A Novel Approach to the Assessment of Limb Loading Symmetry in Adolescent Athletes with and without a History of Ankle Sprains Prior to an ACL-R.

OBJECTIVES: The limb symmetry index (LSI) is commonly used for return-to-sport decisions but focuses primarily on the knee, neglecting intralimb compensations at the ankle and hip. We aimed to evaluate the effectiveness of using energy absorption contributions (EAC) to assess joint loading asymmetry (LSI EAC ). This approach was compared with the conventional method of joint moment analysis in adolescent athletes with and without prior ankle sprains before anterior cruciate ligament reconstruction (ACL-R) and in healthy controls. METHODS: Thirty-nine adolescent athletes were divided into thre

Lower-extremity kinematics during unanticipated side-cutting including before initial ground contact.

BACKGROUND: The biomechanical reasons why lateral ankle sprains (LAS) are more likely to occur during unanticipated side-cutting (UNA-SC) than under anticipated side-cutting (ANT-SC) are unclear. RESEARCH QUESTION: Are there differences in lower-extremity kinematics between ANT-SC and UNA-SC that are relevant to LAS incidence? METHODS: This is a cross-sectional study. Twenty-one male soccer players performed a 45° ANT-SC and UNA-SC. Their ankle, knee, and hip joints and foot segment kinematic data, from 0.2 s before initial ground contact (IC) to toe-off, were analyzed using paired t tests o

Effects of Flywheel Leg Curls on Muscle Structure and Function in Athletes with a History of Hamstring Strain Injury: A Randomized Controlled Trial.

BACKGROUND: Hamstring strain injuries (HSIs) have a high rate of recurrence, highlighting the need for effective tertiary prevention strategies. Flywheel resistance training appears effective in targeting muscular risk factors for HSI. HYPOTHESIS: Flywheel leg curls will result in greater improvements in eccentric knee flexor strength and biceps femoris long head (BFLH) fascicle length compared with conventional leg curls. STUDY DESIGN: Randomized controlled trial. LEVEL OF EVIDENCE: Level 2. METHODS: Twenty-six athletes with a history of HSI within the past 18 months participated in an 8-

The impact of minor crank length adjustments on lower body cycling kinematics.

This study examined the effect of minor crank length adjustments on lower body cycling kinematics and exercise tolerance. Fourteen amateur cyclists performed sub-maximal cycling trials with five different crank lengths, preferred (165.0-172.5 mm), ±5 and 10 mm. An RPE prescription method determined intensity, and three-dimensional kinematics were collected using Vicon motion capture. Statistical parametric mapping was employed to analyse lower body kinematics. Changes in crank length had no effect on the mean power output (199.1 ± 50.5 W). However, minor reductions were associated with signifi

Impact of Fatiguing Exercises on Movement Strategies in Chronic Ankle Instability, Lateral Ankle Sprain Copers, and Controls.

While research exists to induce fatigue using isokinetic dynamometers or simple repetition tasks in patients with chronic ankle instability (CAI), there is a lack of research examining landing movement strategies using fatigue protocols that mimic actual sports. Therefore, we aimed to investigate the effects of CAI and fatiguing exercises on the lower-extremity kinematics and kinetics during single-leg drop landings among patients with CAI, lateral ankle sprain (LAS) copers and controls. A cross-sectional study recruited 20 patients with CAI, 20 LAS copers, and 20 controls in a biomechanics la

Iliotibial band strain while running around a curve.

The iliotibial band (ITB) stabilises the hip and knee joints during running, but excessive strain can lead to iliotibial band syndrome (ITBS), a common overuse injury in runners. This study aimed to determine if running around a curve influences ITB strain using a musculoskeletal model with active and passive ITB strands. Sixteen runners completed trials running straight and around a curve, with kinematic and ground reaction force data collected. An ITB model incorporating gluteus maximus and tensor fascia latae muscles, along with a passive strand, was used. Peak strain values during the stan

Passive hip adduction flexibility impairment measured with the modified Ober test predicts chronicity of iliotibial band syndrome: a cross-sectional pilot study.

