Using pedicle flaps for donor-site closure after harvesting subscapular artery flap in pediatric patients.

BACKGROUND: Flaps based on the subscapular artery are versatile options in reconstructive surgery. However, after extensive flap harvest, primary donor-site closure may be difficult, and skin grafting may be required. In pediatric patients, skin grafting may be associated with additional donor-site morbidity, scar contracture, contour mismatch, and potential growth-related concerns. Although local pedicled flaps have been used for donor-site closure in regions such as the anterolateral thigh and forearm, their application after subscapular artery system flap harvest remains limited. METHODS:

Deep Inferior Epigastric Artery and Vein Interposition Grafts for Free Anterolateral Thigh Flap Reconstruction.

Free flaps for coverage of soft tissue wounds of the lower extremity are often used in the setting of large, complex wounds or failed local flaps. However, in cases of insufficient pedicle length or poor recipient vessel availability, interposition grafts (IGs) may be used to extend the range of flaps. The deep inferior epigastric artery and vein are typically harvested for breast reconstruction with free deep inferior epigastric artery perforator flaps. However, their use as IGs has been described, albeit sparsely, mostly in non-deep inferior epigastric artery perforator breast reconstruction

First Free Anterolateral Thigh Flap for Facial Reconstruction Following Bear Mauling in Bhutan.

Approximately 70% of Bhutan is forested, providing habitat for the Himalayan black bear. A farmer who was mauled by a bear experienced significant tissue loss affecting his left forehead, eyebrow, upper and lower eyelids, and cheek. Immediate management included wound irrigation, antibiotic prophylaxis, and soft-tissue reconstruction using an anterolateral thigh fasciocutaneous flap for coverage. Flap reconstruction facilitated ocular salvage, with retention of excellent visual acuity and a cosmetically acceptable, functional facial outcome. The flap also provided protection and function of th

A Case of Semicircular Lipoatrophy.

We present a case of a 34-year-old woman with semicircular lipoatrophy, characterized by linear skin depressions in a semicircular band-like atrophy on the anterolateral aspects of the thighs. It is still debated whether this type of occurrence is rare or simply underreported because it does not cause overt suffering to the patient, but its estimated incidence has been increasing. The most plausible explanation appears to be repeated external microtrauma, as avoiding its repetition results in complete remission of semicircular lipoatrophy in most cases.

The Association Between Hip Flexibility and Low Back Pain in Ice Hockey Players.

BACKGROUND: Hip flexor tightness is common in ice hockey players because of postural demands. It remains unknown whether hip tightness plays a role in the occurrence of low back pain (LBP). HYPOTHESIS: It was hypothesized that ice hockey players with hip flexor tightness would be at increased risk of developing LBP. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Preseason hip flexor tightness was measured annually for 4 seasons in a boy's high school team, a college men's team, and a college women's team providing a total of 289 player-seasons. A digital level was placed on the

Segmental femoral fracture management using intramedullary nailing combined with plate fixation: a case report.

INTRODUCTION: Segmental femoral fractures are challenging, with a high risk of non-union, requiring tailored treatments such as intramedullary nailing (IMN) and plate fixation. Although this combined approach is promising for tibial fractures, its effectiveness in segmental femoral fractures remains under evaluation. This study aims to evaluate the success rates of IMN and plate fixation in treating these challenging injuries. PRESENTATION OF CASE: A 52-year-old man presented with a deformed, shortened left thigh, moderate tenderness (Visual Analogue Scale score of 4), and limited hip and kne

Metal implantation and high-grade osteosarcoma of the femur: a case report and review of the literature.

BACKGROUND: Osteosarcoma is the most common primary malignant bone tumor, typically affecting adolescents and young adults. While its development is often associated with rapid bone growth and genetic factors, the potential link between metal implants and osteosarcoma remains unclear and rarely reported. This case highlights the unusual occurrence of high-grade osteosarcoma arising at the site of a long-standing metal implant, emphasizing the importance of long-term vigilance in patients with orthopedic hardware. Because implant-associated sarcomas are exceptionally uncommon and causality cann

Topical gene therapy for severe recessive dystrophic epidermolysis bullosa: clinical outcomes and multidisciplinary management in three patients treated with beremagene geperpavec (B-VEC) in Italy - a case series.

