Sport-Specific Considerations in ACL Reconstruction: Diagnostic Evaluation and Graft Selection.

Knee biomechanical demands vary across different sports due to sport- and position-specific patterns of muscle recruitment. To return to performance, athletes must adequately restore knee kinematics to regain control over the same sport mechanics that led to the initial anterior cruciate ligament (ACL) injury. ACL graft selection should therefore minimize donor site morbidity and support sport-specific demands. This study aims to address the available evidence and guide surgical graft choice in athletes. A literature search of PubMed, MEDLINE, Scopus, and Web of Science (up to September 2025)

Reconstruction of the Knee Extensor Mechanism Using a Semitendinosus Tendon Graft and a Free Vascularized Latissimus Dorsi Musculocutaneous Flap for Complete Extensor Mechanism Loss With an Extensive Soft Tissue Defect: A Case Report.

A 58-year-old male sustained an injury that resulted in complete loss of the knee extensor mechanism, with an extensive soft tissue defect on the anterior aspect of the knee. Reconstruction of the knee extensor mechanism using an autologous semitendinosus tendon graft and coverage of the soft tissue defect using a free vascularized latissimus dorsi musculocutaneous flap were performed. The patient returned to his previous work six months after surgery without difficulty, except for a slight feeling of exhaustion when going up and down stairs. Recovery of hamstring muscle strength was sufficien

Dermal Allograft Augmentation for Open Gluteus Medius Repair: A Case Series at 2-Year Follow-up.

BACKGROUND: Gluteus medius (GM) tears are a common cause of peritrochanteric regional pain and disability among middle-aged and older populations. While largely successful, when primary repair fails, it is often attributed to poor tendon quality. Dermal allograft augmentation offers a solution for this patient population by enhancing the biological and mechanical properties of the repair construct. PURPOSE: To evaluate the outcomes of open isolated GM repair with dermal allograft augmentation at 2-year follow-up, with a sex-based subanalysis comparing clinical outcomes in male versus female p

Case Report: PET-avid compensatory tensor fascia lata hypertrophy mimicking metastatic disease in a patient with a chronic Achilles tendon rupture.

In this case report, we describe an unusual case of PET-avid muscle enlargement in the right tensor fascia lata (TFL) muscle mimicking metastatic disease during lung cancer staging. The patient, a 67-year-old woman, had a history of left Achilles tendon rupture, which had been managed non-operatively. A PET/CT scan revealed focal fluorodeoxyglucose (FDG) uptake in a markedly expanded right TFL muscle. The presence of concurrent ipsilateral pelvic lymphadenopathy on the scan initially raised concern for muscle metastasis in this context. However, subsequent MRI of the hip showed diffuse muscle

Pre-landing quadriceps vibration improves single-leg landing mechanics under fresh conditions but fails after combined cognitive-physical fatigue.

PURPOSE: To test whether ultrasound-triggered pre-landing quadriceps focal vibration acutely improves knee mechanics and coordination during single-leg drop landings under fresh vs combined cognitive-physical fatigue. METHODS: Twenty-four healthy males (25.4 ± 2.1 years; 175.0 ± 3.1 cm; 73.2 ± 4.9 kg) performed randomized-crossover single-leg forward drop landings. Quadriceps vibration was off or ultrasonically triggered. Landings were tested pre- and post-combined fatigue (15 min dual-task cognitive +90 s burpees). RESULTS: In the pre-fatigued state, vibration increased peak knee flexion an

Comparison between moderate-load and high-load exercises in the rehabilitation of runners with Achilles tendinopathy: Protocol for a blind randomized controlled trial.

BACKGROUND: Achilles tendinopathy is one of the most frequent conditions affecting runners. Exercise-based interventions are the main treatment options for Achilles tendinopathy; however, the ideal exercise dosage remains unknown. OBJECTIVE: The objective of this study is to investigate the effects of a moderate-load exercise intervention compared to a high-load exercise intervention in runners with Achilles tendinopathy in terms of pain, symptom severity, quality of life, muscle strength, and function. METHODS: Sixty amateur runners will be randomly allocated to one of two groups: high-load

Sample entropy detects altered neuromuscular control in female athletes with patellar tendinopathy.

BACKGROUND: Traditional measures of neuromuscular function, such as maximal voluntary isometric contraction strength and hamstring to quadriceps torque ratio, allow for comparison of peak strength between limbs with patellar tendinopathy and limbs without. However, nonlinear analyses evaluate the evolution of a torque signal over time and, therefore, may provide new insights regarding neuromuscular function. The purpose of this study was to determine differences in neuromuscular function between individuals with and without patellar tendinopathy using traditional and nonlinear measures. METHO

A Biomechanical Comparison of Gluteus Medius Repair With and Without Iliotibial Band Autograft Augmentation.

