The Evolving Role of Artificial Intelligence in Fracture Diagnosis and Surgical Planning in Orthopaedics: Current Insights and Future Directions.

Orthopaedic fracture diagnosis and surgical planning are being revolutionised by artificial intelligence (AI) and machine learning (ML). These technologies enable the automated detection and classification of fractures and provide surgeons with AI-assisted pre-operative decision-making support at multiple body sites. The objective of this review is to synthesise current evidence (from 2016 to 2026) related to the application of AI/ML technologies in orthopaedic fracture diagnosis and surgical planning, to summarise the primary clinical findings from these studies, and to describe the challenge

Critical velocity metric derived from dynamic finite element analysis classifies hip fracture risk in a clinical cohort.

Hip fractures are debilitating and life altering events that substantially reduce quality of life. Areal bone mineral density (aBMD) is currently used to quantify hip fracture risk, along with finite element (FE) based measures such as virtual femur strength (S), and the Fragility Ratio (FR) - which quantifies the nonlinearity of the predicted mechanical bone response. As more sophisticated dynamic FE models become prevalent and can model multiple bones and the soft tissue, these values are either not biomechanically relevant (aBMD), not directly measurable (S), or not easily interpretable (FR

The impact of urinary catheterization on urinary tract infection and renal function in elderly patients undergoing hip fracture surgery: A prospective randomized controlled trial.

INTRODUCTION: Urinary tract infection(UTI) is a common complication following hip fracture surgery in elderly patients. Urinary catheterization is frequently used to improve patient comfort during prolonged preoperative immobilization; however, its impact on UTI risk and renal function remains controversial. This prospective randomized study aimed to evaluate the effect of urinary catheter use on postoperative UTI incidence and renal outcomes in geriatric patients undergoing hip fracture surgery. MATERIALS AND METHODS: This prospective randomized controlled trial included 150 patients aged >

Dose evaluation of medical staff during proximal femur fracture surgery using fluoroscopy.

The use of fluoroscopy in orthopedic interventional procedures has become increasingly common. Although it offers significant clinical advantages by reducing procedural invasiveness, it also exposes both patients and medical staff to ionizing radiation. This study reports occupational dose measurements for surgeons during 40 proximal femur fracture procedures conducted at a reference hospital in Recife, Brazil. The estimated annual effective doses for the leading and auxiliary surgeons were approximately 1.3-1.5 mSv, corresponding to about 7.5% of the annual dose limit (n= 325 procedures yr-1)

Operative Treatment Versus Spica Casting for Treatment of Pediatric Femoral Shaft Fractures in Children Aged 4 to 6 Years: A Large Multi-Institutional Age-Stratified Comparative Analysis.

BACKGROUND: Femoral shaft fractures in children aged 4 to 6 years represent a clinical gray zone between closed reduction with casting and operative treatment. Traditional guidance favors closed treatment under age 6, yet recent reports suggest increasing use of operative treatment despite limited evidence. This study characterized national treatment rates from 2013 to 2024 and compared health care utilization outcomes. METHODS: This retrospective cohort study used the TriNetX Research Network, a federated platform containing nationwide deidentified health records. Patients from 2013 to 2024

Arterial hepatic perfusion is preserved during early resuscitation after hemorrhagic shock in a large animal polytrauma model.

BACKGROUND: Hemorrhagic shock is a major contributor to morbidity and mortality after polytrauma and plays a central role in the development of multiple organ dysfunction syndrome (MODS). The liver is particularly vulnerable due to its high metabolic demand and dependence on adequate oxygen delivery, and hepatic dysfunction is associated with poor outcomes after trauma. This study aimed to assess hepatic perfusion and biochemical liver function following hemorrhagic shock without direct hepatic trauma in a standardized porcine polytrauma model. METHODS: 32 landrace pigs were used in the curre

Multi-ligament knee reconstruction following complex distal femur intraarticular fracture - Case report.

Multiple-ligament injuries of the knee (MLKI), although rare (0.02-0.2% of all orthopaedic injuries), are often devastating. The management of MLKI is intricate and technically demanding, often necessitating a staged approach. In polytrauma situations, MLKI poses considerable challenges to the operating surgeon, especially when combined with complex intra-articular fractures. Accurate initial diagnosis under these circumstances can be difficult, underscoring the in-depth understanding of knee anatomy and injury mechanism for early detection. Furthermore, the presence of multiple fixation devic

Concomitant upper extremity fractures do not worsen outcomes in conservatively managed pelvic fractures: a cohort study of 1840 geriatric patients.

