the epicondylar region through division of branches of posterior cutaneous nerve of the forearm (PCNF) was proposed to alleviate pain without drawbacks of tendon release.
METHODS: The first group included 19 patients treated through dividing PCNF. The second group included 23 patients treated through tendon release. Inclusion criteria included symptoms for 6 months or longer. Clinical diagnosis was based on pos

Extensor tendon disruptions are frequently seen in emergency rooms and hand surgery offices. Traumatic open injuries are more familiar, but spontaneous closed extensor tendon ruptures represent an important clinical entity. Often occurring without any prodromal warning, they can surprise patients and their providers alike. Spontaneous extensor tendon ruptures often occur in the setting of systemic medical conditions or hand and wrist pathology, with the mechanisms that lead to rupture often dependent upon the underlying comorbidities. Remaining knowledge of the factors that place patients at r

OBJECTIVES: Hand and wrist injuries comprise roughly 25% of emergency department visits and generate major costs. Growing incidence and surgical treatment highlight the need to understand resource use and economic impact.
METHODS: We retrospectively analyzed Dutch adults (2017-2022) with phalangeal, metacarpal, or carpal fractures or finger dislocations in a level-2 trauma center. Resource use and treatments were recorded, costs calculated as volume × unit cost, and differences assessed with t tests, analysis of variance, and regression with bootstrap.
RESULTS: A total of 8453 patients were

Background: Patient-reported outcome measures (PROMs) are questionnaires used to evaluate a patient's functional status, examine outcomes and inform clinical decisions. PROMs have been primarily developed with English-speaking populations and should be validated in the patient population in whom they are being applied. As present, the validity of commonly utilised distal radius fracture (DRF) PROMs in patients who speak Japanese is unknown. The purpose was to evaluate the validity and psychometric properties of common DRF PROMs in the Japanese population. Methods: A systematic review was condu

The scaphoid is the most injured carpal bone, with an incidence of 12 per 100,000 per annum in the United Kingdom (UK). Fractures of the scaphoid can be challenging to diagnose, both clinically and radiographically, and have a propensity to fail to unite or develop necrosis. There is debate regarding whether to manage acute fractures conservatively or offer early surgical fixation. The poor predictive value of initial examination and plain imaging not only results in missed fractures but also, conversely, in a fivefold overtreatment, which has subsequent significant socioeconomic consequences.

Background: Wrist arthroscopy is widely used to assess the intactness of ligaments, including the scapholunate ligament (SLL), as it allows direct visualisation and palpation. However, arthroscopy is invasive and costly, while traditional imaging modalities, such as X-rays and MRI, often miss ligament injuries due to their static nature. A promising alternative is four-dimensional computed tomography (4DCT), which allows dynamic, non-invasive visualisation of wrist motion in real time. This study evaluates the diagnostic potential of 4DCT for detecting SLL tears by comparing its findings with

PURPOSE: Kirschner wire (K-wire) infections continue to be a challenge in hand surgery. Our purpose was to quantify the frequency of K-wire site infections of the hand and wrist, to tabulate major and minor infectious manifestations, to identify factors that may predispose a patient to major complications, and to generate an updated algorithm for prognostication and treatment of K-wire site infections.
METHODS: Patients treated with K-wires in the fingers, hand, and wrist who developed infections between 2018 and 2024 at our institution were identified. Information regarding patient factors,

PURPOSE: Accurate and reliable radiographic measurement is essential to diagnose and evaluate surgical treatment outcomes for scapholunate dissociation (SLD). Although published standards for measuring radiographic parameters have demonstrated excellent interrater and intrarater reliability in normal patients, we were unable to find reliability data for patients with SLD. We hypothesized that six radiographic parameters for SLD would have good reliability in normal and injured patients.
METHODS: Wrist radiographs of patients with SLD and a control group of uninjured patients seen at our insti

Arthroscopic treatment of TFCC lesions and stabilization of the DRUJ offer a differentiated range of findings-oriented procedures. Theses include central smoothing/partial resection via peripheral capsular sutures to foveal refixation and, in cases of irrepairable damage, anatomical reconstruction of the DRUJ-stabilizing radioulnar ligaments. The Palmer and Atzei classifications are widely used in everyday clinical practice and offer a structured approach to diagnosis and indication, with foveal integrity being the central criterion for DRUJ stability. Medical history, clinical examination, im

BACKGROUND AND PURPOSE: Cross-country skiing (CCS) is a popular winter activity in Sweden, but little is known about what type of orthopaedic injuries are sustained following accidents in the general population. The purpose of this study was to describe the fracture distribution in accidents during cross-country skiing. METHODS: In this registry-based observational study, all patients who sustained one or more fractures in CCS accidents registered in the Swedish Fracture Register (SFR) with injury date between January 1, 2015 and June 30, 2022 were included. Fractures were classified using the

BACKGROUND: While sports have demonstrated psychosocial and physical health benefits in children, there is little specific research regarding children with symbrachydactyly or congenital hand differences. The aim of this study is to assess sports participation, incidence of injury, and perceived abilities among children with symbrachydactyly, partial, or complete absence of a hand.
METHODS: In this single-center, cross-sectional survey study, patients between the ages of 6 and 18 years with a symbrachydactyly diagnosis were recruited at a pediatric tertiary hospital to complete a survey inclu

BACKGROUND: Focal chondral lesions of the wrist and hand are an underrecognized source of pain and functional limitation in athletes. Their management remains challenging due to the limited healing capacity of hyaline cartilage, the biomechanical demands of small joints, and the lack of standardized treatment algorithms. Arthroscopy allows accurate diagnosis and targeted treatment with minimal morbidity, while biologic augmentation using autologous adipose-derived products has emerged as a potential adjunct to improve clinical outcomes.
OBJECTIVE: To evaluate the clinical outcomes of arthrosc

INTRODUCTION: Pediatric buckle fractures are common. These are often referred to as orthopaedic surgery offices, despite evidence that specialty evaluation is unnecessary. Here, we sought to determine if the provision of an educational video instead of an in-person visit with an orthopaedic provider would be non-inferior for patient-reported outcomes and demonstrate improvement in other outcomes, such as patient, caregiver, and provider time spent on caring for this injury.
METHODS: This was a single-center, prospective, randomized controlled, non-inferiority trial of 41 patients with distal
