Arteriovenous fistulas after replantation surgery.

The delayed appearance of congenital arteriovenous fistula precipitated by local trauma is a rare event. However, these reports may provide some insight into pathogenetic mechanisms responsible for the opening of subclinical arteriovenous communications. This case of multiple arteriovenous fistulas in a previously "normal" hand followed a sharp, incomplete amputation at the wrist level with replantation.

Hand wrist cumulative trauma disorders in industry.

A total of 574 active workers from six different industrial sites were categorised into four force repetitive exposure groups. Workers in low force-low repetitive jobs served as an internal comparison population for the three other groups. Videotapes and surface electromyography were used to estimate hand force and repetitiveness. The presence of cumulative trauma disorders (CTD) was determined by structured interview and standardised non-invasive physical examination. Only workers who had been working on the study jobs for at least one year at the time of evaluation were eligible for selectio

Bone injuries: treatment and rehabilitation.

Solid bony union with full range of painless motion is accomplished by correct early diagnosis, adequate reduction, stable fixation, prevention of edema, and institution of early motion programs. The degree of bony injury and involvement of multiple tissues will dictate the type of fixation and length of immobilization. Restoration of a stable skeleton with a functional range of joint motion must always be accomplished.

Above elbow limb replantation: functional results.

Seven patients with a complete transhumeral limb amputation had their limb replanted. In all seven limbs the mechanism of injury was avulsion, and the ischemic time was relatively prolonged, with a range of 10 to 14 hours. The five patients with surviving limbs achieved useful elbow control. Of these patients, two achieved useful distal function to the wrist and hand and one had a below elbow amputation. All but one patient required multiple secondary operative procedures, with an average of 2.8 procedures for those patients with surviving limbs. We did not encounter any significant life-threa

Three-dimensional imaging of the wrist.

The objective of this study was to determine the diagnostic quality of three-dimensional images of the carpal bones that could be constructed from raw data obtained from a computerized tomography scan. The quality of raw data collected was determined by collimation, slice interval, the number of projections, and x-ray tube operating specifications. The quality of two-dimensional images that were constructed from the raw data was determined by user-specified parameters including zoom or magnification factor, convolution kernels, and centering. The two-dimensional images were modified by erasure

Carpal tunnel syndrome: a review.

Compression of the median nerve in the crowded carpal tunnel is common and frequently incapacitating. Its causes and associated clinical conditions are multiple. Diagnosis is made by the typical history, three frequently positive physical signs at the wrist, and, if necessary by median nerve conduction velocity impairment. Most patients can be managed medically; when surgery is required it can be done usually without general anesthesia on an outpatient basis with gratifying results.

The late functional results of upper limb revascularization and replantation.

The functional results in 25 of 30 patients after successful upper limb revascularization or replantation were evaluated by subjective-patient surveying and objective measurements. Young patients with complete, sharply amputated extremities at the wrist level or those with incomplete injuries and uninjured peripheral nerves had the best functional results. Multiple-level, diffuse crush, or avulsion injuries, even if the injuries were incomplete, and patients with high-level nerve injury had less return of function.

[Extrinsic and intrinsic vascularization of the scaphoid and lunate bones].

This work was performed on 50 wrist dissections. The scaphoid is situated in the territory of the radial artery. Two extracapsular vascular pedicles are constant: --the lateral pedicle arises in the anatomical snuff-box, --the anterior pedicle arises in the "wrist pulse" area and passes behind the flexor carpi radialis. It lies on the radio-carpal ligaments. THe intra-capsular pedicles approach the scaphoid from its proximal pole. There are anastomoses and they lie on the deep dorsal and palmar ligaments. The multiple anastomotic branches may form an inconstant pedicle. The lunate is supplied

Distal avulsion fractures of the scaphoid.

