Endoscopic gluteal tendon repair reduces complication rates while achieving outcomes comparable to open repair: A multilevel meta-analysis.

PURPOSE: The purpose of this systematic review and multilevel meta-analysis was to compare short-term functional outcomes, pain relief, and complication rates between open and endoscopic gluteal tendon repair, expressed as functional improvement normalized to minimal clinically important difference (MCID) units. The hypothesis was that endoscopic repair would provide comparable clinical efficacy with a lower complication rate compared with open repair. METHODS: PubMed, Embase, CENTRAL and Epistemonikos were searched to 15 October 2025. Eligible primary studies reporting postoperative or chang

The Nottingham Hip Fracture Score is an accurate predictor of mortality at 30 days after fragility distal femur fractures.

AIMS: Fragility distal femur fractures have comparable mortality outcomes to fragility hip fractures. Increasing scrutiny is being placed on the perioperative management of these injuries to improve mortality and morbidity outcomes. A key pillar of preoperative consent is the explanation of adverse events associated with the underlying pathology and/or suggested treatment course. The Nottingham Hip Fracture Score (NHFS) is a validated tool used to predict mortality after a fragility hip fracture and has been shown to be predictive of mortality after fragility odontoid peg fractures. This study

Total Hip Replacement Following Unsuccessful Acetabular Repair: A Case Report.

INTRODUCTION: Traumatic avascular necrosis (AVN) of the femoral head is a major issue after acetabular fractures, affecting 20-40% of cases even with precise joint realignment. Performing total hip replacement (THR) as a salvage procedure introduces specific surgical obstacles absent in standard primary THR. CASE REPORT: This report covers a 26-year-old man injured in a motor vehicle collision with an acetabular fracture. After open reduction and internal fixation (ORIF), he had femoral head AVN with superior migration and absorption of superolateral acetabular rim at 18 months, requiring THR

Preoperative joint line convergence angle predicts correction accuracy after medial open-wedge high tibial osteotomy.

BACKGROUND: This study investigated whether the preoperative joint line convergence angle (JLCA) influences post-operative correction accuracy and clinical results in knee osteoarthritis (KOA) patients undergoing medial open-wedge high tibial osteotomy (MOWHTO). METHODS: A total of 169 knees that underwent MOWHTO were followed up for a minimum of 2 years. Preoperative and 2-year postoperative radiographic parameters were used to measure JLCA and the hip–knee–ankle angle (HKA). A post-operative HKA valgus of 0–4° was defined as accurate correction; values outside this range were considered abno

An Exceptionally Uncommon Hip Injury: Femoral Neck Fracture Dislocation with Abductor Avulsion: A Case Report.

CASE: This report presents an extremely rare case of a femoral neck fracture dislocation with associated complete hip abductor avulsion. The patient was treated with open reduction of the dislocation, open reduction internal fixation (ORIF) of the femoral neck, and repair of the hip abductors. The patient initially healed but developed avascular necrosis (AVN) requiring conversion to total hip arthroplasty at 3 years. A Harris Hip Score was 92 eleven years after injury. CONCLUSION: We recommend prompt, anatomic ORIF of the femoral neck fracture dislocation, direct repair of the hip abductor c

Independent Risk Factors for Lag Screw Cut-Out: The Role of Open Reduction in Intertrochanteric Fracture Fixation With the Proximal Femoral Nail.

BACKGROUND Intertrochanteric femoral fractures are common in older adults, and proximal femoral nail (PFN) fixation is widely preferred due to its biomechanical advantages. Lag screw cut-out, defined as the progressive migration of the lag screw through the femoral head with loss of fixation, is one of the most serious mechanical complications after PFN fixation. This complication can result in mechanical failure and require revision surgery. Therefore, this study aimed to identify independent risk factors for lag screw cut-out and evaluate lag screw stability in patients without cut-out. MATE

Acute Total Hip Arthroplasty with or Without Internal Fixation for Acetabular Fractures in the Elderly: A Case Series.

Background and Objectives: Acetabular fractures in elderly patients are increasing in incidence and are frequently associated with osteoporotic bone, fracture comminution, marginal impaction, and pre-existing joint degeneration. Open reduction and internal fixation (ORIF) alone in this population is associated with high rates of fixation failure, post-traumatic osteoarthritis, and secondary conversion to total hip arthroplasty (THA). Acute THA, with or without concomitant internal fixation, has emerged as an alternative strategy aimed at enabling early mobilization and reducing reoperation rat

Knotted Versus Knotless Suture Bridge Repair of the Proximal Rectus Femoris: A Biomechanical Study.

BACKGROUND: A proximal rectus femoris (PRF) avulsion is primarily seen among elite-level athletes, which may require surgery after failed nonoperative treatment. Although suture bridge repair is a common surgical technique to treat these injuries, it is unclear whether a knotted or knotless suture bridge construct is biomechanically superior. PURPOSE: To biomechanically compare the failure load, stiffness, and number of cycles until failure of knotted versus knotless suture bridge repair for PRF avulsions. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 8 pairs of fresh-froze

Endovascular repair of external iliac vein injury caused by acetabular screw penetration during total hip arthroplasty: a case report.

INTRODUCTION: Total hip arthroplasty (THA) is a common orthopedic procedure with low complication rates. However, vascular injuries, though rare (0.1%-0.3%), can lead to life-threatening hemorrhage and hemodynamic instability. External iliac vein (EIV) injuries during THA are exceptionally uncommon, and optimal management remains a clinical challenge. PRESENTATION OF CASE: We report a 62-year-old woman with diabetes and hypertension who underwent right THA for severe osteoarthritis. Intraoperatively, a significant hemorrhage occurred due to the acetabular screw penetration into the right EIV.
