Can N-terminal pro-B-type natriuretic peptide concentration on admission predict the functional independence measure at discharge in patients aged 75 years or older with postoperative proximal femur fracture?

[Purpose] To examine whether functional independence measure scores and effectiveness at discharge can be predicted using N-terminal pro-B-type natriuretic peptide concentrations at admission in postoperative patients aged ≥75 years with proximal femur fractures. [Participants and Methods] This study included 35 patients who were admitted for rehabilitation after proximal femur fracture surgery between April 1, 2020 and September 20, 2023 and were discharged by November 30, 2023. The primary outcomes were the functional independence measure scores and effectiveness at discharge. The explanator

Association between the minimal model of hip structure and risk of hip fracture in Chinese adults.

BACKGROUND: Multiple studies have indicated that the minimal model of hip structure can enhance hip fracture risk assessment. This study aimed to investigate the independent association between minimal model variables and hip fracture risk in Han Chinese individuals. METHODS: This cross-sectional study included 937 Han Chinese patients (248 with hip fractures). Minimal model variables were calculated from the hip structural analysis, including bone mineral density (BMD), femoral neck width (FNW), and Delta and Sigma values. RESULTS: This study included 937 patients (293 men; mean age = 68.3

"Terrible Triad" Injury in an Adolescent Patient With a High-Energy Femoral Shaft Fracture: A Case Report.

Although femoral shaft fractures are typically treated with intramedullary nailing, alternative fixation methods like plating are required in select cases, particularly when nailing may be contraindicated due to respiratory complications or other patient-specific factors. Currently, there are no established guidelines for the optimal surgical fixation method of femoral shaft fractures in polytrauma patients with respiratory dysfunction. We present the case of a young patient with a diaphyseal femur fracture who underwent multiple surgical interventions within three months due to recurrent inju

Simultaneous traumatic bilateral femur fractures in the paediatric population: Case report.

INTRODUCTION: Paediatric femur fractures are well described. Simultaneous bilateral femur fractures are a much more rare occurrence with limited literature on their incidence, treatment protocol and outcomes. This case report outlines the radiological and clinical outcomes of bilateral femur fractures which were managed surgically in our institution. CASE REPORT: We report a 7 year old child that sustained bilateral femur fractures subsequent to an access gate falling on him. He was managed surgically with titanium elastic nailing on the left and a proximal femur locking plate for the right.

A Rare Malignant Case of a Primary Pseudomyogenic Haemangioendothelioma of the Bone.

Pseudomyogenic haemangioendotheliomas (PMH) are exceedingly rare, mostly occurring in soft tissue, with malignant cases even more uncommon. In this report, we present a case of a 28-year-old male initially suspected of having a fibroblastic osteosarcoma of the right femur, which was then correctly diagnosed as a primary pseudomyogenic hemangioendothelioma of the bone with synchronous metastases to other skeletal segments. Molecular analysis through targeted RNA sequencing confirmed the correct diagnosis, revealing a fusion transcript ACTB::FOSB. To our knowledge, this is one of the few reporte

Surgical timing and clinical factor predicting in-hospital mortality in older adults with hip fractures: a neuronal network analysis.

INTRODUCTION: Hip fractures in older adults are associated with a significant mortality rate, which has been reported to be around 35% within a year. Today, the incidence of these fractures is on the rise, and this trend is expected to increase even more owing to the aging of the population. Treatment timing and perioperative management of these patients are typically challenging owing to the presence of multiple comorbidities that are important risk factors for mortality after surgery. This study aims to evaluate the relationship between surgical timing and in-hospital mortality, analyzing th

Combined proximal humerus fractures are associated with greater mortality and morbidity compared with isolated fractures: a retrospective cohort study.

BACKGROUND: Proximal humerus fractures (PHFs) are the third most common type of osteoporotic fracture. One-year mortality rates following PHF are between 4% and 15%. This study examines whether combined fractures involving PHFs and other osteoporotic fractures increase mortality and morbidity rates compared with isolated PHFs. METHODS: This is a retrospective study on patients aged ≥45 years with PHF between January 1, 2011, and December 31, 2020, in Israel's largest health maintenance organization. Fractures were categorized as isolated PHFs or combined fractures involving the proximal humer

Chemical hip denervation using phenol via Pericapsular Nerve Group (PENG) block in palliative non-operative management for frail older hip fracture patients: A multicenter retrospective cohort study.

Effective analgesia is critical yet challenging in non-operative management (NOM) of hip fractures for frail older patients in the palliative setting. Chemical hip denervation with phenol via PEricapsular Nerve Group (PENG) block has been applied to improve pain management. This is the first multicenter retrospective study that evaluated pain experience, opioid consumption and mobility in this population. The study included 185 patients, aged 70 years or older, who received chemical hip denervation with phenol via PENG block in the context of NOM from January 2022 until August 2023 in six hosp

Rickets in the New Age of Food Allergies: A Case Report and Comprehensive Review.

