Pertrochanteric Femur Fracture in a Post-Polio Patient treated with a Pediatric Hip Plate: A Case Report.

INTRODUCTION: Fracture fixation in post-polio residual paralysis is technically demanding due to dysplastic proximal femoral anatomy, osteopenia, and distorted trochanteric morphology. Standard cephalomedullary nailing may be impossible when the medullary canal is narrow, or the entry point is malpositioned. CASE REPORT: A 45-year-old man with childhood poliomyelitis and complete paralysis of the right leg sustained an AO 31-A1.2 pertrochanteric fracture. Radiographs revealed coxa valga and a markedly narrow canal, precluding safe cephalomedullary nailing. Closed reduction on a traction table

The Co-Occurrence of Pertrochanteric Fracture and Acute Coronary Syndrome in a Geriatric Patient: A Case Report and Review of the Literature.

BACKGROUND: Managing elderly patients with simultaneous acute cardiovascular and orthopedic emergencies presents a unique challenge. While ST-elevation myocardial infarction (STEMI) requires prompt revascularization and dual antiplatelet therapy (DAPT), pertrochanteric femoral fractures usually necessitate early surgical fixation to reduce morbidity and mortality. However, the combination of these conditions complicates both standard treatment pathways. CASE PRESENTATION: We present the case of an 86-year-old woman admitted after a low-energy fall, with a radiologically confirmed unstable per

Clinical Implications of ChatGPT-assisted Multimodal Pre-operative Assessment in Elderly Pertrochanteric Fracture Patients: An Exploratory Study.

INTRODUCTION: Given the high mortality associated with femoral trochanteric fractures, reliable pre-operative estimation of post-operative death risk is essential. Commonly used scoring systems demonstrate only moderate predictive ability, largely because they rely on limited and sometimes outdated clinical parameters. ChatGPT (Generative Pre-Training Transformer) may be capable of synthesizing diverse pre-operative clinical information within a single multimodal analytical framework. Therefore, this study aimed to (i) Explore whether ChatGPT-based multimodal integration of pre-operative data

Preoperative Geriatric Nutritional Risk Index (GNRI) and Comorbidity Burden as Mortality Risk Markers After Proximal Femoral Nailing in Older Patients with Pertrochanteric Hip Fractures.

Background: Mortality after geriatric hip-fracture surgery remains substantial. This study evaluated whether preoperative GNRI, comorbidity burden, and CBC-derived inflammatory indices were associated with one-year and long-term mortality after PFN for pertrochanteric/intertrochanteric fractures. Methods: In this single-centre retrospective cohort study of prognostic risk markers, 248 PFN records were screened. After excluding six incomplete records and 25 patients aged <65 years, 217 older fracture/surgical episodes formed the time-to-event cohort; 194 were evaluable for binary one-year morta

Protocol of a multi-centre randomized controlled trial to compare pericapsular nerve group block, fascia-iliaca compartment block and femoral nerve block for pain management in patients with a hip fracture in the emergency department (CPFF-ED).

BACKGROUND: Patients presenting in the emergency department (ED) with a hip fracture are often in significant pain. Opioids are frequently prescribed as part of standardized pain protocols, but are associated with many adverse events, especially in the elderly. Regional anaesthesia with a femoral nerve block (FNB), Fascia Iliaca compartment block (FICB) or Pericapsular nerve block (PENG) may provide a reasonable alternative to these patients. We designed a prospective multicentre study to compare the efficacy and safety of these blocks in the ED setting, which has been registered in the Centra

Knee Osteoarthritis (KOA) Severity Influences Proximal Femoral Biomechanics and Predicts Pertrochanteric Fracture Instability: A Retrospective Radiographic Study.

Background and Objectives: Intertrochanteric hip fractures (ITFs) are common in older adults and frequently coexist with knee osteoarthritis (KOA). Although both conditions share key biomechanical risk factors, the specific relationship between KOA severity and ITF stability has not been well defined. Recent evidence suggests that degenerative knee changes may alter lower-limb load distribution and increase susceptibility to unstable fracture patterns. This study evaluated whether KOA severity, graded using the Kellgren-Lawrence (KL) system, is associated with ITF stability according to the 20

[Cement augmentation in pertrochanteric fractures : Evidence, indication and surgical application].

