Bilateral Femur Fractures in Children: 14 Years of Experience at a Level I Pediatric Trauma Center.

BACKGROUND: Simultaneous bilateral femur fractures (SBFFs) are an extremely rare type of injury, particularly among pediatric patients. Because of their infrequency, pediatric SBFFs are poorly characterized in the literature. Here, we review our center's experience with managing them. We hypothesized that such injuries were likely to be accompanied by additional traumatic injuries to other organ systems/parts of the body. METHODS: This retrospective, observational cohort study was conducted on all patients who underwent treatment for SBFFs at a freestanding, level I pediatric hospital between

Prevention and treatment of post-irradiation fractures and other complications.

Bone may be irradiated either as a target volume in the treatment of metastases or primary bone tumors, or as an organ at risk in soft tissue sarcomas (STS) and other cancers. This review aims to answer 5 key questions: 1. What are the causes, mechanisms, and locations of post-irradiation fractures (PIF)? Radiotherapy affects bone metabolism and surrounding soft tissues, contributing to fractures and complicating their treatment. In STS, periosteal stripping and radiation doses exceeding 50 Gy increase the risk of PIF, especially in the femur, which is the most frequently affected site. Fractu

Anterior Cortical Impingement With 240 mm Cephalo-Medullary Nail (PFN and PFNA-2) in Intertrochanteric Femur Fracture Fixation in the Indian Population: A Problem Statement.

Trochanteric fractures are common in the elderly population, and their incidence increases twice every decade after age 50. Intramural fixation has achieved good clinical efficacy in the treatment of unstable trochanteric fractures, but there have been complications reported in the literature in the Asian population. Most complications arise from a mismatch between the increase in the anterior femoral bow with advancing age and the proximal femoral nails (PFN) on the market, which still have straight designs on the sagittal plane. The non-anatomic shapes of the PFNs sometimes make the surgerie

Combining Multiple Spectroscopic Techniques to Reveal the Effects of Staphylococcus aureus Infection on Human Bone Tissues.

Osteomyelitis (OM) and periprosthetic joint infections (PJIs) are major public health concerns in Western countries due to increased life expectancy. Infections usually occur due to bacterial spread through fractures, implants, or blood-borne transmission. The pathogens trigger an inflammatory response that hinders bone tissue regeneration. Treatment requires surgical intervention, which involves the precise removal of infected tissue, wound cleansing, and local and systemic antibiotic administration. Staphylococcus aureus (SA) is one of the most common pathogens causing infection-induced OM a

Subchondral stress fracture of the femoral head (SSFFH) in young and healthy military recruits: clinical recommendations and fracture configurations on MRI.

PURPOSE: This study aimed to evaluate the clinical and radiologic characteristics of the fatigue-type of SSFFH in healthy military recruits. MATERIALS AND METHOD: We retrospectively analyzed 39 hips from 32 patients who were treated for SSFFH between 2014 and 2018. Clinical variables were analyzed. We devised a categorization system that divided SSFFH into five types (A-E) according to the extent of the fracture line MRI axial view. The femoral head was divided into three parts for the categorization: the anterior third, middle third, and posterior third. RESULTS: The included patients were

The causal effect of schizophrenia on fractures and bone mineral density: a comprehensive two-sample Mendelian randomization study of European ancestry.

BACKGROUND: Schizophrenia was clinically documented to co-occur with fractures and aberrant bone mineral density (BMD), but the potential causal relationship remained unclear. This study aimed to test the causal effects between schizophrenia and fractures as well as aberrant BMD by conducting Mendelian randomization (MR) analyses. METHODS: Two-sample MR was utilized, based on instrumental variables from large genome-wide association studies (GWAS) of schizophrenia as exposure, to identify the causal association of schizophrenia with mixed fractures, fractures at different body sites (includin

Study protocol: biomechanical testing, finite element analysis and prospective, randomized, clinical study of single screw cephalomedullary nailing versus integrated dual interlocking screw fixation for unstable (31A21-3) intertrochanteric fractures in patients > 70 years old.

BACKGROUND: Hip fractures are an increasingly common consequence of falls in older people that are associated with a high risk of death and reduced function. The vast majority of intertrochanteric fractures require surgical treatment to withstand early mobilization and weight bearing, which prevents complications due to prolonged bed rest and aids in fracture healing. METHODS: This study is compromised by two parts, the experimental study and the clinical part. In the first part, a standard 130° nail will be used with the appropriate lag screw(s) and dynamic distal locking in synthetic osteop

Statistical Properties of a Virtual Cohort for In Silico Trials Generated with a Statistical Anatomy Atlas.

Osteoporosis-related hip fragility fractures are a catastrophic event for patient lives but are not frequently observed in prospective studies, and therefore phase III clinical trials using fractures as primary clinical endpoint require thousands of patients enrolled for several years to reach statistical significance. A novel answer to the large number of subjects needed to reach the desired evidence level is offered by In Silico Trials, that is, the simulation of a clinical trial on a large cohort of virtual patients, monitoring the biomarkers of interest. In this work we investigated if sta

Safe Surgical Dislocation and Open Reduction, Internal Fixation with Herbert Screw for Pipkin's Fracture Head of Femur in an Obese Patient: An Interesting Case Report.

Head of femur fractures are relatively rare and tend to be associated with dislocations of the hip and fractures of the acetabulum. Other parts of the femur, namely the neck is also often involved. Only two cases per million are reported on a yearly basis which poses significance due to its extreme rarity. Here, we present a 30-year-old obese male patient with Pipkin's fracture who was treated successfully by us without any major complications.

Changing epidemiology of lower extremity fractures in adults over a 15-year period - a National Hospital Discharge Registry study.