INTRODUCTION: Rickets is a disorder of defective bone mineralization resulting in skeletal deformities, growth retardation, and increased risk of fractures. Nutritional rickets, once a historical disease, has reemerged in select pediatric populations despite modern food fortification efforts. Many of the most common food allergens - dairy, eggs, and fish - are primary dietary sources of vitamin D, which presents a unique risk factor for development of rickets. Children with multiple food allergies are at a heightened risk, particularly if their diets are not adequately supplemented. CASE REPO

High Charlson Comorbidity Index is associated with infection and mortality following surgical treatment of elderly proximal femoral fractures.

BACKGROUND: Proximal femoral fractures (PFF) in the increasingly elderly population have been associated with higher rates of fracture-related infection (FRI) and periprosthetic joint infection (PJI), which are likely to result in permanent functional loss and mortality. This retrospective study seeks to identify independent predisposing factors associated with infection and mortality following surgical intervention in these fractures in the elderly. METHODS: A single-center retrospective cohort study was conducted from January 2019 to December 2023 involving elderly patients who underwent su

The Identification and Work-Up of Nonaccidental Trauma: Practice Variation Across US Children's Hospitals.

BACKGROUND: Nonaccidental trauma (NAT) in children is associated with several warning signs. Since fractures are the second-most common injury caused by NAT, pediatric orthopaedic surgeons are positioned to help identify and treat at-risk children. However, no standardized tools for identifying NAT exist, resulting in national practice variation. The purpose of this study was to compare NAT protocols from Children's Hospitals across the US to understand how institutions vary in screening and treatment. METHODS: We used treatment and identification recommendations for suspected child abuse fro

Use of personalized graft cage and RIA bone graft in a case of acute infection following a grade 2 open tibial shaft fracture: a new option for bone grafting in patients with critical size segmental bone defects.

Critical size segmental bone defects represent a challenging problem. Recently, personalized, structured three-dimensional graft cages have been developed to improve ingrowth of autologous bone material and reduce resorption. A 44-year-old patient presented with an open tibial shaft fracture after a motorcycle incident with minor bone loss. Following intramedullary nailing, an acute infection developed that required removal of ~4 cm bone in a circular mode. After multiple interventions, a personalized 3D-printed graft cage was implanted and filled with autologous bone material, harvested by in

Expeditious femoral nailing prior to vascular repair in fractures associated with vascular injury: A series of four cases.

CASE: Femoral shaft fractures with concomitant vascular injury requiring limb revascularization, although rare, are a limb-threatening condition. Historically, emergent external fixation of the femur fracture followed by vascular repair has been considered the standard of care. We discuss four cases of femoral fracture with an associated vascular injury amenable to nail fixation stabilized by expeditious intramedullary nailing (IMN), followed by limb revascularization. We discuss the timeline and duration of the procedure for this technique. CONCLUSION: Expeditious femoral IMN prior to limb r

Factors associated with the occurrence of postoperative infection of intertrochanteric fracture of the femur and its prediction model creation and validation.

This study explores the risk factors of postoperative infection of intertrochanteric fracture of the femur to construct and validate the prediction model, so as to provide a basis for clinical early intervention. Clinical data of patients with intertrochanteric fracture of the femur treated in our hospital from January 2020 to December 2024 were retrospectively collected. Independent risk factors were screened by univariate and multivariate logistic regression analyses, and a nomogram was constructed based on the regression model. The predictive efficacy was assessed by calculating the area un

Instrumented Timed Up and Go test: A reliable and valid tool for elderly with femur fracture.

BACKGROUND: The Timed Up and Go (TUG) test, commonly used to predict falls, has been enhanced with an inertial sensor, creating the Instrumented TUG (iTUG). Its reliability has been evaluated in a few studies involving neurological and heterogeneous orthopaedic populations. AIM: The aim was to assess the intra-rater, inter-rater, and test-retest reliability of the iTUG in patients with femur fractures and define an interpretative model of the iTUG and a discriminative one to detect patients at risk of fall. METHODS: 201 inpatients with femur fractures underwent the iTUG test before discharge

Triple-Segmental Shaft Femur Fracture: A Case Report.

INTRODUCTION: Segmental femur fractures, an uncommon injury pattern, are usually caused by high-energy trauma such as road traffic accidents. Despite their complexity, early intervention is essential for maximising recovery and reducing complications. CASE REPORT: A 34-year-old male presented with multiple fractures, including a triple-segmental shaft femur fracture, following a high-energy road traffic accident. An intramedullary nail was used for closed reduction and internal fixation of the femur fracture, which provided stable fixation while preserving the blood supply and minimising soft

Reassessing the role of external fixation in polytrauma and severly injured patients with chest trauma and femur fracture: a propensity score-matched analysis from the TraumaRegister DGU®.

INTRODUCTION: Early intramedullary nailing for femur fractures in patients with thoracic trauma has been controversial due to concerns about exacerbating pulmonary complications, particularly in cases of lung contusion. Older studies, relying on conventional radiographs that detected only severe contusions, recommended damage control surgery with external fixation. Today, high-resolution computed tomography (CT) detects even mild lung contusions, potentially overestimating lung damage. Coupled with modern ventilation strategies, this has prompted a reassessment of early definitive fixation ver
