A defatted (thinned) anterolateral thigh flap was designed to reconstruct skin defects requiring thin flap coverage. We used this flap as a free flap for five cases of skin defects, and the outcomes of the reconstructions were all successful. The vascular pedicle of this flap, the cutaneous perforator of the lateral circumflex femoral artery, is about 8 cm long and 2 mm in diameter, and it is ideal for microvascular anastomosis. Closed degloving injuries are uncommon, and the diagnosis may be missed at the time of acute trauma. This study reports on seven patients seen during a 5-year period who presented with a contour deformity a number of months after the initial closed degloving injury. The median time to presentation was 1.4 years. In five of the seven patients, the contour deformity was present on the lateral thigh, and in two patients it occurred on the calf. The records of 43 patients who underwent 64 open bone peg epiphysiodeses for slipped capital femoral epiphysis were retrospectively reviewed. There were 18 unstable and 46 stable slips. The management of tibial gunshot wounds is unique because of the close proximity of the medial aspect of the bone to the skin. The minimal soft-tissue sleeve covering the medial tibia allows bullets to enter medially and strike the bone with greater energy and allows surface contaminants to reach the bone readily unlike areas such as the thigh where soft tissue can dissipate much of the energy. Avulsion fractures of the ischial tuberosity are often mistaken for sprains or a muscle tears. Long urethral strictures raise serious problems in terms of appropriate treatment. Reconstruction techniques using scrotal skin have a high recurrence rate. Leiomyosarcoma is an uncommon tumor that rarely occurred in extremity. Multiple leiomyosarcomas in different extremities is even rarer. We report a 90 year-old male veteran who was found to have four leiomyosarcomas, one in right arm and the other three in right thigh. Sixty closed fractures of the tibia were treated by open reduction and internal fixation with plates and screws. Half the operations were performed with a thigh tourniquet and half without. In the tourniquet group, there were six cases with erythema and induration of the wound; in the other group there were no such complications. Despite negative bacterial cultures, superficial infection of the inflamed wounds was suspected. It is suggested that a tourniquet may predispose tissues to infection, and its use is not recommended during operations for internal fixation of the tibia.

In order to determine the early results of arthroscopically-assisted reconstruction of the anterior cruciate ligament (ACL), a comprehensive follow-up evaluation of 20 athletes with an average postoperative time of 26 months was performed. Clinical knee examination and measurements of thigh circumference, range of motion, and maximum quadriceps/hamstring strength were obtained. A detailed personal interview regarding activity level, subjective ratings, and functional status was conducted. At follow-up, anterior knee laxity was dramatically reduced, with no patients demonstrating a pivot-shift.