BACKGROUND: Demographic changes led to an increasingly ageing population in Germany and thus to possible changes in the frequency of fractures. The primary aim of this study was to report changes in fracture rates of the lower extremities in Germany in 2002 compared to 2017 and to evaluate those changes. METHODS: Inpatient data from the German National Hospital Discharge Registry (ICD10) for 2002 and 2017 were evaluated. Changes in total counts and incidence rates were analysed for fractures in the following locations: femoral neck, pertrochanteric, subtrochanteric, distal femur, femoral shaf

Segmental Defect-Bridging Intramedullary Knee Arthrodesis for Osseous Hydatidosis of the Distal Femur: A Case Report.

Hydatid cyst is a condition endemic to many parts of the world and is mainly caused by Echinococcus granulosus ( E. granulosus). It rarely affects the bone tissue, with the most commonly impacted sites being the vertebrae and the pelvis. Preoperative diagnosis is challenging and very rarely possible because of its similarities with other pathologies. In this report, we present the case of a 64-year-old patient with osseous hydatidosis of a pathological distal femur fracture. The fracture pattern was not recognized on the initial operation and multiple serial debridements were required to contr

Lack of periosteal apposition in the head and neck of femur after menopause in Chinese women with high risk for hip fractures - A cross-sectional study with QCT.

In elderly subjects and in particular in those with osteoporosis the evidence on age related volume changes of the hip is still very limited. Even less is known about bone changes of the femoral head. The aim of this study is to explore associations of bone size of the femoral head and neck with age in postmenopausal women with very high risk of hip fracture and to investigate associations of femoral head and neck bone mineral density. MIAF (medical image analysis framework)-Femur was used for the analysis of CT datasets from 319 females with acute hip fractures age 50 to 98. Integral BMD and

Mixed Reality Combined with Three-Dimensional Printing Technology in Total Hip Arthroplasty: An Updated Review with a Preliminary Case Presentation.

Three-dimensional (3D) printing technology, virtual reality, and augmented reality technology have been used to help surgeons to complete complex total hip arthroplasty, while their respective shortcomings limit their further application. With the development of technology, mixed reality (MR) technology has been applied to improve the success rate of complicated hip arthroplasty because of its unique advantages. We presented a case of a 59-year-old man with an intertrochanteric fracture in the left femur, who had received a prior left hip fusion. After admission to our hospital, a left total h

Fat distribution may predict intra- or extra-capsular hip fracture in geriatric patients after falling.

INTRODUCTION: Hip fractures can be divided into intra-capsular and extra-capsular fracture based on fracture location; these two types of fracture have different pathogeneses, treatments and prognoses. Many factors influence the patterns of hip fractures, including the injury mechanism, areal bone mineral density and the geometry of the hip. However, the relationship between body composition and hip fracture pattern has not yet been discussed. In this investigation, an analysis of the body compositions of geriatric patients with hip fractures were conducted to identify differences between fat

Osseointegrated Prosthetic Implants for People With Lower-Limb Amputation: A Health Technology Assessment.

BACKGROUND: Osseointegrated prosthetic implants are biocompatible metal devices that are inserted into the residual bone to integrate with the bone and attach to the external prosthesis, eliminating the need for socket prostheses and the problems that may accompany their use. We conducted a health technology assessment of osseointegrated prosthetic implants, compared with conventional socket prostheses, for people with lower-limb amputation who experience chronic problems with their prosthetic socket, leading to prosthesis intolerance and reduced mobility. Our analysis included an evaluation o

Breakage of metal hip arthroplasty components: Retrieval and structural analysis.

BACKGROUND: Breakage of joint arthroplasty components are rare, yet during an implant retrieval program we found several cases. OBJECTIVE: In this study we examined the components to determine the causes and mechanisms of breakage of these implants. METHODS: From our collection of 849 retrievals we selected 682 cases with metal parts (503 hip, 79 knee arthroplasties) and identified fractured components: seven hip resurfacing implants, five total hip replacement stems, one monopolar femoral head, and one modular revision femoral stem from. Implants were examined using optical and scanning ele

Resection Arthroplasty Treatment Possibility for a Patient with Fibrous Dysplasia, Chronic Osteomyelitis, and Recurrent Periprosthetic Hip Joint Infection: A Case Report.

INTRODUCTION: Idiopathic Transient Osteoporosis (ITO) is a rare entity that may occur during pregnancy, usually manifesting in the third trimester and is considered a diagnosis of exclusion. When present in the hip, it is usually characterized by joint pain, claudicating gait, and osteoporosis of the femoral head with preservation of the joint space. These early clinical and radiological signs may be confused with those of aseptic necrosis, infection, or bone tumors, making it important to exclude these differential diagnoses. Complete resolution of this condition is usually prolonged and may

[Limb salvage or amputation after severe trauma to the lower extremities : Evidence from the LEAP Study].

Deciding between reconstruction and primary amputation after severe high-energy trauma to the lower extremities is difficult and consequential. The Lower Extremity Assessment Project (LEAP) prospectively included and investigated patients with severe, limb-threatening injuries below the femur, with third-grade open fractures, defined soft-tissue damage and amputation wounds. This paper aims to review the key results of the LEAP study, which were published in several parts, in due consideration of the newer relevant literature, and to deduce the consequences for clinical practice. The main resu

Effects of Hip Geometry on Fracture Patterns of Proximal Femur.

BACKGROUND: Some studies have previously shown that geometry of proximal femur can affect the probability of fracture and type of fracture. It happens since the geometry of the proximal femur determines how a force is applied to its different parts. In this study, we have compared proximal femur's geometric characteristics in femoral neck (FNF), intertrochanteric (ITF) and Subtrochanteric (STF) fractures. METHODS: In this study, 60 patients who had hip fractures were studied as case studies. They were divided into FNF, ITF and STF groups based on their fracture types (20 patients in each grou
