Negative impact of medical ozone therapy on femur fracture healing in a rat model.

INTRODUCTION: The purpose of the study is to evaluate the effect of medical ozone therapy on fracture healing in rats. MATERIALS AND METHODS: 20 male Wistar-Albino rats were randomly divided into two groups as Control Group (n = 10) and Ozone Group (n = 10). The fracture model was created by bilateral femur transverse osteotomy and fixation with an intramedullary Kirschner wire. No medical treatment was applied to the control group, whereas Ozone gas at a dose of 1 cc/kg at a concentration of 20 µg/ml was administered intraperitoneally to the ozone group for 8 weeks. All groups were sacrifice

Pediatric proximal femoral locking plate features superior biomechanical properties compared to those of multiple screws in fixing unstable pediatric femoral neck fractures.

Pediatric femoral neck fractures result primarily from high-energy trauma. Common treatment methods include screw fixation and the placement of a proximal femoral locking plate. However, there is limited biomechanical evidence favoring one method over another for the treatment of unstable fractures. This study aimed to evaluate the biomechanical properties of screws and proximal femoral locking plates for the treatment of unstable pediatric femoral neck fractures using a synthetic bone model. Fourteen synthetic composite femurs were divided into two groups that included screw fixation (S) and

Pediatric Femur Fractures: Flexible Nail, Plate, or Rigid Nail.

Pediatric diaphyseal femur fractures are a fairly common fracture type for which all trained orthopaedic surgeons should develop a treatment framework. Treatment goals are to restore an acceptable limb length, alignment, and rotation. Nuances exist regarding acceptable tolerances depending on the age and remaining growth of the patient. Various treatment modalities exist, including flexible nailing, plate fixation, and rigid nail fixation, each with its own advantages, disadvantages, and limitations. Proper identification of key patient factors and fracture type is important for optimal decisi

Knee Osteoarthritis (KOA) Severity Influences Proximal Femoral Biomechanics and Predicts Pertrochanteric Fracture Instability: A Retrospective Radiographic Study.

Background and Objectives: Intertrochanteric hip fractures (ITFs) are common in older adults and frequently coexist with knee osteoarthritis (KOA). Although both conditions share key biomechanical risk factors, the specific relationship between KOA severity and ITF stability has not been well defined. Recent evidence suggests that degenerative knee changes may alter lower-limb load distribution and increase susceptibility to unstable fracture patterns. This study evaluated whether KOA severity, graded using the Kellgren-Lawrence (KL) system, is associated with ITF stability according to the 20

Treatment of displaced sagittal plane femoral condyle fractures (OTA 33 B1-2) with novel J-plate: technical trick and case series.

Surgical treatment of unicondylar distal femur fractures remains a challenge. Achieving stable fixation to allow early range of motion and weightbearing is complicated by poor bone quality and articular surface comminution, commonly leading to poor outcomes. Multiple treatment strategies have been previously described including small fragment, large fragment, and anatomical plate fixation. However, given the variability of fracture patterns, anatomical implants often lack the appropriate geometry and fixation options required to provide adequate stabilization. With knowledge and application of

Minimally invasive posterior plate osteosynthesis of a periprosthetic distal tibia shaft fracture: case report and review of literature.

INTRODUCTION AND IMPORTANCE: Periprosthetic fractures of the femur and tibia after total knee arthroplasty (TKA) can be difficult to treat due to pre-existing conditions and implant-related factors. The reported rate of periprosthetic fractures of tibia and femur after TKA is between 0.3 to 2.5%, in which the tibia is less frequently affected. CASE PRESENTATION: This case report presents a patient with a distal tibia periprosthetic fracture in the presence of a long-stemmed revision TKA. Surrounding soft tissues were critical due to multiple previous surgeries with existing hardware due to a

Locking plate fixation with fibular allograft augmentation for failed peritrochanteric fractures: A technical note.

The failure cases of peritrochanteric fractures are occasionally encountered due to poor bone stock. For proximal humeral fractures, a strategy using a segment of the fibular allograft (FA) placed endosteally was introduced. Several studies have demonstrated favorable outcomes with FA. Inspired by this, a fixation strategy using FA with a locking plate was performed in patients with unfavorable bone stock conditions. Ten patients with poor bone stock and nonunion of peritrochanteric fractures were treated with FA and a locking plate at a tertiary hospital between 2016 and 2022. We analyzed the

[Cement augmentation in pertrochanteric fractures : Evidence, indication and surgical application].

Cement augmentation of cephalomedullary implants was developed to improve anchorage of the head element in osteoporotic pertrochanteric fractures and to reduce implant-related failure patterns such as cut-out or excessive sliding. The aim of this review article is to provide a clinical classification of cement augmentation in pertrochanteric fractures, with particular focus on clinical and socioeconomic aspects. Biomechanical studies consistently demonstrate improved rotational stability and cut-out resistance of augmented cephalomedullary fixations. Clinically, several prospective studies sho

Age-related differences in liver damage and inflammation after femoral osteotomy.

