Devastating hand injuries often require multiple microvascular reconstructions. We report a patient in whom two flaps were used for late reconstruction of a devastating hand injury involving devascularization of the right hand, severely comminuted fractures of the hand and forearm, and multiple tendon avulsions. We believe the sequential vascular connection of free flaps offers the best method of reconstruction in this severe case, allowing composite tissue transfer, monitoring of the osseous flap, and optimal positioning of the two free tissue transfers.

Fifty consecutive patients who had a history and clinical findings consistent with internal derangement of the wrist were prospectively entered into a study to compare the findings of triple-injection arthrography with those of arthroscopy of the wrist with use of three portals. Failure of bone grafting in scaphoid nonunion presents the hand surgeon a perplexing set of problems. Controversy remains as to the best course of treatment in this difficult situation. Carpometacarpal dislocations of the ulnar four joints are rare injuries that are usually associated with high-energy trauma. Given the small number of cases reported, there exists some controversy regarding their optimal management. The case of a 27-year-old man who sustained a dorsal dislocation (without associated fractures) of the ulnar four carpometacarpal joints as a result of a fall from a height is presented. The literature regarding these hand injuries is reviewed, and the need for anatomic reduction and stabilization is discussed.

We describe the first case of hypothenar hammer syndrome followed by accelerated systemic sclerosis (SSc). A sawmill worker with repeated wrist trauma presented with bilateral digital ischemia and fingertip ulcerations. The objective in this paper is to describe the severity and outcome of arterial occlusion complicating treatment of women with gynecologic cancer. Physical assessment of 146 female workers in highly repetitive jobs found 54% to have evidence of musculoskeletal disorders in the upper limb that are potentially work-related. Many workers had multiple problems, and many were affected bilaterally (33% of workers). Diagnosis of scapholunate instability is made using clinical provocative maneuvers such as the Watson test and a two-compartment arthrogram. The history of treatment options for this instability pattern is reviewed. A technique for arthroscopic treatment is described in which anatomic reduction and multiple pin fixation precipitate fibrous ankylosis to stabilize the joint. Between 1973 and 1991, 19 patients underwent creation of a one-bone forearm at our institution as treatment for radioulnar instability secondary to trauma ("type 1" patients) or tumor resection or congenital deformity ("type 2" patients). Of 231 patients with fractures of the triquetrum, 65 were followed up after a mean period of 46.8 months. The fractures were classified and radiological and clinical follow-up was carried out. The incidence of hand injuries in multiple trauma is not well investigated. Between 1980 and 1986, 782 multiple trauma patients received primary treatment at the University Hospital of Erlangen. We present a case of dislocations of the carpometacarpal joints without associated fractures. Although carpometacarpal injuries are relatively uncommon, it is rare for multiple carpometacarpal dislocations to occur without associated fractures. The injury is difficult to diagnose because of swelling of the hand. A lateral radiograph of the wrist has been found to be mandatory to its precise diagnosis if suspected. In the case presented here early diagnosis and closed manipulation in the accident and emergency (A&E) department resulted in a good outcome.

13 patients who sustained high-energy crush or blast injury of the carpal bones were reviewed after a mean follow-up period of 30 months. These complex injuries resulted in unusual disruptions of the distal carpal row and adjacent metacarpals. Frequent involvement of the carpometacarpal (CM) joints and violation of the proximal carpal row were also demonstrated. A complex hand injury with scaphotrapezial dislocation and dislocation of the trapezoid, third, fourth, and fifth carpometacarpal joints occurred when a motorcyclist was involved in a road accident. The extent of the injury was initially overlooked. Combined injuries of the carpus and carpometacarpal joints can easily be missed at radiographic examination. The true lateral radiograph of the wrist is necessary in the diagnosis of complex wrist injuries.
