BACKGROUND: Postmenopausal osteoporosis (PMOP) elevates fracture risk. Zoledronic acid effectively reduces fractures, but individual bone mineral density (BMD) responses vary. Systematic evidence on independent site-specific predictors of BMD changes across femoral neck, total hip, and lumbar spine remains scarce. This study aimed to identify independent factors associated with 12-month BMD changes at the femoral neck (ΔFNBMD), total hip (ΔTHBMD), and lumbar spine (ΔLSBMD) in PMOP patients (T-score ≤ - 2.5) following initial zoledronic acid treatment.
METHODS: A single-center retrospective co

U-type sacral fractures, accounting for 2%-5% of sacral fractures, typically result from falls. In this single-case technical note, a U-type sacral fracture from a motor vehicle crash treated with a novel fixation technique avoiding lumbosacral extension is reported. A 59-year old female with a Roy-Camille type-2 U-type sacral fracture and multiple long-bone injuries, without neurological deficit, was transferred to the university-affiliated center. Despite a severe sacral fracture, she exhibited no neurological deficits. Surgery began supine to address a distal femur fracture; then the patien

BACKGROUND: Fragility Proximal Humerus (PH) fractures are common and often associated with significant morbidity and mortality. Unlike Proximal Femur (PF) fractures, PH fractures are often omitted from established orthogeriatric multidisciplinary team (MDT) pathways. Combined Proximal Humerus and Proximal Femur fractures (PH&PF) are rare. Combined injuries would be expected to result in worse outcomes compared to isolated PH fractures due to greater injury burden. This study assessed outcomes between isolated PH and combined PH&PF fractures, exploring whether differences in care pathways may c

BACKGROUND: Multiple Hereditary Exostoses (MHE) is a rare autosomal-dominant disorder characterized by the development of multiple osteochondromas. While often asymptomatic, these lesions can cause pain and functional limitations due to mechanical impingement or malignant transformation. Osteochondromas of the femoral neck pose unique challenges due to their proximity to critical neurovascular and weight-bearing structures.
CASE PRESENTATION: We present the case of a 43-year-old Italian woman with MHE who developed severe femoroacetabular impingement due to a large osteochondroma located at t

UNLABELLED: Osteoporotic fractures are underrepresented in administrative data. In the present study, only 1 of 94 admissions for osteoporotic fracture was coded correctly, mostly due to inadequate clinical documentation. Following education of junior doctors, 36% of osteoporotic fractures were coded correctly, with coding of femur fractures particularly amenable to improvement.
PURPOSE: Osteoporotic fractures are underrepresented in hospital administrative data. This study analysed the accuracy of clinical coding of osteoporotic fractures and prospectively evaluated the effectiveness of an e

Fibrous dysplasia is one of the most common skeletal lesions. The wide spectrum of clinical manifestations ranges from asymptomatic conditions (typical of monostotic forms) to severe skeletal diseases with deformity and fractures for polyostotic fibrous dysplasia. The classical radiological features include: an osteolytic geographic pattern, ground-glass bone matrix, cortical thinning/cortical scalloping, bone deformities and enlargement, concavity of margins (evaluated with MRI), and cystic areas (MRI). All the bones can be affected, and the proximal femur is the most common one (about 30% of

Multiple myeloma (MM), the second-most frequent hematologic malignancy, is caused by the neoplastic expansion of clonal plasma cells. Up to 80% of MM patients develop myeloma bone disease (MMBD), a hallmark of which is the development of osteolytic lesions that can lead to skeletal related events such as fractures, spinal cord compression and bone pain. In the Vk*MYC mouse model of MM, MYC is expressed in germinal center B cells, facilitating the formation of malignant plasma cells and development of major characteristics of MM, including MMBD. However, the extent of skeletal changes and under

INTRODUCTION: Atypical femoral fractures (AFFs) are stress-type insufficiency fractures classically associated with long-term bisphosphonate therapy in adults. Reports of AFFs in pediatric patients are limited primarily to osteogenesis imperfecta or other monogenic bone disorders following prolonged exposure. We report the first case of an AFF occurring after zoledronic acid administration in a child with ACTG2 gene mutation-related intestinal failure and severe metabolic bone disease.
CASE REPORT: An 8-year-old male with ACTG2-related intestinal failure, total parenteral nutrition dependence

