Ipsilateral fractures of the scaphoid and radius.

From 1985 to 1990, six patients were treated for ipsilateral fractures of the scaphoid and radius. All these injuries resulted from falls from a substantial height, with multiple injuries occurring in all patients. Two of the patients had minimally displaced fractures (1.0 mm), whereas the other four patients had moderately displaced scaphoid fractures (2.0 mm). The treatment for fracture of the distal radius was dependent on the fracture type, but all the scaphoid fractures were treated with internal fixation. All the fractures united, with the radial fractures healing in 6 weeks and the scap

Treatment of complex fractures of the distal radius. Combined use of internal and external fixation and arthroscopic reduction.

A group of fractures of the distal radius are inherently unstable and cannot be treated by closed reduction and cast or external fixation. These fractures are high-energy motor vehicle accidents often seen in young adults but possible in nearly every age group. They are not typical Colles's fractures because they are intra-articular with displacement of the joint articular surfaces. The goals of treatment are to obtain an anatomic reduction and then to proceed with treatment techniques to maintain the reduction by whatever methods are available or necessary within the experience of the treatin

Common hand and wrist injuries in basketball players.

Multiple injuries to the hand are relatively common in basketball. This article explains the importance of an early and thorough examination, preferably by a hand specialist. It is important to obtain a history of prior injury during the assessment because treatment and prognosis may be very different. Delayed treatment often yields an inferior result and lost time. If an injury is not treated initially, it may require operative treatment or even be untreatable.

[Indications and contraindications of digital replantation].

With the development of microsurgery, replantation surgery have become the method of choice in treating the digit amputations. Although better than 90% of successful rate can be achieved in replanting guillotine type amputation, the final functional recovery is the main goal. The indications include: (1) thumb amputations, (2) Zone I amputation, (3) multiple digits, (4) bilateral amputation, (5) hemi-hand amputations, (6) hand amputation at wrist to upper forearm, and (7) pediatric amputations. And the contraindications include: (1) life-threatening associated injuries, (2) technically impossi

Aquatic hazard Mycobacterium marinum infection.

We describe two patients with Mycobacterium marinum infection and review the pertinent literature. M. marinum infection follows trauma, often trivial, in water or from marine life. Clinical manifestations include superficial cutaneous lesions which are either solitary or multiple in a sporotrichoid distribution, involvement of the deeper structures of the hand and wrist and disseminated disease. Biopsy of infected tissue reveals a mixed suppurative-granulomatous reaction with sparse to absent acid-fast bacilli. Definitive diagnosis is achieved by growing the organism from appropriate specimens

Dislocation of the lunate, triquetral, and hamate bones. Case report.

A patient sustained multiple injuries during a road traffic accident, including closed palmar luxation of the lunate together with dislocation of the triquetrum and hamulus of the hamate bone. Initially the injury was treated closed, but redislocation necessitated closed reduction and stabilisation with K-wires. Three years later movement was painless and slightly restricted despite persisting palmar dislocation of the lunate and triquetral bones as well as ulnar translocation.

Simultaneous laceration of the median and ulnar nerves with flexor tendons at the wrist.

Simultaneous laceration of both major nerves with flexor tendons at the wrist is a combination injury familiar to any hand surgeon, which has received scant attention in the English-language literature. The causes and pattern of injury in twenty-six patients treated for this condition over a 10-year period were reviewed. Intensive reassessment of eight of these patients revealed good restoration of hand mobility and strength from the repair of tendons, but the results of nerve repair were less reliable. Tests of hand function indicated that the majority had compensated well for their impairmen

Injury patterns of female competitive club gymnasts.

Gymnast and injury information on 178 competitive female gymnasts was collected through questionnaires and interviews in a 3-year prospective epidemiologic study. The injury rate was 30/100 gymnasts/year, .52 injuries/1000 h. Injury rates excluding risk exposure increased with competitive level, but the top level gymnasts had the lowest rate per 1000 h of practice. Fractures of the wrist, fingers and toes were most common, followed by sprains of ankle and knee. Nearly 40% of the sudden-onset injuries occurred in the floor event. 'Missed move' was most frequently cited as the injury mechanism,

Transcarpal carpometacarpal dislocations, excluding the thumb.

During a 9-year period, we treated 13 patients with major fractures of bones of the distal carpal row creating instability of the corresponding carpometacarpal joints. Diagnosis of the injury was made early in only eight patients whose treatment consisted of open reduction, pin or screw fixation, and immobilization from 6 to 8 weeks. They showed no complications and had an excellent functional recovery. Diagnosis in five (38%) patients was delayed 1 week to 8 months after the accident. At initial presentation, symptoms included pain, loss of grip strength, and mild limitation of finger motion.

