A shrapnel migration from a peripheral vein to the right ventricle: case report.

BACKGROUND: Foreign bodies that migrate into the heart may include medical devices dislodged from their original location or, rarely, external particles (shrapnel and other foreign bodies) that penetrate the vein, remain intraluminal, and migrate via the venous blood flow to the right heart. Most reported entry sites of these external foreign bodies were in the torso, thigh, or neck; none of them penetrated through a distal extremity of the body. We report a case where shrapnel was found in the right ventricle (RV) following penetrating injury to the hand. CASE SUMMARY: An otherwise healthy 2

Midcarpal Impaction Syndromes as a Rare Cause of Ulnar-Sided Wrist Pain: A Review.

Ulnar-sided wrist pain remains a commonly encountered diagnostic challenge, and its successful management requires a comprehensive understanding of the multiple conditions that can present with this symptom. Midcarpal impaction syndromes in the form of Hamato-lunate and Triquetro-hamate impingement have both previously been reported as rare potential causes of ulnar-sided wrist pain. Despite this, they remain poorly recognized and incompletely understood. This article reviews existing literature that describes the diagnosis and management of these clinical entities.

Improving manual dexterity using ergonomic wearable glove in patients with multiple sclerosis: A quasi-randomized clinical trial.

One main problem faced by people with multiple sclerosis (PwMS) is upper limb dysfunction, which can occur in the first decade of the disease and with the highest prevalence of disability in the progressive type of the disease. Then, PwMS may benefit from personalised and intensive treatment as provided by robotic devices. These innovative devices have increasingly been brought into the neurorehabilitation field, due to their ability to provide repetitive and task-oriented training. In this quasi-randomized study, we aim to evaluate the effects of robotic-assisted hand training, using the Hand

Factors influencing surgical treatment of De Quervain's tendinopathy: A retrospective cross-sectional observational study.

BACKGROUND: The literature surrounding how different patient, sociodemographic, and anatomical factors influence surgical treatment of De Quervain's tendinopathy (DQT) is limited. PURPOSE: We hypothesised that different patient, anatomical, or sociodemographic factors influence the management of DQT with regard to non-operative vs. surgical management. METHODS: This retrospective cross-sectional study reviewed 155 cases of patients with DQT seen over a 10 year period. Patient-specific factors included age, gender, hand affected, dominant hand, steroid injection given and mean number of injec

Arthroscopic algorithm for acute traumatic triangular fibrocartilage complex (TFCC) tears.

The triangular fibrocartilage complex (TFCC) is crucial for stability and acts as a shock absorber and load transmitter at the distal radioulnar joint (DRUJ). It is often injured in wrist trauma, particularly in young athletes. Clinical assessment involves patient history, physical examination, and imaging modalities like MRI, with wrist arthroscopy as the gold standard for diagnosing TFCC tears. Multiple classification systems categorize TFCC tears based on location and arthroscopic appearance, guiding treatment decisions. Surgical options are recommended for tears refractory to conservative

Improving Patient Care and Streamlining Follow-Up: Compliance With National Institute for Health and Care Excellence (NICE) Guidelines for Pediatric Distal Radius Buckle Fractures.

INTRODUCTION: Torus fractures, also known as buckle fractures, are among the most common types of fractures seen in children who present to the emergency department (ED). These injuries usually occur when a child falls onto an outstretched hand, resulting in the compression and buckling of the dorsal cortex of the radius while the volar cortex remains intact. These fractures generally have a good prognosis and heal well with simple immobilization with a low risk of complications. However, current treatment practices often involve using a rigid cast and scheduling multiple follow-up clinic visi

Wide Awake Local Anaesthesia No Tourniquet Surgery of the Foot and Ankle: A Review of Indications, Technique, Patient Satisfaction, and Complications.

Wide-awake surgery of the hand was surrounded by a lot of apprehension, mainly over concerns around using epinephrine near digits and its potential to cause digital ischemia and necrosis. With multiple reports underlining its safety and effectiveness, it is now being widely adopted in hand and wrist surgery. The British Society for Surgery of the Hand has already published guidelines on operating outside of main theatres, with an emphasis on wide awake local anaesthesia no tourniquet (WALANT). However, the same cannot be said for its use in foot and ankle procedures. There have been a handful

Expanding functional workspace for people with C5-C7 spinal cord injury with supernumerary dorsal grasping.