BACKGROUND: Patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and comorbid asthma have more severe disease and are difficult to treat. However, the molecular endotypes associated with CRSwNP with comorbid asthma (CRSwNP + AS) are not clear. This study aimed to investigate the characteristics of type 2 inflammation and the molecular signatures associated with CRSwNP + AS.
METHODS: A total of 195 subjects; including 65 CRSwNP + AS patients, 99 CRSwNP-alone patients, and 31 healthy control subjects; were enrolled in the study. Nasal tissues from patients with CRSwNP + AS, CRSwNP-al

Pathological fractures should be suspected in young adults presenting with low-energy femoral fractures. We report a 39-year-old woman who sustained a subtrochanteric fracture after trivial trauma while exiting a car. Imaging demonstrated multiple lytic lesions involving the femur and pelvis, with pulmonary nodules suspicious for metastases. Laboratory evaluation showed biochemical hyperthyroidism. Intramedullary fixation was performed after discussion with the Oncology Multidisciplinary Team (MDT), and histopathology of femoral reamings confirmed metastatic thyroid carcinoma, likely follicula

INTRODUCTION: Ipsilateral "floating lower limb" (FLL) -coexisting pelvic/acetabular, femoral, and tibial fractures- presents rare, high-energy trauma with complex limb-threat physiology. A standardized diagnostic framework could improve early triage and operative sequencing.
OBJECTIVE: To develop and preliminarily evaluate the diagnostic floating lower limb ipsilateral protocol (D-FLLIP) as an ordinal severity classification and early decision-support tool for FLL.
METHODS: Retrospective, single-center case series at a tertiary trauma referral hospital (January 2018 / December 2023); n=28 co

BACKGROUND Periprosthetic femoral fractures and complications after joint replacement pose serious therapeutic challenges, especially in older patients with multiple comorbidities. In cases where conventional stabilization and revision procedures fail, implantation of a total femoral replacement (TFR) prosthesis may provide a limb-salvage option. CASE REPORT We present the case of a 77-year-old woman with multiple comorbidities who underwent repeated treatment for fractures and complications involving the left femur over a 20-year period. Due to poor bone quality resulting from multiple previo

A 3-month-old female Boxer puppy presented for bilateral inguinal lymph node enlargement, vulvar swelling and hip pain. Pelvic radiographs revealed moth-eaten to permeative lytic lesions affecting the right hemipelvis and right femoral head and neck. An incomplete pathological fracture of the cranial proximal right femur, an incomplete right femoral neck fracture and a suspected folding fracture of the right ischium were also noted. The dog was euthanized and submitted for necropsy. The fractures were confirmed at necropsy and were surrounded by extensive dark red areas in the skeletal muscle

Osteogenesis imperfecta is a genetic disorder caused by defects in type I collagen. Long-bone plastic bowing results from bone fragility, microfractures, deforming muscular forces, stress fractures, and malunion. The primary goal of treatment is deformity correction and stabilization to prevent recurrent fractures and progression. The management of long-bone deformities commonly involves corrective osteotomies with intramedullary stabilization. Although telescopic rods are widely preferred for their ability to accommodate skeletal growth, their availability and cost may limit their use in many

BACKGROUND: Postoperative delirium (POD) is a frequent complication following hip fracture surgery in older adults and is associated with adverse clinical outcomes. Early identification of high-risk patients using the Comprehensive Geriatric Assessment (CGA) may improve perioperative management and reduce morbidity.
OBJECTIVES: To determine POD incidence and identify clinical, geriatric, and perioperative predictors using CGA-based assessment.
MATERIALS AND METHODS: This prospective observational study included 350 patients aged ≥65 years who underwent hemiarthroplasty at a tertiary care hos

BACKGROUND: Cutis laxa is a heterogeneous group of rare connective tissue pathologies characterized by dermal flaccidity and diminished cutaneous elasticity, often associated with phenotypic features of premature aging. The inherited variants of cutis laxa have various modes of genetic transmission and phenotypic heterogeneity and are categorized into three main categories based on inheritance patterns: autosomal dominant, autosomal recessive, and X-linked recessive forms. Autosomal recessive cutis laxa type 1B (ARCL1B) is associated with the gene EGF-containing fibulin-like extracellular matr