Avulsion fractures of the distal scaphoid occur in children and young adults and comprise nearly two per cent of all hand and wrist fractures coming for primary care. The avulsion occurs following dorsiflexion-ulnar deviation stresses. The radiological diagnosis can be difficult without multiple projections and meticulous technique. Various controversies concerning this poorly understood fracture are discussed.

[Nerve lesions following injuries of the upper limbs in childhood (author's transl)].

A total of 93 cases of nerve lesions in the limbs in children is reported. After blunt trauma (n = 53), there was most often damage to the ulnar nerve (n = 28), especially in the area of the elbow joint, followed by the radial nerve (n = 14) and the median nerve (n = 11). Causes were primary pressure and scar compression following fractures of the elbow and forearm, where ventral re-siting and/or neurolysis were necessary in serious denervation processes. Among the cutting injuries (n = 40), the lesions in the area of the underside of the wrist severing the ulnar and median nerves, more rarely

Hand and wrist injuries in the athlete.

Injuries to the hand and wrist are probably among the most common injuries incurred by the athlete. Neglect of such injuries may result in irreparable damage to the hand or wrist. This summary of the more common injuries to these areas seen in the athlete, with a description of the recognition and treatment, gives the reader insight into the multiple considerations the athlete and coach must have. Even seemingly minor injuries must be carefully evaluated and treated by a physician. Early diagnosis, accurate precise treatment, and proper rehabilitation are extremely important to regain optimal

Multiple carpometacarpal dislocations. A review of four cases.

The cases of four patients with carpometacarpal fracture-dislocations of the ulnar four joints are reported, in all of whom there was extensive additional trauma. Open reduction was necessary in three patients, and only one was treated successfully by a closed reduction. He then had persistent but painless subluxation of the metacarpal bases. On end-result evaluation, two of the other three patients had occasional discomfort and diminished grip strength. One patient had a diminished range of motion of the wrist joint at one-year follow-up, and one patient had narrowing and sclerosis of the car

Primary care of the injured hand, part 2.

Nerve injuries in the forearm and hand are common, especially with concomitant tendon lacerations. The optimal time for repair of a clean sharp laceration is within the first 24 to 48 hours using magnification to achieve precise surgical reapproximation. Transfer to a facility equipped to perform replantation is indicated for multiple digit amputations, any thumb amputation, transmetacarpal amputation, wrist and arm amputation, and major amputations in children. Acute paronychial infections are treated with warm soaks, antibiotics, elevation, and immobilization. Surgical drainage is often indi

Multiple volar carpometacarpal joint dislocation. Case report of traumatic volar dislocation of the medial four carpometacarpal joint in a child and review of the literature.

Traumatic dislocation of the medial four carpometacarpal joints in a child is an unusual injury. Descriptions of this injury in the literature have been reviewed and a case is presented to illustrate the pertinent anatomy, biomechanical aspects, and details of management. Specifically, the importance of collateral circulation to the hand and an effective technique of pin fixation are described.

The ligaments of the wrist.

The ligaments of the wrist were studied by dissecting ten fixed and seven fresh frozen wrists. In three other specimens multiple cross-sections were prepared. These studies show that the wrist ligaments can be classified into two groups: extrinsic and intrinsic. In the extrinsic group, the deep volar radiocarpal ligaments are three strong and very important structures connecting the radius to the capitate, the radius to the lunate, and, in a deeper layer, the radius to both the scaphoid and the lunate. A ligamentous deficiency was noted frequently between the capitate and the lunate. There als

Saving amputated digits. Current status of replantation of fingers and hands.

Since the advent of microsurgery in the 1960's it has become possible to sucessfully repair vessels as small as 0.5 mm in diameter, which makes the replantation of totally severed digits possible. Some centers have reported 50 to 60 percent survival of completely severed digits and up to 100 percent survival of amputated hands and of partially amputed but otherwise non-viable digits that were reattached. In view of this success, severed members should be considered as potentially replantable.THE RECOMMENDED INDICATIONS FOR REPLANTATION ARE: (1) multiple digital amputations at or proximal to th