Fractures of long bones such as the femur are a common and serious health concern in the elderly, triggering immune responses essential for healing but also affecting remote organs like the liver. With age, the risk of fractures and immune imbalance increases, raising the likelihood of organ damage, infections, and mortality. To better understand age-dependent hepatic responses to bone injury, this study investigates early immune responses in the liver following femoral osteotomy, used here as a reproducible model of fracture. In a mouse model, young (17-26 weeks old) and aged (64-72 weeks old

Torsional Stability of the Femoral Neck System for Pauwels Type III Femoral Neck Fracture Fixation: A Finite-element Analysis.

INTRODUCTION: Femoral neck fracture fixation remains challenging, particularly for Pauwels type III fractures that require robust implants to withstand vertical shear forces. The femoral neck system (FNS) offers greater stiffness and more stable load sharing than multiple screw fixation; however, it has a lower load-to-failure than plate systems. We evaluated the torsional stability of the FNS using finite-element analysis. METHODS: We analyzed a transcervical Pauwels type III femoral neck fracture without a fracture gap, fixed with one of three constructs: multiple screw fixation, a dynamic

Functional and Radiological Outcomes of Ipsilateral Femoral Neck and Shaft Fractures Treated with Proximal Femoral Nail Antirotation-2.

BACKGROUND: Ipsilateral femoral neck and shaft fractures present a unique challenge due to their complex biomechanics and the need for optimal implant selection. Traditional dual-implant fixation has been widely used, but a single implant approach, such as the Proximal Femoral Nail Antirotation-2 (PFNA2), offers potential advantages, including reduced surgical time, minimized blood loss, and lower implant-related complications. However, the stability provided by PFNA2 and its impact on early functional recovery remain areas of ongoing investigation. MATERIALS AND METHODS: This prospective stu

Implant fracture of the Trochanteric Fixation Nail Advanced (TFNA) lag screw through a cement fenestration: a case report.

INTRODUCTION: Cephalomedullary nails (CMNs) are commonly used to treat proximal femur fractures. The Trochanteric Fixation Nail Advanced (TFNA, DePuy Synthes) is a CMN system, introduced in 2015, featuring significant design modifications compared to its predecessors. These include a new Ti-15Mo alloy and a fenestrated lag screw that permits cement augmentation of the femoral head. Nevertheless, device-specific failures have emerged. We report a unique mode of failure: breakage of the lag screw through a cement fenestration. CASE PRESENTATION: An 81-year-old woman with an AO/OTA 31-A2.1 pertr

Alternative Osteoporosis Therapies After Bisphosphonate Associated Atypical Femur Fractures: A Retrospective Analysis.

INTRODUCTION: Bisphosphonate medications reduce the risk of osteoporotic fragility fractures; however, prolonged use increases the risk of atypical femur fractures (AFF). As the population ages and the burden of osteoporosis rises, the relative incidence of AFFs is expected to grow. Current expert guidelines recommend discontinuing bisphosphonates and initiating alternative anti-fracture therapies (AFT), such as denosumab, teriparatide, or raloxifene, following an AFF to support skeletal health and reduce future fracture risk. However, it is unclear which patients receive these medications aft

Comparison of treatment options with long intramedullary nail and dynamic hip screws for metastatic subtrochanteric fracture: A cohort study.

BACKGROUND: Pathological subtrochanteric fractures pose challenges due to the high biomechanical demands of this region and the systemic complexity of metastatic disease. At present, the optimal fixation method remains controversial, particularly between intramedullary nailing (IMN) and dynamic hip screw (DHS) fixation. This study compared the mechanical, oncological, and functional outcomes of these two treatment strategies. METHODS: This retrospective study reviewed patients treated for metastatic subtrochanteric fractures at a single tertiary referral center between 2002 and 2018. Demograp

Surgical management of a subtrochanteric fracture in a patient with 50-year longstanding intra-articular hip ankylosis and ischiofemoral synostosis: a case report.

BACKGROUND: We present a rare, previously unreported case of total hip arthroplasty (THA) implantation in a patient with an ankylosed hip and pelvic-femoral synostosis who sustained a subtrochanteric fracture. CASE: A 70-year-old woman had previously undergone right-sided infectious coxitis treatment, leading to hip ankylosis and pelvic-femoral synostosis. The patient had experienced no hip joint movement from her 20s for over 50 years due to bone fusion at 2 levels. Walking was feasible with compensation, given the ankylosis at an "ideal position" of 15° of flexion, 10° of abduction, and neu