Cement augmentation of cephalomedullary implants was developed to improve anchorage of the head element in osteoporotic pertrochanteric fractures and to reduce implant-related failure patterns such as cut-out or excessive sliding. The aim of this review article is to provide a clinical classification of cement augmentation in pertrochanteric fractures, with particular focus on clinical and socioeconomic aspects. Biomechanical studies consistently demonstrate improved rotational stability and cut-out resistance of augmented cephalomedullary fixations. Clinically, several prospective studies sho

Implant fracture of the Trochanteric Fixation Nail Advanced (TFNA) lag screw through a cement fenestration: a case report.

INTRODUCTION: Cephalomedullary nails (CMNs) are commonly used to treat proximal femur fractures. The Trochanteric Fixation Nail Advanced (TFNA, DePuy Synthes) is a CMN system, introduced in 2015, featuring significant design modifications compared to its predecessors. These include a new Ti-15Mo alloy and a fenestrated lag screw that permits cement augmentation of the femoral head. Nevertheless, device-specific failures have emerged. We report a unique mode of failure: breakage of the lag screw through a cement fenestration. CASE PRESENTATION: An 81-year-old woman with an AO/OTA 31-A2.1 pertr

Proximal Femoral Fractures Following Ipsilateral Transfemoral Amputation in Working-Age Men: A Report of Two Cases.

Lower limb amputation is associated with progressive loss of bone mineral density (BMD) in the residual limb, particularly at the amputated-side proximal femur. However, proximal femoral fractures in working-age individuals with transfemoral amputation who are independently ambulatory with a prosthesis remain insufficiently described. We report the cases of two working-age men who sustained ipsilateral proximal femoral fractures after minor falls at home while not wearing their prostheses. Case 1 sustained a left femoral neck fracture classified as Garden grade I six years after amputation, wh

Pertrochanteric fracture in case of Sheehan syndrome: A rare case report.

Sheehan's syndrome is caused by ischemic necrosis of the pituitary gland following hypo-perfusion to the pituitary gland. The prevalence is around 3.2% in women <40 years of age and is relatively common in developing countries. The diagnosis is made with the help of a suggestive obstetric history, clinical manifestations, laboratory findings of reduced hormone levels, and pertinent radiological features. The treatment is to replace the patient with deficient hormones and prevent the complications associated with it. We present the management of a patient with Sheehan's syndrome with pertrochan

Mortality risk factor in centenarians with proximal femoral fractures.

BACKGROUND: Given the increased life expectancy, an increasing number of proximal femur fractures (PFFs) will occur in centenarian patients. In these patients, PFF has a serious impact on quality of life with high morbidity and mortality and an increase in total cost of care. Therefore, it is important to understand the surgical outcome for this group of patients defining preoperative risk factors. MATERIAL AND METHOD: From January 1, 2010, to December 31, 2020, 33 centenarian patients with PFF were treated with locked nail or hemiarthroplasty. Mortality risk factors were assessed. Several su

Preventive fixation of the greater trochanter in the intramedullary nail for unstable pertrochanteric fractures of the femur: Xander's technique.

INTRODUCTION: Proximal femoral fractures (PFF) represent a global public health burden as more than 10 million cases per year are globally reported. Kyle 3 and 4 or AO/OTA 31.A2 pertrochanteric fractures account for 10-15% of all PFF. This specific fracture pattern is characterized by intrinsic mechanical instability as a result of the presence of a detached greater trochanter fragment and is usually treated using a cephalomedulary nail (CMN). However, the unstable greater trochanter fragment makes the insertion of the CMN guide wire challenging. HYPOTHESIS: The aim of this surgical technical

Maintenance of Postoperative Reduction in Trochanteric Fracture Fixation: A Prospective Comparison of Helical Blades and Screws in a Single Cephalomedullary Nailing System.

BACKGROUND: Cephalomedullary nailing systems, such as the trochanteric femoral nail advanced (TFNa), are standard of care for trochanteric fractures. The TFNa system allows for the use of either a helical blade or a lag screw for femoral neck fixation, but there is ongoing debate regarding which component provides superior outcomes. The aim of this study was to compare the performance of these two components in maintaining fracture reduction and preventing mechanical complications. METHODS: A prospective cohort study enrolled 201 patients aged 50 years or older with type 31A1-31A2 (pertrochan