OBJECTIVES: To investigate predictive features of passive hip adduction flexibility impairment measured during modified Ober's test on chronicity of pain in runners with Iliotibial Band Syndrome (ITBS). METHODS: Twenty male runners with ITBS were evaluated for self-reported pain characteristics and passive hip adduction angle using a digital inclinometer during the modified Ober test. The receiver operating characteristics (ROC) analysis identified the cutoff value for hip adduction angle that predicts ITBS chronicity, and linear regression determined the relationship between these two variab

[Sports injuries and overuse syndromes of the groin].

Injuries and overuse complaints of the groin are a common issue in competitive and professional sports. Clinically, they typically present as pulling to stabbing pains that radiate to the inner thigh and the outer side of the scrotum. Sports that involve rapid changes of direction, sudden accelerations, and strong rotational movements of the torso are at particular risk for the development of groin complaints. Diagnosis is based on clinical, sonographic, and magnetic resonance imaging (MRI) examinations, and can be quite challenging, requiring an interdisciplinary approach. An inguinal hernia

An 8-week injury prevention exercise program combined with change-of-direction technique training limits movement patterns associated with anterior cruciate ligament injury risk.

Knee ligament sprains are common during change-of-direction (COD) maneuvers in multidirectional team sports. This study aimed to compare the effects of an 8-week injury prevention exercise program containing COD-specific exercises and a similar program containing linear sprint exercises on injury- and performance-related variables during a 135° COD task. We hypothesized that the COD-specific training would lead to (H1) stronger reductions in biomechanical variables associated with anterior cruciate ligament (ACL) injury risk during COD, i.e. knee abduction moment and angle, hip internal rotati

The Value of Strength as a Predictor of Musculoskeletal Injury in Canadian Armed Forces Basic Infantry Candidates.

INTRODUCTION: Musculoskeletal injuries (MSKI) impact military organizations by threatening their operational readiness, warranting investigation into relevant factors to inform risk reduction strategies. While several self-reported and physical performance measures have been associated with MSKI among military personnel, few have been validated and none have been reported in Canadian basic infantry candidates. The purpose of this study was to investigate associations between self-reported and physical performance measures and MSKI, and determine their validity as predictors of MSKI, in Canadia

A biomechanical report of an acute lateral ankle sprain during a handball-specific cutting movement.

Biomechanical measurements of accidental ankle sprain injuries are rare but make important contributions to a more detailed understanding of the injury mechanism. In this case study, we present the kinematics and kinetics of a lateral ankle sprain of a female athlete performing handball-specific fake-and-cut manoeuvres. Three-dimensional kinematics and kinetics were recorded and six previously performed trials were used as reference. Plantarflexion, inversion, and internal rotation angles were substantially larger than the reference trials and peaked between 190 and 200 ms after initial ground

Imaging of soccer injuries in adolescent female athletes.

Soccer participation is increasing among female adolescents with a concomitant rise of injuries in this group. Age- and sex-related factors, such as the transition from skeletal immaturity to skeletal maturity as well as anatomic, physiologic, neuromuscular, and behavioral differences between females and males, all play a role in injury patterns for these athletes. Anterior cruciate ligament (ACL) tears in this group have received a great deal of attention in the medical literature and the media in recent years with increasing knowledge about causative factors, surgical management, and injury

Gender-specific hip strength disparities correlate with injury patterns in NCAA men's and women's soccer players.

BACKGROUND: In National Collegiate Athletic Association (NCAA) soccer athletes, men have higher rates of hip and groin strains, whereas women have higher rates of knee ligament injuries. Strength imbalances of the hip and thigh, specifically in agonist-antagonist muscles, are known risk factors for these injuries. OBJECTIVE: To perform hip and thigh strength assessments in NCAA soccer players to evaluate for differences between genders and correlations with gender-specific injury patterns. DESIGN: With a handheld dynamometer, weight-normalized isometric strength of six muscle groups (hip abd

New Insights Optimize Landing Strategies to Reduce Lower Limb Injury Risk.

Single-leg landing (SL) is often associated with a high injury risk, especially anterior cruciate ligament (ACL) injuries and lateral ankle sprain. This work investigates the relationship between ankle motion patterns (ankle initial contact angle [AICA] and ankle range of motion [AROM]) and the lower limb injury risk during SL, and proposes an optimized landing strategy that can reduce the injury risk. To more realistically revert and simulate the ACL injury mechanics, we developed a knee musculoskeletal model that reverts the ACL ligament to a nonlinear short-term viscoelastic mechanical mech