Recessive dystrophic epidermolysis bullosa is a severe inherited blistering disorder characterized by chronic wounds, fibrosis and systemic complications. Beremagene geperpavec (B-VEC), a topical HSV1-based gene therapy delivering full-length COL7A1, has shown efficacy in clinical trials; however, real-world data remain limited. This retrospective, descriptive case series reports on three patients with genetically confirmed recessive dystrophic epidermolysis bullosa - two children and one young adult - treated between 2024 and 2026 at two Italian tertiary centres. All patients had extensive ch

Clinical outcomes after surgical treatment of open thigh muscular tears in skeletally immature patients.

Open thigh muscle lacerations in skeletally immature patients are rare, and evidence on optimal management and outcomes is limited. Among sports, skiing represents a frequent mechanism of injury due to sharp ski edges, yet data on pediatric cases remain scarce. We aimed to describe injury patterns, surgical management, and recovery of pediatric patients with open quadriceps tears. In a retrospective two-center cohort, we included skeletally immature patients with open thigh muscular lacerations treated surgically. Mechanism of injury, contamination status, muscle involvement, and complications

Reconstruction of chronic Achilles tendon ruptures using a doubled peroneus longus tendon free graft and interference screw fixation: a technical note.

BACKGROUND: The surgical management of chronic Achilles tendon ruptures (CATR) with significant tissue defects (Myerson Type II/III) presents a clinical challenge. Traditional reconstructive methods relying on free hamstring autografts or flexor hallucis longus transfers are often limited by donor-site morbidity, infection risks, or are unavailable in patients with a history of anterior cruciate ligament (ACL) reconstruction. This technical note describes a refined, tissue-preserving surgical technique utilising a doubled free autologous peroneus longus (PL) tendon graft. METHODS: Under regio

Ankle arthrodesis following bone transport as a definitive limb salvage strategy after multiple failed microsurgical reconstructions for infected tibial pilon fracture nonunion: a case report.

BACKGROUND: Reconstruction of large segmental tibial bone defects remains one of the most challenging problems in orthopedic trauma surgery, particularly when complicated by infection and soft tissue compromise. Various techniques, including vascularized fibular grafting, induced membrane formation, and bone transport, have been described for limb salvage in such complex situations. Bone transport based on distraction osteogenesis enables biological reconstruction of bone defects while simultaneously improving the local biological environment. Clinical and radiographic outcomes of robotic-assisted direct pars repairs for spondylolysis.

BACKGROUND: Spondylolysis is one of the most common identifiable causes of low back pain in young athletes. Direct pars repair offers a motion-preserving surgical option, and robotic-assisted minimally invasive surgery may reduce invasiveness and improve precision compared to traditional open techniques. METHODS: We performed a single-center retrospective review of patients with symptomatic spondylolysis who underwent robotic-assisted direct pars repair from 2023 to 2025. A reference array was placed on the spinous process of the affected vertebrae through a < 2 cm midline incision, and screw

When Life-Threatening Injuries Overshadow Soft Tissue Trauma: A Missed Morel-Lavallée Lesion.

Morel-Lavallée lesions are closed, shearing injuries that occur in polytrauma patients, most commonly affecting the thigh, hip, and pelvis due to mobile skin over thick fascia. These lesions are often missed upon initial evaluation due to the severity of accompanying traumatic injuries and the focus on life-threatening conditions. We present a case of a missed Morel-Lavallée lesion in a Level 1 trauma patient and our innovative approach to treatment. A 36-year-old female patient was air-transported to the emergency department after a motorcycle accident, where she was struck by a vehicle. She
