Posterior Hip Dislocation with Ipsilateral Subtrochanteric Femur Fracture: A Rare Case Report.

INTRODUCTION: Posterior dislocation of the hip associated with an ipsilateral subtrochanteric femur fracture is a rare and technically challenging injury pattern, most commonly seen after high-energy trauma such as road traffic accidents. The associated proximal femoral fracture disrupts the normal femoral lever arm, making closed reduction difficult or impossible and often necessitating open reduction. Delay in diagnosis or treatment may result in complications such as avascular necrosis of the femoral head, post-traumatic arthritis, sciatic nerve injury, and poor functional outcomes. CASE R

Fracture-Pattern-Guided Implant Selection in Extracapsular Proximal Femoral Fractures: A Descriptive Single-Centre Study of Dynamic Hip Screw and Proximal Femoral Nail Fixation in Older Adults.

INTRODUCTION: Dynamic hip screw (DHS) and proximal femoral nail (PFN) are both used to fix extracapsular proximal femoral fractures, with implant selection in routine practice typically guided by fracture morphology. This study describes perioperative outcomes and the pattern of implant selection in older adults treated at a single UK trauma centre. MATERIALS AND METHODS: This single-centre retrospective descriptive study included all eligible patients aged 65 years or older undergoing fixation of extracapsular proximal femoral fractures during the 12-month study period (1 January to 31 Decem

Micromotion at a femoral osteotomy stabilized with a retrograde interlocking intramedullary nail.

BACKGROUND: Retrograde nails are becoming used increasingly in the trauma setting and the indications are expanding. The goal of this study was to evaluate micromotion at fracture sites in the proximal half of the femur stabilized with a retrograde intramedullary nail to determine the effects of fracture location, canal fill, and screw configuration. METHODS: Composite femurs were loaded in simulated single-leg stance while measurements of micromotion were made using linear variable differential transducers and motion analysis. Part one of the study compared differences by fracture location (

Profunda Femoris Artery Pseudoaneurysm After Proximal Nailing: A Case Report.

Hip fractures in elderly patients are common after low-energy trauma, while vascular complications remain rare but potentially life-threatening. A pseudoaneurysm of the profunda femoris artery may occur when displaced bone fragments come into close contact with the vessel. We report the case of a 68-year-old man who sustained a displaced subtrochanteric fracture treated with gamma nail fixation. The initial postoperative course was uneventful; however, a secondary fall on postoperative day seven led to implant displacement, progressive anemia, and thigh swelling. Computed tomography angiograph

Lateral decubitus positioning was associated with improved efficiency and functional outcome compared with traction table positioning for cephalomedullary nailing of subtrochanteric femur fractures: a retrospective comparative study.

BACKGROUND: Subtrochanteric femur fractures are challenging to treat due to complex anatomy and high mechanical stresses. Cephalomedullary nailing is the standard treatment, often performed with the patient supine on a traction table, but this can prolong setup and cause complications (pressure or nerve injuries). Lateral decubitus positioning without a traction table may facilitate reduction and reduce traction-related risks, yet comparative data in subtrochanteric fractures are limited. This study compared outcomes of lateral decubitus versus traction table positioning for subtrochanteric fr

Factors Affecting Transfusion Rate for Patients After Hip Fracture.

BACKGROUND: Hip fractures are a common indication for red blood cell transfusion among elderly patients. Blood transfusion may be clinically necessary, yet it carries risks and should be anticipated according to patient and surgical characteristics. OBJECTIVE: This retrospective study aimed to identify clinical and perioperative factors associated with the need for blood transfusion among patients hospitalized with hip fracture. METHODS: The study included 150 patients with hip fracture treated at the Orthopedic Department of Bnei Zion Medical Center between April 2014 and June 2015. The tra

Locking plate fixation with fibular allograft augmentation for failed peritrochanteric fractures: A technical note.

The failure cases of peritrochanteric fractures are occasionally encountered due to poor bone stock. For proximal humeral fractures, a strategy using a segment of the fibular allograft (FA) placed endosteally was introduced. Several studies have demonstrated favorable outcomes with FA. Inspired by this, a fixation strategy using FA with a locking plate was performed in patients with unfavorable bone stock conditions. Ten patients with poor bone stock and nonunion of peritrochanteric fractures were treated with FA and a locking plate at a tertiary hospital between 2016 and 2022. We analyzed the

Comparison of treatment options with long intramedullary nail and dynamic hip screws for metastatic subtrochanteric fracture: A cohort study.

BACKGROUND: Pathological subtrochanteric fractures pose challenges due to the high biomechanical demands of this region and the systemic complexity of metastatic disease. At present, the optimal fixation method remains controversial, particularly between intramedullary nailing (IMN) and dynamic hip screw (DHS) fixation. This study compared the mechanical, oncological, and functional outcomes of these two treatment strategies. METHODS: This retrospective study reviewed patients treated for metastatic subtrochanteric fractures at a single tertiary referral center between 2002 and 2018. Demograp

Surgical management of a subtrochanteric fracture in a patient with 50-year longstanding intra-articular hip ankylosis and ischiofemoral synostosis: a case report.

