Testosterone Therapy and Tendon Injury: Clinical Evidence, Biological Mechanisms, and Sports Medicine Implications.

Testosterone therapy increases lean mass and can improve muscle strength in men with hypogonadism. This review examines the relationship between testosterone therapy, tendon adaptation, and tendon injury, including the possibility of a mismatch between changes in muscle strength and tendon capacity. This narrative review critically evaluates the clinical and experimental evidence relating testosterone exposure to tendon injury. The present review focuses specifically on tendon-related safety and is not intended to assess the overall benefit-risk profile of testosterone therapy. Regenerative Trifecta: A Multimodal Regenerative Approach to Soft Tissue Injury Recovery and Return to Sport.

Soft tissue injuries - affecting tendons, ligaments, and muscles - are the leading cause of missed games, lost seasons, and early career endings in competitive sports. Injury rates across major professional leagues have risen significantly over the past decade, with the knee, groin, hip, and thigh bearing the greatest burden. The financial impact is enormous at the high school and collegiate levels alone, while at the professional level, soft tissue failures have decided playoff outcomes and derailed championship-caliber teams. Standard treatments - rest, physical therapy, nonsteroidal anti-in

Evaluating Thigh Muscle Volume and Compensatory Muscle Changes in Bone-Patellar Tendon-Bone, Hamstring Autograft, and Allograft Anterior Cruciate Ligament Reconstructions: A Minimum 5-Year Follow-up.

BACKGROUND: Graft selection in anterior cruciate ligament reconstruction (ACLR) may lead to long-term, graft-specific muscle adaptations, yet their effect on functional outcomes remains poorly understood. Magnetic resonance imaging (MRI)-based 3D modeling enables precise quantification of muscle volume changes, offering insight into the relationship between postoperative muscle preservation and patient-reported outcomes. PURPOSE: To radiologically evaluate thigh muscle volume changes and patient-reported outcomes at minimum 5-year follow-up among patients after primary anterior cruciate ligam

Structural and Functional Principles in Quadriceps Reconstruction.

Quadriceps muscle and tendon injuries are a significant cause of impairment of the knee extensor mechanism, ranging from minor muscle strains to complete tendon ruptures and extensive defects following oncologic resections. This narrative review provides a comprehensive analysis of contemporary concepts in anatomy, biomechanics, diagnosis, surgical management, and rehabilitation, with a particular focus on reconstructive techniques and functional outcomes. While most muscle injuries respond well to conservative management, complete quadriceps tendon ruptures typically require surgical repair t

Sport-discipline and age-related differences in knee extensor musculotendinous morphology and function.

BACKGROUND: Sport-specific mechanical demands and progression from youth to elite competition may influence musculotendinous morphology and function, that potentially contribute to knee pain and injury in athletes. The objective of this study was to assess sport- and age-related differences in the knee extensor musculotendinous morphology and function, and self-reported knee pain and injury incidence in competitive athletes. METHODS: A total of ninety male athletes volunteered for the present study: cyclists (CY), basketball players (BP), and ice hockey-players (IH), with 15 youth (U19 teams

Complete Proximal Rectus Femoris Avulsion Repair in a Division I Collegiate Track Athlete: A Case Report.

CASE: A 20-year-old track athlete experienced a pop in her left hip with immediate pain and weakness during a race. She had a palpable defect distal to the anterior inferior iliac spine (AIIS). Magnetic resonance imaging demonstrated a complete tear of the rectus femoris direct head tendon from the AIIS, with 3 cm of retraction. Given her age and activity level, she underwent successful proximal rectus femoris avulsion repair and experienced a full return to sport. CONCLUSION: For athletes with an isolated avulsion of the direct head of the rectus femoris and significant tendon retraction, su

Injuries to accessory head of rectus femoris in professional football players: anatomical description, imaging findings, and clinical implications.

BACKGROUND: The quadriceps femoris muscle compartment comprises four muscles, including the bi-articular rectus femoris, which is particularly prone to strain in kicking athletes. Anatomical variations such as accessory heads occur frequently but are seldom identified clinically. This retrospective case series describes injuries involving a previously undescribed accessory rectus femoris head in professional footballers. MATERIALS AND METHODS: We retrospectively reviewed a series of cases in professional footballers from 2022-2024 with acute anterior thigh injuries. Inclusion criteria require

Knee and hip joint dynamics differ between sprinting and Nordic hamstring exercises.

Sprinting and the Nordic hamstring exercise (NHE) are common training modalities used to reduce hamstring injury risk, but the differences in the biomechanical demands of sprinting and the NHE are unclear. We conducted an exploratory analysis comparing knee and hip joint kinematics and kinetics, and biceps femoris long head (BFLH) muscle-tendon unit length and velocity during the flight phase of sprinting and the NHE. We collected motion capture and force data from fourteen athletic participants (8 males, 6 females) running (4-8 m/s) and performing the NHE. We used this experimental data and m

Augmentation of hip abductor repair with fascia of vastus lateralis.

BACKGROUND: Hip abductor tears are common and can cause severe limp and pain. Repair of gluteal tears often requires augmentation. Although current methods of gluteal augmentation have good reported outcomes, they can be costly and add an additional layer of complexity if they become infected because they rely on non-autologous materials. Based on the case series below, we report an alternative technique to augment gluteal tendon repair that uses vastus lateralis fascia. METHODS: Between 2022 and 2025, 23 hip abductor repairs augmented with a vastus lateralis fascia flap, following a primary

The Deep Myofascial Tissue Network and the Quadriceps Femoris Tendon Autograft: A Clinical Commentary.

BACKGROUND: Quadriceps tendon (QT) autograft use for anterior cruciate ligament reconstruction (ACLR) has increased. Like other ACLR tendon autografts, this tissue is a component of the deep myofascial tissue network. CLINICAL QUESTION: How does QT autograft use for ACLR compare to hamstring tendon and bone-patellar tendon-bone autograft use from a deep myofascial tissue network perspective? KEY RESULTS: The QT is a histologically and morphologically less homogenous tissue than bone-patellar tendon-bone and hamstring tendon autografts. Harvest site damage to QT aponeurosis fusion points during

Direct Anterior Approach to Hip Resurfacing Arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p

Determining the incidence of abductor tears in total hip arthroplasty via direct lateral approach: a prospective analysis.

INTRODUCTION: Each surgical approach for primary THA has unique advantages. The direct lateral approach (DLA) offers increased visualisation of the gluteus medius and minimus tendons and, consequently, has been suggested to facilitate management of intraoperatively identified abductor tears. However, there remains a paucity of data exploring the frequency of abductor tears in patients undergoing primary THA via DLA. The purpose of this prospective study was to: (1) report the incidence of abductor tears in the DLA patients; and (2) identify risk factors for abductor tears in patients undergoin

Anatomy and Pathology of Anterior and Lateral Hip Compartments Bursae: An Ultrasound Structured Approach in Sports Medicine.

When assessing hip pain in athletes, it is important to focus on the extra-articular soft tissues that may clinically mimic joint pathology. One such extra-articular structure is the synovial bursa. Bursitis can clinically be misdiagnosed as joint-, tendon- or muscle-related pain. These pathological processes are a result of inflammation that is often secondary to acute trauma, overuse injuries, arthritis, or infection. Traumatic and overuse hip bursitis occur frequently in amateur and professional athletes and clinical presentation is often nonspecific. After clinical assessment, imaging play
