Long-Term Functional Outcomes of Posttraumatic Complex Extensor Tendon Reconstruction in Hand and Forearm Using Composite Anterolateral Thigh Free Flaps: A Case Series and Literature Review.

Complex dorsal hand and forearm injuries with extensor tendon and soft tissue loss are challenging to manage. Single-stage reconstruction using composite anterolateral thigh (ALT) fasciocutaneous free flaps with vascularized fascia lata may reduce the need for multiple surgeries and accelerate functional recovery, but clinical evidence is limited. We conducted a retrospective case series of nine patients with complex extensor tendon and soft tissue defects who underwent single-stage reconstruction with composite ALT flaps between 2009 and 2018. Six patients were available for long-term follow-

Specific fixation beyond the watershed line for distal radius fractures: A comparative study.

PURPOSE: Various fixation methods are available for intra-articular distal radius fractures, depending on column involvement. Some rely solely on ulnar hook plates, while others use specific "extra distal" plates that address both columns. To date, no comparative studies have evaluated these techniques. This study compared the fixation by ulnar hook plate combined with one or two percutaneous screws (Group 1) in the radial styloid with extra distal plates fixing both columns (Group 2). Functional outcomes were hypothesized to be similar, whereas complication rates were expected to differ. MET

The influence of plate removal on functional and patient reported outcomes after scaphoid nonunion surgery.

INTRODUCTION: Volar locking plate fixation with cancellous bone grafting is an effective treatment for scaphoid non-union. However, the plate can impinge on the volar rim of the radius, causing pain or restricted movement that often necessitates plate removal. The primary aim of this study was to evaluate the effect of hardware removal on wrist function and patient-reported outcomes after plate fixation for scaphoid non-union. METHODS: This prospective study assessed 49 of 113 patients who underwent plate fixation for scaphoid non-union and later required plate removal due to functional impai

Resource Utilization in the Management of Distal Radial Fracture.

PURPOSE: In a survey- and scenario-based experiment, we addressed the factors associated with surgeon recommendations for specific distal radius fracture treatment options, including patient financial insecurity and insurance coverage. METHODS: One hundred sixty-two surgeons were with presented seven sets of radiographs of dorsally displaced fractures, accompanied by a scenario with randomized elements regarding the patient and circumstances of the injury. Surgeons then rated the likelihood they would offer reduction and cast immobilization, percutaneous Kirschner wire fixation, and volar pla

Current Concepts: Lesser Digit Carpometacarpal Joint Fracture-Dislocations.

Carpometacarpal fractures and dislocations are commonly unrecognized injuries. Treating physicians need to maintain a high index of suspicion regarding this particular injury as delay in diagnosis and treatment can lead to a suboptimal outcome. Mechanism of injury often entails high-energy trauma or an axial blow to a closed fist. Thorough physical examination and adequate imaging are necessary during initial evaluation. Closed management and immobilization can be considered if the injury is recognized early; however, surgical reduction and stabilization are more commonly warranted. This artic

A Review of Hand Neuroanatomy, Anesthesia, and Bedside Blockade.

Many hand-related pathologies can be appropriately managed at the bedside without operative escalation, thereby reducing resource allocation, healthcare costs, and exposure to general anesthesia. Plastic surgery trainees have variable experience and knowledge in managing hand pathologies outside the operating room. A comprehensive clinical reference for hand anesthesia has yet to be seen in the current plastic surgery literature. This review aims to guide the safe and effective administration of regional anesthesia in the hand and wrist for bedside procedures. This review was guided by clinica

Complete Recovery of Iatrogenic Radial Nerve Palsy in a Child: A Case Report.

Supracondylar humerus fractures are among the most common paediatric injuries. Although neurologic complications are often seen in these fractures, iatrogenic nerve injuries following closed reduction and percutaneous pinning remain relatively uncommon. Radial nerve palsy is particularly rare in this context, and most cases represent neuropraxic injuries that recover spontaneously. This report describes a rare case of iatrogenic high radial nerve palsy following closed reduction and K-wire fixation of a comminuted supracondylar humerus fracture in a child, with complete functional recovery und

Attrition rates in hand and wrist trauma trials: a systematic review and meta-analysis protocol.

BACKGROUND: Hand and wrist injuries are increasingly prevalent and impact individuals' abilities to participate in society and generate income. Attrition in the hand and wrist trauma treatment time course and trials is a recognised issue, with patients disengaging from follow-up due to various logistical and socioeconomic barriers. While several studies have addressed this, the timeline and causes of attrition in trials remain inconsistent. This meta-analysis aims to evaluate the attrition rates among patients with hand and wrist trauma across all prospective randomised controlled trials (RCTs

A proposed surgical strategy for complex articular distal radius fractures: The Marseille approach for radius salvage (MARS).

INTRODUCTION: Complex intra-articular distal radius fractures with severe comminution or fragment rotation may be difficult to reduce using conventional fixation. We describe a dorsal salvage strategy, the Marseille Approach for Radius Salvage (MARS), enabling direct articular visualization in selected cases. HYPOTHESIS: The MARS strategy allows reliable reduction and acceptable short-term functional outcomes in fractures not amenable to standard fixation. MATERIALS AND METHODS: Single-center prospective case series (May 2021-October 2025). CT assessed reduction quality (step-off ≤ 2 mm). Fu

Forearm Compartment Syndrome Following a Thermal-Crush Injury Treated With a Dorsal Ulnar Artery Flap: A Case Report.

Compartment syndrome (CS) is a condition that requires immediate attention; if diagnosis is delayed and treatment is not provided promptly, it can lead to complications resulting in permanent functional disability or limb loss. As the name suggests, once a muscle compartment is affected by an injury, this syndrome may develop, manifesting as progressive inflammation and edema, leading to reduced blood flow to the limb. Diagnosis is primarily clinical, based on signs of impaired perfusion. Fasciotomy is the treatment of choice, helping to prevent amputations when performed in a timely manner. H