INTRODUCTION: Pelvic fractures in elderly patients are associated with significant pain, functional decline, and prolonged rehabilitation, with the majority managed non-operatively. In the conservative management paradigm, intact upper extremity function is critical for mobilization with assistive devices; concurrent upper extremity (UE) fractures may therefore complicate recovery. The aim of this study was to evaluate the clinical outcomes associated with this injury combination in patients with conservatively managed pelvic fractures. METHODS: A retrospective cohort study was conducted at a

Rare Novel Genetic Variants of the OFD1 Gene Associated With a Familial Form and a Sporadic Case of Long Bone Atypical Fractures.

We describe a family with 11 members affected by oral, facial and digital findings, as well as adult-onset multiple long bone insufficiency fractures mimicking atypical femur fractures (AFF) on radiographs. The analysis of the pedigree of this family suggested an X-linked dominant inheritance. We performed whole-exome sequencing in this family. A rare novel variant, p.Ala642Asp (NM_003611, c.1925C>A, hg19) of the OFD1 gene was the only candidate on the X chromosome segregating with the phenotype within the family. The OFD1 gene targeted sequencing was carried out on 49 patients with AFF and 10

Bone microarchitecture and microfractures in disorganized bone in a young female with pycnodysostosis and atypical femur fractures: A case for novel fracture prediction technology using ALIGNOGRAM1.0 analysis.

Pycnodysostosis (PYCD) is a rare genetic bone disease characterized by impaired osteoclastic bone resorption. The inability to resorb bone leads to sclerosis, with increased bone mineral density (BMD) but paradoxically increased bone fragility. We report a case of a young female with PYCD with multiple fragility fractures including bilateral atypical femur fractures (AFF). Areal and volumetric bone density were supranormal, measured by dual-energy x-ray absorptiometry (DXA) and high-resolution peripheral quantitative computed tomography (HR-pQCT), respectively. However, bone disorganization wa

Efficacy of Tranexamic Acid in a Large Population with Proximal Femur Fractures-A Matched Pair Study of 7394 Patients from the Registry for Geriatric Trauma (ATR-DGU).

Background: Perioperative blood loss represents a major challenge in elderly patients undergoing hip fracture surgery. Although tranexamic acid (TXA) has been shown to reduce bleeding complications, evidence from large registry-based cohorts remains limited. This study assessed the efficacy and safety of perioperative TXA administration in geriatric proximal femoral fracture surgery and evaluated in a subgroup analysis whether repeated TXA administration provides additional benefit compared with a single intraoperative dose. Methods: Data were derived from the Registry for Geriatric Trauma of

Analysis of Postoperative Knee Dysfunction Following a Distal Femur Fracture Managed by Different Surgical Methods.

Background Distal femur fractures, in particular, are complicated injuries that have a major effect on knee biomechanics and functionality. As all fractures primarily affect older females from low-energy falls and younger males from high-energy injuries, patients treated with open reduction-internal fixation (ORIF) and intramedullary nailing (IMN) had their postoperative knee dysfunction and functional results assessed in this study. Objective This study aimed to analyze postoperative knee dysfunction and functional outcomes following distal femur fracture managed by ORIF and IMN and to evalua

Rapid transfer to specialist orthopaedic ward reduces mortality in hip fracture patients, but several factors reduce the ability to achieve the < 4-hour target.

PURPOSE: Hip fractures are one of the most common fracture types in adults. Rapid admission to orthopaedic wards from ED (< 4 h) and optimal timing for surgery within 36 h, is linked with good patient outcomes. This study aims to identify factors that influence achievement of timely admission of elderly hip fracture patients to specialist wards at a UK tertiary centre. Additionally, it seeks to examine the impact of timely admission on 30-day mortality. METHODS: This retrospective, single-centre study of a cohort of 6,170 patients aged over 60 who were admitted to our hospital’s ED with hip fr

Effect of Marker Ball Positioning on Templating Accuracy in Hip Arthroplasty: A Closed-Loop Audit.

Introduction Positioning of external calibration devices (ECDs) in anteroposterior (AP) pelvic radiographs influences magnification correction and therefore the accuracy of digital templating in hip arthroplasty. Orthopaedic departments rely on radiology teams to accurately position ECDs on pre-operative radiographs, but due to multiple factors, placement is not always satisfactory, which may incorrectly guide selection of the hip prosthesis size. This closed-loop audit assessed the accuracy of templating femoral head size in patients with intra-capsular neck of femur (NOF) fractures before an

Consequences of Periprosthetic Fracture After Hemiarthroplasty of the Hip.

Introduction Low-energy femoral neck fracture (FNF) in the elderly is a serious injury. For patients with multiple existing comorbidities and frailty, the risk of complications, poor functional outcomes, and premature death may be higher. The treatment aims for immediate weight-bearing, preferably by hemiarthroplasty (HA). Previous research has focused on prosthetic joint infection and dislocation after HA. In this study, we aimed to describe the possible surgical consequences of periprosthetic fracture after HA. Methods HA patients with a periprosthetic fracture occurring between 2009 and 201