BACKGROUND: We present a rare, previously unreported case of total hip arthroplasty (THA) implantation in a patient with an ankylosed hip and pelvic-femoral synostosis who sustained a subtrochanteric fracture. CASE: A 70-year-old woman had previously undergone right-sided infectious coxitis treatment, leading to hip ankylosis and pelvic-femoral synostosis. The patient had experienced no hip joint movement from her 20s for over 50 years due to bone fusion at 2 levels. Walking was feasible with compensation, given the ankylosis at an "ideal position" of 15° of flexion, 10° of abduction, and neu

Result of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Nail.

Subtrochanteric femoral fractures account about 10.0% to 34.0% of all hip fractures with a high complication rate. Anatomically the fracture is proximal to adequately control with implants for femoral shaft and distal enough to control with implants used for intertrochanteric fractures. Inherent instability and forces of muscles giving the fracture a tendency to varus collapse. Due to high rate of implant failure; extramedullary and intramedullary implants are not suitable these fractures. Recently Proximal femoral nail (PFN) has been applied in treatment of subtrochanteric fractures. It has a

Occult Femoral Arteriovenous Fistula and Pseudoaneurysm after a Gunshot-Associated Subtrochanteric Fracture: Transfusion-Refractory Anemia as the Sentinel Sign - A Case Report.

INTRODUCTION: Infected tibial shaft non-union after internal fixation is a difficult orthopedic problem because infection control, fracture stability, and bone healing must be achieved simultaneously. Ilizarov external fixation is a valuable limb salvage option in such complex cases. This case is important because a successful union was achieved despite persistent infection, skin necrosis, and the need for subsequent skin grafting. CASE REPORT: A 26-year-old Bangladeshi male presented with an infected non-union of the left tibial shaft after plate fixation performed at a private clinic. Posto

Pathological Subtrochanteric Femoral Fracture as the Initial Presentation of Metastatic Thyroid Carcinoma in a Young Adult: A Diagnostic Challenge.

Pathological fractures should be suspected in young adults presenting with low-energy femoral fractures. We report a 39-year-old woman who sustained a subtrochanteric fracture after trivial trauma while exiting a car. Imaging demonstrated multiple lytic lesions involving the femur and pelvis, with pulmonary nodules suspicious for metastases. Laboratory evaluation showed biochemical hyperthyroidism. Intramedullary fixation was performed after discussion with the Oncology Multidisciplinary Team (MDT), and histopathology of femoral reamings confirmed metastatic thyroid carcinoma, likely follicula

Diagnostic challenge of occult subtrochanteric femoral fracture in an elderly patient with negative initial MRI: a case report.

INTRODUCTION AND IMPORTANCE: Occult fractures of the proximal femur represent a significant diagnostic challenge in elderly patients when initial imaging is negative. Delayed diagnosis can lead to fracture progression and increased morbidity. This case highlights the limitations of initial imaging and the importance of maintaining a high index of suspicion in elderly patients presenting with persistent hip pain and an inability to bear weight. CASE PRESENTATION: A 76-year-old female with a background of hypertension and type 2 diabetes mellitus presented with a 3-day history of progressive le

Predictors of Non-union in Subtrochanteric Femur Fractures: A Multivariate Logistic Regression Analysis.

Over the last 20 years, the role of adjuvant pharmacotherapy in optimising outcome in rehabilitation programmes for alcohol-dependent patients has become increasingly evident. New avenues for rational drug treatment have arisen from better understanding of the neurobiological substrates of alcohol dependence, including adaptive changes in amino acid neurotransmitter systems, stimulation of dopamine and opioid peptide systems, and, possibly, changes in serotonergic activity. Disulfiram, naltrexone and acamprosate are currently the only treatments approved for the management of alcohol dependenc

Application of a humeral intramedullary nail for subtrochanteric fracture in a patient with poliomyelitis sequelae: a case report.

OBJECTIVE: To investigate the clinical feasibility and efficacy of using a humeral intramedullary nail for the treatment of subtrochanteric fracture in a patient with ipsilateral poliomyelitis sequelae. METHODS: A case of a 54-year-old female patient with a left subtrochanteric fracture caused by a traffic accident was reported. The patient had a 50-year history of ipsilateral poliomyelitis, leading to developmental deformity and severe stenosis of the femoral medullary canal in the affected limb, which could not accommodate a conventional femoral intramedullary nail. Therefore, we innovative

The Off-Label Use of a Humeral Nail To Treat a Subtrochanteric Femur Fracture: A Case Report.

The use of humeral nails for femoral subtrochanteric fractures is rarely reported. We present a case in which a humeral nail was employed for a subtrochanteric femur fracture alongside contralateral prophylactic stabilization of the femoral neck and shaft.  A 29-year-old female patient with a history of bilateral varus-producing proximal femoral osteotomies, complicated by symptomatic hardware indicating bilateral removal, sustained a subtrochanteric fracture. Given her 98-degree neck-shaft angle, a humeral nail was selected for right femur fixation. She also underwent prophylactic stabilizati