Spinal cord injuries (SCI) substantially affect sensory, motor, and autonomous functions below the level of injury, reducing the independence and quality of life for affected individuals. Specifically, people with SCI between C5 and C7 cervical levels encounter limitations in voluntary finger and wrist flexion, reducing grasp capability. Compensatory strategies like tenodesis grasp, whereby wrist extension passively closes the fingers, remain; this is effective for small and light objects but insufficient for heavier ones. Typically, wearable assistive exoskeletons are designed to actuate a pe

Spaghetti Wrist Injury: Past, Present, and Future.

This study will present our experience in treating "spaghetti wrist," a complex injury of the distal forearm, with laceration of tendons, nerve(s), artery(es), and soft tissues. Also, we will present a review of the literature summarizing preoperative preparations, surgical and rehabilitation therapy, and immediate and delayed postoperative outcomes. The patients had complex injuries located in the distal 1/3 of the forearm, regardless of the physical etiologic agent, encompassing breach of the deep antebrachial fascia, at least one nerve, tendons, and, in some cases, arteries, along with sing

Complications of flexor tendon repair.

This article reviews and highlights complications of flexor tendon repairs. Although the outcomes of flexor tendon repairs have improved over the years, fair or poor functional outcomes are seen, especially in patients whose trauma involves multiple structures of the hand and in zone 5 with multiple tendon lacerations. Rupture of the flexor tendon after repair is no longer a major problem if current repair principles are carefully adhered to. Different degrees of adhesion formations and interphalangeal (IP) joint stiffness still occur in a few patients. Early active postoperative mobilization

Arthroscopic Management of an Extremely Rare Wrist Fracture-Dislocation Pattern.

BACKGROUND: Axial dislocation involves the displacement of carpal bones along the longitudinal axis of the wrist joint. There are three major groups: (1) axial ulnar, (2) axial radial, and (3) combined. On the other hand, radiocarpal dislocation entails disruption of the articulation between the radius and carpal bones. There are two groups: (group 1) it is either a pure radiocarpal dislocation or a minor avulsion of the radial styloid and (group 2) a fracture-dislocations characterized by a significant radial styloid fragment. CASE DESCRIPTION: The patient, a 66-year-old female involved in a

Surgical intervention for distal radial fractures with concurrent ulnar styloid fractures may be beneficial for the elderly patients: a retrospective study.

BACKGROUND: Distal radius fractures are highly prevalent among older adults. Ulnar styloid fractures frequently accompany distal radius fractures, raising concerns about treatment outcomes due to the risk of distal radioulnar joint (DRUJ) instability. This study investigated the necessity of surgical intervention (open reduction internal fixation; ORIF) for distal radius fractures in very old patients with coexisting ulnar styloid fractures. MATERIALS AND METHODS: A retrospective analysis of 96 patients aged ≥ 80 years with AO classification 23-A2 to 23-B3 distal radius fractures with concomi

A staged approach to managing a combination of a terrible triad injury and an Essex-Lopresti fracture dislocation: an open forearm crush injury-a case report.

BACKGROUND: Open and crushed forearm injury is a complex and rare injury affecting the upper extremity. It results in damage to various structures, including bones, soft tissues, and neurovascular bundles, ultimately leading to functional impairment. Typically, these injuries occur owing to high-energy trauma. CASE PRESENTATION: A 19-year-old South Asian male was seen at our trauma center emergency department following an accident where his arm got entangled in a machine belt. He sustained a serious open injury on his left forearm, spanning from the elbow to the wrist, affecting the proximal

Evaluation of predictive factors of septic wrist to avoid overdiagnosis.

BACKGROUND: The existing diagnostic criteria for septic wrist are nonspecific, exposing patients with noninfectious etiologies to surgical morbidity. This study aimed to identify predictors differentiating septic wrist from other etiologies. METHODS: An institutional review board-approved retrospective review was conducted on patients with a presumed diagnosis of septic wrist (2003-2022). Bivariate and multiple regression analyses were performed to identify correlation between confirmed septic wrist and comorbidities (autoimmune diseases, immunosuppression, crystalline arthropathy, intravenou

Arthroscopic Management of Metacarpophalangeal Joint Synovial Chondromatosis.

Synovial chondromatosis is a rare condition where cartilaginous nodules form within the synovium, which can detach and cause loose bodies within the joint. While it typically affects larger joints, involvement of the metacarpophalangeal (MCP) joints is rarely described. We report the case of a 42-year-old female patient who had a slow-growing mass in her right third MCP (MCP III) joint, associated with limited finger motion, swelling, and pain. The diagnosis was confirmed using imaging studies. The patient underwent arthroscopic excision of multiple tumors and synovectomy via dorsal radial and