BACKGROUND: Gluteus medius tears are an increasingly recognized cause of lateral hip pain in active individuals, yet no gold standard repair technique has been established, and failure and retear rates remain high. Iliotibial band (ITB) graft augmentation is a validated option in rotator cuff and hip labral repairs for stronger repair construct, but its biomechanical utility in gluteus medius repairs has not been investigated. HYPOTHESIS: It was hypothesized that ITB-augmented gluteus medius repairs would demonstrate superior ultimate load to failure and increased repair stiffness as compared

Case technique report: an allograft "sandwich" reconstruction technique of a massive retracted chronic quadriceps tendon rupture.

INTRODUCTION AND IMPORTANCE: The surgical reconstruction of chronic quadriceps tendon ruptures can be challenging, particularly in the neglected massive retracted tears and in previous failed surgery. Chronic quadriceps tendon ruptures result in the retraction of collagen fibers, leading to a progressive loss of their multilayered structure. The failure of acute quadriceps repair is an uncommon complication, with recorded failures in about 2% (range: 0-8.3%) of cases. CASE PRESENTATION: This report aims to describe a novel reconstruction procedure for chronic quadriceps rupture that uses an a

Can a Clinician Accurately Diagnose Iliopsoas Tendinitis on a Physical Examination?

BACKGROUND: Iliopsoas (IP) tendinitis is an underrecognized but important cause of anterior hip pain in active patients. Some studies have reported that a physical examination (PE) has limited reliability in making a diagnosis of IP tendinitis. Differentiating anterior hip pain due to IP tendinitis from intra-articular sources is critical to appropriately and expediently treat patients. HYPOTHESIS/PURPOSE: PE techniques can detect IP tendinitis, and we sought to quantify the diagnostic utility of these techniques using responses to an ultrasound-guided anesthetic injection as the reference st

The Assessment of the Tendon of Insertion of the Iliopsoas Muscle in Dogs Using Diagnostic Ultrasound Has Good Intraobserver Consistency but Lacks Interobserver Consistency.

The iliopsoas muscle (IPM) is a flexor of the hip joint in dogs that is vulnerable to injury. Intraobserver and interobserver reliability of musculoskeletal ultrasound when evaluating the tendon of insertion of the IPM was studied. The IPM tendons of insertion of a randomly selected cohort were screened by one investigator and recorded. Musculoskeletal ultrasound recordings were separated into two groups of 20 dogs with presumptively normal IPM tendons of insertion and 32 dogs with one or two presumptively abnormal IPM tendons. Recordings were anonymized. The 104 tendons from these 52 dogs wer

Revision anterior cruciate ligament reconstruction using an autogenous quadriceps tendon graft is associated with substantial improvement in patient-reported outcomes.

INTRODUCTION: Revision anterior cruciate ligament reconstruction (rACLR) presents multiple technical challenges, including having to contend with limited graft availability and compromised bone stock. The quadriceps tendon (QT) autograft-often preferred for its large diameter, high collagen content, and reduced donor-site morbidity compared to patellar tendons-has emerged as a promising option due to its biomechanical strength and reduced donor site morbidity. OBJECTIVE: To evaluate clinical and functional outcomes of rACLR using autologous QT grafts with a bone block, focusing on improvement

Progressive tendon-loading eccentric exercise therapy in athletes with patellar tendinopathy improves postural control, quadriceps strength, and pain: A randomized clinical trial.

INTRODUCTION/OBJECTIVES: Athletes with patellar tendinopathy (PT) have impaired postural control and an increased subsequent sport injury risk of sports injuries. Improving their postural control is crucial to reduce future sports injuries. The aim of this study was to investigate the effects of a 12-week eccentric quadriceps rehabilitation program using decline squats with progressive loading on postural control in athletes with PT. METHODS: In this randomized controlled trial, 40 athletes from local volleyball, basketball, and handball teams suffering from PT were randomized to either the e

Editorial Commentary: From Tendon Wars to Mars: Why the Quadriceps Might Be the Last Autograft Standing.

In 2026, we are sending probes to Mars, mining metals from asteroids, and debating whether artificial intelligence should write our operative notes, yet we are still arguing about which tendon to use for anterior cruciate ligament reconstruction. Amid the noise, the quadriceps tendon has quietly re-emerged as the pragmatic choice in a field long dominated by graft dogma. With its broad cross-sectional area, favorable collagen alignment, and lower donor-site morbidity, it offers an appealing balance between biomechanics and biology. Recent evidence shows comparable stability and superior patien

Abductor Tendon Tears in the Native and Prosthetic Hip.

Gluteal tendon tears are an increasingly recognized source of laterally based hip pain. These tendon tears are typically degenerative in nature and require careful history, physical examination, and imaging interpretation for accurate diagnosis. After a trial of nonoperative management, open or endoscopic repair can predictably improve pain, though expected functional improvement is correlated to the degree of preoperative muscle atrophy. The authors advocate for an open double-row suture technique to reliably visualize the full extent of the tear, restore the anatomic footprint of the avulsed
