BACKGROUND: Femoroacetabular impingement (FAI) is a common cause of hip pain in young, active athletes. American football players place high demands on the hip due to the sport's requirements for pivoting, cutting, and loading in deep flexion. Although arthroscopic treatment of FAI in professional National Football League (NFL) players has shown favorable outcomes, football-specific outcomes in non-NFL athletes remain limited.
PURPOSE: To report the return-to-sport (RTS) rates and functional outcomes at a minimum 2-year follow-up for active American football players competing at multiple leve

Acetabular fractures are becoming more common, especially in the elderly population. Standard goals of management include restoring stability and articular congruity to the hip joint in an effort to minimize the risk of posttraumatic arthritis. This is most commonly achieved through open reduction and internal fixation, from which outcomes are generally positive. However, several preoperative risk factors have been identified that correlate with poor outcomes, such as severe acetabular impaction, femoral-sided lesions, and older age. Furthermore, goals of management in the increasingly encount

AIMS: Fragility hip fractures are a growing concern, increasing year on year by just over 1%. These injuries carry high mortality rates and often result in poor functional outcomes, especially in frail patients. Early mobilization after surgery has been linked to better survival and shorter hospital stays. While Enhanced Recovery After Surgery (ERAS) protocols are well established in elective orthopaedics, their application in hip fracture care remains limited due to the frailty of this patient group.

BACKGROUND: Periprosthetic proximal femoral fractures (PPFF) are expected to increase substantially due to demographic changes and the rising number of total hip arthroplasties. While early surgery is clearly recommended for native hip fractures, the optimal timing of surgery for PPFF remains controversial. The present study aimed to evaluate the effect of early versus delayed surgery on perioperative outcomes in patients with PPFF treated by open reduction and internal fixation (ORIF).
METHODS: We conducted a retrospective analysis of 110 patients with PPFF who underwent ORIF at a Level 1 tr

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a common pediatric hip disorder primarily managed with transphyseal screw fixation. Opioid pain medication is often used postoperatively, but there is little evidence to guide prescribing practices. The purpose of this study was to describe opioid utilization and prescribing practices for patients after screw fixation of SCFE.
METHODS: This was a prospective observational study of patients with SCFE who underwent transphyseal screw fixation. Patients and guardians were contacted on postoperative days 1, 3, and 5 for data collection. Post

BACKGROUND: Periprosthetic femur fractures (PFF) are a significant complication associated with Total Hip Arthroplasty (THA) and Hemiarthroplasty (HA). There remains a gap in the literature regarding a consensus on which method of stem fixation yields higher rates of periprosthetic fractures—cemented or cementless. The aim of this study is to analyze the rates of comminution, stem loosening and requirements for full femoral component revision in periprosthetic femoral fractures when comparing cemented implants to cementless implants. METHODS: A retrospective cohort study of all femoral PFFs be

BACKGROUND: A thorough understanding of posterior hip anatomy is essential for safe and effective posterior hip endoscopy. The sciatic nerve runs laterally to the ischium and hamstring origin. While sciatic nerve injury during proximal hamstring repair is uncommon, the increasing use of endoscopic techniques has led to a rise in iatrogenic injuries. This highlights the need for a structured approach to identifying and protecting the sciatic nerve.
INDICATIONS: Surgical intervention is indicated for complete proximal avulsions involving all 3 hamstring tendons, partial avulsions involving ≥2 t

PURPOSE: The purpose of this systematic review and multilevel meta-analysis was to compare short-term functional outcomes, pain relief, and complication rates between open and endoscopic gluteal tendon repair, expressed as functional improvement normalized to minimal clinically important difference (MCID) units. The hypothesis was that endoscopic repair would provide comparable clinical efficacy with a lower complication rate compared with open repair.
METHODS: PubMed, Embase, CENTRAL and Epistemonikos were searched to 15 October 2025. Eligible primary studies reporting postoperative or chang

AIMS: Fragility distal femur fractures have comparable mortality outcomes to fragility hip fractures. Increasing scrutiny is being placed on the perioperative management of these injuries to improve mortality and morbidity outcomes. A key pillar of preoperative consent is the explanation of adverse events associated with the underlying pathology and/or suggested treatment course. The Nottingham Hip Fracture Score (NHFS) is a validated tool used to predict mortality after a fragility hip fracture and has been shown to be predictive of mortality after fragility odontoid peg fractures. This study

INTRODUCTION: Traumatic avascular necrosis (AVN) of the femoral head is a major issue after acetabular fractures, affecting 20-40% of cases even with precise joint realignment. Performing total hip replacement (THR) as a salvage procedure introduces specific surgical obstacles absent in standard primary THR.
CASE REPORT: This report covers a 26-year-old man injured in a motor vehicle collision with an acetabular fracture. After open reduction and internal fixation (ORIF), he had femoral head AVN with superior migration and absorption of superolateral acetabular rim at 18 months, requiring THR

BACKGROUND: This study investigated whether the preoperative joint line convergence angle (JLCA) influences post-operative correction accuracy and clinical results in knee osteoarthritis (KOA) patients undergoing medial open-wedge high tibial osteotomy (MOWHTO). METHODS: A total of 169 knees that underwent MOWHTO were followed up for a minimum of 2 years. Preoperative and 2-year postoperative radiographic parameters were used to measure JLCA and the hip–knee–ankle angle (HKA). A post-operative HKA valgus of 0–4° was defined as accurate correction; values outside this range were considered abno

CASE: This report presents an extremely rare case of a femoral neck fracture dislocation with associated complete hip abductor avulsion. The patient was treated with open reduction of the dislocation, open reduction internal fixation (ORIF) of the femoral neck, and repair of the hip abductors. The patient initially healed but developed avascular necrosis (AVN) requiring conversion to total hip arthroplasty at 3 years. A Harris Hip Score was 92 eleven years after injury.
CONCLUSION: We recommend prompt, anatomic ORIF of the femoral neck fracture dislocation, direct repair of the hip abductor c

BACKGROUND Intertrochanteric femoral fractures are common in older adults, and proximal femoral nail (PFN) fixation is widely preferred due to its biomechanical advantages. Lag screw cut-out, defined as the progressive migration of the lag screw through the femoral head with loss of fixation, is one of the most serious mechanical complications after PFN fixation. This complication can result in mechanical failure and require revision surgery. Therefore, this study aimed to identify independent risk factors for lag screw cut-out and evaluate lag screw stability in patients without cut-out. MATE

BACKGROUND AND PURPOSE: Currently, there is no specific surgical treatment strategy established for spastic hip displacement in ambulatory patients with cerebral palsy. We aimed to evaluate the outcomes of our hip reconstructions, specifically designed to address hip displacement within the context of single-event multilevel surgery (SEMLS).
METHODS: We conducted a retrospective study on patients with Gross Motor Function Classification System (GMFCS) levels II (n = 27) and III (n = 28). Surgical procedures involved various combinations of open or closed reduction, iliac osteotomy, proximal f