Wrist injuries: correlation of clinical and arthroscopic findings.

Wrist arthroscopy was done for one hundred nine patients with chronic wrist pain, averaging 22.8 months in duration. History regarding mechanism of injury, painful crepitus, pain with activity or at rest, localization of tenderness, visible swelling, and instability with examination was compared with arthroscopic findings of ligament damage, articular cartilage damage, and synovitis. Both pain at rest and swelling correlated significantly with synovitis. Tenderness did not correlate with specific ligament injury. Wrist ligament injuries and/or cartilage damage were noted in 96.3% of these wris

[Special utilization potentials of the external fixator in severe complex hand injuries].

Complex injuries to the hand may cause considerable problems in the aftertreatment. Two possibilities are described, which may facilitate and optimize the postoperative care of the severely injured hand: 1. In case of an inguinal pedicle flap, the temporary stabilization of the hand against the pelvis by means of external fixation is advocated. 2. After multiple extensor tendon repair, the temporary immobilization of the wrist, leaving the fingers free to move, by means of a small fixateur externe is suggested.

Reconstruction of the burned hand.

Several basic principles of burned hand care must be kept in mind at all times. Intervention should be early and aggressive, small splints should be placed within 24 hours, and early tangential excision of the burn should be done within 72 hours. Hemostasis should be absolutely meticulous prior to grafting. Depending upon the availability of donor site skin, full-thickness skin grafts, split-thickness grafts, or meshed split-thickness grafts (expanded or not expanded) are preferred. I have found the functional and cosmetic results to decrease with use in exactly the order stated. The skin graf

Tire explosion injuries.

Twenty-five patients hospitalized with injuries sustained from tire explosions from 1980 to 1987 were reviewed. Injuries occurred directly from the tire rim or parts of its assembly, from the patient being thrown against adjacent unyielding structures, or a combination of these. Lethal or life-threatening injuries do occur, so these patients initially must be triaged as polytrauma patients. Resuscitation and expeditious attention to life-threatening injuries must be provided when necessary. Serious head, facial, eye, and upper extremity injuries occurred frequently. Pelvic and long-bone fractu

Flexor tendon ruptures in patients with rheumatoid arthritis.

One hundred fifteen flexor tendon ruptures were reviewed in 43 hands with rheumatoid arthritis, one hand with psoriatic arthritis, and one hand with lupus erythematosis. Ninety-one tendons were ruptured at the wrist, four ruptures occurred at the palm, and 20 ruptures occurred within the digits. At the wrist level, 61 ruptures were caused by attrition on a bone spur and 30 were caused by direct invasion of the tendon by tenosynovium. All ruptures distal to the wrist were caused by invasion of the tendon by tenosynovium. Patients whose ruptures were caused by attrition regained better motion th

Traumatic volar dislocation of the second, third and fourth carpometacarpal joints: mechanism and manipulation.

Traumatic volar dislocation of the second, third and fourth carpometacarpal joints without associated fracture is extremely rare. A single case, treated successfully by manipulative reduction supplemented with splint immobilization and isometric and isotonic muscle exercise is reported, with restoration of complete anatomical alignment revealed by postmanipulative radiographs. The radiology of carpometacarpal joint dislocation is discussed. The conservative traction-dorsiflexion manipulative reduction for multiple volar carpometacarpal joint dislocations is described and recommended before usi

Injuries caused by pigs in Papua New Guinea.

Pigs are intelligent animals that can be formidable adversaries to humans because of their sharp tusks and their ability to attack swiftly. Domestic and feral pigs have an important role in the ecology of village life in Melanesia. A six-year review of all injuries that were caused by pigs that were referred from the villages in Milne Bay Province, Papua New Guinea, to the Provincial Hospital was completed. Some of the injuries that were seen among the 20 patients who were studied included: three penetrating abdominal injuries with prolapse and strangulation of the intestine; a "sucking" chest

Traumatic brachial plexus palsy in the adult. Retro- and infraclavicular lesions.

The author reviewed 420 adults treated with surgery for traumatic palsy of the brachial plexus. The results of 105 patients (25%) who suffered plexus injury behind or beyond the clavicula are presented. In 69 patients, lesions affected secondary trunks and in the other 36 affected terminal branches. Simultaneous fractures or vascular injuries were not uncommon and often influenced overall prognosis. Decision criteria for nerve grafting or neurolysis are presented. Surgery is generally performed three weeks to six months after injury. Most patients present extensive damage due to traction lesio
